Beard grafts depend upon the available density. Fromthe shadow area and anterior neck in the average patient, upto 1000 can be harvested. No, my practice of BHT esp. of the beard has remained the same. if you mean the whole body as a donor area, I still do it, but in the most desperate situations only. Not otherwise.

what do you consider the optimum ratio between beard hair grafts and scalp hair grafts when doing a treatment for a patient with advanced hair loss? (NW5, NW6, NW7)

I would say 1:4 is the best but when the baldness is extensive and the only resource available is body hair, I have done 1:3. 1:2 is done only if the patient is wanting to keep hair short and supplement with adjunctive scalp micropigmentation.

#AskDrBhatti Since finasteride is a DHT blocker and beard and body hair grafts grow under the influence of DHT. Is there any point in taking finasteride post op for BHT patients?

No one does only BHT. That will look weird! But for academic reasons, body hair do not need Finasteride.

Body grafts are taken from the beard and chest. These grow reliably. Others do not.

Do you plan to make a video about microneedling/dermarolling (either for better absorption or stimulating growth) ? What's your opinion on prostaglandin analogues like latanoprost or bimatoprost for hair growth or topical antiandrogens like RU58841 or CB 03-01 more known as clascoterone (Breezula) ? I'm currently using 0.5mg per day dutasteride (Avodart),1x day 5% minoxidil+ 0.015% tretinoin,2-3x a week 2% ketoconazole shampoo and other days Neutrogena T-Gel or other shampoos with selenium sulfide or zinc pyrithione,also using dermapen 1x week at 1.5mm and a couple of days with 1mm gently to increase topical absorption.After a year results are great and still improving !

Good to know your regimen. I use lanatoprost only for the eyebrows and the effect is encouraging and consistent always.

We do not treat the back of the head if you have a hair transplant in mind and plan to use these hairs after treating retrograde alopecia.

#AskDrBhatti Hello Dr. Thank you for another excellent video with information I have not seen anywhere else. I have a few questions: I am taking oral minoxidil 2.5 mg every day and it is working well. I have noticed the common side effect of an increase in body hair. For some this is undesirable however in my case I have mostly body hair transplanted on my head from all over my body so I think this is helping my scalp density in unique ways. Some people have suggested I use finasteride too however since my scalp is covered mostly in body hair which seems to thrive on DHT I am worried finasteride would actually make my scalp hair which is mostly body hair thinner and that oral minoxidil is actually the best drug for me? Do you have ay opinion on taking oral minoxidil vs finasteride for those who have had extensive BHT to scalp hair transplantation? You mentioned scalp, beard, chest are the ideal donor sources. However when it comes to the hairline would nape, abdomen and even leg hair be good candidates so that the hairline has a soft and natural appearance?

Very good question : Body hairs are robust grafts. Though they thrive on DHT, Finasteride does not seem to hinder their growth. We do BHT all the time and have not seen them doing better in patients not on Finasteride. You can continue Finasteride in tandem with OM without any problem in my opinion. Nape of neck hairs in most people are temporary. They are not DHT resistant. We routinely did this in the initial years of our FUE practice but when the leading edge of the hairline started to vanish in a few years, we gave it up completely. Abdomen and leg hairs have a long cycle and may take 2 years to grow. Most do not have harvestable body hairs other than in the beard and chest. The best known way to have a natural hairline today is placement of singles along the leading 2 rows of the hairline- grafts meticulously dissected under magnification to remove telogen hairs hiding within the grafts.

What about thick shoulder and back hair for head transplant vs chest hair?

Loving your videos. I have a question, say if i dont have much body hair but have a decent amount of hair under the jaw area. Can multiple procedures be done? Like the grafts being taken out, transplanted in, then wait until new hairs grow out of under the jaw area, and transplant them again? If this makes sense?

I cannot comment without pictures. But theoretically if body hairs are available in abundance alongwith grafts from the permanent scalp donor zone, a full head of hair for all times can usually be assured in grades 5 & 6.

So if chest hair and leg hair is thin, can the procedure be done only with hair from only the neck area? Of course it is not enough hair, but what if the procedure is done more than once? Let neck hair regrow and transplant them again? Can this be done?

@@KudoWoo Yes, but only if you intend to keep hair short; or else the characteristics of body hair will show up. We usually plant body hair mixed with scalp hair in a particular ratio s it loses its unique characteristics.

This is not a permanent zone since with age these hairs fall off. We earlier took nape of neck hairs for softening the hairline but experience that they surely and slowly fall off.

We do this almost every day and they do grow long. We do not transfer hairs into depleted donor areas since in baldness progression will always happen and these precious grafts should not be wasted.

No this is not done. You should keep hair shaved close or get scalp micropigmentation done.

It is very rare that hyperpigmentation will happen n the Caucasian skin after harvesting grafts. Abdomen and armpit hairs will be useful if they are of adequate thickness. Fine hair are of no use.

Yes theoretically it is, but what purpose will it serve? Will you not need more grafts for the baldness as it progresses? You can try SMP.

This is the theory of donor dominance which has been widely discussed. But through clinical experience I can tell you they retain mostly the original characteristics hence should be mixed with scalp grafts in the proportion 1:4 to camouflage the unique characteristics of Body hair. Also thick beard grafts should not be used for the crown.

Thanks for watching. Yes over time hairs transplanted resemble those of the scalp as far as morphological characteristics go. Anagen phase and proclivity to susceptibility to DHT go unaltered.

​@@tastybrownie7448 They in fact take 1-2 years to grow fully. Other hairs do not show reliable regrowth after relocation and hence are never used.

Why not doctors are thinking on buzz look ( one cut on scalp top) and zero cut on side and back of scalp. Top scalp will be covered with side and back scalp hair through transplant and then doing smp for fuller look. Side and back scalp will be covered with SMP. This technique should be in case of high grade baldness.

The life cycle of hairs is composed of 3 phases- anagen, catagen and telogen. Hairs will fall off but the roots will not- they will keep rowing new hairs. Short anagen phase means that the hair is seen for a shorter time.

Yes it grows longer than scalp hair when taken from the beard. Chest hair grows 3-4 inches maximum.

An average beard gives 600-800. A lush beard gives 1500 Seborrheic dermatitis can undergo treatment when the condition is quiscent consequent to treatment, not otherwise.

Sir ji , you are saying that Donor Dominance Theory is Not True , as the Grafts Taken from the Beard area (Anagen grafts) Do acquire the Character of the Recipient Area over time and loose their Old Character as we change their Location . So in that case it will be safe to assume that Even the Anagen Grafts removed from the Donor area of the Head and transplanted to the Recipient Bald Area in the front would get the same characteristics of the Recipient area AND in such become Susceptible To " DHT " and fall out over time as the Recipient Area would change the Anti DHT Character of the donor hair as the Location Now stands Changed ? So Recipient Dominance holds more substance . Sir ji as you are vastly renowned and respected in the Hair Transplant Industry , who better than you , could Help us understand this Better . They are anagen selected hair. Just like anagen selected hair from scalp are permanent grafts and don't fall out. Scalp grafts improperly selected or transplanted at a young age when scalp permanence is not established, only fall off.

Problem is they have a long hair cycle. We will not know if they worked or not till then. I have done a lot of limb hairs in 2011 and 2012. Not encouraging.
