How long does it take an MCL injury of the knee to heal?

Many people want to return from injury as quickly as possible to play sports or to exercise. On the other hand, many patients are simply concerned about when they can do their normal activities. In this Ask Dr. Geier video, I address that concern regarding an MCL injury of the knee.

Bhavesh asks:
I suffered an MCL injury while doing squats. It has been 2 weeks, and the pain is less compared to the first day of injury. How many days will it take to heal so I can just walk and do my work? I apply an ice pack 3 times a day and rest and do some normal exercise. Is that OK?
The MCL is the medial collateral ligament. It is a thick, strong ligament along the inside, or medial side, of the knee that protects against side-to-side stress. You can injure it cutting to change directions while running or when someone hits you on the outside of the knee.
TREATMENT OF AN MCL INJURY
MCL injuries almost always heal without surgery. Your orthopedic surgeon might put you in a hinged knee brace to allow you to regain knee motion while protecting the ligament as it heals.
TIME FOR AN MCL TEAR TO HEAL
How long it takes the MCL to heal depends on the severity of the injury. These injuries are graded 1, 2 and 3 which roughly corresponds to mild, medium and severe. Grade 1 injuries often heal in 1 to 2 weeks. Grade 2 MCL injuries often take 2 to 4 weeks, and grade 3 injuries can take 4 to 6 weeks or more.
In this video, I discuss the timeline of healing for an MCL tear based on severity and location on the injury. I also discuss return to daily activities like walking as well as return to exercise and training for sports.

Dec.2nd i fell on my ramp. Orthopedic doctor said i sprung or sprained my mcl .i still can't straighten or bend my knee that far. Its still swollen some. The last visit i got a knee brace like after you have knee surgery. I'm thinking about getting a 2nd opinion. Cause it still hurts and can't bend or straighten knee.

I injured my MCL about 2 months ago playing football. I gave myself plenty of time to rest, did light exercises for the knee, and slowly continued with my yoga practice. After a month and a half I was slowly jogging (barefoot) and my yoga/flexibility/mobility was improving... I'd say I was at about 85% with some mild pain when bending or extending. I gained a little bit of confidence and decided to play volleyball with some friends. There was one play where the ball got tossed close to the net which I instinctively jumped off one leg to recover... and "pop"...down I went. That happened yesterday and I am now having to restart this painful process over again. DO NOT RUSH THE HEALING PROCESS! I learned the hard way. This time I am giving myself a year before even thinking about sports.

Currently recovering from an MCL injury. I didn't have any physical injury to the knee but I have moved incorrectly whilst getting into bed the first time. The pain was excruciating, far worse than a normal tweek of a knee. The pain continued for three weeks solid unable to fully weight bare let alone walk. Finally went to seek medical assistance after a few days of no improvement, they said I just badly tweeked my knee. A few days later it happened again for the second time, but the pain was far worse and had to attend a&e where they said I had suffered a severely sprained MCL ligament to possible grade two MCL injury/small tear. No knee brace was required so i strapped it up with support bandages which helped to support my knee, rested and iced three times a day. At present it's now three and a half weeks on, I'm now fully recovered ish, but need physio to strengthen my knee ligaments.

I am 61 and in pretty decent condition. injured the knee walking my 2 dogs , about 60 and 75lbs, walking over a small bridge after a rain and the bridge has a foot wide strip of indoor outdoor carpet. The carpet was wet pretty soaked had my right leg on the carpet and left on the wooded bridge. dogs saw something and were pulling me to get it and my right leg slid out on the carpet the left knee bent sideways stretching the MCL. I started with ice and after a week moved to heat. Also have a stretch type brace. Knew does stiffen up when not being moved. Been about 3 weeks and I still walk the dogs. Knee feels best after movement and walking helps. Still very stiff after sitting any length of time and when getting out of bed.Have bought a heated knee brace. Has a rechargeable battery so portable but at this time usually use when stationary sitting down. The heat does help. I have self diagnosed but if not better in a few weeks will follow up with Ortho. It is feeling better each day but I am impatient. Want to be able to play tennis again but do not want to put any side to side stress on the knee.

