Hair Transplantation and Diffuse hair loss of DUPA & DPA | Darling Buds Clinic in India

Hair transplantation for diffuse hair loss is always not possible and especially in DUPA (diffuse unpatterned alopecia). To know moe we need to know about the Grades of Baldness on Norwood Scale and perhaps also the Ludwig scale for hair loss in women.
Androgenetic alopecia (also referred to as male pattern baldness or female pattern baldness) is characterized by a shortened anagen phase and hair follicle miniaturization.
Diagnosing and differentiating DUPA and DPA early is important since-
1. They both respond well to medication.
2. DPA patients make for good hair transplant candidates. DUPA patients almost never do.
So let us know more about them:
DPA
It is an androgenetic alopecia manifested as diffuse thinning in front, top and crown with a stable permanent zone at the back and sides of the head. The entire top of the head thins (miniaturizes) out without passing through the typical Norwood stages. It never progresses to complete baldness. The hairline does not recede significantly. The stable permanent zone differentiates it from DUPA.
It is seen very occasionally in females since the cause is the male sex hormone and the genetic make-up of women makes them mostly resistant to the damaging effects of DHT on hair follicles.
Diffuse patterned hair loss (DPA) is found more in men than diffuse unpatterned hair loss (DUPA). As the hair appears thinner at the side and back it differs from male pattern baldness but the initial cause is the same, genetic hair loss.
DUPA
This is also androgenetic alopecia but lacks a stable permanent zone. It is faster in progress than DPA and is found more commonly in women. It ends up in a horse-shoe pattern resembling NW VII but unlike it the horse-shoe pattern looks almost transparent due to low density of hair at the back and sides of the head.
DUPA normally advances quicker than DPA resulting eventually in horse-shoe shaped baldness on the top with thinning hair around the sides and back.
Treatment:
1. Hair Loss due to DPA and DUPA can start as early as the late teens in men but if caught in the early stages there is a higher success rate of being able to stop the process through medication.
2. DPA in many patients is amenable to hair transplant surgery.

    What do you think about aspirin against dupa? since finasteride seems to be useless because it's not related to dht.

    never tried Asprin for hair loss. Nor will try any such medication since people have very small windows in which we have to work and they do not have the patience for long term use. So only tried and tested medications which I have personal experience with over many years are what I wil continue to suggest. Treatments come and go but only Finastreide and Minoxidil have been steadfats in their utility, efficacy and patient tolerance and affordability.

    Do you think the efficacy of generic finasteride is lower than Propecia? I am using finax which doesn't seem to help. I have DPA (or DUPA, time will tell). Do you think I should increase the dosage to 1.25mg if 1mg of generic is not working?

    Propecia is standardised and a good product with the best salt. Generic Finasteride if taken from a reliable manufacturer would be equally effective.

    Does the oral minox work well for a dupa? Do you have positive feedback on the oral minox? because finasteride does not work on the dupa. A frequency of 2 mg every 2-3 days is good for the oral minox?

    Finasteride has been working well for mu cases for DUPA. OM can be added for better results. OM not being a DHT blocker ill have its drawbacks since will not prevent progression of baldness.

    Hello Dr. Does finasteride for DUPA work better than dutasteride. I have DUPA and dutasteride almost seems like I’m using no DHT blocker, I am getting extensive miniaturization everywhere.

    The salt is the same and hence the effect will also be the same. They are packaged and sold separately for ease of dispensing since Proscar is for Prostate and it needs 5 mg dosage.

    Does It mean that if treated in time then diffuse type is much easier to treat than the typical ? Does it also mean that diffuse thinners respond well even to micro dosing and can be on finestrde for long long time without losing much efficacy?

