Flexor pronator strain: Signs, symptoms and treatment of this elbow injury

A flexor pronator strain is a common elbow injury in baseball pitchers. It is a strain or tear of the origin of the flexor-pronator muscles off the medial epicondyle, or the bony prominence many know as “the funny bone.” These muscles flex the wrist and pronate the forearm, or turn the palm toward the floor.

SIGNS AND SYMPTOMS OF A FLEXOR PRONATOR STRAIN
A flexor pronator strain can either result from an acute event or can develop as a result of overuse. You might remember a specific throw where you felt sharp pain on the medial side of your elbow. Or you might not remember a specific injury but notice pain there that gets worse with continued pitching. Your pain will usually be located just below the bony prominence on the inside of your elbow. You might have pain flexing your wrist or pronating your forearm.
DIAGNOSIS
If you see an orthopedic surgeon or sports medicine physician, he will likely order an x-ray, even though it is usually unremarkable. An MRI can show a strain or tear of the tendon as well as rule out an injury to the ulnar collateral ligament, also known as the Tommy John injury.
TREATMENT OF A FLEXOR PRONATOR INJURY
Usually a flexor pronator strain resolves with rest from pitching to give the tendons time to heal. Ice and physical therapy can also be helpful to decrease the athlete’s symptoms. As the pain improves, the pitcher will be started on a long toss program prior to returning to full pitching. Occasionally a complete tear of this tendon could require surgery.

    For most of my life, my sports injuries were lower body related. But, since starting VR fitness 3 years ago (I love it!), I've had to learn my upper body like never before. Currently suffering my first even elbow-related injury... likely from overuse in VR. I've felt what you describe before, by this feels deeper, like in the space between the back of the connecting bones and diminishing overall strength and stability of movement. Not severe pain or disability, so just resting it. Thanks for the informative video!

    Hi Doc, I'm 45 and never had arm issues but I'm pretty sure it's the flexor pronator. I pitched 2 weeks ago but played catcher last week and I started feeling pain after a throw down to second base (which was actually on point but my 2B was out of position). I was able to throw a tiny bit more during the rest of the game, but I'm in between weeks and looking to heal up. I'm resting it now, but any nutrition and/or stretches for a speedy recovery would be greatly appreciated. Thanks!

    This video just confirmed that it’s my mouse at work doing this. I’ve always had a thumb ball mouse but my coworker had a thumb ball mouse that had optional heights. The way you described the rotation of the palm to lay flat and how it attaches to the elbow nailed my forearm pain. I realized that while I was off for a week for the holidays I didn’t have any forearm pain till I had to use my mouse for updating my insurance. The pain instantly came back. I have to find another mouse that doesn’t cause me to rotate my wrist the way this one does.

    I have this, I had started using a very heavy hydraulic nail gun at work, as well as very strenuous work with my hands.. It’s been about six weeks now and it’s getting a little better from what it was that morning I woke up and my two fingers were completely numb, on both hands actually but the muscle grip strength of my two fingers on my right hand hasn’t full come back yet.. I can feel the pain like I pulled a muscle on the inside of my elbow though,..

    This happened to me as a kid when I was pitching in baseball. I put a lot of pressure on myself to be the best. When I was pitching at a game I threw a pitch that hurt a lot, but It was a strike. I didn’t want to mess it up as it was one of my first games as a pitcher. I threw another and started jumping around in agony. I couldn’t throw a baseball for 6 months after that. I probably tore it but never went to the doctor.

    I got it from drinking a 12-pack of Corona and trying to carry a 36" Sony Trinitron TV to my upstairs by myself so my kids could hook up the old Nintendo and play duck hunt. The TV is a monster. The Corona beer was the courage. And now I find it hard to pick up a piece of paper without screaming pain. Lesson learned. The pain is located right by my funny bone. And it is the most miserable pain in the world. Feels like pliers pulling on my muscles from the elbow. So let that be a lesson. Get help moving old tube TVs.