I had to lay up for a while after prostate cancer surgery which followed a career of constantly being on my feet and running around and noticed that my knees started to feel very stiff! Then one day when I was working in my yard I had to squat down all the way and heard something crack in my right knee! I'm sure it was a tendon problem because it's swelled up real bad the next day but I've been keeping it elevated, icing it three times a day, using my hand gun massager got a knee brace and also some compression wraps! Been a couple of weeks now and starting to feel better!

Both knees skiing Feb 2019 after a skier decided to wipe me out! Anyway I would say still recovering a year on, as in still gaining strength,(I would class myself as fit, I work out 5-6 days a week). Took me a long time to walk correctly again with one leg. I wear Cambivo braces which have helped when working out, going on long walks and hikes. Put me out of action for a good 6 months and had to work myself back into my HIIT routines. Took a long time till I could kneel and crouch down. Just keep doing your exercises and I found spinning has been the best thing....if I miss a couple of days my knees now get stiff. Never used to happen till this injury. I saw my Dr the other day and laughter about the recovery time of 8-12 weeks, but then he said I wasn’t normal as I had to do both 😸😸 Glad that I am not like I was when it happened, then I thought I would never walk right again....but it all came back together, not the same as before accident have more pain in knees, but now getting ready to hit the slopes again....yeah think I maybe crazzy! 😂 but it could have been worse! Phew one life to live....so go live it!! Good luck to all recovering from this injury you can do it!

My second degree MCL injury was estimated to be 4-6 weeks but it didn't feel "right" for almost 8 months for normal walking. Little foot slips, say on a wet floor, would cause sudden pain. Nothing long lasting but generally foot twists and deep squats took over a year to feel normal again.

I caught my toe on a mat and fell while quickly walking, so I twisted my ankle, twisted my knee and smashed it. No popping sound and while it really hurt I was still able to limp around and finish work. The next day I was using crutches and it was super coloring up on the inside and outside of my knee. I used ice etc... I still had to work as I own my own business. 8 weeks later i'm off crutches but there is still pain and limping. I wear a brace while working. I'm hoping for better days in another 8 weeks. :\

I had already some knee crepitus that was more intense and painful lately. One day, doing a external bending stretch it was like the inner femur and inner tibia split and I could not put my feet on the floor. Pain was excruciating and I got a inflammation of the knee that I treated with ice and ibupruoen. However still x2 weeks and I have pain specially walking w shoes and more than 10 minutes. The knee is not stable and add a patellar tendonitis in both knees. Perfect storm. Working strength and flexibility now.

I'm healthy and fit. Always exercise, and mine took a solid 4 to 6 months to get to about 75%. Initially there was significant swelling that subsided after about a week. For the next several months there was mild but visible swelling, pain on the inside of the knee, and reduced flexibility. The knee was stiff constantly, especially after sitting or laying down. I was able to walk stiff-legged. Never took any time off. Pain was intermittent from about month 6 to 9 and the knee still didn't feel solid, and twice, a hard step off a curb knocked me back down to square one for about a week or so. I was afraid to run on it or overload it in anyway. By about one year from the initial injury it had healed and I no longer felt anything. I never saw a doctor. I bought a brace, which helped greatly. I wore it for about the first 3 to 4 months. I followed suggested self-treatment regimens on KZread. Once the weather warmed up, cycling helped to loosen the knee up substantially. That was probably the biggest help in getting back to normal. Turmeric also helped greatly in reducing tightness after periods of immobility, such as sleeping. Instead of waking up stiff-legged and sore, I woke with reasonable range of motion and mobility and was able to pop right out of bed. It may have gone quicker had I seen a doc. But, the pain was tolerable if inconvenient, and I was never immobile. But, it definitely took quite a while.

thankyou for you report on your experience. I crashed on ice and pavement on my bike and tore all the ligaments . I had to fight for any medical attention at all - no pain meds - i was throwing up for a month i was in so much pain. Finally i got them to get me in physiotherapy. All the while they are saying 'you'll be even stronger in 6 weeks' Well. 8 months later - i can ride my bike but walking is impossible. The knee is swollen - ALWAYS - and clunks loudly when i move at all .The doctors are ignoring me - won't even give me an X-ray. Your story sounds much more realistic of a 'holy shit that hurt' injury. hope you are well - and i will keep doing what i can.