    Hello good doctor. I have DPA, started medication with an advanced progression of hairloss, but luckily responded quite well. Now i have a good hairline and some good hair on top that allows me to comb to hide hairloss with some volume. I am considering a HT, but i fear a HT can damage existing hairs, since I have no slick bald areas. All new grafts need to be planted to areas where there is hair already. What do you recommend in my case? Much appreciated. 🙏🏻

    Sir what is the right time !? I mean after how many months it will be difficult to manage dupa !? I started minoxidil after 7 months , am i in the safe time zone? And sir I am only 14 years old so my doctor thought it would be telogen. Effluvium so I haven't used minoxidil or any other medications but I have used dermaroller , supplements and a healthy diet with vitamins ... Sir plz tell me am I too late? It's been and 7 months I am having hairloss but haven't started minoxidil ! Is it too late?

    Live a healthy lifesyle and consider going on a DHT blocker at age 21. A hair transplant can be considered a few years later after evaluaion of grade of baldness and level of DPA that you seem to be having..

    As long as you value hair!!!! In genetic baldness there is no cure. The problem exists and if you stop medicines the thinning recurs.

    Diffuse pattern has hair all over without any clear cut baldness. The thinning also exists in the scalp 'permanent donor' area.

    DUPA is connecetd to male hormones too. Yes Dutassteride can be added after a doctor's in depth consultation.

    Hey I am not sure if I have retrograde alopecia, it is very hard to tell...and I am using topical finasteride for 12 months and it helped me so much with my diffuse thinning on top. Do I need to change to oral version to try to stop potential retrograde alopecia or topical finasteride still goes systhemic and its basicallythe same effect as oral? Cuz I am putting topica fin just on top od head...not nape and neck

    Retrograde alopecia can be retarded with Minoxidil and oral minoxidil as well. Reportedly the latter is more effective but carries some downsides- cardiovascular effects. The need to take the medication on prescription is mandatory.

    Hello dr. i started loosing hair ao i started taking minoxidil for almost a year and a half and added micro needling and fenasteride 0.75 3 days a week like a year ago i had amazing results almost restored all my hair back,i started working out at gym without adding any supplements or hormones and everything changed started noticing massive shedding and miniaturization and weak hair and lost base so i stoped working out im still on the same regimen but added dutasteride so 2 days fenasteride and 1 day dutasteride i guess its telogen effluvium because of working hard and i had corona so 2 3 days of fever so what you think i lost some base any help can i restore back what i did with medications i had before?

    Hi Dr, Great Video!!!. I have DPA starting since 16 years old and am now 35. I still have a good hair line. I have hair on top, which is thin and my crown which is very thin almost not there. Treatments I started beginning of this year are Minoxidle/ Fin spray, but switched to just Minoxidil Oral tablet 3 months ago. I also started PRP sessions. Have I started treatments too late? Should I add Finasteride again? If I continue with my treatments, should I expect good results? Thank you Dr.

    Not really in my experience. We use it for graft storage before they're moved into the slits. Graft survival and handling is better.

    Exercise caution with these drugs. You need to see a doctor and get your cardiovascular system examined before going on oral Minoxidil. Minoxidil is not a DHT blocker and hence though the condition of hair will improve, progression of baldness will not be affected in any way.

    Excellent video, I have a question though, If the donor area in DUPA is treated with Minoxidil, and hair restored , would a hair trabsplantation would make sense ? Another question, could someone suffer from both DUPA and DAP on the same time, I have had a regression of my hair line since my twenties, but my dobor area was great till since 2 years I starred to notice thinning in my back of scalp ( am 31 )

    There is no point doing a procedure on a DUPA patient without Finasteride. Regression of hairline is genetic and not related to balding. As you advance in age, the ghairline resets itself. No, DUPAand DPA do not co-exist.

    If they are intact or remain intact after stopping Finasteride, he never had DPA. If DPA donor area is improved with Finasteride, it will soon deteriorate if the medication is stopped. It is common sense.

    I am also diffuse thinner and have had heavy dandruff which caused hair fall but I detected it early the doctor gave me minoxidil + finasteride + keto shampoo 1.5 years passed and i have maintained good head of hair hope so the medication sustain the results in long run and I don’t know whether i am dpa or dupa

    hello doctor i am in very early stages of dupa,my hairline is still norwood 1 but my hair is slowly becoming more thin on the back,sides and even top,i recently started taking 1 mg of oral dutasteride and also 5 mg of oral minoxidil i belive they are the two best things for dupa but is there anything else i can do to make sure my hair does not continue to miniturize?

