    I’m five days post op, both bunions removed so bed bound for a little while longer. I’ve had bunions since I was a teenager so I’ve never really had any proper flexibility in my big toes. As horrible as this part of my recovery is I’m so excited for when I can start my physio journey and I’m very grateful to you for sharing your knowledge free of charge.

    Im about to do a double and i have no help at home. Did you find that initial pain etc subsidies quickly? How long until you were back in shoes comfortably?

    @@cassieashley7476 I won’t lie, I relied very heavily on my partner for a lot of things in the first six or so weeks of my recovery. You can’t drive with both done so he did the groceries and took me to doctors appointments. Just getting up for the bathroom, preparing medication and food is a challenge. I’m pretty tough but these things just weren’t physically achievable. I’m not trying to scare you but I guess it’s important you consider these things and maybe have a nurse or someone who can come by and check up on you. Initial pain took a good several weeks so really subside and get back to normal activities, albeit much slower and with rest breaks- probably 6-8weeks? And about 12 before I could get around almost like normal. I’m nearly 10 months post op now and doing things I haven’t been able to do in years, still have some swelling occasionally and stiffness but it’s been worth it for me :) good luck with your surgery!

    Thank you so much for this video! I am 10 weeks post-op with my left foot Lapidus Bunionectomy (my second surgery, right foot, is scheduled in 2 weeks). I have been struggling with this very issue - sore big toe. I have been doing my own physical therapy, but it has not had any lasting effect. Now I understand why - your video and explanations make perfect sense to me! Wish my doctor would have shared this kind of information with regards to my rehab - this is a game changer!

    Hi Beth, I was wondering how it's been doing the two Lapidus procedures so close together. This week I am 16 weeks post op on my right foot and doing wonderfully, I have 2 more weeks of no running/jumping then I'm clear. I am doing my left foot in March hopefully (consult in 2 weeks as well). I am not sure if they're doing Lapidus again, or a different procedure since my left foot isn't as severe. How are you doing at this stage about 2 weeks post op? I still get swelling in my foot and rarely pain and was nervous about doing them close together. I am not as worried now since I got my right foot done first, so I'm hoping the second time around will be easier since I will be able to drive and not be stuck for 3 months. I'd love to hear how you're doing and how your experience has been thus far!

    Hello! how are you after two years? Is everything back to normal? I hope so! I am just on my first surgery (will do the second in 3 months) . I am a bit worried of not being able to go back to my Argentine tango dance since pivoting and turning uses the big toe a lot!

    At what point after surgery should you start? That should be mention on the video because some people don’t understand that it takes time for the bone to heal and you should not be messing with the bones while healing.

    6 to 8 weeks after surgery. Your doctor usually takes an x-ray and will release you to activity so you don’t have to make a decision on your own. Your physician will tell you when you’re ready.

Alot more swelling ankle and foot..your advice about swelling and scars. Seeing surgeon in 4 days for check X ray and pin removal from adjacent toe. Will treatments after this. If I lived nearer your treatment centre and not in UK I would come for foot therapy!

    This video helped me SOOOOO much after surgery. And it was my SECOND bunion surgery (other foot years earlier). I had a good PT, but this time I needed extra support and advice. I adore this video and am grateful for it!!! Even the scar advice was helpful (as was the additional scar video). Unlike my last surgery I had a centimeter length of my scar that began to appear raised. The CICA gel recommendation wasn't mentioned by my PT. It was so informative. In short, this video helped me with issues that I had the second time I had bunion surgery that were never an issue the first time. One just never knows what variables may pop up each time. Thank you for the videos!

    Three weeks is way too soon to start these exercises. Wait until you’re fully healed. Then you won’t wince, well maybe a little.

    Hi! I am 6 weeks post op today from lapiplasy also. I am in a boot 2 more weeks then transitioning to a sneaker. I Just started doing stretches after the doctor telling me I should have been doing them sooner. It was their mistake. How are you doing

    @@hodgepodgemomentswithrenee6857 I too didn't get much direction on recovery from my Dr. Glad I found this wonderful video. I went to a post op shoe at week 6. Firm sole shoe at week 8. I had noticeable swelling under my big toe until week 10. Some overall swelling still but tolerable. I'm still wearing toeless compression socks. So glad I had lapiplasty. Having my left foot done in December.

    @P R Causey what's with these doctors??? I got next to 0% instruction either. I told him about it too. Nicely, of course. Especially, after he told me I should have been stretching it earlier and they never told me too!😡 I have 2 more weeks in the boot then I transition to a good sneaker. That will bring me to 8 weeks. This stretching makes me cry it hurts so much. He wants me to go to PT but I'm going to wait till I work on it myself some first. It's just too painful now.

    @@hodgepodgemomentswithrenee6857 I've been using a heating pad to apply heat to my foot before stretching. I also use natural remedies. Arnica oil topically and homeopathic arnica. I'm seriously considering the laser therapy after the left foot surgery. I'm sorry yours is so painful. Hang in there! Keep at it.

    Really interesting video thanks for showing it , Quick question I have bunions and one is much bigger with a angled toe that position under my 2nd toe and I have really limited up movement can this kind of physiotherapy work when you have bunions ? My bunions are not inherited my dr said its shoe related because I wear heels alot that I find more comfortable for my legs than flat footwear , thanks Sharon.

