Understanding Knee Buckling

Knee buckling can happen in different ways. In this video, I discuss...
👉Inward (valgus) buckling of the knee which is common with MCL and ACL injuries.
👉Forward/backward buckling of the knee indicative of a quad injury (tear), Patello-femoral dysfunction, effusion, knee pain, patella tendon disorders and anything that cause reflex inhibition of the knee.

Both my knees have completely failed twice, buckling forward and dropping me on them with no pain, one time while working in a -40 cold storage (I was sweating and had warm clothing on) and once while hiking with shorts on at higher altitude. There was no warning and I had to have help walking afterwards to a location where I could sit until they started working again. Now they’re getting really sensitive to weather changes and ache when it does.

This sounds exactly what I've got. It's like when I turn my left foot as far to the left and then rotate my leg in that direction (to rotate left) my left knee buckles on the inside (Right side of left knee) as if it's about to pop out of the joint. And immediately after that happens, I can sometimes walk on it normally, as if nothing has happened but my leg becomes extremely weak, as if it had no strength to hold me up anymore. Now that I'm older (been dealing with this for like 10 years now)... my leg sometimes feels like it wants to bend in the opposite direction of where it's supposed to bend when standing up straight. Can sometimes walk normally without pain or instability if I get into a good stride, but starting and stopping can feel very dangerous. Been using an ace bandage to get around at work, but I've lived the last 10+ years with the fear that I might one day break my knee/leg because of this issue. It's caused me to fall down stairs, or even miss a curb and fall into the street (when it feels like it's going to pop out of place), just to name a few times. Hoping someday to visit a doctor to solve the issue. But I guess my question is; what exactly based on the above could be the issue? I know there is some inflammation, and no physical (bone) damage to the leg as I went to the hospital at least once where they did an X-Ray after a particular bad incident and found no damage to the knee in the X-Rays. They gave me a long-leg brace, and after 2-3 weeks I was able to barely walk again (As after that incident I couldn't even put weight at all on that leg, as if it were broken/sprained). Or rather, what else can I do to strengthen the knee or better stabilize it? I've broken my ankle before, and physical therapy helped with that. But not sure what physical therapy I could do for a knee?

There are sooo many videos out there for knee pain, but they don’t address the buckling Mine buckles inward/outward with excruciating pain, so much that I can’t walk on it for 3 to 4 days. It’s becoming more frequent now occurring once a week or so just walking. What is my next step to address how to fix it?

Im going through your videos hoping I find something that can help lead me in the right direction. I’m 29 and already have tons of muscle, nerve and pain issues. I’ve been absolutely miserable for the past year especially. My knees buckle often, what I call “bambi legs”. If I don’t stand a certain way my knees will do multiple little buckles while I’m standing still. Parts of my legs feel numbish, my fingers and toes will feel numbish like when you’re outside in the cold for too long. Shooting, stabbing, tingling, trickling sensations from my head to my toes. EXTREME headaches and pressure that last 2 weeks are longer with no relief from OTC meds 😣

Hi Urvi, I think I mentioned that it is a loss of control between the hamstrings and the quads. Usually because of swelling or pain. It can also happen with ACL tears.

@@OrthoEvalPal could it be ACL tear? My knee buckles mostly in the forward way, it seems to give out, but in a forward direction. And when it comes back, it wobbles just a little bit, right before it goes into hyperextension. Basically I have to focus and bring it back by tightening my muscles, otherwise it feels loose, weak and buckles forward. I had a partial ACL tear a long time ago that never really seems to get better despite all the PT I have done. A month ago, the doctor asked if I had any buckling, I replied with no, and was prescribed gym exercises like leg extension, leg press etc. As my leg muscle strength was weak. Now despite doing some work in the gym, my knee seems to keep getting weak, maybe due to swelling, and now even buckling is there.

My knee definitely buckles forward then whips back kinda going down stairs and on occasion on flat surfaces and this is with a dislocated knee injury back in December 2023 from overused knee extensions going up and down stairs wells all day as a delivery driver where I wasn't physically fit to do it.

I play basketball and my left knee buckles inward and rotates outward when I am jumping to shoot, my right knee on the other hand is perfectly fine and seems to even be stronger and more stable than my left, In certain movement my left leg is a lot less stable than my right. I think that the stability issues might go away as I strengthen my knees, I am still going through early puberty and I am assuming that will go away in the future as my leg gets stronger. When I jump normally both legs are perfectly fine but when I'm gathering more power to shoot the basketball it seems like my left leg buckles inward. I don't think that it it due to injury as well, and i dont feel like it hurts either

Hi Jonathan, absolutely. Mild swelling can cause stiffness and swelling inside the joint could be an indicator of anything from an ACL tear to a meniscus tear to damage of the cartilage lining the bone.

I tore my ACL and sprained my MCL 7 months ago. Non-operative management so far, but it still feels a little unstable. Meniscus and cartilage are fine. Can buckling cause additional injury? It hasn't done this yet, but the potential is there I think. I'm athletic, but at age 60 I'm disinclined to have a reconstruction.

I had a torn acl and had a repair. Directly after the sports accident my "only" issue was subluxation/inward buckling with every step of normal walking. Due to this instability I did not walk at all for the 3 weeks waiting for the OP and just started full weightbearing 2 month after OP again (= in total almost 3 month not fully weightbearing). I never had this subluxation/inward buckling anymore, knee feels stable in general, just sometimes it buckles backwards/forwards, which I did not have at all right after the acl tear, where all my muscles were still strong. This is why I attribute this to muscle weakness and/or slight swelling related. Do you see this backwards/forwards buckling in post OP knee patients with weak muscles and it resolves after they build up their normal muscle strength? Or is it very strange and uncommon to have after a knee surgery?

@@raghavsharma9838 Hi I am happy to report any uncontrollable backwards buckling of the knee was completely related to post OP muscle weakness in my case and completely disappeared now 1 year post OP. The first 3 month I prioritized getting back full ROM symetrical to the other side. starting at 3 months post OP I focused strength training, but really slowly (leg press started with 15 kg haha, now one year post OP I am able to do 140kg again). I had a checkup at my surgeon at 4 month validating the acl is fully stable. I think the key is to get back full extension first, then build your muscle slowly back to normal. It is normal this takes 9-12 month. My PT saw semi professional footballers who rushed the training and now struggle with chronic problems. be aware for flexion it is normal to take a bit longer to fully come back, sitting on my feet with fully bent legs took 10 months to feel comfortable again, but I worked on this until it was possible again :) Also trust your feelings- in the beginning the knee might swell after too much strength training. Give it a rest of 1-2 days and go slower . Then I am sure you also will be able to combat the post OP backward buckling if it is also muscle weakness related Wish you all the best 👍😊

Does this have some kind of underlying condition? I dislocated my right knee around 3 years back and had surgery on it to fix some problems. Then a little over a year after that it happened to my OTHER knee, except I didn’t dislocate it, it just gave out after buckling like that. Now I have issues with my left knee buckling inwards and it makes it super difficult because I’m involved in theater which requires a lot of dance, and I can’t run either or my knees will buckle. It doesn’t necessarily hurt, but it’s super uncomfortable and if it DOES give out it feels incredibly weak to walk on for a little while afterwards.