    Thank you Dr David for describing exactly what I am suffering from. Mine came gradually from working out in the gym. I can't pinpoint which exercise caused it, it just got gradually worse. It's a very strange injury. For example, I flat fly with 26kg dumbbells and it's the initial lifting them on to my knees before lying flat on the bench that gives me the most pain. Once I'm doing the exercise, I suffer from no discomfort. I only get the pain when I'm moving my arm. Also, when I bend my arm with my fist up to my shoulder, there's barely any pain when I prod and poke the inflamed area. By the way, great to see you're a Liverpool FC fan. That's my team and I was born there.

    Bro this is the exactly the pain I'm going through, I've just got back into gym 2-3 days ago after a back injury. I worked out on upper body mainly and I was fine. Second day I couldn't stretch my arms out 100% and today my right arm is dead. It's almost like a pain in my forearm muscle and the joint. What's helped you to get rid of the pain and recover from the injury?

    @@DrDavidGeier I get intense pain in lower part of my elbow the bony bit I heard grinding and a click and felt out of joint heart when elbow stretched. Back to normal but it happens again every time I put to much pressure so it's no a year or tennis elbow

    The inside of my elbow hurts when I press my fingertips on anything or I lift a tray of mail or magazines. I work for the post office. I try doing stretching and motion of range movements to warm up the muscles but sometimes I wonder if I'm hurting myself more.

    Dr. Geier, Thank you for your time. I have none of these diagnosis. What I can only do is describe the elbow situation as on the outside of the elbow or pain situated in the muscles /ligaments in the joint area. It occurred while making a tennis shot. I'm in my sixties and usually get over these pains as I have with the other tennis elbow epicondilitis pathology. What's a mother to do???

    I am an arm wrestler (puller) and my move (lane) of choice is a top roll which is mostly pronation. After training hard with teammates who are much more experienced and stronger I must have over worked my flexor pronator.

    Trying to figure out what’s going on with my 13 year old’s elbow. He’s coming off of an injury (lumbar fracture) and has not thrown much at all in the past 8 months. He’s trying to get back into pitching, but he keeps getting occasional pain on the inner elbow. It’s definitely not overuse…strain? UCL?

    Thank you for your video. I have a 50% tear. MRI confirmed. A traumatic injury I am having surgery I'm two weeks. I have given it five months of rest-activity modifications cortisone injection. Minimal help. I'm very active and workout daily. 😊

    great video, dr Geier. My injury on the flexor pronator was due to overuse and somewhat calcified or arthritic funny-bone area (over 20+ years of use) which made the ligament pop back and forth over the tip of the elbow, hence the strain and eventual small tears. Time to visit a surgeon. :-)

    Hey I believe that I just pulled my inner tendon, very similar to what he was explaining. Did you end up getting surgery? What did they say about your strain? Maybe I could find similar solutions for myself through what happened to you.

    i think that i am also suffering from flexor pronator, or i just guess like as you mentioned in vid. So if i need to describe the case of happen, it might be occured while i was swimming one or two hours for 1 week. Because i cant remember any cause rather than this case. i hope it will not getting worse in weeks, it took 1 or 2 weeks and it always got worse.. any ideas ?

    My injury happened while doing chin ups. I was doing this while wrapping a towel around my pull-up bar in order to get a larger grip. this was ment to help sort of engaged the forearm with a static hold. As soon as I felt the pain in my arm I stopped. Hope it gets better.

    @@heal4066 8 months later. There's no longer any pain. But I still Would see a doctor of are you. I strained by tendon. I can do pull UPS now but I can only Curl A maximum 30 pounds Without my forearm hurting.

    @@joshmonroe5740 I would recommend light weight high rep wrist curls to help get blood flow into the pronator/tendon and all of the forearm flexors.

