    Hello yes you want to envision smashing the scar like it would be smashing a pea. So that you break down the height of the scar tissue.

    @@allthingsreview777 Hello. That is because it has lots of vascularization. There is a pulsed dye laser that would make that scar look great. It would be cosmetic of course. I have not got a PT remedy for the vascularization.

    Not too sure. Pain at the top of my right hip is regular but fairly mild, sitting down position is OK. But more severe pain which also moves across to a more central location when standing up. Once I have stood up the pain goes back to mild. Getting out of bed is problematical with the standing up type pain inflicting a reap pain jab as described above.

    Hello sounds like SI joint sacrum probably a little twisted and the pain across the top of the hip is muscular pain from some muscles being overworked due to the strain of the hip bones being slightly out of line. I would definitely find a practitioner who does muscle energy technique And get that sorted out you will be very happy. If it's been going on long time, it will take a bit of maintenance and you may find yourself and PT kind of regularly to keep yourself but PT takes less than an hour, most people think it's worth it to keep things in line. Plus, remember the long-term consequences of potentially deteriorating a joint and then having arthritic pain later so I think it's worth it.

    No one ever addresses how unsightly this issue is. It's embarrassing. Who wants a deformed foot? Even when it's not totally painful because sometimes I I go for long without it flaring up but it still looks like I have a red ball on top of my big toe joint and it feels so bony when I touch it. it's really ugly. ☹️

    @peekaboots01 yes podiatrists know what to do with this problem but podiatrist have different surgical allowances in different states. So it depends on where you live whether a podiatrist is allowed to operate it on on it. You could also seek out an MD who is a foot and ankle specialist and they will be able to correct it for you. It sounds like yours is severe enough based on your description that you would be a surgical candidate. Although it would hurt for a little while you would be much happier when it was all over and no more bright red ball on your toe. Orthotics can help a little bit right now but since it sounds like it's very severe. I'm not sure how much. The orthotic must have a forefoot post and a first Ray cut out with fill in and you can tell your practitioner that. Orthotics will help with recovery afterwards and will give you many years on your new toe. If you don't wear orthotics after the surgery, you risk it gradually returning because the reason for the bunion is your foot mechanics. The orthotic fixes the reason why you developed the bunion in the first place, but it would take five or 10 years for it to begin to reform.

    Thank you. I am 8 weeks out of surgery and can't walk without a severe limp. I only found today from KZread that I should be doing flexes on my toe. I thought I would be running by now but walking to get the mail is slow and somewhat painful.

    Hi! Well at eight weeks, your bones have only just fully healed so now would be the big progression of activity. So definitely I would not have expected to run by now. I would expect you to be able to run in months. The first goal is to be able to walk comfortably. One of the most important things after surgery to be able to walk is wearing shoes that will protect your foot. It sounds like physical therapy could definitely help because we can speed the healing. Take down the inflammation. Decrease the pain and give you pointers on where you should be and what you should be doing. If you need help and you are far away, you can schedule a video consultation or maybe call around to your local physical therapists and see if anybody has treated people after bunion surgery. Don't lose hope I know it's frustrating and I know it's slow, but you will get there!

    What happens when air bubbles appear during treatment between the transducer and the area treatment is being applied to? What does this indicate? TIA

    Hi, it's an indication of inefficient transduction of sound waves and it is called cavitation. Cavitation is what makes the little air bubbles. If your patients are still feeling results, I'm sure it's fine. If not, I would have your equipment inspected.

    Hi :) is there a way to test for femoral neck stress fracture without getting mri? I have really sharp pain on initial foot contact with every weight bearing activity. Hip flexors are completly painless when acitivated as well as my adductors....

    Hello, iontophoresis will definitely help with swelling but not with bone healing. The thing that will help with both swelling and bone healing is the MLS class IV Laser. Any cold laser will help but the original ones were class three and the class for as much more power. You can search for one near you on the Internet. I know the company that we bought one from has a search tool like that in the name of that company is cutting edge.

    Is Iontophoresis safe while bone is still healing? I have 4 more weeks to 12 weeks was going to wait until then but I would really like to continue treatment if I can for hyperhydrosis on my feet. Thanks

    Hello yes the iontophoresis for the hyperhidrosis is safe while you are healing. The only thing you wouldn't want to do is use the dexamethasone sodium phosphate because dex can impede healing.