This sounds like my injury. Not sure what specifically caused it, though I was up and down a ladder several times before it happened. I'm 41 and pretty healthy and exercise a good bit, have always had strong-looking legs, at the very least, thanks to soccer and biking. But my knee blew up insanely big, looked like a bad case of bursitis. After about a week, which is how long the swelling took to drop down about 90%, bruising set in on the inside of my knee and all the way down my leg. Wore a compression sock starting the first week and am just under two weeks in wearing a non-locking hinge brace as rec'd by my PT buddy, who said it's likely a Grade II tear, and am walking a bit stiff-legged but not having a problem bartending three days a week. Sleep is surprisingly the most painful part of my day thanks to the sensitive stir-swollen knee and the different gravitational directions while lying down. This feels like it's going to last months, though, and I'll be shocked if it doesn't stay tricky for a long time.

I'm 75yo, 5'10", 162#. 7 Weeks ago I slipped on basement stairs and the left ankle held me from falling all the way, but damaged my MCL. Probably type 2 from your description. Used ice/heat and a sleeve for 2 weeks. No relief. Called orthopedist and took 2 weeks to get in. Hurt the whole time. Got a brace. Wore it for 3 weeks. 3 days ago I saw him again and he had me stop the brace and will start physical therapy next week. So, 7 weeks out it's not too bad as long as I don't twist it which is painful. There was never any swelling or redness. Ibuprofin helped but gave me stomach pains. Tylenol didn't help for some reason. The biggest problem for me is that I haven't been able to get a good night's sleep as my knee aches no matter what position I lay in. I use a pillow under my knee but it barely helps. Still can only walk slowly and difficulty with stairs.

My son recently suffered a right knee injury during football practice while pivoting to his right. Initially after initial assessment, the doctors thought he had both torn his ACL along with a dislocated knee cap. The MRI results just came back and fortunately, the ACL is intact and the patellar is normally located centrally within the trochlear groove with signs of recent dislocation. The 3 issues diagnosed however include: complex horizontal tear of the anterior horn of the lateral meniscus, a Grade 2 tear of the MCL near the femoral region, and lastly, evidence of a complete tear of the medial retinaculum at the femoral region. I believe all of the these issues individually are not as severe as an ACL tear and collectively, the recovery period even with arthroscopic surgery is much shorter than had it been an ACL replacement. My question is, can he continue to exercise and finish the football season and delay surgery to fix the meniscus and medial retinaculum with the aid of a brace as long as he does not feel pain ? Ultimately, he is only 15 years old and we probably should repair the 2 issues. Thanks in advance.

Did not injure my MCL doing anything other than just waking up! Went to a specialist and had x-rays and MRI (after 3 weeks of icing and not being able to put my full weight on). The MRI showed a slight sprain and shortly after my MRI I was able to walk again. Put ice on it and did the exercises prescribes. All together it was about 6 weeks before I could put all my weight back on it and walk normally. I still don't know where it came from (except I played lots of sports when I was younger and College Football and Baseball). I sure would like to know how it got sprained? Help! Looking for some concrete answer. Thanks!

I got injuried on 7th of Feb 2020 along a skiing downhill at 4 p.m. I cried of pain and was not able to raise myself alone from the ground. A sleepless night followed. I put something to chill the area and then some plant packages on it. After 26 hours I could walk for about 50 meters and gone up and down 100 steps, just to feel how it is .... I try to bend the leg (extensions & flexions). Rotations are also possible. Walking is not possible yet. If it doesn't get better in 1-2 days, I go to the doctor. I also use mental stuff (we are the placebo after all and I experienced this before) I am pretty sporty (trail running and cycling, female over 50), so this was the part of a good chance in this accident. I will keep posting :-) Thanks for your video, I use any information I get (I am also a sports coach and a bit familiar with injuries).

injured both knees, MCL tears (still waiting for MRI results) skiing. Skis were too tight and did not come off, so essentially did the splits and knees went sideways. Very hard to walk for the first week, iced, rest etc. was able to walk longer stretches after 2 weeks. Started working out very gently and seemed to gain some mobility but still stiff after sitting for prolonged times. Still can't fully extend the right knee which was more severely injured, and occasionally have some tenderness there, although I don't recall there being a lot of swelling even within the first 48hrs. I am very active and of course worry about recovery times, and fully regaining function to the knees.