    Sir, my MRI revealed a flexor tendon strain. If feels a little better, but now I have a bit of pain near my tricep where it meets the joint. Is this something the MRI could have missed, or could it all be related? I have been limiting activities but am still lifting light weights without issue

    On July 25th of this year, I hit a drive and I had a lightning strike of pain in the area you are talking about. I couldn't believe the pain, it was 10 on the scale. Doctors said that I had tendonitis and gave me a cortisone shot. They didn't listen when I told them that I'm sure I tore something. The cortisone shot did nothing, after 3 weeks I told the Dr. that I think more testing should be done so he ordered an ultra sound. It shows a frayed tendon so he recommends physio , after several treatments I started developing tendonitis on the outside of my elbow joint and still have pronator pain . I am going to talk to the Dr. on Monday about PRP treatment which he recommended. So after 3 and a half months my elbow is in worse shape than when I injured it. I'm glad I saw your article on KZread. No Dr. ever mentioned what you described and that's exactly my problem. Thanks so much for posting this. Love 💗 and peace ✌️☮️

    I have this on my right arm from my one arm chin-up training on the gymnastics rings. I'm going to floss my elbow on a daily basis and just take a rest from chin-ups for the next 4 weeks. might be time to take some rest after focussing on this one exercise for the past 5 months.

    I’ve had pain on the inside part of my elbow for about a year and rest isn’t cutting it. I’m a door installer so that requires lifting heavy awkward positions I’ve always thought it was golfers elbow but this video made me look into an MRI. Or the test to know if it’s UCL tear

    Absolute must is rest maybe even a splint/ strap, depending on severity. Then after a few days try gentle stretching and very light gentle muscle movement, repeat, rest, stretch blood supply, muscles heal about 4 weeks bones 8 weeks tendons 12 weeks and ligaments 16 weeks. Don’t rush it . If pain is really severe stationary or you have a functional deficit ice you can’t move a finger no matter how much you try, you may need surgery as injury may be complete tear/ rupture.     Hello Doctor! I had a bad fall on one of the trail runs. I fell with my arms outstretched. It's been four weeks and the inner side of elbow still hurts when I fold my completely, like say when I brush my teeth or comb my hair. So which exercises would you recommend to releive this pain?

    I carried something heavy on my left hand and I hadn't notice that my elbow and forearm were at pressure where it was rotated, in an uncomfortable position. That same afternoon my arm became weak and numb sort of. I got scared cuz I thought I was gonna have a heart attack 😅

    @drdavidgeier I’m not able to get an MRI at the moment or for at least 4 months. I can move my arm about just fine. I heard a pop when I strained it. I have a bump only under the forearm. My bicep did slightly move up. Is there anyway your bicep can move up without tearing your distal bicep tendon? Thanks!

    hey! i’m 15 years old and i play girls fast pitch softball. after a little bit of throwing, i feel horrible pain in the inside of my elbow. i took a couple days off of no throwing only batting, but after 2 or 3 days the pain has slowly started to come back. (told to by my athletic trainer) it hurts so bad to the point where i feel like i’m going to throw up. it also hurts during the day when i press on it, and when i wake up, it’s already aching. i’ve been seeing it could be very serious or just take getting used to. if you could help in anyway that would be great. thanks!

    My left arm was hurting,went to the hospital, got n xray,and a sling, didnt do nothing for me,got an electric muscle stimulator, pain is minimum, but my index finger is still numb.any other suggestion.

    Hi, Im having issues pronating where I feel pain coming from the funny bones front (to the hand). I do a lot heavy lifting being an ex powerlifter. I also hit my funny bone some 8 weeks ago. Im not getting better, fully rested 3 weeks+ with NSAID. Doctors visit is 3 month away im sweden :(

    I have managed to strain this muscle and the medial tendon from doing poorly controlled knee-ups on a high bar at crossfit. My back muscles should have been taking the load but they're not strong enough so they weren't engaged and my hands and forearms took the load. I now have months of rehab ahead of me. Not only can I not do crossfit for a while but I am in constant pain with this. Even exercise bike cardio makes it throb and I'm not even using my arms doing that! NSAIDs are my friend right now.

