Smoking Can Affect Healing and Survival of Hair Grafts after a Hair Transplant

A 26-year-old man had a hair transplant fo 2600 grafts. One month after the transplant, he started smoking 3-4 light cigarettes a day as he says it is not possible to quit. He asks if smoking will have any side effect on his hair transplant, and if it will cause problems with his hair growth.
New York hair restoration specialist Amiya Prasad, MD reviews his question in this video, explaining what happens in a hair transplant, and how smoking can affect survival of hair grafts:

    excellent video. i'm assuming this also includes vaping because the nicotine is still present, however is it identical to cigarettes in it's potential to damage a healing hair transplant or are cigarettes significantly worse? if i am able to drink a dramatic amount of water to consistently flush the nicotine out of my system will this help? basically i'm looking for an excuse for the occasional vape because it will be very difficult to quit entirely cold turkey.

    Hi Jared, As a medical doctor, I can't recommend doing anything that can harm your body. Nicotine is in your bloodstream, so you can't flush it out with water, and drinking so much water in attempt to do this can reduce electrolytes in your system to a dangerous or even fatal level.     Hi AL Gaz, Kratom is not available in the US, and I'm not familiar with its immediate or long-term effects. It seems to be a controversial substance which I don't want to get into.

    Hi son ofspardaz, It is OK to quit smoking at any time. Smoking is particularly damaging when one continues to do it after any surgery as it affects healing. I wish you the best of luck with quitting, and I hope you can get cigarettes out of your life for good!

    Hi Palak Mehra, As a medical doctor, I advise that you just quit smoking for your general health.     Hi asit srivastava, Shedding after a hair transplant is dependent on the individual patient, and how transplant surgery was performed, so there is no set time for shedding for everyone, and shedding does not happen with every transplant. I hope this clarifies things for you.

    Hi Andi Susilo, I hope you understand that as a doctor, I can't recommend anything potentially harmful to you. As far as I know, vape liquids still contain nicotine, which is very addictive. Vaping is also not recommended as current studies are showing that it makes you more vulnerable to lung infections, including Covid. Perhaps getting the most out of your expensive transplant is a motivation for you to quit completely. I hope this helps you.

    Amazing doctor! what about second hand smoke? I have done hair transplant 20 days before. I go to nightclubs on weekends with friends but no alcohol :) I am worried if the smoke in nightclubs (3 hours on Friday and Saturday) affect my hair grafts and results. Please let me know.

    Hi Jsksks Jdjdjd, While quitting smoking can help with your transplant results and healing, there are other factors for a successful hair transplant. You shouldn't be under age 30 if you're getting a transplant, otherwise you are losing hair faster than it can be transplanted, and coverage won't look good as you lose native hair rapidly. How your transplant surgery is performed, and treatment after your transplant for progressive hair thinning are also important.     Sir i have a question regarding hair thinning and severe hair fall. I have recently experiencing it since 2020. I have visited severely dermatologist and nothing helped. Lastly i was asked for mesotherapy and done one sitting and left as it was quite painful. Will mesotherapy really help me regain my all lost hair back?     Hi Anushka Kundu, Mesotherapy is not practiced in the US. Mesotherapy usually consists of vitamin treatments, but genetic pattern hair loss is caused by genetics, not by nutrition, so mesotherapy does not address the genetic causes of pattern hair loss.     Hi Arman Patel, There is no set time for hair shedding after a transplant as does depend on many variables, such as occurrence of shock loss, collateral loss where native hairs are damaged during transplant, where you are in your hair growth/shedding cycle, transplant technique, rate of graft survival, etc. For any concerns, I recommend you follow up with your transplant surgeon as they know the details of your transplant. Kind of you to help Please give the guidance on Shampooing after transplant I am 2 months in after surgery and currently using Johnson baby shampoo indirectly--as in making a foam of it in a mug first and then applying it on the scalp Is it the right time now to just shampoo my hair directly as it is done normally without worrying about the foam.     Hi @connorapple006, I do have my hair transplant patients use baby shampoo during the early stages after a hair transplant, using a cup or a mug. If you want to shampoo your hair directly, or change shampoos, you should really be discussing this with your hair transplant surgeon as they know details of your hair transplant surgery, and your scalp.     Hi Panchal jeet, E-cigarettes or vaping is still smoking, and it still introduces toxic chemicals to your body, so as a doctor, I still don't recommend you do it.     Hi doc, I just received fue hair transplant 5000gfs 15days later grafts looking good,. But I can not help myself from not smoking shisha before bed time. I was a heavy cigarette smoker I quite cigarettes in 2017 by starting shisha as alternative, I'm now nervous will I have healthy hair regrowth I have had PRP and I'm currently on course of hair growth pills vitamin D3 biotin and will start kirkland minoxidil 5% drops next week. Also accidentally I applied pure Amla fruit oil my doctor told me not apply any sort of oil now. I have not told him that I'm smoking shisha.. Is shisha going to harm my grafts