    Hi, Yes, if your physical therapist knows a lot about feet and bunions it can help. It is not common to find physical therapists that know feet and bunions very well. I would have to do I biomechanical evaluation of your feet to see if I agree with your doctor. It is very rare for a shoe to cause the problem.     @@14lishazz goodluck!!! you'll be fine! I am already walking on week 3. being in a wheelchair was a little tough but as long as you keep yourself productive you'll be okay. stay ahead of the pain and take your pain meds every 4 hours. norco helped me a lot the first week

    I had my bunion surgery on 12/4 and finally got the green light on 1/18 to start putting weight on my foot. Im having such a hard time with recovery and shocked you are having surgery on both ?!! This entire process for me has been mentally and physically draining

    @@mayrar2321 mentally and physically this is way too much!I am healing well but...this hurts in so many different ways.Even though it's been rough at times,there would be no way that if I did 1 foot I would have done the other.I dropped a heavy pot right on my foot the other day..I saw stars!My doctor said to try wearing sneakers..I can not yet.

    Hello unless you only had a cheilectomy it is too soon to start the stretches at three weeks. You want to wait until the bone is knit. If you’ve had an x-ray and it’s well healed and your surgeon has said it’s OK then it’s OK otherwise you need to wait 4 to 6 weeks or until the surgeon clears you. It’s too soon.

    Thank you so much for sharing this info. I had a lapis bunionectomy on my right foot on Nov 2020 and it is now Feb 13th 2021. My fiancé has helped me start this exercise today because my right big toe is stiff but I can still lift it a little. After doing the first and second motion, I started to walk on my foot and my right ankle seems to crack when walking.. do you think this is normal after being off my foot for so long? Also when do you think I should start phase three?

    Hi. Yes you are 3 months out, that is a long time so things may have changed a bit. Your ankle may not be in the perfect position anymore. Hopefully it will sort itself out by walking on it, but your ankle may need some skilled hands to correct its mechanics too. You can work on motion, swelling and scar all at the same time. I wouldn't try strengthening until the toe moves well enough because that can be frustrating. But definitely work on balancing flat footed on that foot.     @J Petz It depends on which surgery you have. Some surgeries are more extensive than others. Do you know which one you are having? Is it a distal bunionectomy? The proximal crescential? If your bunion is very severe it will require a bigger surgery and require 6 to 8 weeks of nonweightbearing. If you only need a little bit of bone clipped off it would be one or two weeks. It could be somewhere in between.

    Hallo, I had my hallux rigidus sugery (cheilectomy + decompression osteotomy) 17 days ago. I'm still not able to put my foot on the ground since weight bearing is too painful (the painful area is around the first three metatarsal heads). Is this delay normal?

    @Stefan Drago, No, it isn’t normal. That is usually an easier surgery to recover from. Make sure it is not infected and check with your surgeon. If everything checks out with them and they say it is all okay I would keep icing, start progressively desensitizing and gently increasing the amount of weight on your foot. Like start with 30lbs and increase by 30 lbs every 3 days. There are other things you could do. If you want us to have a look at it and you are far away we could always do a telemedicine visit.     Hi! It's been almost a year since my first bunion surgery which was a 'simple' chevron osteotomy with one bone cut, moved and fixed with one screw. I was VERY sore and swollen for over 6 months which made any kind of rehab almost impossible -although I did try everyday. Today it's healed but my dorsiflexion is sadly nowhere near where I would like it to be, despite trying really hard to bend the toe once I was able to. It's easier to bend the toe when it's "warmed up" rather than e.g when I just get out of bed. My doctor tells me it won't get any better due to lack of rehab... Would this technique still be able to help me? I had my second foot operated >3 weeks ago and it's nowhere near as swollen and painful as the first one!

    Hi Rain, yes it will still help. I have been able to help people with bunion surgeries that are very old. Just make sure that it doesn’t hurt on the top when you’re stretching it. If you need help you can always call us if you are not in the area now with the advent of telemedicine we are more set up to help people anywhere! But go ahead and give it a try it is a little hard to get as much leverage on your own toe as it is to get somebody else to do it so if you can get someone else to do the stretch for you it should help more. 🤞

    @@Biomechanicphysicaltherapy Thank you so much for responding so quickly! I'm a former dancer, 33 y o, so to not have a 90 degree flex on my big toe was a big bummer... My pain mainly originates over my former bunion and closer to where the bone was cut off, so I can't tell if it's scar tissue, sinews or the toe joint causing the stiffness. I will give this a go starting tomorrow! I will convince my boyfriend to help as it's like you say difficult to do on oneself.

    What is a stiff-soled shoe you would recommend? I have not really found anything that works. Birkenstocks are ok. Most athletic shoes, even hiking boots are too flexible. Thank you!

    Hi when I go shoe shopping I basically call it “ squeezing the Charmin”! I don’t no if you have seen that commercial. But when you’re shoe shopping just bend the shoes. One of my favorites online is KLOGS. Combat boots are also usually stiff soled and anything with a thick lug sole or a sole that is one piece of molded plastic or wood.

    Where is this available? I have bunions, not terrible, but I also have the second toe hammer toe. I was diagnosed as having a plantar plate tear in my left foot second toe and have just been using orthotics to help tolerate it. What surgery would you recommend? Hopefully the least invasive and not one that I would have to be on crutches for weeks.

