    Retired dental assistant here. I've only seen one patient come into our practice who was in a state similar to this. Absolutely horrific, and the patient was in so much discomfort. Even after the procedure - which was very necessary as the patient's life was in danger - it was sad, because with the calculus (tartar) gone, the patient's teeth were just flapping in the breeze. Please, Please, PLEASE, folks, do not let your teeth get anywhere near this bad.

The patient wanted the minimum done to resolve the serious infection in the mouth because there was a risk of the patient going septic (having the infection spread from the mouth to the rest of the body). We felt terrible letting the patient leave the office in that condition, but that's what the patient insisted on.

    @@mistiffiecation There are more factors than just hygiene, though that's a big one. There are also genetics. Everyone has slightly different chemicals in their saliva, and that means some people are more prone to forming calculus (tartar) quickly. The patient I mentioned above and one other who was almost as bad were both just in their mid-twenties. There were serious hygiene issues and neither had ever been to a dentist before, but to have such an enormous amount of calculus at that age, there had to have been genetic factors as well. So a person can have teeth like this as early as their 20s, but they would need to have no dental care, minimal hygiene (there are a lot of people who do nothing more than rub toothpaste on their teeth with a finger - seriously), and saliva that calcifies quickly. People who aren't dealing with a genetic predisposition to form calculus quickly may never get to this state so long as they practice acceptable dental hygiene and visit a dentist regularly. People who do form calculus quickly can still keep their teeth and not get into a dire situation like this as long as they are rigourous with their hygiene - brush AND floss everyday - and visit a dentist for a check-up and cleaning every 6 months.

    Answer me this please, because I’m confused. If this person has such horrible dental hygiene then why are his teeth and gums in great condition. Look closely, those teeth look very healthy. No bad spots or dark areas and his gums are nice. But then there’s this epic stuff on his teeth that’s supposed to be plaque. I’ve seen a hundred videos on here with people having the most rotten, poorly taken care of teeth you’ve ever seen and they don’t have plaque build up at all. I don’t see how you can not brush your teeth and have zero cavities, healthy gums but with epic levels of plaque build up. I wonder if some of these videos are fake, where they put something on their teeth to look like build up. I mean look at how perfectly clean those teeth are

    @@jtmemphismoney Ooh this is such a good question! I apologize in advance because I get real excited about these kinds of questions lol. Tldr; cavities and tartar buildup/periodontal disease are two different disease processes caused by different things (like type of bacteria). You can have one without the other, you can have both, or if you’re really lucky you have neither. I guarantee you these videos are not fake, as crazy as they are! The longer answer! There’s a couple main facts to know to understand this: 1- Plaque is mostly bacteria and food debris. It’s soft and sticky and it hardens into tartar over time. 2- Tartar is mineralized plaque build up on the teeth, like a hard water stain. The minerals come mostly from your saliva. 3- Our teeth are strengthened by minerals. 4- Cavities form when bacteria turn food we eat into acid, which then pulls minerals out of our teeth. Some people have more minerals in their saliva. This leads to more buildup and sometimes fewer cavities because the minerals are strengthening the teeth. This isn’t always the case though. Some people have more acidic saliva which causes more cavities. Different bacteria cause cavities than cause gum and periodontal disease and not everybody has the same exact bacteria in the mouths. Cavity risk can also be affected by diet. You’re right that the teeth themselves do look in pretty good condition. However, the gums are not. The redness, swelling, bleeding and the loose/floppy appearance all tells us that there’s a major infection going on in the areas of tartar buildup. Probably more important though is the bone that’s actually holding the teeth in place. This level of infection causes the bone to basically disintegrate around the teeth, which is why you can see some of them moving as the doctor is cleaning. In this case the person has so much bone loss and so much periodontal disease that even though the tooth on its own is healthy, they’re probably gonna lose some of them because of the condition of the bone, there just isn’t enough to hold them in place.