    Hi @Biomechanicphysicaltherapy, I had lapiplasty 12 weeks ago on my left foot and I have pain under the big toe when walking, sometimes it feels like pain under second metatarsal, it's hard to tell. I can flex the toe fine and I have no pain when I do that. The foot is still swollen but mostly on the right side so when I walk the foot feels uneven. I'm concerned that this is a chronic pain at this point and I have to live with it. I can walk in sneakers (I can only fit my shoe in sneakers with shoelaces untied or loosely tied) but my walking is not pain free. I don't have any pain when I'm not walking and my toe is not stiff. I can flex it and bend it. Do you think these exercises are for me or should I be doing something else?

    Hello, 12 weeks is very recent... it is okay for you to do controlled exercises and yes these are fine for you. My advice is, respect your pain, don't push it too hard, ice it when you do. If you do too much and keep yourself in pain all the time you might end up in a chronic pain situation. But it is not chronic pain now and is not a harbinger of chronic pain. It is early days yet, be nice to it for 6 - 9 months more.

    Hello @leevize6045 I am sorry to hear that. It is possible that the nerves will heal. There is a laser on the market, MLS Laser by Cutting Edge Technologies that is the only proven thing to heal nerves. It is also possible that you have a little bit of innervation left in which case physical therapists can help stimulate those muscles to be able to lift the toe again. Good luck to you.

    You could have more than one thing going on. But the other thing I’ve discovered. When the inflammation or problem has gone on for a long time, the whole darn thing hurts. So as soon as you get under treatment and start getting the pain and the inflammation handled, the pain will start to get more specific from where it actually started as. Maybe you can remember which pain it was when it first started? Or you could try to find a good Physical Therapist with an MLS cutting edge Laser to decrease your inflammation and help focalize it.

    Im 6months post op and have started getting very worried my toes have stiffened too much and im still limping because my toes dont bend anymore. Heel lifts just hurt but now i know i need to extend the toe to create space in the joint. How often should i do these exercises being at advance recovery stage.. Thank you so much

    Hello, it would be best if you do this twice a day. You might want to start with a professional PT to see how hard you are supposed to push. Best to do most stretching in first 6 months so get started right away!

    Hello physical therapy is helpful to regain your range of motion to speed your healing, and to decrease your amount of pain and to ease the process of getting back to what you like to do. Not everyone does physical therapy though.

    The i-walk is intended to remove the weight-bearing only on your foot or ankle or lower leg. You weight bear through the knee with the iwalk.

    @@rosebudelfjohnson4637 I started walking like 2 weeks out, but that was with crutches and I was doing 10% on my surgery foot, then I would increase with weight via my Drs. orders. I currently still have foot problems so it's def a 50%50 shot I'd say.

    ​@rosebudelfjohnson4637 I was weight bearing at week 2, but I was only allowed to put 10% of the weight of the surgery foot. I then increased the weight via my doctor. But for the love of God, do physical therapy idc what they tell you you NEED physical therapy especially with a foot surgery, I had to figure this out the hard way and now I'm 8 months and still have foot pain.     i have had this off and on for 5 years. i stand for a living. i got custom orthotics but they don't help much and i can nver find the correct shoe. only thing that helps is the bulky lace up afos which day to day stink to wear

    Hello @jimbrow83 sometimes the orthotics are not aggressive enough, so maybe you can look at getting a device with an arch which is as high as your arch when you are not weight bearing on it and combine that with a lace up combat boot instead of using an AFO.

    They don’t always take a lot of supplements,that was misinformation. Mentioning that it takes years and years of consistent training to build a lot of muscle mass would have been the thing to point out.

    I'm diagnosed with pttd recently on stage 1,can I reverse this through wearing custom orthotics and strengthening Mai tendon? I'm so much worried..

    Hello, YES! You can reverse this. If you wear an aggressive custom orthotic it can allow the tendon to heal. You may need a period of immobilization. If you do, your doctor may have recommended wearing a walker boot. Do put your custom orthotic in your walker boot as it is very flat inside. Some docs disagree with me, but I stand very strong in my belief!