I'm so happy I read these comments I've been thinking I'm going to need surgery. I guess it just takes longer than they say. I had a grade 2 tear in my mcl and pcl. After 12 weeks my pcl feels fine. My mcl still feels loose when I'm completely relaxed. It's weird I'll be rolling out of bed and I'll feel I slide around and it hurts but I can lift weight work construction all with great stability even practice may thai just bag work though and it feels fine its just when it's completely relaxed in bed my mobility still is about 6 inches away from my good leg though

Years ago I had LCL injury from martial arts. Its the supporting leg from a kick which I was wearing a running shoes which cause the whole thing. I did all the therapy that was available for months and got better. I switched to competitive mountain biking and never had a problem with my LCL for many years. Two year ago I started playing soccer again and realized my LCL was hurting especially on tall grass in the field. I know what it was and realized I didn't strengthen my LCL injury from the martial art incident many years back. My solution was to stop playing soccer, but I didn't do much therapy then and went on mountain biking which doesn't hurt anyway. Last year I revisited my martial art which I was really good at it long time ago and I'm still very flexible. Well you probably assumed what happened next, I re-injured the same LCL from 15 years ago from a similar kick which is the supporting leg, but I was not wearing any shoes this time. I was hopping with one leg coming from a complicated kick. Yes I knew immediately what just happened. So I started therapy again like I remember, but something odd happened when I was kneeling down working on my car while I was still recovering, my MCL got injured too just by kneeling or something. Now its was a bigger problem and doing therapy for both LCL and MCL. For almost one years I struggled on recovering and avoiding all sports. Today the LCL feels much better, but MCL still telling me its still weak. I can ride a bike, hike, jog, balance with one leg with slight pain. I'm still afraid of fast running, high jumps or quick side to side. What's wrong with me or what em I not doing. My first LCL injury was at age 35 and today I'm 48 still active. Thanks for reading my story!!!

I did my MCL 2 weeks ago while playing football, hard tackle sending the foot one way the knee forces itself the opposite direction, very painful the first 3 days almost could not walk. It has improved but nowhere near as good as being mobile, taking the stairs still very hard, the reason for not recovering is cos I keep working and can not stop, so my advice is! if you can then take some time off and elevate it, hot and cold treatments should help. if you keep moving as I do, it will take a lot longer. Thanks for reading.

First I'd like to say Chelsea all the way. Second its been a year since I've dislocated my patella on my left knee and I've sprained my MCL in that knee twice since then. I've just finished my second round of PT and I've begun working out on my own. My dr. says I've got muscle atrophy in my leg and it just doesn't seem to get better no matter how much I work out. Are there any tips or exercises I can be doing to help heal muscle atrophy. Before my second MCL sprain I was able to do a lot of leg weights however I can barely raise 20 pounds now. Also my injury was initially in my left knee however recently my right knee has been acting up and feeling a lot like my left knee was feeling before PT but the weird thing is, is that I don't know why because I haven't injured it anyway. So far I've just been applying the same exercises I do with my left leg to my right left as well. Anyway any advice would be helpful, thanks for the videos.

I just had a skiing injury like that, torn MCL near tibia, two places, high grade tear, torn ACL, sprained LCL, tibia micro fracture, bruised tibia. Im a week out from injury, still slightly swollen but walking around OK with knee brace. They limited brace angle between 10 and 70 degrees. Doc said MCL needs to heal tight, not allowed to stretch leg to prevent MCL from healing loose. Ortho i saw referred me to NFL team surgeon to do patellar ACL repair, bone ligament bone. I think they want my MCL to heal quite a bit before they operate on the ACL...:((((

Is there something that I should be doing more? Or less?