    @norma gonzalez we have two locations in Virginia Beach and in northern Virginia. The surgery depends on the severity of the bunion. The severity of the bunion is determined by how far apart the long bone in your foot that goes to the big toe and the one that goes to the second toe how far apart those two bones are. If you wear the right kind of orthotics maybe you can reduce the size the bunion before you have surgery. You could also do intrinsic foot strengthening exercises to help support that plantar plate tear. Unfortunately it is a painful surgery and you do have to stay off of it. If you can find a place (like us) that has a high powered non-visible infrared spectrum laser like the MLS laser you can cut your healing time in half.

    Thank you so much for the video I had bunion surgery about a year ago now that I can get my heels on it still hurts underneath the ball of my big toe when I walk how can I stretch this or make it better?

    @MIZZ VENUS It could be your sesamoid bones, there are two small bones underneath the bowl of the big toe that might need to be moved around and also it could be inflammation. If I were seeing you for physical therapy I would be checking the movement of the sesamoid bones and applying an anti-inflammatory directly to the area using Dexa with iontophoresis. I don’t know if you are starting with shorter heels but if you can wear shorter heels around your house first as an exercise you might be able to toughen up the ball of your foot. The trick is not to do too much. Sometimes it’s hard to answer these questions without me touching your foot myself. If there’s any more data you can give I might be able to help more.

    Hello physical therapy is helpful to regain your range of motion to speed your healing, and to decrease your amount of pain and to ease the process of getting back to what you like to do. Not everyone does physical therapy though.

    Thank you for this video! I had a scarf with akin osteomie surgery about 8 weeks ago. At this moment i can't wear normal shoes because if i do i have a lot of pain in my big toe and the next toe.     I am so happy I found your video. Two and a half weeks out from my Lapidus procedure. My big toe joint has motion, but it is kind of crunchy when I move it backward. Is this a result of the osteoarthritis in the toe joint? What do you recommend for this?

    Hello yes it probably is those remaining rough surfaces but they are also still healing. Please wait 6 weeks before you try much. The bones need to knit & wiggling the ends of the cut bone / motion slows down the knitting of the bones. Your surgeon may also have smoothed the joint surfaces, so they may be more smooth after healing. If you want to move faster, I would look for an MLS laser near you. Company is “cutting edge”. It can cut the healing time down by 45%.

    I appreciate this video. I had bunion surgery 2.5 weeks ago. I am doing the stretches. I am hoping this foot gets back to normal soon! Oh and I’ve cracked my metatarsals twice! Again I appreciate the advice!

    Hello @Carole Robin Bartly if you are only 2.5 weeks out, it is too soon to be doing the stretches. Take a rest and allow some healing. It takes 4- 8 weeks for the bone to knit and jostling the ends of the bone is not productive. So, unless you have a cold LASER (non-visible infrared), just ice and don't move it. When you get your next x-ray, ensure if it is "well healed" or just "healing". Once the bone is solid, then start stretching. If you need more help, just call.

    @@Locoandchooch Hi it depends on which type of surgery you have how long it takes to recover. If they’re going to do just clipping off a little bit of bone you can return to work within a week, if they are going to repair the bunion down close to the bunion you can return to work as soon as you’re comfortable just walking on your heel which would probably also just be a couple of weeks. If they’re doing the surgery where they repair the bunion but they have to cut the bone up closer to the middle of the foot it’s a larger surgery and if you return to work you would be on crutches because you have to be nonweightbearing for 4 to 6 weeks for that surgery. So it also depends on what type of work you do. If you can go to work and be on crutches you can go to work as soon as you’re out of surgical pain which- regardless of the type of surgery- should be a week or two depending on how you do with pain. It is also easier to return to work if you have a desk job where you can prop your foot up. Because it can hurt to have your foot down and hold the blood pumping at the surgical area.

    Thank you. I am 8 weeks out of surgery and can't walk without a severe limp. I only found today from KZread that I should be doing flexes on my toe. I thought I would be running by now but walking to get the mail is slow and somewhat painful.

    Hi! Well at eight weeks, your bones have only just fully healed so now would be the big progression of activity. So definitely I would not have expected to run by now. I would expect you to be able to run in months. The first goal is to be able to walk comfortably. One of the most important things after surgery to be able to walk is wearing shoes that will protect your foot. It sounds like physical therapy could definitely help because we can speed the healing. Take down the inflammation. Decrease the pain and give you pointers on where you should be and what you should be doing. If you need help and you are far away, you can schedule a video consultation or maybe call around to your local physical therapists and see if anybody has treated people after bunion surgery. Don't lose hope I know it's frustrating and I know it's slow, but you will get there!

    I had a bunion removal surgery 3 months ago. I had a pin in my foot for 6 weeks. I am now about 5 weeks since my pin was removed and thanks to this video I regained full flexibility in my toe but I have a terrible pain in the ball of my foot now mainly under the big toe. Is this pain temporary? Is it because my foot is still healing? I hope I'm not stuck with this pain because it would prevent me from running or wearing heels.

    @Monica Danci i’m sorry I missed your question two weeks ago! But yes it is common to have pain on the ball of the big toe often because of the sesamoid bones, they were in the wrong place when you had your bunion and now they usually need to be moved around to make them comfortable in the new place. You could also still be healing. The other thing I would do would be to wear arch supports in order to better distribute the weight on the bottom of your foot. You could also wear a low heel around the house as an exercise then take the shoe off when the toe starts to hurt. Gradually increase the amount of time in the shoes and then gradually increase the height of the heel. Make sure that you do not just beat it up though, what you’re trying to do is gently and slowly increase the tolerance of that part of your foot to weight-bearing.

