    Another point to make in your teen years, make sure you get your wisdom teeth out if they’re impacted. I avoided getting mine out because I was scared of the surgery, combine that with bad oral hygiene and I lost almost all my teeth at 23 😮

    One tooth posterior dentures are not too comfortable. Their only purpose is to keep your remaining teeth from shifting. You can chew on it, but you won't find yourself using it that often if the denture is only replacing a single posterior tooth.

    I plan to possibly have my front four teeth removed for a partial (can’t afford implants or caps which led to this scenario), would the metal partial be very visible since the clasps would most likely be around my canine teeth?

  • @johnjazayeri
    @johnjazayeri12 күн бұрын

    Hey there. You can have the denture extend to the back teeth and place the clasps on the back teeth only. This would require some planning and discussion with your dentist. You can also opt for a flexible denture with pink clasps in the front. These are the most common ways of getting around a metal clasp showing on your front teeth!

    Hi there. Thanks for the detailed explanation. I wish you would mention which tooth it is that is bothering you. Anyways, I personally hate root canal retreatment. If the root canal did not work the first time, why should it work the second time? There are exceptions of course, but generally a root canal retreatment does not clear the tooth from infection (exceptions are one or two nerve teeth where the root canal was performed below the standard of care and there is no infection on the tooth). Having said that, there is a good chance that your tooth is still infected and the root canal is only masking the symptoms. The only way to tell if this is the case or not is a CBCT of the tooth. The infection could be hiding behind the tooth or in between the nerve, and that only shows when you take a CT scan. I just don't understand why people insists on spending so much time and money on extending the life of an infected tooth when they have the option of removing the tooth and replacing it with the implant? This goes for patients, but more so for the dentists. I hope this helps. Feel free to send me an X-ray of the tooth and I will evaluate it more carefully. good luck either way!

    He’s correct,corporations are making implants a big business bc it’s near 5 billion in 2024 and will be 7.4 billion in 2027 in USA alone. Best to seek dentist that owns the practice- but many still price gauge & fail to possess the SKILL & tech needed to do best job bc u do not want to pay again for failed implants- more common now due to high demand.

    Holistic dentists claim to use only natural ingredients. But many infections cannot be cured with tea, tree or clove oil. DNA & one’s immune system determines if I holistic will even work effectively.. 80% of the population grind or clinch unknowingly- so a crown with no metal will crack easily. DIET- huge factor in oral health.; eating hard foods, sugary drinks & foods, acidic foods/ fruits and drinks, even 1 nut can chip or crack a tooth/molar, especially as one ages. Also, even if born with great DNA ORAL ENZYMES, medical treatment/ accident can adversely affect one’s oral health, especially saliva which carries the good flora to protect against bacteria and entire mouth. If one is in INDIA or Malaysia, they have done tree that naturally cleans teeth IF one is young, healthy & has no sugar diet, eating like natives. Not ALL natives have great teeth- they have dentists there too- it’s usually a lot cheaper than USA. Austria has great national health system that includes dentistry. Some USA ppl travel there bc got family/ friends to stay while getting implants / more costly procedures., paying all cash.

    Implants in 2024 is 5 Billion $$ business in USA& insurance companies state implants costs will increase 2.3 Billion each yr forward, to 7.4 Billion by 2027. You are kind to explain these costs, but in truth few insurance covers most dentistry procedures despite demand & new costly tech . I know all the new tech has monthly maintenance paymnts too, like most software. At least u have real experience- so many get botched jobs bc ppl cannot afford over $10k for 1 implant by specialists.

    Thanks for the information doctor. I got into a bike accident and my two front teeth were broken at gumline. It's so heartbreaking because they were healthy and now I have to get a rct. Is there another way of saving my teeth?