    Hi Charming007, Shisha still contains tobacco, and nicotine, and it has been linked to similar health problems as cigarettes, so I don't recommend you do it. You should be asking your surgeon about finasteride to slow progressive hair thinning as it is more effective in treating progress hair loss than the other treatments you mention.     Hi Suzaul Karim, Tobacco does cause free radicals in your system, so it is always good to stop the habit. It may interfere with your graft healing, so perhaps now is a good motivation to quit tobacco entirely.

    Hi Irresistible Libra, It's still early after your transplant, but you can start scrubbing your scalp this week, upon advice of your surgeon. Scrubbing can help with the itch and with dead skin on the scalp. I hope this helps you.

    Hi Mikey Mulla, Cannabis is shown to have an effect on the metabolization of anesthetic and sedation. However, how smoke it, say vaping, may have less of an effect on hair grafts. You basically want to avoid the carbon monoxide getting to your hair grafts. There isn't too much information available yet on cannabis and the healing of hair grafts, but cigarette smoke is certainly toxic, and can affect healing. I hope this helps you.

    Hi Susheel Sharma, You can have your hair cut when your transplant has healed, and it's easy to tell you have healed as you get growth from your hair grafts. You don't have to keep your hair long, depending on how your transplant surgery was done as short hair may potentially reveal donor area scarring. I suggest you follow up with your transplant surgeon as they can physically examine the problem, and also know the details about how your transplant was done, and in what areas of the scalp.     Hi momo momo, It won't lead to a failed transplant, but smoking will affect blood supply, and introduce free radicals which won't help with healing. People quit smoking all the time, so it's not impossible to do. I hope this helps you.

    @@AmiyaPrasadMD another question doctor , I saw your video about types of shedding but I want to know in details because I experience hair breakage from the first week to one month after the beard transplant, almost all my hair are broken i can see the root so i'm sure it s not shedding. Is that normal ? does it affect the growth of the hair ? And also I see the roots so is this normal that after a month I experience only breakage and not shedding ? I'm worried that smoking has an effect why my hair is not shedding but breaking . Thank you in advance doctor

    Hi momo momo, When I describe shedding in the video, it is for scalp hair, which behaved differently than body or beard hair. If you have concerns about your beard transplant, you should address them to your transplant surgeon as I do not perform beard transplants, so my experience is limited in this area.     HI Doc, I have dandruff condition. Will this hurt the regrowth of hair after shedding phase? in your opinion is it ok to return to anti dandruff shampoo 1 month after hair transplant? Tx

    Hi sharan chugani, Dandruff is a mild form of seborrheic dermatitis, and does not affect hair growth. It is usually OK to continue with dandruff shampoo one month after a transplant when scabs are already healed, but it is best to first consult with your transplant surgeon as they know your scalp, details of your surgery, and where you are in the healing process.     Hi Jayesh .shanbhag, Smoking can affect your healing from the transplant, which does affect results, but it won't lead to complete failure of your transplant surgery. If you are really worried about the effects of smoking on your transplant and your general health, you can just stop. It may not be easy to stop smoking, but it is certainly possible.     Hi Dr. Prasad, I am turning 29 and did a transplant (FUE) for 3000 grafts in two sittings (6 weeks & 4 weeks ago). I started smoking 2 weeks after the latest transplant. I smoke IQOS (Heated tobacco), ~10 a day. Is this going to affect my transplantation? Also, what is the effect of Marijuana if I am smoking without tobacco (e.g., green tea). Will this have an adverse effect?