    Good video. It is a pity bunions often treated with a surgery that requires so complex rehabilitation. Wouldn't it be great if bones can be put to proper place w/out surgery?

    They can often be put into their proper place without surgery. It just depends on how far you let it go. If you wear orthotics that correct the reason why the bunion forms in the first place then you can actually reduce your bunion on your own. The orthotics need to have a specific feature.

    Hi there! I had a bunion surgery five months ago and I’m still having healing issues. I have pain in the ball of my foot under my big toe every day. I got a staple and two screws placed. I’ve been in PT for three months now doing dry needling, massage, and using tape. I still have a very swollen big toe joint and not much range up. I did a cortisone shot in the joint recently and still no relieve. Any advise would be great!

    Hi @Holly Z that’s kind of a hard one to diagnose without actually touching your foot muself. I’m not sure what they would be dry needling or why though. Dry needling is for muscle spasm generally. Doesn’t sound like a muscle spasm problem but I know dry needling is all the rage right now and some use it for non-intended purposes. Are you protecting your foot with a stiff soled shoe? Maybe you’re beating it up too much with too much walking activity? Pain in the ball the foot can also be due to the sesamoids. You have two little tiny kneecap bones underneath of that and sometimes they need work. Are you wearing custom arch support? One of the things you can do to help the ball of the foot recover is to distribute the weight over your whole foot rather than on the ball in the heel which can be done using a custom arch support.

    Thanks for you’re reply! I Not sure why PT is choosing to dry needle other than to help with swelling maybe? I do a lot of walking/ standing for my career and so I am wearing brooks gym shoes all the time with a custom insert - my dr even added a higher pad/ support to the bottom of my insert to help take stress off the sesamoid bones. Hoping that will give some relief.

    @@ProjectAccount1000 I would give it a go without dry needling. Dry needling is for muscle spasm and tendinitis. You don’t have that. It does cause inflammation because you are inflicting a new wound. So maybe you should take a break from dry needling for at least two weeks and see if it is better.

    7 months after lapiplasty on left foot, i have a rigid big toe and ankle swelling and puffy top of the foot. I can make an indentation on the swelling with finger and it stays dented in for a minute. I stopped wearing compression knee hi socks4-5 months after surgery. Do I need to continue compression socks and get into rehab for flexibility. I am 70 , taking longer to regain normality? Dr told me all bones are healed and i have no restrictions but it could take a year for swelling to go away?

    Hello @Chris Keller I would definitely seek out physical therapy! Hopefully somebody who has worked on bunions before because if they haven’t they will just give you butt exercises or ankle exercises. That is a sure sign they don’t know what they’re doing! The best thing for the swelling right now would be to have somebody do retrograde massage it pushes the swelling out and does an incredible job and then wear your compression sock after somebody has pushed the swelling out. The swelling can last for a year especially if you had a lot of swelling before surgery but it does sound like it needs to be managed. Then if you could have somebody do the stretches for your toe or try to do it yourself which is kind of hard that would be important. It is OK if the toe doesn’t move you were just wear stiff sold shoes because getting a great toe fusion is a common solution so not having range of motion is not terrible. I find that people very wildly in how fast they recover it’s not necessarily an age thing.

    Hi it is most likely scar tissue from the surgery. The second toe of course is very close to where the fusion was done. Once your fusion is fully healed per x-ray, then you can massage it between your toes and try to soften that.

    Hi @Biomechanicphysicaltherapy, I had lapiplasty 12 weeks ago on my left foot and I have pain under the big toe when walking, sometimes it feels like pain under second metatarsal, it's hard to tell. I can flex the toe fine and I have no pain when I do that. The foot is still swollen but mostly on the right side so when I walk the foot feels uneven. I'm concerned that this is a chronic pain at this point and I have to live with it. I can walk in sneakers (I can only fit my shoe in sneakers with shoelaces untied or loosely tied) but my walking is not pain free. I don't have any pain when I'm not walking and my toe is not stiff. I can flex it and bend it. Do you think these exercises are for me or should I be doing something else?

    Hello, 12 weeks is very recent... it is okay for you to do controlled exercises and yes these are fine for you. My advice is, respect your pain, don't push it too hard, ice it when you do. If you do too much and keep yourself in pain all the time you might end up in a chronic pain situation. But it is not chronic pain now and is not a harbinger of chronic pain. It is early days yet, be nice to it for 6 - 9 months more.

    Thank you for this info. Came across your page whike looking for help. Can it help still after 8 yrs post surgery? I suffer extreme pain still. Especially when laying down. I had my second hammered toe corrected at the same time so it has a pin and cannot curl. This makes it extremely difficuilt to flex or curl either of them. Foot still swells in footware. I'm so distraught and looking for any help I can find. Thank you.

    @Shirl Starg it’s not too late! Something can’t be done about it. I don’t know where you are but you can find a physical therapist and then you could have them call me and I can tell them what they should do. I have done it many times were physical therapist called me for a protocol. It’s totally fine as long as they have the ego to be willing to do that. You might be able to find a physical therapist who already has experience with bunions but they are not very common. If you have a person who is good with their hands they could also be with you and telehealth with me and I could tell them what to do. But I think a physical therapist who has access to the tools that you need such as ultrasound and iontophoresis would be the best.

    Three weeks is pretty early. I would start now with just trying to get the inflammation down and just healing. You can try icing it. Ice just 10 minutes Because it doesn’t take long to cool down the toe. If you have access to someone with a cold laser that will help healing. It cuts the healing time in half for both bone and soft tissue.

