That is the joke. It's highlighting the self-defeating folly of trying to put any concept beyond the remit of comedy. This was one of Ricky's best sets, precisely because it addressed the whole issue of bigotry in comedy. The people it mocks had decided to be offended by it before hearing it.

What's the conundrum? A lesbian probably isn't interested in a real penis, so it wouldn't matter if the transperson had one. Pus a strap-on isn't the same as the having a real penis hanging off you, so the trans-person isn't going to have such a problem with it. The joke doesn't really make sense.

Ricky Gervais is putting his career on the line here. I believe that his explanatory and extended delivery is to allow the naysayers time to catch up. He is not delivering jokes folks. He is doing his best, to his detriment , to wake us all up to the ridiculousness and the dangers of woke culture. From my perspective he is a brave man , speaking the words out loud that many of us agree with but do not have the intestinal fortitude to say out loud ourselves. Knock him if you like because you don’t find this funny but at least give the guy credit for sacrificing his his normal comedic routine and probable loss of income in order to bring awareness to the idiocy of the woke ideology. He has the platform to do this that I don’t have but if I did I would be on it. Kudos to Ricky Gervais and shame on the critics who don’t have the mental agility to understand what is actually happening here.

Speaking the truth has never been socially acceptable. Victorian society is famous for repression and hiding the ugly parts of humanity, but we still do it in numerous ways - there's so many taboo topics that it's riddiculous. I mean sure, not talking about defecation at the table has a valid reason behind it, but so much stuff is not justified. Why not talk about salary, politics, religion, death, age and looks? Comedians get a pass for anything nowadays, luckily and people like Gervais, D. Chapelle, or the great George Carlin are the closest thing we can get to philosphers, because they can speak the truth on anything as long as they make it humorous.

First, there are the old-fashioned-non-political transsexuals/transgender people whose gender dysphoria won't stop after puberty or young adulthood, a good example is Blaire White. Second, there are those who have been confused to think they are transgender by the social media and the political context, a good example is Chloe Cole. Were someone from these groups of people to kill themselves they would probably do it for different reasons: The first group would do it because they are seen and judged differently as what they would like. The second group would do it because they regret deeply what they did to their bodies. The only common factor might be that they feel isolated and different, which is not the punchline that you refer to. And yes, Ricky is the best.

Usually "kicking yourself" refers to feeling regret at a missed oppurtunity or wishing you had done something different. It implies that you're upset at your past self for making the bad decision, and wish you could kick your past self to express that anger. For example, I could say "Jake must be kicking himself right now, wishing he hadn't broken up with Taylor, she's so rich now." In this video, Ricky is kicking himself because he wishes he hadn't gotten the surgery to remove his penis, as now he's trying to replicate a penis by using a strap-on dildo.

It doesn't really make a lot of sense, but as a vague sort of observation the joke is about how the trans-version of himself would use a strap-on dildo, but then kick herself for getting rid of the penis to begin with.
