I am a certified lymphedema specialist, the behavioral changes required to address lipo-lymphedema are very challenging for all my patients. A lot of people at that weight or much lower end up with significant and life threatening wounds on their legs that require months to treat if not years. Being consistent is the key to success. Glad she seems to be doing well.

Most of the patients on that show seem to have some form of lipedema, which the surgeon either doesn't understand or ignores. Legs like that are not a result of overeating. They're half lymph and scar tissue.

Honestly, i don't think we will make much progress on the physical effects of obesity until we finally have better approaches to the underlying psychological causes. Psychiatry has been terrible at addressing food behaviors unless it's bulemia or anorexia. It's also just not terribly good at etiology to begin with. Dieticians are completely ineffective in addressing the fundamental behavioral aspects of dysfunctional eating, except in a small patient population who are lucky enough to respond well to their nutrition-focused approach. Bariatric interventions have been a good advance but it's horrible to have to wait until things get so bad that surgery is necessary.

I got the gastric sleeve surgery in 2021. I lost 105lbs in one year and reached my ultimate goal of getting pregnant. It changed my entire life and I am so thankful for my gastric doctor for giving me the surgery and the opportunity to lose weight. Carrying so much excessive weight is so hard on the body. I struggled with breathing, going up the stairs, infertility, acne, varicose veins in my legs, and clothes not fitting me. Now, at 9 months pregnant, I am still at a healthy weight, I am more active and living a much better life. I am just so thankful and happy that I made the decision of surgery. It changed everything.

He reached about 45, and his knee started hurting, and he got so depressed and always acted like his life was basically over because he always hurt. Then he went through a bunch of stuff in his life and lost a crud ton of weight. his knee stopped hurting, and he feels so much better and younger even though he's about to be 50 soon. I honestly had nightmares when i was a kid that he was going to die young.

@thatgirlwiththegreenjacket i agree and would add that it would still be cheaper for the state to fund this than to deal with all the conséquences of obesity. It has been proven many times and the fact that they do not take action is definitely a choice. Funny how capitalist logics aren't even the more efficient economically. It happens way more often than people think (with privatising public services for example or actually supporting dynamics that make middle and lower class people poorer and poorer when they 're the basic ground of our economies and if they can't buy most things anymore so one would be there to buy them and huge parts of the economy will collapse... Talk about inflation, crazy rents... All of those are political choices (to do nothing about it/have laughs that allow/encourage it)

The mental toll and recovery you have when you get gastro surgery is next level. It forces you to confront every anxiety you have towards food. For me, I had a hatred for food and, if I could, I never wanted to eat again. It made me realize how my body is in need of nutrients and that's how I have to look at my food and my body. Gastro surgery has a terrible rep and it deserves a lot more credit then it is given!!

I just had the gastric bypass surgery last month and i dont know how she isnt more emotional lol. Its been very emotional for me, from having imposter syndrome over being able to even get it done (i grew up in poverty) to dealing with the pre-op diet, the surgery itself (its the first surgery ive ever had) to the liquid diet etc its a lot. But totally worth it. If youve been trying to lose weight for a while and its not working see if you can get it done. And dont let anyone talk you out of it. Ive had to pull several family and friends up because they tried to gaslight me into thinking i dont need it and im just lazy. They dont know what ive been through. They havent seen me having breakdowns and contemplating suicide because i cant get control of my weight. I dont care if they think its the easy way out. I want to get healthy and live a good life and not feel like i cant go outside on days im feeling down. Its just an aid at the end of the day and its up to you to make sure you lose the weight and keep it off. Its not a guarantee. You will be assessed by your doctor, the surgeon, a dietitian and psychologist and they will determine if weight loss surgery is best for you. Its expensive, very expensive but join weight loss surgery facebook or discord groups and ask about it and the people in those communities will help you figure out your way around everything.

The people who get THIS overweight where they’re almost or completely bed bound and the enablers keep over-feeding them…I feel like it’s abusive. You’re literally helping them eat themselves to death. It’s a very slow self-ending. And if you help, you’re an accessory. I’m not talking about the people who can still walk around and do things for themselves, I’m talking about the ones who can’t live alone, walk, or sometimes, can’t leave the bed.

