What does monophasic flow mean Does it show stenosis
Thanks for your comment. The presence of monophasic flow generally indicates a significant stenosis or an obstruction in the arterial segment above the site where the Doppler waveform is sampled. For example: if you find monophasic flow in the tibial arteries you should suspect disease in the femoral popliteal segment. Hope this helps. The ABC team

Why not also mark the smooth echolucent plaque at the anterior wall which also seems to be present at the distal CCA? The marked plaque looks like an ulcerated plaque, there is a piece missing at the posterior wall, you see the eddy, stenosis will be >70% (but to be sure you have to know the velocity in the distal (unaffected) CCA, view from multiple scanplanes and power Doppler could also usefull in this case). And when measuring you should adjust the PRF to the height of the velocity (where is the velocity at its max, could also be distal to the plaque, measure through the stenosis).

The maximum PSV was recorded in relation to the posterior wall plaque, hence focus on the posterior wall plaque for this case. Not so convinced about the ulceration as there is not significant excavation of the plaque. CTA did not report any ulceration. Well done

Floating thrombus
The plaque is well attached to the arterial wall. It is indeed hypo-echoic which can look like a thrombus, however this is just a hypo-echoic plaque causing a stenosis of the internal carotid artery. Well done for answering