  • @johnjazayeri
    @johnjazayeri12 күн бұрын

    Hi there. I'm sorry to hear about your accident. If your front two teeth have broken at the gumline, then the only way to save them is with a root canal, post and crown. You still need some tooth structure above the gumline for this solution to work long term. Otherwise, if it's really broken at or below the gumline, then removing the teeth and dentures or implants is the better choice. Here are a few tips if you really want to save these teeth: 1. Consider connecting the two crowns: If there is hardly any tooth structure left, then the crowns may fall off. Connecting the crowns improves their longevity. 2. Crown lengthening: If there is almost no tooth structure above the gumline, crown lengthening can extend the useable tooth structure to improve the chances of your crowns staying on. These treatments are not cheap. Make sure that the teeth have a pretty good prognosis with root canals and crowns. Otherwise, remove the teeth and replace them with a denture or implants. So the most important factor here is how much tooth structure is left above the gumline. The more, the merrier! Feel free to send me your Xray if you want me to look more carefully at your situation. Best luck either way!

    if the implant is done by cbct guided imagery thing is the implant hitting the nerve not an issue?

  • @johnjazayeri
    @johnjazayeri12 күн бұрын

    Hi Carl. It is very rare to hit the nerve during implant surgery. Using a CBCT guide is helpful to position the implants. It reduces the chances of hitting the nerve, but it does not completely eliminate the risk. Keep in mind that the nerve is only an issue for dental implants placed in the lower posterior region. The good new is, if the implant that has hit the nerve is removed right away, the nerve will mostly regenerate itself. How can you tell if this has happened? If you receive a dental implant in the lower posterior, and the numbness does not wear off after 24 hours, then the implant may have hit the nerve. This numbness could also be from the needle or the drill, but I would rule out the implant sitting on the nerve immediately. Contact your dentist and have them take a CT scan to see if the implant is on the nerve. If it is, have it removed right away. Otherwise you could be looking at long term, or even permanent, ,numbness of the jaws!

    Hello, I'm concerned about a very bad smell developing in my mouth, specifically between my teeth, after a root canal treatment. The smell is particularly noticeable when I use dental floss. Could this indicate an infection or something else? X-rays show that the two root canals I had done so far on my lower molars were not done properly. My new dentist suggests that they need to be redone but says she won't do it and that this requires machine-assisted endodontic treatment, which involves a longer waiting period to get an appointment. The unpleasant smell started two days after the root canal, and prior to that, I had severe infections in those teeth and the entire right side of my mouth. Additionally, small red spots have appeared on the right side of my face at the level of my upper and lower jaw. Based on this history, could you explain what might be going on and what I should do next? I would greatly appreciate your advice. Thank you!

  • @johnjazayeri
    @johnjazayeri12 күн бұрын

    Hi there. Unfortunately, your symptoms do not sound good. First off, root canals do not work for severely infected teeth. Period. Trauma, sure. Mild infection, maybe. But a tooth with severe infection does not need a root canal. It needs to be removed. Everything else you mentioned about your teeth points to the fact that the infection is spread well beyond the tooth and into the jawbone. Keep in mind, you can not see the extent of the infection on traditional X-rays, despite most dentists wrongly assuming that you can. You MUST take a CT scan to see if the infection is still there, or if it has been cleared. IMO, cut your losses and remove the teeth. Your body will thank you for it. Feel free to send me your X-rays if you want me to take a closer look at the teeth. Either way, good luck!

Because I have lost 1st molar tooth#46 that is almost 10 years ago. So I ask my doctor if I need bone graft on my molar and he said it’s better. He do countour bone graft (sinthetic)together with the implant. Only he put bone graft on the side of cheek .I ask him if I need membrane he said no. He stitch it with nón resolvable suture. My question is why the doctor don’t do bone graft with membrane? Do I will have good results without membrane? Even I follow the post instructions that you said on other vidio to how to takecare the implant for healing so I will have good result? Appreciate your response. And Thanks 🙏 for your effort for giving us guidance when it come to dental 😀

  • @johnjazayeri
    @johnjazayeri12 күн бұрын

    Hi there. The purpose of a membrane is to keep the graft from moving. A membrane is usually only needed when performing tooth extraction or extensive implant surgery. If the implant and graft are placed at the same time, you probably do not require a membrane. If the doctor can proximate your gum tissue and achieve primary closure, then the membrane is redundant. I hope this explains the situation. I sure hope that my instructions help LOL! Best of luck.