    I got that problem from arm wrestling and I just rest my hands for five weeks without putting too much pressure on it and it got better, but if I put any overload on the arm it will get inflamed again. For example whenever I go to the gym and do benchpress or biceps curls it start acting up again

    Every time I do a pushup I heard a grind and click in my elbow and it hurts a lot and when I stretch out it feels like bone on bone and within a few minutes back to normal can do normal things only pain with strain it's on the lower side only too it felt like it out of joint... Haven't done pushups in a while

    Hurt my forearm trying to do incline dumbbell press with 125 pound dumbbells. Couldn't get the one in the left arm fully up when I laid back and it pulled something in my forearm. It hurts when I try to pull something.

    My flexor has been bothering me for months maybe more than 6 months and it’s because of boxing. When I do my left hook my elbow starts to hurt after a workout. I’ll try these solutions and see if they help. I’ve taken a couple weeks break but it never went away. I think holding my phone also caused some pain after long hours of holding it

    Bro im a professional boxer and ive been having this problem here recently too.. Its only when i throw left hooks. I sparred 4 days ago and decided to throw it and i suffered a severe sprain.. Inner flexor was a big knot, then it bruised all the way just underneath my wrist. I still have function and its gettin better. Probably a grade 2 strain.

    So the other day I was playing basketball and when I dunked, I just barely got enough height to wrap my pinky and ring fingers on the hoop. Well, barely hanging on for a split second was long enough to really strain something, because now when I bend my fourth and fifth fingers it causes incredible pain. Should I go the the doctor, or just see if it’ll heal in about a week?

    It sounds like a tendon strain and will take a good 3 months potentially at a minimum. With just rest without rehab I had this injury for 6 years. Patience and not jumping into quick to training will be something to master. Look up Thera band as tool to potentially aid your rehab.

    Dr. David, good morning. I have a grade 1 Flexor pronator strain. I notice when I did this at the gym. I was doing a back movement. Will some thing like this require surgery? It’s still swollen on the inside of my elbow. It’s been a good 6 months or so.

    I love digging... I dug to deep and my left elbow gone bust, anti inflammatory works, only for soo long I have not done 5 hours of digging for 5 years. I cut my left hand half off when in my early 20's, Docs always said I would have trouble with it later in life... I also dislocated my right elbow, when the casked me up. They never put it in right... 3 weeks of pain. Now a 20cm scar and chunks of bone left in my elbow. Elbows to me are like knees

    So I play tennis and when I play and I am hitting serves that mainly flares up and causes my elbow area to throb very painfully. It will usually happen around the second match if a day or weekend, and it used to only flare up from serves but now if I play a tournament over a weekend with lots of serving obviously sometimes usually 2-3 matches, then during the following week it flares up without hitting any serves and is painful unless I take like a 4-5 day break and then play again without serving at all the pain thins out a little. It’s gotten to points during my highschool tennis season where it flares up and starts throbbing really bad to where I don’t have the strength to lift up my water bottle to drink, I will get shakes and fell like my arm is going to collapse I would have to drink with my left hand.     Hello! I recently had baseball tryouts, and I made the team, but I threw to ball too hard trying to impress the coaches. Before tryouts, i hadn’t thrown the ball and practiced in a really long time, but then i immediately went out to tryouts and started throwing hard and it caused my injury. Just yesterday I was doing warmups and out of no where i started feeling this awful, tight, stinging pain in my elbow and the pain traveled down my arm into my fingers. Do you have any clue what I could have done? If you could respond it would be really helpful. Edit: Went to the Orthopedic and they said i’ve got tendonitis in my elbow........can’t throw for a month

    I’m a softball player I’m an outfielder when I throw it feels like a deep sharp pain in my elbow and I have no idea where it comes from and it is getting worse and worse every time that we throw we have a game almost every day for the next 3 months.