I hope she can get to a healthy weight and stay there. I know three people who had bariatric weight-loss surgeries. They each lost incredible amounts of weight in the two years following their surgeries. (One unfortunately lost most of the weight because she had some pretty serious complications that required several follow-up surgeries, and she couldn’t eat or didn’t feel like eating most of the time.) Sadly, in the years following all three of my friends’ weight losses, they’ve each gained back the majority of weight they lost. One started at about 450 lbs., lost about 260 lbs., but now weighs in the 370-380 range again, more than 10 years after her surgery. Ten years ago, when I’d eat a meal with her she ate very small portions. She had had gastric bypass surgery and had a golf ball sized pouch for a "stomach." Because she had lost much of the digestion and absorption of nutrients in the food she ate, she had to supplement these tiny portions with vitamin and mineral supplements at every meal. Now she eats about the same "normal" portion sizes as everyone else and she no longer needs the supplements. Her body seems to have adapted and has become capable of fully digesting her food again. That tiny golf ball stomach has stretched back to the size of a regular stomach. I guess I’m sharing this because these surgeries aren’t always the miracle cures they’re made out to be. They require far more work on the part of the patient than is even shown on these TV shows. It seems pretty easy for these people to fall back into old patterns and behaviors. Hopefully surgical techniques have advanced in the past decade so that patients can avoid some of these pitfalls.

Of course people lose weight on 800kcal. But we know that weight loss after such extreme restriction isn't long term. And most people regain beyond their previous highest weight after restricting so severely

I a m not nor have I ever been overweight or obese, I have congenital lymphedema tarda, and right on schedule, it jumped on me at 35. I'm now 43. I can hardly walk, plus I have developed cellulitis and huge ulcers on my lower limbs which haven't healed in 5 years. I haven't worn shoes in years. It's a disabling and disfiguring condition. I'm basically confined to my home , and wear long dresses or skirts to hide my barefeet when I have to leave my home to see my wound care doctor. I've become an agoraphobic shut in basically. There's no cure, and where I live there aren't any lymphedema doctors to get decongestant therapy. I hadn't a lymphapress pump that I use twice a day, but can't afford appropriate leg compression garments, so I have a home nurse helping me, , so I can keep my legs elevated, also my insurance only pays for physical therapy once a month for complete decongestion therapy. I'm usually hospitalized several times a year with either, staph, pseudomonas, and even gangrene and nearly lost my leg. I'm so depressed, I feel like this lady, like I have no hope anymore. I've had people stare, and even ask " omg.what's wrong with your legs and feet ?" It is so painful and isolating. I have a one year old grandson whom I haven't even babysat yet , because I fall so often. I'm at my wit's end with this horrible and cruel disease. My doctor says it has become fibrotic in my feet and ankles. I don't think I'll ever wear shoes again. Dr. Youn, thank you due this video, there is little information about it on the internet, except for basically what I already know. I can't decide which of my current emotional /mental anguish is the worst. The week long pity party staying in bed and staying all day for two weeks. The anger and rage I feel, my uselessness ro even take care of myself. Thank God I have a nurse who is here every day. All this extra fluid has caused severe urinary incontinence. I'm only 43, and I literally cry daily. I'm in near constant pain. Doctors here mostly dismiss me. I need help of some kind so badly. I'm bawling my eyes out as I type in this discussion. I feel so alone, with nobody to talk to who actually cares or understands . Dr. Youn or anyone knowing anything that can be done to help me, please share, because I g Have never felt so hopeless. I feel no joy or pleasure, or even interest in anything anymore. Lymphedema has stolen what should best years of my life. I never dreamed at 43 I'd be disabled , and unable to even drive to go to the store or anywhere else. I'm so so so angry, STILL. I was diagnosed January 2017. I can't seem to move past the depression or anger at st all. Everyday I pray for a miracle to take away this illness and all of this physical and emotional pain I feel. Sorry for typing y'all a novel, but I had to get some of this out, bcuz I feel /am keenly aware of my basketcase stream of consciousness mind. Peace😢 thanks for listening, or rather reading if u made it all the way through.

Maybe he is just a really good actor, lol, but the empathy and care in Dr Youn's eyes is so refreshing. This coming from someone who sees physicians all the time now and so often the attitude of doctors treating you is comparable to the illnesses causing me so much Hell and taking up much of my time I have left. Can't tell you the difference just a slight amount of care or compassion would make during a doctor's visit. I understand that docs must be honest and informative but often that doesn't even happen. So thanks Dr Youn for any time you have showed compassion to your patients. It's huge and so important.

What people don't realize is that people who go through emotional eating have a problem that is similar to excessive drinking or drug use. They are not lazy slobs surrounded by nothing but enablers. They are hurting