    I am so happy I found your video. Two and a half weeks out from my Lapidus procedure. My big toe joint has motion, but it is kind of crunchy when I move it backward. Is this a result of the osteoarthritis in the toe joint? What do you recommend for this?

    Hello yes it probably is those remaining rough surfaces but they are also still healing. Please wait 6 weeks before you try much. The bones need to knit & wiggling the ends of the cut bone / motion slows down the knitting of the bones. Your surgeon may also have smoothed the joint surfaces, so they may be more smooth after healing. If you want to move faster, I would look for an MLS laser near you. Company is “cutting edge”. It can cut the healing time down by 45%.

    Hello mam I play Cricket (sport like baseball) while playing i was having chronic hip pain on right side, pain is on front of hip ( lower abdomen) near inguinal ligament and inner groin or attachment bone of adductor muscles. After MRI it shows cortical irregularity, partial tear and hip Synovitis . It hurts while running and change of direction movement. I have been suffering with it since 10 months now. Taken Physio therapy treatment but it is not getting better. I am unable to play. Please help with any guidance or videos. Thank you.

    Hello, you might want to try a different physical therapist as all the physical therapists have different techniques. Different techniques work for different people. If you have already tried several different therapists without results it’s possible that you are a surgical candidate.

    Neo vaginal scar Many different areas merged at one of the two points with heavy sutures that go from Deep internal to external in layers. So area of the suture are interconnected like a knot that makes it immobile to seperate the different layers that spread out like a webbing. While the outer ends have become Lumpy and firm. These areas have made the canal difficult phytherapist are concerned with the mobility of blocking tissue which does no good if it’s blocking on all accounts for size or movement. The center of the knot is rooted into itself making motion useless as it pulls all offending connections. Surgeon revised to remove a small bit of the tissue 1x1.2x1.4 cm & 2x2.2x2.4cm Which is nothing given if I could rip it out would be as big or bigger than my index Dry needling therapist won’t take the risk at a few points and the surgery was so insignificant I can’t maintain dialation anyways so the thick tissue is almost all back from what was removed I wanted to take a risk when I was in the or when I approved for a transfusion if it came to it but they only wanted to go based on comfort of safety. Like I don’t care if I’m signing a waiver and approving things in case of something I wanted more results I’m just venting but I do wonder if a liquid injection or electrical treatment would help

    That depends on how extensive your surgery is and how physically capable you are yourself. If you are non weight-bearing, then you may not be able to put weight on that foot. If your bunion was very severe, then that would be the case. If it was not very severe, and they’re only clipping off a small piece of bone. Then you will be able to weight bear on your heel. It depends on how fit you are, how prepared you are to walk with crutches with only 1 foot on the floor. Peoples level of fitness varies wildly. So if you’re bunion is very severe and you are going to have hardware afterwards and you are not good with crutches then I would think you should have some help.

    I put tape on my feet and wrapped it up when I want to take it off they tore a half of the top of my skin off at the top so be careful when you take it off

    Yes, It can tear up your skin, especially if you have fragile skin or tap frequently. Pro tip; fold the tape back on itself & follow with your other hand rather than pulling the tape away at a 90 degree angle.

    I had MIS bunion surgery a month ago. When should I start doing these exercises? I’m doing some already but not as vigorous as these.

    As soon as your bones are well healed, you can start the exercises. Usually that is 4 to 6 weeks but you may have also seen your doctor to get a follow up x-ray. If your follow up x-ray says you’re well healed then you’re ready.!

    I sometimes have intense pain in front, or the groin area when standing from a seated position on both sides of my pelvic area. My first few steps are particularly painful, then it goes away completely after a minute or so. Sitting with my knees elevated above my pelvis, or leaning forward a lot while seated exacerbates the condition when I do finally rise. I am searching for helpful information as to the medical reason.

    Happy to help - Sounds like femoroacetabular impingement - could be some arthritis. Usually IR deficit too. Core and hip strength and some specific stretching without causing the pinch is the treatment. Sounds like a more chronic case for it to be B and severe, maybe lumbar compression making it worse. A good mechanical, manual physical therapist would be able to help!
