Treat Knee Cap Pain (Patellar Mobilization Technique)

Dealing with knee cap pain? Here are some quick techniques to mobilize your patellae and keep your knees healthy.
I got a question from Jackie inside the Precision Movement VIP Lab on Facebook, who said she'd had a fractured patella, and now she wants to make sure it's moving properly.
I wanted to make a video on knee cap pain because it is common as we age. Taking steps to address (or prevent knee pain) now means that you don't develop compensatory mechanisms down the line. Because compensation, if left unchecked, usually means pain and stiffness down the road.
This technique takes three different approaches to rehab the tissue around your kneecap. First, you’ll get your patella moving passively. This also lets you feel for anything abnormal like swelling, stiffness, or scraping.
Next, you’ll work on auto self-myofascial release of your patellar tendon (which inserts into your tibia). Finally, you’ll break up the tissue in your quadriceps tendon, which attaches the top of your kneecap to your quads.
ASMR breaks up any scar tissue or adhesions in the tendon, allowing the fascia to glide smoothly.

    Hi Coach E - Jacquie with the fractured patella here - this is incredibly helpful for me and hopefully for others - I actually have been avoiding really moving the patella with my fingers, probably because of the pain it previously went through but this has given me something I can regularly do and get back to not being scared of it.     YES good point - pain can often keep us away from trying things, but as you have just demonstrated it's all about dipping your toes in little by little.

    This was helpful. I am recovering from a displaced tibial plateau fracture. My knee is terribly stiff. I only was allowed to started PT yesterday, 8 weeks PO. Doing these exercises, my knee said, "Oh yeahhh, I am feeling a bit better!"

    Great video. My patella moves in all directions except down and is painful after playing soccer. Which tendon would it be most important for me to work on? Thanks.

    Hi, Love your videos. They are super helpful. I have a severe Patella Alta and have had a previous MPFL injury which led to patellar dislocation. Also have problems with patellar tracking. My knee cap moves around a lot and I have to wear a brace while playing. Will patellar mobilization exercises be detrimental for me since I want my patella to move around to a lesser extent? Appreciate your help. Thanks!

    I’d say yes, but to be sure come out on Thursday at Noon EST so you can ask Dr. Boynton (ortho surgeon) to be sure - she’s the expert on post-surgery rehab.

    Definitely stop anything that causes pain if you cannot modify it to do it pain-free. Modifications include moving slower and smaller.     I tried doing this but whenever i move my patella downwards it makes a deep pitched, somewhat quiet pop sound, any idea what causes this?

    It is likely that pieces of connective tissue are rubbing over each other making a noise similar to when you snap your fingers. - Coach Joshua, Team PM

    Injury about 15 years ago is flaring up now feels like the cap wants to float down. Can't even barely touch it

    Try applying hot and cold water over the affected area in alternation for 30 sec each for a total of 5 min. You can do this daily. Move the patella as much as you can even if the movement is very small. Keep us posted on your progress :) - Coach Joshua, Team PM

    i cant move my kneecap when i have legs relaxed. but i can when im standing a certain way...i have patellar tendonitis.

    I didn't get it checked out. I was struggling with sum1 an I was actually lying down when I felt a move on my knee to the left side. Is no longer swollen is just not stable an kinda weak

    Might be a silly question, but how can one identify where their patella is? Maybe mine is just immobile/stiff, but around my knee I can’t seem to make anything “move”

    It's not clear why your patella is stiff but with time and practice you should improve its mobility. Make sure that you are completely relaxed when doing the exercise. Keep us posted on your progress! - Coach Joshua, Team PMC

    @@adityabindal3163 Per my MRI, I have cartiledge tear and patella tilt and my pain is in knee and hip, comes and go. I did this video/exercise first time yesterday. BUT I have been doing lot of other things- No workouts (rest mode), Collagean, MSM supplements, Turmeric water, 15 mins under sun (for D), yoga, stretches, u name it I am doing it...so pain is almost gone so I am not sure what makes pain go away and what not. I just want my pain to go away!
