Patellofemoral Pain Syndrome Evaluation and Treatment

This patient has lateral patella compression syndrome so I thought I would demonstrate Patellofemoral Pain Syndrome Evaluation and Treatment. This gentleman's history fits the picture quite well. Here you can also see how quickly this can be evaluated and how ruling out other knee issues localizes the problem.

I've been through two orthopedic doctors and PT's, and they haven't even come within 1/10th of the level of detail and thoroughness as Paul. The orthos looked at my knee for about a minute, mainly just ruling out meniscus and other major tears (not really trying to actually diagnose me), and the PT's just throw template exercises at me without focusing on a plan tailored for my specific PFPS diagnosis (that hasn't even been established). They just do what most PT's do, stabilization, hit the VMO, hit the hips, not providing any at home regiments... Pain isn't going away. If only Paul was near me.

When I sit for too long it hurts I can't wear my heels for too long, I limp and I can't get on my knees the pain is bad. I also can't put my leg back to stretch 😫. The examination you did here is far more than what my physician did. 😒 All he gave me were anti inflammatory and pain killers.

I did not hurt my knees, I am not an athlete nor do I run. Equipped with this video, I self-diagnosed, and (Patella grinder clark sign? I am sure I am not saying it right) but right around the 3.15th minutes of your video and it hurts, that's it. Is there any recommended treatment, exercise for this. Is this a long term situation or something that could be treated? I am worried, I'd be grateful if you are able to help respond. Thanks much!

Firstly, thank you for this amazing and clinical eval sir, just had a few questions being a physical therapist assistant student. So there will be lateral patellar tracking most prominently with reduced medial patellar mobility?

  • @OrthoEvalPal

    @OrthoEvalPal

    4 жыл бұрын

Yes typically but not always. The first of the quad muscles to shut down is the VMO. Therefore the quad pulls the patella more laterally and the most common area of discomfort is the supra-lateral patella where it contacts the lateral femoral condyle.

Have the same problem due to old acl sprain when i was a kid. I didn’t know it caused a patella tracking problem so 20% of patella cartilage gone. I had no pain so the cartilage was grinding away minutely for over 30 years. Started playing tennis and suddenly pain started like this gentleman described. Seen orthopedic doctor and scans. It turns out that IT band was pulling my knee ever sos slightly causing the tracking problem. Saw PT and turns out i have weak glutes. 6 weeks of intense glute, quad and hamstring exercises made a huge difference. Combined that with foam rolling of IT band.

But I can squat under load (185 lbs x10 for few sets) and walk up and down the stairs. One PT diagnosed me with PFPS but I'm not sure since two orthodontist had no idea what's wrong with my knee. X-ray came clean. It started to hurter after a basketball game, also that week I had like 3 games in 7 days. If anyone has any idea I would appreciate your answer.

After we saw no improvement they scheduled and appointment for mri. The mri results showed that I have chondromalacia of the patella. They prescribed PT 2x per week for 6 weeks. Scheduling appointment with PT at home is very challenging and im worried that the days go by and soccer season is almost starting for next semester and I want to get back to playing ASAP. Hi Dr I noticed others was asking and you replied so thought I'd get mine in. I feel unstable, have pain around my knee caps but inside and legs wobble when walking, standing or using steps. Sometimes I get swelling too around the knees it also feels like my knee caps are going in towards the back when I bend. After watching your video I commented saying thanks now I know it's patello. Do you think this is the case? I'm due to see the physiotherapists on the 7th January. I've had knee pain since 2016 but it's just got worse with more symptoms.

Hi Keegan, Absolutely! Reflex inhibition plays a major role here. The VMO shuts down first along with lateral retinacular tightness leading to too much lateral movement. Check out this video. Classic. kzread.info/dash/bejne/eHposJd-ibHapsY.html Best, Paul

I’ve been dealing with this pain for about 8 weeks. I play high school basketball and I train a lot. I’ve taken about 3 weeks off and now I’ve started training again. I still feel pain on the left knee when I go up for a dunk. I don’t know what to do. I can still move fine but I mostly feel pain when doing isometric movement and jumps off of my left leg. Plzz help. I’m pretty sure it is this because it’s on my upper left knee

Ortho EVAL Pal With Paul Marquis I have a question about 8 months ago I had a subluxation in my knee and then I went to PT for it and left with it being ok. But a month or two after I heard cracking in my knee when ever I bend it and there is sharp pain when I keep it bent for too long. Also if I’m sitting down for a while and I stand up I here a large crack from it and hurts a little when that happens I’m curious as to what it is.

The diagnosis is still most likely Patellofemoral Pain syndrome (PFPS). ~~~ (Still go see your primary care physician) The initial management of PFPS includes 1. activity modification (eg, reduced intensity of exercise) and 2. nonsteroidal anti-inflammatory drugs. 3. stretching and strengthening exercises with an emphasis on the quadriceps and hip abductors. Exercises (as per www.webmd.com/pain-management/knee-pain/injury-knee-pain-16/slideshow-knee-exercises) 1) Straight Leg Raises. If your knee's not at its best, start with a simple strengthening exercise for your quadriceps, the muscles in the front of the thigh. ... 2) Hamstring Curls. These are the muscles along the back of your thigh. ... 3) Prone Straight Leg Raises. ... 4) Wall Squats. ... 5) Calf Raises. ... 6) Step-Ups. ... 7) Side Leg Raises. ... 8) Leg Presses.

I wonder if you could advise - I have been having some knee joint pain and I think it might be Patellofemoral pain syndrome - squatting doesnt hurt - nor does bending the knee and most of the excerises you did dont hurt. The only time I get a sharp pain in my knee is when you did the hyper extension when my leg is dead straight I get pain in the front of my kneecap. I have poked the cap, moved it etc and bent it and no pain in other ways - it does also happen a bit on the excerise you did with him standing on one leg and putting pressure on it :( cant go to the doctors at the moment due to lockdown lol!!!

I’ve been struggling with knee pain since I was ten, I’m 28 now and it seems to get worse and worse. I was an avid skier and cheerleader into my late teens. I got diagnosed with patellofemoral syndrome about three years ago. I thought finally my pain will go away! But the physical therapy hasn’t helped and it’s to a point where they get swollen and I have to ice them almost every night. The X-ray I had three years ago showed my patellas were both off center, leaning towards the exterior of my legs. Is there anything else I can do besides manage pain? I can no longer ski, bike, lunge, or even manage a flight of stairs without being in considerable pain

when I had it many years ago I found this helpful: strengthen VMO (the lower part of your inner quad), stretch out hamstrings (but only when the muscle is already warmed up), also stretch out butt muscles and calves, and finally, when walking, try to remain very upright with like a 'book on your head' posture, keeping abdominal muscles and butt muscles activated while walking. I found that really helped me a lot. I haven't had that problem since

I’ve been dealing with this for a long time. Be prepared to give up high impact (running, jumping) for at least 6 weeks. I’m at the point where I can’t even do quad exercises at all because the pain is that bad. The key lies in most likely your hips, ankles can play a part too. There are 4 things I do daily that are helping immensely: 1: slow backwards walking on an incline treadmill (10 mins) 2: lying leg abductions for glute medius 3: banded clamshells for glute medius/tfl 4: whatever quad loading you can handle, for me it’s straight leg isometric holds and straight leg raises