    I don’t brush every day, and once I didn’t brush for five months, but I never get this hardened buildup, because I am plant-based - dairy / meats / eggs etc are the main cause of cavities and moving teeth and hardened buildup and tonsil stones and most other h-problem, and it isn’t sugar or carbs, like, I eat a lot of sugar and carbs almost every day, and I never get new cavities or hardened buildup, only a bit of soft buildup which can easily be scraped off, and it also comes off after eating apples, and, the only thing that sugar can cause is temporary toothache in sensitive teeth, as it irritates the nerve a bit, so I try to brush regularly because I get ache in my broken upper wisdom tooth after eating most types of sweets and granolas, as it is very sensitive, but luckily no new cavities and no moving teeth in over a decade, since going plant-based! (Also, I haven’t washed most of my body and my head in over one year, even though I am OCD, and I don’t even have smęlls because I am the pure being, and my breath also never has smęlls, even when I wouldn’t brush for months, but, the reason why I haven’t washed myself is, because I cannot do anything, and I am stuck in an impossible situation at the moment, so, many times, not showering and not brushing can be due to a mt block, or due to not having proper conditions and a proper house, and not necessarily due to fear, though it can also be due to fear or due to a combination of multiple other factors!)

    Honestly, ppl have a breath / body etc that naturally smęll, so even brushing / showering won’t make it go away, and maybe just reduce it, and most cannot tell that it still has smęlls, but I can immediately smęll it if one is too close to me or if one talks or breathes and is one meter away or two meters away or half a meter away, because I have a very developed sense of smęll etc, so I don’t get too close to ppl as I am very sensitive to smęlls - however, I never have smęlls in my breath, not even when I don’t brush for months, and even my sweat and the other things that come from my body have no smęlls, as I am very pure and I also am plant-based, and, by the way, I used to not brush for months in the past, but now I brush regularly, at least every few days, or after eating sweets that give me a toothache in my broken wisdom tooth, but even when I didn’t brush for five months I still had no smęll, like, my breath just naturally has no smęlls, which was also tested and confirmed by others, not just by me testing it, and right after having something it smęlls just like the food or drink that I had, so right now my breath smęlls like Alpro blueberry yogurt with peanut butter and orange / mango juice as I just had those foods and drinks, and an hour later it shall have no smęll at all, as usually about one hour after eating and drinking it smęlls like nada (warm air without smęll) again!

    It can be horrendous indeed....and sadly for the patient can be extremely painful especially when the gingiva recedes so much due to the tartar stacking against the tooth and just increasing like a huge cyst that just increases.....I would not be too surprised if that patient had issues while he ate food.

    I think dental hygienist completed that cleaning actually. Dentist usually do not do cleaning ( at least in USA) . By the way the procedure is not completed yet. This patient need at least one more - fine scale.

    Here is someone who takes pride in their work. Excellent job. I’m so surprised by the condition of their teeth, after the removal of the tarts. Teeth weren’t yellow or had many cavities. I’m betting the patient won’t stop smiling or looking into a mirror for a very long time. Thanks for taking the time to give them back a good smile.

    Tontonan sebelum tidur, biar nyenyaakk😅😅 Btw, minggu kemaren habis scaling setelah 7 tahun lamanya ke dental clinic, dulu cuman di puskesmas after scaling cuman dikasih amoxilin biar mengurangi ngilu, kemaren di klinik gigi minta di-fluoride treatment utk ngurangin pembengkakan dan buat bisa nutup gigi berlubang yg kecil bisa bertahan sampe 6 bulan ke depan, ini ampuh sih tp ga boleh makan abis treatment itu jd langsung aku buat tidur

    That wasn't gum material, it was tartar. Basically left over chewed food. That's why there was corn in there as well.

    Это не рыхлая десна, а некроз ( отмирание тканей). Удивительно, что камень не повредил зубы, это такая зараза, он образует кисты и дыры в зубах, просто проедает! У этого парня железо- бетонные зубы, но вот боюсь, что убрав этот каменный фундамент, зубы начнут качаться. Много лет он растил эти глыбы у себя во рту!

    Its interesting because different bacteria cause decay than calculus. Mostly the calculus "shelters" the decay and dentinal sensitivity. Seems like this demographic of patients have more calculus than decay.... I always wonder if he uses LA... 🤔

    This dentist was quite gentle, given he had huge chunks to remove, so, bravo! Well done! Dentists are unsung hero’s, in my book! They spend years hunched over, doing their work in a tiny place. Then, once they’re near retirement, need back surgery, or need help with their bad backs later on! From those of us who care, thank you! Last thing - I feel so sorry for this young man, he’s going to really hurt, the next day! He’s so brave having it done, and am tickled they could afford to get this done!! Whew, dentist are SO expensive! Maybe it’s why this boy waited for so long to have it done!!