I have always been thin or had an “athletic type, rectangular body build”. I was 120 pounds at 5 ft 7 inches until I had my child in my mid 20s, and then I gained up to 134 or so and could never get much lower than that weight all through my 30s. In my 40s, I gained up to around 144 pounds which fluctuated from 143 to 148 in my late 40s and early 50s. I’m now in my mid 60s and have gained up to 160 pounds at 5 ft 6 inches ( I’ve lost height in old age although my bone density tests say I’m in the normal range. ) I try not to eat much, and I’ve done intermittent fasting. All my life, I’ve tried to eat healthy and stay active and fit. But as I’ve aged, it has become harder to continue the levels of exercise I used to do…aches and pains, also my asthma acts up, etc. I have joined a gym, but it’s not a very good gym..it’s 30 mins away, no pool, you have to ask for access to various machines, etc. ( I live in a rural area; you can walk in the spring and fall outside, but it is way too hot in summer and too cold in winter to walk outside [ my asthma acts up in hot weather and in extreme cold ]. Also, one important point id like to make is: this health and weight thing is a LIFELONG commitment, and so many things conspire to work against you as you try to stay healthy and fit ( not even taking in to account family, upbringing problems, emotional issues, money and anxiety problems ). We are bombarded with food…delicious food…on the streets, at ballgames, at fancy Sunday dinners, holidays, through advertising, etc. Hard to resist. Your health and fitness has to be almost like a religion to you or you’ll lose the lifelong struggle. And I know for a fact that some people are just born with genes that cause them to be bigger. I taught school for 35 years and kids are so different in their makeups…even kids from the same families. I’m not saying they should excuse themselves from eating well and excuse their “fat” away and just blame genetics, but some people will never be rail thin, no matter what they do. And when you’re an older woman, it’s just unbelievable how you can eat a salad without dressing and a handful of grapes..and NOTHING ELSE except water…for 24 hours and gain two pounds the next day. Our metabolism slow down so much as we females age that we have to starve ourselves to get back down to just what we weighed in our 30s. Why isn’t more research being done on this problem?

There's a strong link between avoidant personality traits and binge eating disorder. Eating is a form of escapism. It's no different than using ellicit drugs. It's a dopamine oxytocin serotonin high. Especially serotonin, as your gut is the second producer of serotonin in the body. Eating like that is more akin to heroin use, and stopping is just as hard, speaking from experience. I remember bonding with a patient suffering from heroine addiction and schizophrenia over the shared high we would get from our chosen drugs and how much it hurt and sucked to get off them. Food is a drug. I remember having dreams of eating so vivid I woke up and ran to the toilet to vomit. I swore I must have been eating in my sleep. It is fucking hard, because I spent all that time eating to run away from my abusive shitty life. And taking control of my weight and my food was the literal first step in reclaiming a sense of power in my own life and getting the help I needed.

I have Lipo-lymphedema. I gained quiet a bit at puberty, yet I was extremely active. I kept on saying to the Drs that there must be something wrong with me. So started my self hate and emotional eating journey. Once I knew that I had this condition I stopped hating myself and life coached myself into losing 30kgs over 3 years. I could only lose during the winter months because spring, Autumn and Summer I had to fight the swelling due to our hot weather. I started by first getting a body composition scan done, which helped me to see MY BODY facts and learn how much I needed to lose for my body. I was shocked that my ideal bodyweight was 81.5kgs and not the 60kgs that I was told by weight loss companies. Talking about them, I couldn't lose weight on them at all. I knew I was an emotional eater, due to the fat shaming I received thus making me turn on myself. I ended up with Depression, Anxiety Disorder as well as Social Anxiety Disorder. Talk about enablers, I would tell my partner to not bring home biscuits or chocolate etc, but he brought it home which made it available when I was anxious, I got into a viscous circle, eat, hate myself, eat etc. I find it hard that people even have a go at me for losing 30kgs over 3 years, but considering it took me longer to gain it, and I couldn't lose weight with help, I actually feel proud of my efforts. I had lost 35 but I gain 5 thru the stress of COVID. I have basically maintain my new weight for almost 2 years. This winter I am going back to the routines I had in place, the strategies I used before and I know I will lose my 5kg put on, which I have already lost 2, and my goal is to lose 5kgs, this will make my average of 10. If people were to butt out of my life and keep their hate speech to themselves then this will be easy. But the next time you think it is ok to speak hatefully to a person or fat shame, just remember that YOU can be an enabler of anxiety and them to emotionally eat.

That is also why they also ask for significant weight loss prior to surgery. In addition to reducing the risks and difficulty of the surgery, the weight loss is requested because if you can’t control your intake to some degree, you can definitely lose only a little, or even just continue to gain, after the surgery. Most doc will decline the surgery if you can’t lose, considering it unethical to have the risks of the surgery if the lack of weight loss shows the patient may not have any benefit (although varies case by case).

This sort of weight issue is so much more complex that we or the medical field can even fathom. How is it that some people are able to put on that much weight? For example, Dr. Youn, you could stress eat 24/7 and probably have great difficulty ever getting past 250 lbs. Whereas she might be needing to keep her eye on things to prevent getting to 800. She looks like she might also have lipedema - a disease discovered at the Mayo Clinic in 1949 and then largely forgotten by the US medical community but nevertheless widely studied and treated in Europe. Oh, and it affects 11% of women worldwide. Anyway, I really feel for this young woman.
