Does an AC Separation or a Separated Shoulder Require Surgery?

AC separations, also called a separated shoulder are fairly common shoulder injuries. If you landed on your shoulder and now you see a bump then you likely have an AC Separation. Many of these injuries can be managed without surgery. Some of these injuries are best managed with surgery. How do we tell which might lead to a more functional and less painful shoulder.

@@muhammadarifkhan6120After further investigation my injury was reclassified into grade 5. As the result after 2 weeks and 1 day following my injury I had a surgery done. I did it at my own expense as my insurance expired 1 month before that happened (talking about being lucky). I had a LARS (Corin) which is a French prosthetic ligament fixed on two titanium screws. In the US you have a similar surgery called a LockDown. You can also do a DogBone but only if it has cord, not a bolt. Now I am 23 days after the surgery and slowly recovering. Post surgery recovery is approximately twice slower. MOST IMPORTANT: Don't freak out (like I did) and try to ask your doctor for good painkillers - I had co-codamol (before surgery) and dihydrocodeine (after) ! )) This injury is treatable in the 21st century.

Now with all the knowledge and experience I would definitely support a decision for a surgery (mine was LARS). I removed them no problem.

Thanks for the great explanation. I separated my shoulder abut 18 months ago and it has not healed. There is still a big bump in certain positions. I have regained maybe 80% strength and most of the pain is gone. My local orthopaedic surgeon said they cannot make it any better. I am using KT tape to help stabilize the joint for everyday activities and that seems to help, but is not enough for heavy weights or gymnastics. Do you have any recommendations?

Doctor, thanks for discussing the stable versus unstable grade 3 AC separation. Complication rates appear high (30% or more) for grade 3 AC reconstruction, using the common surgical techniques. Is there any consensus about the success rate, using the double endobutton closed loop (Struhl) technique? Why do the western orthopedic journals avoid discussing this newer technique?

Went back in a couple weeks ago after doing some serious gardening lol. I’m worried because my ex rays def show a separated shoulder but my shoulder muscles really hurt and I do have bouts of numbness and tingling in my arm/hand. I’m regularly in CO but in NJ/NYC for the next few weeks so rescheduling with them is going to be useless as you can imagine. 6 months to get where I’ve gotten. I’m super frustrated and in tons of pain. Any thoughts?

Does the ac joint go back to how it was originally in such injuries or do the muscles compensate to give stability to that joint, in that case will it cause problems in the long term?

Can you form a type 2 or type 1 separation just by sleeping? I originally thought my shoulder was dislocated it in my sleep, but went to the ER and the doctor said it looks like my shoulder is in the socket, then he asked me if I was having pain at the front of my shoulder at the end. Which I thought was funny cause why yes that where most of my pain is. He said that he thinks he sees a slight separation of my “AC joint” which I had to look up I’ve never heard of it before. Anyways he did exactly what you said put me in a sling and I have to call the orthopedic surgeon tomorrow. I guess I’m asking because it sound like most people don’t get injured just by sleeping. I ask even more so because I can lift my arm any further then level with my shoulder, and pivoting my arm is hard it locks right up. Doesn’t hurt to move it’s just aches really.

A week ago, I fell on my head and shoulder during basketball, and dislocated my right collarbone. When I got up, my collarbone was sticking out almost comically, so I would say that was clearly a grade 5. However, later that evening I managed to position my body in a way that made the bone settle down, and look more like a 3, and it has been like that since. Does it make sense for an AC separation to "downgrade" from a 5 to a 3 by itself, or am I still looking at a 5 that "mimics" to be a 3? I know the doctor can't answer medical questions like that here, so perhaps other commenters have thoughts on this. In any event, I've been considering surgery, but if it is indeed more of a grade 3, I think I'll let it heal by itself and hope that works out the best. Narcosis and the (small) chance of complications are not things I'm willing to accept unless necessary.

Hi, I have been first diagnosed with a type II AC separation in the emergency room, and then re-diagnosed as a type III later by an orthopedist. I was given a sling; that helped a little bit, but that didn't really work very well. I tried a shoulder support brace, and that didn't work either. I found out that when lying on my back the clavicle fall in place almost perfectly. So what I'm doing now is use the straps of the sling to create some type of brace that go around both arms and connect in my back to keep my shoulder back as if I was lying down at all time. And that seems to give the best results. The drawback is that while wearing this, the motion of my arms is limited. Now, my question is: doing this as much as I can, will it help the ligaments to reconnect, or will they never reconnect on their own? I suppose age is a factor, and for the record, I'm not young any more. PS: thanks for the very informative video

It'll get better. There has to be a new ligament formed at the tip of the separation which will take time , and it'll take some more for it to seal itself back to its original position. But it does heal back and will start to look and feel like it was prior to injury. The funny thing is, your body knows it's damaged despite it being long.

So, on a Grade 3 do the ligaments reconnect on their own?

No, sadly not, but the structure around it can be strenghtend in order to achieve a certain stability.

When your surgeon who happens to be nationally regarded as the best asks, "is it ok that i takes photos of your X-rays to share them with my colleagues." you know your ac is f'ed. FYI Type 5 and to put it into perspective 4 posterior rib fractures, burst lung and pneumothorax.

@@almiighttrack3158 I recommend getting some exercise tips from a physical therapist to increase shoulder strength. My Dr did not recommend surgery. the only issue is the protruding bone from the accident, but there is no pain or limitations. Only purpose of surgery is cosmetic to put bone back into place.