    My husband had tarter almost this bad & he was in his 30's at the time. He is now 60yrs & still doesn't take care of his teeth. He does brush them once a day in the morning but that's it. I wore braces for 7yrs as a child & I take very good care of my teeth. My husband can't understand why I won't kiss him anylonger. His teeth make me sick!!! Once some tarter broke off the back of one of his lower front teeth & he thought his tooth had broken. I told him it was tarter but he wouldn't believe me. He did go to the dentist finally. The dentist cleaned his teeth it was the first time they had EVER been cleaned (I blame his parents), he cried & wined about the pain afterwards for 3-4 days. I told him that if he would just go to the dentist twice a year he wouldn't have been in so much pain. He has now gone to have his teeth cleaned a total of 2x's in his entire 60yrs of life. He still won't listen to me, it is so sad. Thanks for reading an old woman's (62yrs), bitch about her husband & his teeth.

    It was watching videos like this as a form of exposure therapy that got me over my fear of the dentist. :) 1 root canal and 5 fillings later and it's just routine visits for me now! If you are afraid of the dentist let me first note that my root canal was not only painless, but it felt like a much needed flossing, as though I could --while numbed--tell there was some relief going on. It was a spa day and a half, and I absolutely love visiting the dentist now. I wish you all the best on your road to dental health!!

    How can the teeth of these people be in this condition? It's amazing, I've never seen tartar like this and it's still surprising that the teeth survived all this punishment.     Mind you, girl, the Tartar works as a protection wall, and its peculiar chemical composition serves to neutralize exactly the sort of acids that cause karies. In more extreme cases, the teeth have loosened to the point of falling out if it weren`t for the Tartar keeping them in place.

    I’m so surprised by the condition of his teeth, after the removal of the tarts. Teeth weren’t yellow or had many cavities even with that massive tartar!!! By the way... Great job!!! I wonder why did the patient let his teeth got like that.

    На самом деле зубной камень как панцирь, обволакивает и защищает зубы. У меня тоже удаляли зубной камень, Я переживала, что там под ним, как мои зубы. Но все оказалось не так уж плохо, зубы были здоровыми и белыми. Правда, камня было в разы меньше. Мне 57, у меня все зубы свои. Нужно просто чистить их правильной пастой. И обязательно на ночь.

    How does one's teeth get that bad and none of them break of fall out. And here I am brushing mine, taking care of them to the best of my abilities, and I still have to get a filling and one or two of them pulled..... Plus my gum is preceeding and there's nothing I can do to fix that.

    Pretty sure that patient plaque is the only thing preventing cavities from the intense gum loss. All those plaques that were removed covered parts of the lower tooth that doesn't have enamel. They definitely look "better" but I pray their teeth aren't starting to rot now.

    What causes this? I'm sorry but I have never seen anything like this and the dr being as gentle and thorough. Steady and gentle as possible. Great job on the skills. I know y'all must have some strong stomachs. I hope the patient heals quickly and doesn't let this happen again.

    When you don't brush after waking up or before going to bed you'll see a sticky build up on your teeth. You can feel it after waking up in the morning and feeling your tooth. If you don't brush this soft gooey layer on your teeth hardens up after 42 hrs and forms plaque and tartar. The tartar grows and keeps hardening every time you don't brush slowly and keeps eating your gums and bones holding the teeth away. That is why in severe cases the teeth are all wobbly after scaling. This dude though prolly hasn't brushed even once in 30 years. That's why it's so bad.

    I'm sure this young man won't take care of his teeth since he seems not to have been taught as a child. I know that my husband wasn't taught & he still only brushes his teeth once a day in the morning.

    RDH here. Great job! Need Deep cleaning. There’s a lot of mobility on mandibular anterior teeth and some slight mobility on some molar teeth. Hopefully once tissue regains health and tightens up it will be better but for long term prognosis to save some of the teeth looks not so good.
