I made a mask with your template, it's great! A word to the wise: make sure you take into account the bulkiness of the mask when you make the hat. It fit great on my head, but not with my mask on too

Go to google/firefox/safari whatever you use and look up. 'flared crown hat'

What are the dimensions of the foam sheets - 9"x12 or 12"x18"? Also, do you think 3mm would work?

12 X 18. 3 mm may work, I think it would hold it's shape, or you could layer it for the brim.