    Hi there, thanks so much for this video. I had surgery 3 weeks ago, which consisted of: hallux valgus surgery, First tarsometatarsal joint (TMTJ) arthrodesis, akin osteotomy, cheilectomy, and plantar fasciitis surgery. I don't even know where to start on my rehab process, so I think I will see a physical therapist. Are there any recommendations for where I'm at currently? I've been resting my foot this whole time. My doctor told me to start putting a little bit of weight, but it is too painful at this point. There is still a lot of swelling, so should I be doing the icing and hot water treatments? I miss driving and going to the gym so much hehe. Also wondering when it would be safe to ride the bikes at the gym? I'm sure my physical therapist will help, but maybe having some knowledge before going in. Thanks :)

    First off you’re only three weeks out it should be swollen and painful! Arthrodesis means joint fusion. Your foot Hass to carry three times your body weight when you’re walking. Allow the fusion to heal. I would not put weight on it unless you have an orthotic that is perfectly fitting the bottom of your foot or are in a hard cast, fiberglass. Not a boot. The healing can be speed it up by getting class three or class for nonvisible Red spectrum laser. Not the silly stuff you can buy on the Internet. You are not ready to start with physical therapy until the bone is well knit. At the fastest that could be four weeks the usual is 6 to 8. You could at this point wiggle your toes and move your ankle and touch it if it is hypersensitive.

    That depends on how extensive your surgery is and how physically capable you are yourself. If you are non weight-bearing, then you may not be able to put weight on that foot. If your bunion was very severe, then that would be the case. If it was not very severe, and they’re only clipping off a small piece of bone. Then you will be able to weight bear on your heel. It depends on how fit you are, how prepared you are to walk with crutches with only 1 foot on the floor. Peoples level of fitness varies wildly. So if you’re bunion is very severe and you are going to have hardware afterwards and you are not good with crutches then I would think you should have some help.

    As soon as your bones are well healed, you can start the exercises. Usually that is 4 to 6 weeks but you may have also seen your doctor to get a follow up x-ray. If your follow up x-ray says you’re well healed then you’re ready.!

    I just went to my 6 week post op bunion surgery, lapiplasty, and found out I was to be stretching my joint. I didn't know. I screamed when he tried to bend it. I am to go to physical therapy but honestly, I don't want the therapist to cause me that much pain again. I want to do some on my own GRADUALLY. Are all the exercises shown good for me if I have plates in my joint? He only showed me the up and down one. Thank you.

    Hello the best thing to do to start gently is first to start touching it. Wash it with a washcloth wash between your toes really get it desensitized to touch and texture. Massage it. and pull straight out on the toe. Do that several times several times a day. When that is no longer a problem then do the wiggle. Will you move the toe flat in line with the foot, parallel with the foot and wiggle it up and down. Do that for a couple of days. When you’re comfortable with that then do the whole combination pull out do the glide and then bend backwards. Most physical therapist don’t know what to do with toes so you should call around and make sure that you find someone who has worked on bunions before.

    Hello @leevize6045 I am sorry to hear that. It is possible that the nerves will heal. There is a laser on the market, MLS Laser by Cutting Edge Technologies that is the only proven thing to heal nerves. It is also possible that you have a little bit of innervation left in which case physical therapists can help stimulate those muscles to be able to lift the toe again. Good luck to you.

    Im 6months post op and have started getting very worried my toes have stiffened too much and im still limping because my toes dont bend anymore. Heel lifts just hurt but now i know i need to extend the toe to create space in the joint. How often should i do these exercises being at advance recovery stage.. Thank you so much

    Hello, it would be best if you do this twice a day. You might want to start with a professional PT to see how hard you are supposed to push. Best to do most stretching in first 6 months so get started right away!

    Hello it would probably not be best to wear a heel higher than 3 inches. You might be able to do it after a year of healing. The thing that speeds the healing up the most is to use a non-visible infrared laser such as the Med-X or the MLS laser. It cuts the healing time and half for both bone and soft tissue. You could call around to doctors and therapists near you to see if you can find someone who has a laser that you should start using immediately post- op.

    @Elsy K .P - sorry you cannot start any exercises with your foot, the first thing that you need to do is let the bones knit. If the ends of the bone wiggle it will not heal so that’s why it needs to be immobilized and rested. If you really need to do something you could do straight leg raises or sit ups keeping that foot in the air or tricep dips on a chair. You could sit on the weight bench with your foot elevated and do shoulder presses or bicep curls. But most importantly, let your bones heal for four weeks. If you have access to a cold laser (non-visible red spectrum) such as the MLS laser you can cut your healing time in half.     I had a big toe fusion that I’m really regretting. It’s 4 months after surgery and I can’t walk correctly or even wear a shoe. How will I ever walk again if I can’t flex my toe? My ankle is now being affected as well because of the way I have to move. Thanks for any help.

    @LivingaDream101 It takes 8 to 12 months to get used to a fused joint. It would be nice to know what angle the big toe is fused on. That’s very important. The other thing that you can do is wear really stiff thick lug soled shoes to prevent any stressors on your toe until the 8 to 12 month mark has passed. I know it sounds like a long time but if you just wear a big sole (I will try to attach a picture) and change your style perhaps for a little while. It is usually a very successful surgery. If you want to talk to me live about it you can call the office. This week I am at 757-917-7857. OK it looks like I cannot attach a picture but you know those shoes that are popular right now with the white soul that’s like 1 1/2 inches tall. They’re like fashionable sort of sneakers. Or you could wear combat boots. The bottom line is you don’t want the sole to bend at all.