    It’s the fault of the patient - so it’s you who lost the guaranteed outcome. Life though is complicated, and sometimes simple survival is all that some can be focused on. I do believe we create the protocols with our demands. Instant affordable fix. So I’ll first say stop blaming or putting pressure on dentists. It’s maybe somewhere we need funding to support different treatment plans. It’s a tough role as a dentist with statistics to match. Not to say there isn’t the odd poor quality dentist. I’ve had that too. Drilled too close to the tooth, a bridge of filling connecting two teeth and an incomplete root canal. Having said all that… A cone ct scan is the true way of seeing an infection.. i just use a neck sleeve for the thyroid protection. The studies are becoming interesting on how many infections are picked up. Mine has failed. I’ve suffered for years with an auto immune condition. Your organs are so focused on this infection.. metabolism, hair, hormones.. they stop becoming the priority. I’ve been eating clean, active for years. In a serious way. Stopped working. This seems to be my root cause. Hope is round the corner.. so is expected expense. So it will be that I’m working on next. Wishing you all healing.

    I should add , I have read for two months on apectomy on a upper first molar (left side) to decide between an apectomy or zirconia dental implant. I do not like infected seal tooth but if I go for an implant and it fails , I will endt ed up with a denture. My tooth is quite asymptomatic, and it is a root- canal treated teeth of 14 years. The infection is in the jaw bone that has been discovered using an Cone Beam. Thhe endodontist who made the root-canal told me that it was a small inflammation and the only thing he suggested was monitoring the tooth. The other endodontist recommends an apectomy with a 95% chance. I have an apointment for an apectomy for the tooth, but I am still unsettled.

    There are apectomy for a failed root canal, you are not at all a proponent for that procedure?

  • @johnjazayeri
    @johnjazayeri12 күн бұрын

    Hi there. Apicoectomy, is a procedure that removes the infection in the jawbone without messing with the tooth or the root canal. It used to be a very popular treatment. But nowadays, with the widespread use of dental implants, it is becoming more and more obsolete. What is my opinion? I would only use them on front teeth under rare circumstances. Here are the issues I have with apicoectomy: 1. Let's say the apicoectomy is successful and the X-ray looks better over time. Does this mean the infection is gone? And what if the X-ray does not show improvement? How can you tell if the procedure was a success? Honestly, you can't! 2. An apicoectomy requires removing bone and gum tissue. Should the tooth fail, there is now less jaw bone and thus, more complicated to place a dental implant. 3. Costs add up. Why not get a permanent fix instead of a patch up job? Especially one that is quite costly as well (don't get started with insurance coverage, they only pay because they don't care if you have an infection and it is a much cheaper solution that a dental implant). I would only consider an apicectomy on a front tooth with minor signs of infection. Back teeth have multiple nerves and this procedure rarely removes all the infection. Hope this helps. Either way, good luck. And feel free to send me your Xrays so I can better see your situation.

    Using the Phrase "saves the tooth" with a root canal is misleading. The root canal kills the tooth, and then a crown is placed over the dead tooth. Holitic Dentists are of the opinion that having a dead black tooth in your mouth, underneath the crown, can lead to health problems later on. Dentists should STOP telling patients that a root canal "saves the tooth" Canals keep a dead tooth in your mouth so that the crown has something to attach to. Its a big decision to make for yourself.

If you go online Dentists will say that the immune system cant reach the tooth yet if you study medical articles they say there is a immune response and the immune system has the ability to clear infection, remove dead pulp, and even seal the tooth if need be. Also if the infection can spread as you guys say white can’t the immune system enter the tooth? So only bacteria can leave but white blood cells can’t enter?

If so, then sure, go for the root canal. Or do you have a history of root canals failing on you? If so, then it's best to opt for the dental implant. Personally, I had three root canals, two which failed and had to be replaced with dental implants.Therefore I would opt for implant over root canal. But every case is different. Hi there. Sorry about losing your two teeth. Don't worry, not getting a bone graft does not mean you have no bone left! Bone graft helps your body generate more jawbone. If you don't receive a graft, your jawbone still heals, but you will have less jawbone. If you decide to get dental implants in the future, this may pose a problem. Although there are other factors that come into play. But don't stress too much, you still have jawbone if in case you decide to go for dental implants. Best of luck!

It's always best to wear your dentures for a week or so before getting lots of adjustments. I usually limit my adjustment visits for a new denture to 15 minutes. Then, I see the patient every week or two until they are used to their new teeth. Getting used to a new denture is like riding a bicycle and takes some time. Good luck and I hope to get used to them sooner than later!