    Very difficult to differentiate the two. DUPA responds well to Finasteride.

    @@virendrapandey2282 Hair thinning nd miniaturisation is the same process. Chronic telogen efflvium can stop but hair that are lost in the background of androgenetic alopeca wll not return.

    Hello doctor! I have dupa and i took finasteride for a year and 2months(didn't have great noticable results). And I my doctor told to stop fin for 3 months and prescribed a (redensyl procapyl and caffiene) serum for 3 months... I am losing more hair that when I was on finasteride i belive doctor and if I'm not wrong I won't get same results if i start fin again. What should I do doc? Prp? Gfc? Or fin again?

    You will need to continue treatment with the same doctor. I don't know anything about your case. Other treatments mentioned may not be effective when Finasteride has failed.

    Finasteride does help in androenetic alopecia in women. However, it is best taken in the post non-child bearing age group.

    If you are eligible for a hair transplant, it will be done after 3 months on Finasteride. Very little chance of damage in this scenario.

    I had a severe sun burned on my scalp 14 months ago it was red , and with water bubbles ,i was 8 days on a cruise. The next month or so after the event my hair started thinning and shedding quite fast .before the sunburned i had no thinning or shedding at all . Shedding lasted 13 months it now stopped but hair is still thinned on top .can I recover from it fully ?

    Finasteride and dutasteride really work on DUPA ? It's not related to DHT.

    They work on DUPA quite well. In fact the only thing that works in most cases. DUPA is androgenetic alopecia, hence DHT modulated.

    Hello Dr. because of my hairloss I started taking oral finasteride 1.25mg/topical minoxidil 1ml daily. I started having great results for up to 5-6 months, since then I have started losing a lot of hair again. It has been about 1 year since I started. I sent photos to a hair transplant clinic and they said I have DUPA. They said taking brandname finasteride (Proscar) may help instead of generic finasteride... Do you think it is true? And is there anything else I can do?

    Dr,I have receding hairline .so, I used minoxidil for 4 months and then I started finasteride .so,as soon as I started finasteride I was loosing my hairs all over the even from sides and back .I stopped it .it's been two months I stopped it but still I'm loosing hair from sides and donor but it is little bit slower down now.so,it means I have DUPA sir Please reply

    in dpa is same as male pattern baldness i means in male pattern baldness reach grade7 its last stage in dpa diffuse thining also progress up to grade7 area and donar area also thin in dpa means in dpa hair transplant not possible if done hair transplant in.dpa dht blocker are need to take life time

    DPA is akin to AA. Only thing is the hair are miniaturised and majority will not withstand relocation through hair transplant. However, there are cases who have done well with hair transplant but then they are committed to taking Finasteride for long periods of time.

    They can have any grade of baldness, but they definitely do not keep grade I for life.

    My hair transplant surgeon canceled my hair transplant surgery today. He looked at my photos I had sent him and said my hair is to thin on the back of my head. He said it looks like I have DUPA. He said if he transplants the hair from the back of my head to my thinning temples that the transplanted hair might just fall out. I have been on 1 mg of Finasteride/day for 7 months now and it has not helped my hair at all.

    Retrograde alopecia is a terminology restricted to the back of the head, which decreases the scalp donor and is an indication of extensive baldness.

    My father had DUPA I suspect. His hair loss continued to his eyebrows, chest and arms in his late 70’s where all of that hair was totally gone. He had no eyebrows left, no arm and chest hair left. It was bizarre. He resembled a chemo patient. He never got diagnosed. I fear his unusual DNA inspired loss will be my burden. I’m a NW 5-6 at 59. My hair loss began significantly at 50. My donor is decent but will visit a HT specialist to get opinion.