    Hi Rajeevan Rajendran, You are still smoking tobacco, which is still toxic to your system, so the effects are explained in detail in the video.     Hi@@rajeevanrajendran1077, You're welcome. If by regenerative you mean PRP alone, that will not be enough. You will need a DHT-blocker like finasteride to slow thinning, and a treatment better than PRP alone.     Man then only why m I suffering from hairloss due to smoking? I m a smoking since last 10 years and so are my close friends but their hairs are getting thicker day by day I have 26 male friends who are heavy chain smokers smoking since many years and their hairlines and overall hairs are intact and in fact their hairs are getting stronger day by day.Then why only me?😭😭😭😭

    Hi Jeet, While smoking is bad for your health in many ways, it's not the cause of hair loss. Pattern hair loss is a genetic condition, not one caused by external factors.     Hi@@VikramjitSingh924, Smoking does compromise your oxygen levels, does constrict your blood vessels, and ruins your health in countless ways, but it's not going to cause genetic pattern hair loss. You should quit smoking for many health reasons other than your hair.

    Hi Ramvath Kumar Kumar, It's important to have the cause of your hair loss first diagnosed by a doctor. If you have male pattern hair loss starting in your 20s, you are likely highly DHT-sensitive, and need a DHT-blocker to control it, and slow hair thinning. The Hair Regeneration treatment can thicken thinning hair, and stimulate growth from dormant hair follicles, but it cannot block DHT-formation. Hair transplants are generally not recommended for someone as young as you because you will lose hair faster than it can be transplanted. Expectations of hair density in people as young as you are also higher, and cannot be achieved by even multiple hair transplants. I advise you do not get a hair transplant, but consider stabilizing your hair thinning with finasteride to slow it down, and Hair Regeneration to thicken thinning hair, and stimulate growth of dormant hair follicles.

    Hi jagesh singh, Kergaglo and follihair are both supplements, mainly biotin. You can talk to your doctor about it since they prescribed it, but they look similar to me, so I'm unsure about why it was switched. I don't normally prescribe supplements to my hair loss patients. You should continue with finasteride daily to control DHT. I hope this clarifies things for you.

    I've on 7th month after FUE and do not like the results and wanted to truly understand the impact of smoking after the first month as most surgeons tend to emphasize the first month mostly. This sounded very generic and politically correct to me, not really what i came to hear. But thanks for reinforcing the benefits of finasteride. I started taking it about a month ago and home to see some improvements when I reach my 10 months mark.

    Hi Mohib Siddiqui, There is a difference in hair shedding, and hair thinning, with progressive hair thinning being the real sign of pattern hair loss. Shed hair does grow back, but the problems associated with pattern hair loss are when it grows back thinner, and there is delay in regrowth. That being said, hair transplants do not treat progressive hair thinning at all, and only transfer hair from the back of the head, to the scalp. You need to have additional treatment like finasteride to slow progressive hair thinning, and possibly a treatment like TrichoStem Hair Regeneration to thicken thinning hair, and stimulate hair growth from dormant but still viable hair follicles.     Hi@@MrRocky6303, The chances of side effects from finasteride are pretty rare at about 2% of users having them. I've had patients who were on finasteride for 20 years who didn't suffer from side effects. There are more common causes of erectile dysfunction, which also affect men who are not on finasteride, including stress, excessive alcohol intake, smoking, lack of sleep, lack of exercise, an unhealthy diet, and aging. Often, men on finasteride overlook their lifestyle choices if they encounter problems, but a healthier lifestyle needs to be considered as well. I hope this clarifies things for you.

    Hi kunji meena, Pattern hair loss is a genetic condition hardwired into the DNA, so it can’t be completely cured or stopped.     Hi Chantel H, While it is difficult for many people to quit smoking since the body can be physically addicted to nicotine, people quit smoking permanently all the time.     That's really great to hear,@@chantelh532! Although I've never been a smoker, one of my staff told me he quit by resisting each craving one a time, and he's been a healthy, non-smoker for over 15 years!