    @@Biomechanicphysicaltherapy Thank you so much for your reply. I was under the impression that I needed to try to use my foot more so it would get the mobility back. I don’t have any shoes that fit because of the swelling. I’m wearing a boot like Ugg boots because it’s the only thing that fits. I’ll have to shop for the type you recommend. My toe is set just off the ground and not angled upwards at all. The last x-ray showed disuse osteopenia so I thought it was good to use it. Nobody told me it would take so long to be able to walk. Doctors make it sound like you’ll be back in jogging shoes in 6-8 weeks. I will take your advice and get stiff sole shoes. I really appreciate your help. I couldn’t find any information on recovery from a big toe fusion. I think you were referring to Timberland work boots. I’m going shopping for shoes asap.

    @@SaltySherry Hello, yes the disuse osteopenia is from not weight-bearing. But that toe is never going to move so you don’t want to work on any flexibility for that. Sure work on flexibility for your ankle. Do weight bear on it. But protect the toe from motion. It sounds like the Thow is in a good position. I know the surgery is easy for the surgeons they don’t often consider how long it takes to recover. Happy shoe shopping! Glad I could help. If there’s nothing on the Internet perhaps I should make a video and a blog about that specifically.

    I'm 5 months post Lapidus surgery. I've been going to PT and doing my exercises. My dorsiflexion is about 40 degrees. My doctor's office says with that I should be able to walk without any visible defect. But that's not my reality. I'm limping. When I walk slowly with sneakers you might not be able to tell I'm limping. But if I pick up speed, I limp for sure. I'm not able to walk fast. It's really disheartening. I'm afraid I might never walk normally again. Anyone else with this experience? How long does it take to walk normally?

    Hi there can be several reasons why you limp. it would be best to have an in person evaluation with someone who is good at this. it’s possible that it is because you have an old habit that you haven’t broken, the nicest answer the easiest one to fix. It could be that you actually need 60° of dorsiflexion at your hallux which is the normal number that you would need for roll over. You could try just wearing really stiff sold shoes and that might give you a normal walk.

    @Heather Marconi You can wait until your doctor says that you are well healed on x-ray. If you have access to a cold laser or if you’re getting physical therapy and they have a cold laser it can cut your healing time in half. So if you’re getting cold laser it could be as soon as 3 to 4 weeks if you’re not getting cold laser it will be approximately 6 to 8 weeks. Or when your doctor says your bone as well healed.

    Twice a day would be good. I would do several stretches, starting with the distraction, then the mobilization, then the backwards stretch. So three sequences of stretches twice a day.

    Thanks for the video... I'm very worried as my dr. Didnt give me any physio and I noticed my toe wasn't flexing back and I was limping.. I'm 8 weeks out . My toe is very stiff. Am I too late?

    Ailbhe Large No it’s not too late. Not very many therapists really know what to do to treat bunions though. What you are looking for is starting with heat to warm your toe up then you’re looking for ultrasound to be done around the big toe joint, massage, pulling the toe straight out, and carefully bend it backwards without any pain in the joint. You need heat and the ultrasound before the stretches to make the stretches more effective. Do not do heel to stretch it. Beware if you find a therapist that just gives you exercises like straight leg raises to strengthen your butt. It’s a common thing to do. You might even be able to start stretching right now on your own. If you can’t find a therapist who will work directly on your toe for your range of motion in real life you can schedule telemedicine with us. We can walk you through exactly what to do.     Dear Dr. I have a painful bunion and stiff big toe joint which occurred due to a trauma on the bunion 4 months ago.. my MRI and X-rays show that bones and soft tissues are okay but the joint still hurts and stiff when walking.. can I use these exercises to enhance the range of motion in big toe joint even if I haven’t done bunion surgery?

    Hard to say without a physical exam but assuming there are no red flags like excessive heat from the joint, significant pain, preceding infection, history of gout, etc., then this type of manual therapy is typically going to be just fine. It sounds like a health professional has looked at it already but it's still hard to give advice over the internet.

    Hello. It sounds like what happened is that the joint has become inflamed from the injury. Since your MRI and x-ray also everything is OK I would think the most important thing is to get the inflammation down first. That could be icing, there’s an excellent treatment and therapy called iontophoresis which would work for you very well! You could do other anti-inflammatory Treatments. But yes you can do the exercises to increase the range of motion. I am so sorry for the delay in the reply for some reason I did not get a notification!

    @@rosebudelfjohnson4637 I started walking like 2 weeks out, but that was with crutches and I was doing 10% on my surgery foot, then I would increase with weight via my Drs. orders. I currently still have foot problems so it's def a 50%50 shot I'd say.

    ​@rosebudelfjohnson4637 I was weight bearing at week 2, but I was only allowed to put 10% of the weight of the surgery foot. I then increased the weight via my doctor. But for the love of God, do physical therapy idc what they tell you you NEED physical therapy especially with a foot surgery, I had to figure this out the hard way and now I'm 8 months and still have foot pain.     I had surgery about 10 weeks ago on my left foot. I did not get any physio so have done the exercises on this very helpful video . However, I went back to see the surgeon yesterday and he told me that my toe will need er bend at the bottom joint again. I wonder , is it therefore pointless for me to do these exercises? I feel they have helped, I can walk fine and hiked 2h the other day but I would like it to vend more of course. I have a plate an screws in my foot now. Thank you in advance, Anna

    Hi @Anna Eriksson I don’t know why he said your toe would never bend at that joint… Did he fuse it? Where are the plate and screws? Did he do a pseudo-fusion and pinning and moving it is not advisable?. I’m afraid I cannot answer your question without asking many other questions. A postop x-ray and a recent x-ray would answer my questions.

