Partial rotator cuff tears are very common. Many people over 40 who have an MRI for shoulder pain, particularly those of you with pain at night will notice that the radiologist mentions a partial thickness rotator cuff tear on the MRI report. This is usually not a cause for concern. Many of these partial cuff tears are an age appropriate change that takes place within our rotator cuff as we age. Most people with rotator cuff tears and rotator cuff tendinosis will respond to a physical therapy program for your rotator cuff. Some may chose to proceed with injections too. Surgery for partial rotator cuff tears should be your last option. Research shows that when necessary, the surgery is often (not always) a cure for the night pain in your shoulder associated with these issues.

    I love that this video endorses the possibility that a rotator cuff tear can heal--however, a video of yours from a few years back also mentions that a rotator cuff tear can become larger. Would you mind producing a video regarding factors involved in both outcomes--tear healing or getting larger based on your observations?

    Im a 21 yo male who started doing chalistenics in this quarantine, i think i torn my supraspinatus tear 2 month ago, i did all the test for supraspinatus and it was 100% positive: weakness, pain, muscle atrophy and just fear to move my shoulder in certain directions have been my symptoms. It just sucks thst at my early 20's i damaged my RC... I will be getting an MRI tomorrow and will go to my orthopedic surgeon in 7 days... Will see if that's a tear or just tendonitis, but im pretty sure its the first one. Also i have been dealing with severe depression/suicidal thoughts since this injury, its amazing how an injury like this can affect your mental health.

    Hey man how's it going? I fucked up my shoulder about 10 weeks ago from weight lifting and I haven't been able to go to the gym and I've been in terrible pain sense. I got an MRI next week and then physical therapy after. How'd your MRI go?

There's way morse stuff than having a great physique, at least for me, i got accepted in one of the worlds best universities, now i have s new goal and im motivated. What im saying is: find something else and fine new interesting things to do. Im pretty sure that with all the medicine advance and technologies it is very possible to recover from this, its gonna be tough, but we can. The first thing to do is an MRI, will be getting it tomorrow

    @@paladindanse98 the same, i got some medication from an specialist, but its quite the same, ill be doing PT in the next 1 or 2 months, and if that doesn't work, ill be getting surgery

    Thanks for your video, I have a partial tear under the muscle, tried therapy and injection, not much of an improvement, everyone one I asked about this who had their rotator cuff done, regret doing it! however the Dr. says it depends?! Thanks again.

    I have a partial tear from a calcified shoulder. The pain and limitation of movement was gradually onsetting. I waited too long working with anti-inflammatory medications till i couldn't no more and left work for the hospital. The xray showed the calcification and M.R.I. showed the partial tear. I had 2 cortisone shots and I'm done 2 months of physiotherapy. I start healing for a couple weeks, then with false uncontrollable movements i go back into inflammation and restart the process. I wish i could have a brace to hold my shoulder position longer enough to be healed stronger, but the focus seems to be enforcing moving all the time. I understand some if that, but when my exercise are increased, i go downhill in pain and have to start over again. But an operation is brought up all the time if i don't heal fast enough.

    They just found 6-8 cm size tear . Can i prevent surgery and use a shoulder tool treatment to get blood & oxygen flow to treat it at that size tear?

    I got partial tear 2.5 years back while doing weighted pull-ups. It pains slightly but I am unable to lift heavy doing pushups is painful.

    Mine healed! 2 tears, when I was much much younger. Ok not that much. Early 20’s. Physio was extremely beneficial and likely the key to success. As well as youth on my side 😬

It is an awesome one off product for getting rid of your rotator cuff pain without the hard work.     Mine healed as well..........MRI indicated complete/partial tear of the Supraspinatus with surgeon indicating surgery was the only way to go. Extensive rehab and ice got me through it but it took almost two years

    mines happen in 2013 and it's 2022 and i hurted my right shoulder but i believe it healed cause i can still throw the football and play drums. i want to start lifting weights again but i want to know by lifting weights will it help it get stronger or i shouldn't lift

    Does 30 - 40% supraspinatus tear require surgery? Can it regenerate itself if left without a surgery? Are there any patients of a young age who had it grow back on its own? Or it never grows back because it’s impossible from the anatomy and biology standpoint and with special exercises we only make what is still attached to the bone stronger and the torn part remains torn forever?

    I have a full thickness tear for 3 years now and I was told that now it cannot repair it (three separate surgeons) and a replacement surgery is my only option.