Hi. I'm a female ice hockey and floor hockey goalie. Both knees suffered injuries earlier this year and although I've been told that by now they should be healed, my left knee, the "grade 1 mcl tear" still hurts a bit when I am in the butterfly position. It's different from when I had the injury in that when I'm in butterfly and upright (back straight) I have no pain at the point where the injury was. Before it used to hurt to kneel on that spot or push on it. When I sink my butt back I feel pain on the side where the mcl is. I'm wondering if this is just meaning it needs to be stretched more or of after 11+ weeks a grade one could still be torn. The MRI states "thinning of the proximal medial collateral ligament with a tiny chronic low grade partial thickness proximal mcl tear. No high grade or acute tear. Also says mild proximal patellar tendinosis and minimal insertional quadraceps tenionosis without tears. Minimal lateral compartment chondrosis. And last but not least a tiny bakers cyst". The orthopedic surgeon I saw, who did my meniscus surgery in 2015 on same knee, basically only focused on the mcl, said it was low grade, and PT for 4 weeks and I'm good to go. I've been doing PT two to three times a week for the last 11 weeks and 4 weeks before that so they would allow me to get the MRI. So, lots of PT. The PT I'm seeing said try my lower gear on and get into position and see how it feels. Everything else feels great but this painful tension I'm getting when I drop my butt back. It feels more like it just needs to be stretched, so I'm hoping that's the case. My right knee/calf was the one that had the most wrong with it and it feels awesome. But this left one still making it hard to decide if I should try playing, even thought they say I'm good to go. They're telling me it's ok to go for it. But I'm looking for anyone else who's been off their sport for this long with an mcl tear (grade 1) and having some tightness in the mcl when going back, is it even possible for the mcl to tighten up. Specifically if you're a goalie with this same strain. I've sprained this knee before and I'm 41 so I don't know if that has any impact on it. I know you can't give medical advice, but if you can offer some insight I'd appreciate it. I can't afford another $100 copay to have them say you're good to go when I'm still having pain. Unfortunately not alot of people understand the goalies butterfly position.

Hey there, I'm an Ice hockey goalie as well and I have injured both knees (MCL) while in the butterfly position. I never tore anything to my knowledge, just stretched the ligament way past it's limits to where my knee cap would swing out and around the outside if my knee. It hurts very bad and will swell up to where I can't walk for a couple weeks on it. My experience is once you injure your MCL, the chances of it happening again are huge. The ligament will never be as strong as it was.

Also, I would focus on stretching your hips a lot more. Its the only thing that has helped me still be able to play. If your hips are more flexible it can potentially ease the stress built up on your knees in the fly. You would start to sit a little lower in the fly, but less of a chance of injuring your MCL!! :) Good luck!

I had a partial meniscectomy 2 weeks ago (injured during a violent fall 3 years ago) and the surgeon, in manipulating my knee during the scope, accidentally tore my MCL. I was told “it’s not an uncommon occurrence”. What should have been a 2-3 week recovery has turned into a 6-week recovery. On the bright side, the surgery does seem to have eliminated the excruciating pain caused by the torn meniscus. Just hope the ligament heals well. Two weeks in and I still can’t walk far, and will be using a cane for at least another 2 weeks. Not a happy camper.

Since then it happens maybe once a year if I do some huge activity, that causes the dislocation. Now my question is I was quite heavy at some time and it happened often. I'm asking if I can repair my ligaments without surgery, or is this going to happen for life and I have to wear knee brace for the rest of my life?. That's what I'm afraid off. But at the age of 16 I wasn't big, I was just not active person as I am now.

2 weeks ago pulled my MCL and also fell onto it, at speed off my bicycle, so the skin was road rash, the MCL bruised and sprained. The Femur side’s attachment point hurts the most, right along the edge of the femur’s knuckle head. Never had a problem putting weight on it, but night one I couldn’t sleep any movement was level 7-10 pain. Body weight on the joint compresses it and the ligament isn’t stretched as much, so right now I can bend it like halfway without pain if I’m standing with weight on it, but only 1/5 at rest. The ligament is clearly the only pain source and it seems to only hurt when knee bending stretches it. (If I slowly grit thru the pain to a folded knee it doesn’t hurt there either). I got lucky I guess, no loss of stability to the joint and don’t think I’ll need a brace. If anything my concern is this healing too tightly so gonna start walking up and down gradual inclines to help orient the healing. Been full resources on this for 2 weeks; rest, elevation, taking lots of all vitamins, collagen, hyaluronic acid, turmeric, aircast cryocuff (cold and compression), and lots of food and arm-based workouts for growth hormones. I’d say I’m 70% healed and 50% functional this point. I can walk normal-ish if I pick up my toes (not with shoes on tho). I can just barely walk up stairs, but not down. Ruled out meniscus with that stand and twist test (on the injured knee, zero pain there). I tried that metal bar therapy (used a big wrench lol) which helped ID exactly where the pain was, but I’m not sure it helped heal anything. Also that kinesiology tape. It helped like 5% but sure hurts to remove from hairy legs. Good luck everyone 🙏