I believe it is a fusion surgery, he removed some parts of the big toe joint that he said was too worn down so it's held with a plate and some screws. He said the top part of the toe will bend which it is starting to do but not the bottom. I do feel your exercises are really helping though so I think it is safe to continue.

    Hi. Sorry for the delayed response your swelling may be gone by now! For some reason I did not get the notification. There are 3 main ways to handle the swelling. The safest one considering the type of surgery would be to do the contrast bath. A contrast bath is putting your foot in ice cold water and warm bath water alternating with for instance 10 seconds in the ice and 30 seconds in the hot going back-and-forth for several minutes. Another way is retrograde massage, you probably would not be able to do this on your own but it involves pushing the swelling out with your hands. The third way would be to use Coban or co-flex wrap, wrapping it in the morning when your swelling is the least and that will hold it down during the day.     Hello if it’s well healed on x-ray you can begin therapy the screw is level with the surface of the phone so it won’t be a problem. You may not want to pull on your toe this early but most likely it’s OK. you could follow along with the steps in the video.

    Hi, it depends on what type of bunionectomy you have had. If they only cut off bone and you don’t have any hardware you can start as soon as you can tolerate it. If you have hardware you need to wait until that all heals which could take 4 to 6 weeks.

    Short answer hell no. Lol I just had mine July 9th. With hardware. The first week is the hardest with pain and swelling. The second week you can control the pain easier because you’ve had a week to get to know what feels best (elevation, positions, how much meds to take away the pain) but my personal swelling was still very apparent into the third week. And I wasn’t feeling pain from surgery in the second week, but any excess movement or if my dogs or kids bumped it would take me OUT! There is a LOT of healing going on after this procedure. So expect to not be ready for any manipulation at the surgery site for a solid 3 wks

Stay ahead of the pain by following the pain medicine prescription exactly, use ice, don’t try to do too much. Let yourself heal.

    @@dr.farrahzahid3239 Yes but you should expect to wait a full year before you do something like running. You should have a full recovery and you can always use sturdy shoes to protect you. With sturdy shoes you can do just about anything! It is OK to have the cast on for six weeks. it is great to have physical therapy afterwards even if your doctor really doesn’t believe in it.

    After doing these exercises is it better to ice your feet or heat them? I'm in PT for this issue right now, and I'm afraid my PT has no idea what he is doing. He is not using this technique... I am 6 years post surgery for bunions and have very limited range of motions. Hoping if I start doing this it will help! Also, how many times a day can I do this exercise? Thank you!

    Hello @Taylor Miller if you are sore you can ice after. However, if you are not sore you may want to skip the ice because once you have gotten your toes moving and more flexible, icing can sometimes stiffen them up again. Ice is great after you have been on your feet enough to make them sore. It doesn't take much, so only ice for 10 minutes. Heat is best before the exercise. You can do the stretches 3 - 5 times a day. Most PT's don't actually know these techniques, I have specialized in foot & ankle for almost 25 years. I do make sure that all of my 11 PT's who work for me know these techniques though! 😉

    Hello, if you have only had a piece of bone cut off and you don’t have any plate or screws or pins then you can start the exercises. If you do you have a plate and screws it would be better if you waited for the bone to falling it before you start the exercises so give it a couple more weeks.     @MeNevar what is the reason why you can’t wear heels? Is it because the toe doesn’t bend? Is it because the ball of the foot hurts? Is it because it’s swollen? Is your scar causing a problem? Theoretically you should be able to wear heels now so whatever problem it is needs to be specifically addressed.

    @@menevar7529 Hi, so it sounds like you need to get more range of motion. It is hard to do the stretch on yourself, but not impossible. Try warming your foot up in a nice warm bath or after a shower, then do the stretch shown starting at 2:15 on the video. Do that several times, preferably twice a day. Try to have a way to measure how much your toe bends back now, then stretch for a week, measure it again, keep doing that until there is no longer increase in how much your toe bends back -- or until you can get into your favorite heels! An alternative to heels for now would be to wear a wedge/platform sole type shoe. You will have the hight without the toe bend.

    @Dakota L only the smallest of procedures would require no screws where they just trim off the bone. And usually that’s not technically a bunion. The problem is is that the definition of a bunion is that that long bone in your foot deviates so in order to get it back where it belongs you’ve got to cut it in half and screw back together. So yes, if you truly have a bunion and not just a bony overgrowth. See our video on turf toe to see if you have a bunion or not. Or send me a picture.

    @Dakota L then maybe you should just try changing shoes, wearing orthotics, and trying to live with it first. It’s not an emergency. If you have pain you could try to find a therapist who knows what to do and they can see if they can help you. Not all therapists know what to do with a toe, if they give you leg exercises you know you’re in the wrong place. It is an ordeal to go through post surgery; for some people it’s easier & for some people it’s really hard. Wait until you’re ready.

