Neurophysiology

Neurophysiology

This channel is primarily dedicated to topics relevant to Clinical Neurophysiology, medicine's best kept secret. General and topical health issues will also be covered from time to time.

The primary aim of this channel is educational.

As the channel has grown, I have an ever increasing 'in-box' of questions. If it's a general question relating to a given topic that hasn't been asked before, feel free to post it by the relevant video and I'll try to answer. They each have their own communities and all can benefit from the Q&As which are often quite interesting.

However, if it's a personal health question - please ask your own local doctor and also please do not email me directly.

Simon

Dear Doctor. I’m currently suffering from peripheral nerve issues as a result of a reaction to Ciprofloxacin. I’ve been tingling for a few days now. Is this likely to settle down?

  • @DrSimonFreilich
    @DrSimonFreilich17 күн бұрын

Hi, it's now accepted that as a rare drug reaction, ciprofloxacin can cause these symptoms as a presentation of a peripheral neuropathy. The general advice is to discontinue taking it (you'll need to see your doctor re a replacement for whatever reason it was prescribed) and usually it settles by itself. There are no obvious reasons to predispose it occurring, and no specific treatments other than letting the body settle things down. Hope that helps, bw Simon

@@DrSimonFreilich God bless you Doctor. Thank you very much for taking the time to reply. This is very reassuring to know that the nerve issues will hopefully settle down after a while. I stopped taking it after the first dose. It only took one 500mg tablet to cause the reaction. FWIW your videos were a great help & comfort to me pre & post bilateral cubital and carpal tunnel surgery between 2018 and 2019

Managed to cure mine 🎉🎉🎉 Tried almost everything. Even doing squats before bed, that worked for about 10 min Multiple sets of all sorts stretches also , 10 min relief . Cut down on sugar , then after day 3 , went "cold turkey" Adding no sugar on anything . Used honey to sweeten whatever needed sweetening ( can't believe how the taste of fruit changed ) Normally nice , turned into sweet and delicious No soft drinks (that was hard ) Started switching food brands and bought the brand with the lowest sugar content (it's almost impossible to find food without processed sugar ) Cut out processed (take out ) foods as well . Except for grilled chicken and Chinese take out (I'm only human) Even though it isn't a 100% sugar free life . It dropped by about 90% Than was good enough Restless leg syndrome dissipeared like magic . Withing 2 days ,just gone . If a cheat day is enjoyed and maybe a milkshake is had ( really once in a blue) Then it feels like the rls wants to start up again . Had a bit of gout and starting sighns of arthritis. That's also a memory now. Please try cutting back on the sugar and see if it helps you . Peace and good health to all . Love from south africa . ❤

Thanks for the talk. RLS-UK follows The Mayo Clinic Algorithm 2019, which is now regarded as best line treatment. Iron infusions are first line therapy. The new American Academy of Sleep guidance , due to be released soon, will relegate the dopamine agonists to 'end of life scenarios' where short term benefits outweigh the inevitable long term severe complications. Nearly everyone on them WILL end up suffering severely. NICE & NHS guidance are now quite outdated. RLS-UK is campaigning to get RLS included on the curriculum at medical school and during GP training and to stop dopamine agonists from being prescribed, except in end of life scenarios. There are too many elderly patients on high doses of dopamine agonists and suffering severe, unbearable, augmented RLS. GPs do not follow up after prescribing these dangerous drugs. Impulse Control Disorder affects up to 38% of patients and there have been hundreds and hundreds of successful legal cases against GPs and neurologists for failing to warn patients of the high risk of gambling, impulse spending, etc on these drugs. Time for them to be stopped and iron infusions and gabapentinoids used first line, with long half life opioids ( methadone and Buprenorphine) used for refractory RLS. The economic savings in reduced NHS appointments/ medications and increased productivity will far outweigh the cost of training GPs properly in this common disease. I do hope new treatments are researched and trialled now that the recent gene study by Dr Steven Bell, et al, has been released.

This is passed down through your DNA from your mother or your father, We know this... Why would people who have this horrible thing going on and their bodies have children? What in God's name are adult people thinking when they know they're passing this on to their children to suffer every day of their life.? It's selfish, Nor socistic, and unforgivable. I have something similar, Is my parent knew this, They did not have to have children and pass this c*** on down for them to live miserable lives. That's my opinion...

Sorry kinda ranting, but the truth must be told.

I have been suffering for years with what I now think is what I just watched. Been to every doctor, who I've been told is the "Best" including the over rated JHU circus. What I have realized is every time I go to another "Specialist" they read the notes from the prior "Specialist", tell me to push, pull and squeeze their fingers. No one looks at my skin, atrophy etc. This doctor who made this video has provided answers to every question I have without asking. I'm sorry he's not in our country, which is too busy letting politicians destroy it and get rich as we are the cattle and sheep make to shut up and get in line. Thank you for the video

I was a chronic alcoholic for 5 years in my early twenties. I’m now 27 and I get these spasms all over. I think it’s neuromyotonia or issac’s syndrome. I also have enlarged calves due to the constant muscle twitching. Even in my sleep. Blood work comes back normal and everyone writes it off as severe anxiety and depression. Sucks no one helps.

I was told Intravenous IV-IG can reduce Small Fiber Neuropathy via regular Intravenous administration. Recently I was introduced to a oral capsules by METACURE. The Revive Alpha Lipoic Complex with Curcumin is much faster than Intravenous IV-IG and ALA. The SEVERE NERVE PAIN, TINGLING AND BURNING are reduced about 90% to 100% in one to two weeks. The NUMBNESS is slowly going down though takes much longer.

I seem to have just gone past the first stage, following a slapped cheek infection. After two trips to A&E (the second involved giving me morphine, which only mildly dulled the pain, but at least let me sleep for 2 nights), the fourth consultant came up with this. I still have to see someone face to face about it, but the symptoms and painlevels ring true. When the shoulderpain started, I noticed that the soft tissue under the collarbone was extremely sore. Is this a usual presentation of this ? The worst for me was the lack of sleep...one can't function on 1 hour of sleep and my liver is ringing warning bells from the cocktail of pain killers. I just hope that the referral appointment comes through soon.

This umbrella of diseases are very cunning and sometimes move really slowly , please make sure you monitor your CK levels - specifically CK-MM which indicates skeletal muscle damage , there are three types of ck ‘s . Monitor aldolase levels and all the other typical inflammatory markers . Some variations of this despises cause a lots of pain and weakness , make sure you find providers that understand it will take a village to take care and monitor this conditions, multidisciplinary approach if you could find it . Keep moving , do not stop moving . Reach out to the Myositis Association/ very helpful. Sometimes there’s overlapping diseases , they don’t come alone ( SAD) . Can happen to anyone from children to elderly, don’t give up asking to be identified, sometimes is a long journey. Ask questions , send messages , advocate for yourself. Keep track , collect data , identify flare ups and get tested during this windows of time . Take care