    Hello Doctor! I've been on 1mg fin for almost a year now. I'm 21 and have DUPA and I still loose a lot of hair(about 120 just by agitating for few mins). Hearing this my doctor prescribed 2mg of fin and a hair serum (with some saw palmetto and peptides - Logihair Hair Serum). But I don't that is doing any good either, my hair is below baseline. How do I recover my hair at least to some extent? Should I ask my doctor for dutasteride? How will DUPA hairloss respond to Minox, PRP or GFC?

    Please do not increase dosage. Watch: kzread.info/dash/bejne/iaWltMmnpqbedag.html DUPA will only respond to a DHT Blocker. Yes you can in consultation with your doctor initiate once a week Dutasteride as well in place of Finasteride for that day.

    @@hairtransplantinindia So doctor since dupa will respond only to dht blockers, will the logihair serum containing saw palmetto (a natural dht blocker i believe) or topical finasteride work? or should I ask my doctor for dutasteride itself ?

    Sir I'm suffering severe hairfall suddenly since 4 months around 200-250 hair a day my hairline isn't receding but my top scalpe started visible I'm soo much worried is it DPA or telogen effluvium. I'm 23yrs male

    Yes it could be Telogen effluvium if you are undergoing stress. Definitely not DPA. See a doctor and if it is androgenetic alopecia you will ned to start medication.

    @@hairtransplantinindia i have done trichoscan and the results are Telogen hair 77% anagen hair 23% Vellus 17% terminal 83% Is this telogen effluvium or androgenic alopecia? i will b really thankful if u reply Dr

    @@hairtransplantinindia so if a person is just started to DPA then does he needs to hold the future hair loss with medications OR he should let the hair loss with time and have hair transplant after full thinning ( eligible for transplant ) and proper time ? What should a good choice ? Whether before and after transplant he has to be on medication , hasn't he? So does it helpful that he should start medication in early stage rather than let it happen ( further hair loss) . So even though transplant is way ( final ) for DPA but not a permanent solution ( cause a person has be on medication even after transplant) . So if he stops medications after transplant hair might be loss further , isn't it?

    Only Finasteride 1 mg on alternate days will be of any benefit.

    @@hairtransplantinindia sir my Tests level is incredibly low. I can't take Finasteride. Another alternative for Dpa. Along with that my vit d level is low(17), iron (85) and have hypogonadism. Which problem should I address first?

    @@tapasmajumder9302 In that case you should not go for Fiansteride. You are also not a candidate for a hair transplant. You should go for a hair piece, shave your head or keep head trimmed and get SMP scalp micropigmentation done. Vitamin and iron supplementation will bring your other low parameters back in 3 months. For low testosterone please start TRT i consultation with a doctor. Do you have erectile dysfunction?

    no sir. I haven't felt that. I am 24 year old. I have a very patchy beard. But I haven't faced any problem with urges, erection, morning erection. But for a couple of years, I have been facing gynecomastia problem( that's probably for high estrogen but didn't check estradiol level) And throughout the day I feel sleepy and don't get enough sleep at night. And recently have checked for thyroid (TSH -5.82) may be the hypothyroidism also. Don't know what to do? I think there is no way no curb my hair loss with so many complications.

    Some patients have had good things to say about it. However its use is not established. Why would you like to spend money on something which has little supportive data and clinical experience to back it.

    @@hairtransplantinindia i live in Nepal and here Finasteride 1mg is not available so i took dutasteride 0.5mg. Only 5mg fin is found here. So do you think dutasteride and minoxidil combination will help?

    Products come and go. I have not heard of this. A product that can withstand the crutiny of time is a good product. Rest are all commercial itinerant profit-making concoctions. A hair loss sufferer lives on hope.

    Usually DUPA does not allow a hair transplant. But Finasteride will help.

    @@hairtransplantinindia sir I have been having finasteride and applying minoxidil from 2014. My hair loss is stable for 5 years now. I wanted to increase my density so that it would match my native density. Medication are not increasing the density anymore.

    In advanced cases of DUPA we feel it is not working. If there is no miracle you are expecting, try stopping for a month and see whre your hair loss goes.