Coming up on one month and the pain/stiffness varies a lot these days. Overall still progressing in capability and pain reduction. Anders Sjöström so I was just about back to normal within 3 months. Interestingly, using my body/knee sped up healing of the last bit. Light/normal use like walking. Crossing my legs took more time to not be painful. These days 99% of the time I have totally forgotten it ever happened, but if I carry heavy things upstairs I can later be reminded with echoes of the original pain point. I do think there’s some very minor permanent damage on the outer edge of the meniscus but luckily not enough to worry about.

I have ligamentous laxity ( not severe). I was roller blading a month ago and i 've fallen down in W position. I've heard immediately the pop, I 've fell burning medially , and my proprioceptive sensation was that my right lower leg was a little bit lateral instead perfectly aligned ( on spite of it was looking like aligned) I was able to stand up without problems, I put my own weight over and continue roller blading cautiously for other 10 min or so. I got a MCL injury grade 2 . Now, i am able to do treadmill flat, elliptical, and up and down stairs without problem, however twisting movements are still painful. Based on your experience, how long can It takes to disappear those sensations? Ive been applying ice 3x a day, wearing braces since the fall, and I am doing therapeutic exercises . Thank you very much!

I had my first MCL injury 15 years ago when I was 34 yrs old. I teach kickboxing and one day a was wearing a rubber shoes with narrow sole and when I did a side kick my supporting leg with the shoe turned outward causing my knee pop out and back in place very quickly. I had therapy using wobble board and worked very well. I stopped martial arts and was on competitive mountain biking and never had a problem again. I knew why because I don't have any lateral movement that much. After 13 year I went back training kickboxing and after months of conditioning and stretching I was ready to kick some heavy bags. Kicking the bags was not a problem! Then after weeks of getting my skill back, I did a fancy spinning kick with a small hop which was cool. That's not the end of it, I re-injured my MCL at the same spot. Why? at that time I was 47 years old. I started a similar therapy but did other stuff too. It's been 2 years now age 49 and it's not 100% better maybe 80%. I can hike and jog/run not too fast but I'm afraid of trying kickboxing or any one leg jump which might injure it again. I'm not sure why it took that long to get better. Will I get complete recovery or not? Do I still need surgery? What did I do wrong in the therapy or what was missing with my therapy? Thanks. I never had any MIR for the knee. Thanks!

Injured my knee playing football, got tackled pretty hard. My second degree MCL injury was estimated to be 4-6 weeks but it still doesnt feel good now almost 3 months after... Little foot slips, say on a wet floor, causes sudden and great pain. It does not last for long, but still. Im maybe 70% healed now after 11-12 weeks, can walk, run and do squats pretty normally. Things like changing direction sharply while running and kicking something, as a football still causes big pain and some unstability:/

I am 59 and have never had this, but had an acl 20 y. ago on that side and a dislocation of the patella slipping on wet grass playing soccer 40 years ago. I notice that the relatively new tennis shoe sole had come unglued slightly before and did not address this (re-gluing it). maybe I should have.

Hey Dr. David!! love your videos! So here's my situation: I tore my MCL a while back and the fix involved using a ligament from the front of my knee as a graft to replace the MCL. Fast forward, and during some parkour, that graft failed plus I ended up with a cruciate ligament tear. Not my best moment. Now, I'm pretty sure I need another surgery, but I'm curious about the type. I've been researching donor grafts too. What are your thoughts on this? Using kinesiology tape in the meantime and keeping to handstands, but I'd really value any insight you might have. Planning on seeing an ortho specialist soon, but your opinion would be gold. Thanks a bunch!