    Stop playing and definitely get it checked out. I was in the process of doing a lot of calisthenic workouts and I was doing 30 pull-ups consistently for a week and 3 days but on that third day I pulled it nice and good! I continue to work out a few days after doing pull-ups and dumbbell raises but it made it really bad. I would definitely get it checked out.

    I felt a pop and pain in that pronator tendon while using a dumbbell. Within an hour I had bruising and massive swelling/ fluid from inside of forearm upwards into biceps. Did you have bruising?

    i was moving a table with my son yesterday and felt kind of like a pop in my upper forearm, it was painful, took some inflammatory and now woke up with pain in the upper forearm where then albow bends... i have a range of motion but its definately eweak when trying to grip and pick something up..wa scs is ivebeen resting my right shoulder because i have a slight labral tear so i think imay have uver used my left arm. what should i do? rest or go see my doctor asap?

    My strain came when I was walking a new rescue dog and he lunged at another dog & as I was trying to restrain him he dragged me several feet. It has been 5 months and it still hurts. Don’t have insurance so haven’t seen a doctor.

    Mine gets irritated doing bicep curls - less so with hammer curls. Also, when I use my hand/fingers a lot, like when I use a mouse or type for long hours. It is not getting better

    I have no idea what I did to my elbow but it’s been like this for a few months and I get the shot of sharp pain once in a awhile and I’d I run my elbow lightly on my bed mattress or something it feels weird and it’s on my left a elbow and I’m right armed/handed I’m super scared and I don’t know what I should do….     I been Having pain in my arm can't lift anything I been to three different doctors they have not done any x rays or MRI they just go by tennis elbow so I did everything they ask and the pain has worsen I can not get to a specialist until three months is there anything I can do until then.

    your problem is may be a mytotendinous junction issue and so massage with a golf or spikey ball, the area. Rest for 10 days from gripping and if you do gym, then keep your wrists neutral don't twist the hand as this will aggravate it. I have this injury and am telling you what my physio has advised me. Also, get a massage gun such as a theragun and massage your arms.

    Hi. I’m having pain around my biceps tendon on the inside, right by my forearm. I did it when I was trying to lift a shetters and I felt a pull in that area straight away. It’s been 3 weeks now and the pain is still there. Every time I try to lift a bag or anything, it hurts. It also hurts when I tense my arm or when I flex my arm to a 90 degrees. Any tips of what is wrong with it please? And any remedy for it pls ? Many thanks

After 4 days of following iceing 3 times a day the swelling and little pain is gone. But there is still little swelling and pain . I can't even make a proper movement on wrist, finger and elbow.     I was squeezing something with my hand and all of a sudden I felt as though a very strong elastic band slapped my posterior forearm right in the middle. Now I get pain in my Elbow if I lift anything above shoulder high and it’s getting worst (even light objects like a coffee cup) and I guess weakness or just inability to lift higher.     Hii, can someone tell me if forearm numbness and tingling is dangerous? Im a pole dancer who recently did a movie that requires forearm and it’s been 2 weeks my forearm is numb and when there’s a bulging vein when I touch it kinda gives this electric shot feeling but there’s no pain. Hospitals in my town are not attending unless im dying which it makes very difficult to know my problem…

    I hade fell on my wrist skateboarding and hade the same experience I ended up slightly breaking my wrist but it was a hairline fracture I would go to a doctor if you can

    I don’t know where this thing came from but I’m having a lot of pain all around my elbow. With tingling and numbness in my fingers. When I stretch my arm out( hurts) you can see a bulge in the crease of my arm. Happy 4 th of July to me.

    pretty sure I just did this to myself last evening, tripping on my shoelaces and taking the brunt of the impact on my right arm. It's really painful but not so bad to suspect a break or fracture so I don't plan to see a doctor