    Hello @Mezzo Wannabe you should be able to start the exercises now. If you have had an x-ray for your six week follow up and the doctor said you are well healed you can definitely start them. Please note the stabilization of the hand on the foot. It will prevent you from putting any torque on the surgical site with hardware. It will make it more comfortable and more secure feeling. It is a little bit hard to do on yourself because it does take a bit of force. Be sure to do all three motions. You can start with just pulling the toe out, hold it for 10 seconds and do that 10×3 times a day. Do that for a few days. Then add the pulling the toe out and the glide. Same thing hold 10 seconds do about 10×3 times a day. After you’ve done that for about a week start pulling out to the glide and bending backwards the whole combination. Hold that for 30 seconds if you can. Make sure it is stretching on the bottom. No pinching on the top.

    @@Biomechanicphysicaltherapy Oh my god thanks a lot for the detailed explanation. I have been doing the excersises as you told for around 2 weeks but there is almost no change in the stiffness. It's like the fusion is starting my joint or there is a rock instead of my join :( If I continue doing these, do you think even if in a year, my joint can be like before? Like the 65degree bending out (like running position) ?:( Another doctor said it is similar to the teeth implant so there is no connection with the joint. But it's impossible for me to understand how a joint can be THİS stiff only due to the torque or something :( Again thank you very much. Sending so much love and my best regards

    @@mezzowannabe543 Yes sometimes the joints are very, very stiff after surgery. I have had this experience before I usually say it feels like somebody poured concrete in your joint. You could start by soaking your foot in a warm Bath for 15 minutes before stretching. If you were in physical therapy with us we would start with heat then we would do ultrasound which moves fluid with sound waves then we would do the stretches and massage then we would do laser and iontophoresis to take down the swelling and inflammation and speed healing. Maybe you can find a physical therapist near you who can do all these things. Most of them don’t know how to stretch a toe so you would have to show them the video. keep stretching and keep stretching aggressively your best window of opportunity is the first six months. Press on.

    @@mezzowannabe543 Yes absolutely keep stretching! Every day preferably twice a day aggressively the only thing you don’t want is a pinch on the top of the joint and try not to bend the joint within the big toe backwards that can make it sore.

    @Megan Copeland if you have not worked on stretching it then you should get started right away. If you can’t do it yourself you could try a physical therapist in your area. Six months is kind of the end of the scar tissue remodeling phase. It’s not that you wouldn’t be able to make progress after six months but you should definitely do something about it at this point if you have not already. So there’s two types of bending the toe though; one would be bending it as in doesn’t have any range of motion and the other would be bending it using your own muscles. In my answer above I assume you are referring to the fact that it doesn’t move well but that you can activate your muscles?

    Going on 10 weeks and my toes cannot plantar flex at all. Slight dorsal flex, toes overall stuck and swollen. feeling of separation inside where the bunion use to be every time I plantar flex. Worried I might never get my range of motion back

    Hi Tonyfitness motivation, I know 10 weeks might seem like a long time but it’s not too late. It does sound like you need physical therapy for some retrograde massage and some swelling management and to get the fluids moving in there with ultrasound and joint mobilization. If you feel like you can do it yourself then we could help you with that live or you could follow with what the video says. It is hard to do it to your own foot just because of the position. You could also teach a friend or companion how to do it. I do think it would be important to do some intervention at this time especially since it looks like you’re into fitness and at 10 weeks your bone should be fully knitted and you should be ready to go. I would not expect to run until 6 to 8 months after surgery. And right now you should be in stiff soled shoes so that you can do all the walking you want without taking a hit on the toe. The swelling does sound like a problem.

    I would question why you were still in a tall boot at 10 weeks. Does your x-ray not look good? If your x-ray shows you’re healed a tall boot will only keep you swollen. Even if your x-ray doesn’t look good I would be taking off the boot and pumping the ankle several times a day. There are things that can help with speeding healing such as the cold laser (it cuts the healing time in half. we do have a video on it if you want to see more) if you are having a hard time knitting the bone at this late date your insurance company should pay for a special ultrasound that will also help speed bone knitting. But In general and normal healing it would be incorrect to be in a boot at 10 weeks.

    @@Biomechanicphysicaltherapy My X-Ray in my opinion is horrible because right now the toe is floating in the air and I have to push it down in order for it to touch the ground. X-Ray shows that the Hallux bone pointed upwards as well. Highly doubt that is suppose to be the result. Also, I had to call and request a Physical Therapy because they never gave me one.

    Hello, I am sorry to hear you are in pain! It is hard to answer without much data. Like, are you in a cast? Have you had your post-op visit? Has your dressing been changed? Are you taking pain meds? Did you "fall behind" on your pain meds? Hopefully it is just post-op pain but it would be nice to be able to contact your surgeon's office to see what they think.

    @@Biomechanicphysicaltherapy I had bilateral bunionectomy I am not on pain medication for the most part the pain is well managed. No cast but I have to wear a splint. Dressing and staples have all been remove. I just get these random and quite shocking shooting pain in the big toe joint when I move its really painful and that just started yesterday. I'm praying it just part of the healing process.

    @@KarenJewels Yes I would imagine the shooting pain would be when the non-healed edges of the bone bump together. They do have to cut through the bone and then screw it back together usually. You’re only at 2 1/2 weeks then I would imagine it is not knit yet. I guess it’s surprising that you are only in a splint. Maybe that is your postop dressing? They often will do splint and lots of padding immediately Postop and then when you go for your postop visit they’ll take that off and put you in a cast or a boot. That will help keep it more immobilized.. It needs to not be moved in order for the bone to knit. I hope it helps just to know what might be going on. Keep in touch with your doc & hang in there. 🦶