    I got a funny story about healing, a true story. I tore my whatever in my knee over ten years ago, I lived with it as it basically was just uncomfortable, and really uncomfortable when I was sitting and crossed my legs for any amount of time. So my goal was recently , I'm 53, wait till I retire before surgery. I was sure I needed surgery, but not willing to take time off work since it only bothered me to a small degree. I could walk just fine. It just was not healing at all. Same for over a decade, no improvement, and forget about jogging, I just couldn't do it, I tried. But walking was relatively pain free. So on to next injury, maybe 3 months ago, ...thought I tore my rotator cuff, ...only hurt with certain exercises. So knew it wasn't requiring surgery, and decided to just wait it out, quit those exercises, and started reading about vitamin supplements that might speed up the healing. So added calcium and vitamin d and after about a week my shoulder still jacked up, but my bad knee was completely 100 percent pain free, in any position, in any circumstance, completely better, after over a decade... True story.

    Could be calcium buildup -- calcific tendonitis. If you're still having issues, see if they can do an x-ray or MRI on it. If it hasn't hardened, they can probably take care of it without any surgery. But hopefully your body has taken care of whatever issue it is. I just thought I'd mention the calcific tendonitis because it feels like a rotator cuff tear. But you wouldn't know without an x-ray/MRI.

    I had a surgeon tell me I had a FULL tear cuff with surgery required or other problems would soon follow. That was 6 months ago. I'm 68; active and in shape. I'm now completely pain free, back to playing my sport, have full range of motion, but am being very considerate to make 100% certain that the shoulder is completely healed. All this WITHOUT surgery or therapy (except for my own at-home therapy).

    laverdadesmejor Yes I had MRI & this is the findings: “IMPRESSION 1. Impingement upon the rotator cuff. There are focal areas of tendinopathy and atrophy of the supraspinatus tendon. I believe there may be a small full thickness tear of the distal anterior supraspinatus tendon. In addition, there is a small full thickness tear through the distal infraspinatus tendon at the 2 o'clock position relative to the humeral head.” 2. Degenerative change with a possible degenerative tear in the superior glenoid labrum near the bicipital anchor. 3. There is chondromalacia involving the articular surfaces of the glenohumeral joint. 4. Partial tear versus capsulitis inferior glenohumeral ligament.

    The biceps is often a pain generator in people with rotator cuff issues. Like the rotator cuff, the biceps can fray as well. Sometimes that hurts people, sometimes it doesn't. Many people will improve their pain and function with PT alone.     i got a cortisone shot in 2017 i believe and the pain went away but my shoulder still hurts but on a scalp from 1 to 10 it's a 2 but i want to know is there something i can do to make it go away for good. they said i have a partial thickness tear but they never mentioned what that means

    Thanks for sharing your knowledge doctor. The common reaction fear based is to freak out and think of surgery as the ine and only option. I will take all the suggested steps starting with a healthier diet and physiotherapy to see how my partial supraspinatus tendon tear evolves 👍🏻👍🏻👍🏻👍🏻

    @@ki8417 Thanks for asking, much better. Pain is gone and range of motion is near 90% after three months of consistently working out and eating well balanced and nutrient dense food. I am a certified personal trainer so I started all over again,.. stability, mobility, core exercises, etc.. every daily life activities (movements) can be performed in an obsessively meticulous way, aiming for the perfect posture and functionality, everything! Once we start re-educating our neuromuscular patterns magic happens... It takes a bit of effort and practice to become aware but it is totally worth it. Now I am enrolled in a Functional Training course which is fascinating. I would definitely recommend you to get one of the text books if you can, is called The Pain Free Program by Anthony B. Carey.

    I had a partial shoulder replacement. Basically the humeral head is cut off and replaced with a titanium smooth surface. Now, that being said. What if you tear your supraspinatus tendon? How is the tear repaired?

    Those tears can often be treated the same way as with a normal shoulder. The part of the shoulder (greater tuberosity) where the rotator cuff attaches is not replaced with metal during a humeral head replacement/resurfacing.

    @@HowardLuksMD I’m a very active person. It’s been a 1 1/2 since my surgery. I also had my left one replaced a few years ago. Absolutely no pain whatsoever. I stretch a lot, I think maybe this is what caused the tear in the newly replaced shoulder.

    Great video. I was diagnosed with a full thickness supraspinatus tear. That happened working out with bb curl. The doc says surgery is needed but I have almost no pain at all and full ROM (the extra rotation is not 100%). I can do overhead press and db press with minimal discomfort. I am 54 and been training for yesrs with serious strength training. What do you think? Should I do surgery? Can I strength train? Thanks a lot.

    Hii sir please do help me with this.I am a Volleyball player and I'm 16 years old. The impression I got in my mri was -1)Interstitial tear with tendinosis of supraspinatus tendon. 2) superior labral antero-posterior tear. 3) Marrow oedema within the greater tuberosity. I've been resting from 8 months and it's better now as I used to deal with pain before. I will be starting physiotherapy in few days. Can it heal and can I start playing volleyball in few months sir?

    Hi! Have a minor supraspinatus tear in my left shoulder from a fall. Started PT but have been doing rotations and strengthing for like 2 months at home. Since I started the PT it hurts a bit more. Is that normal? Thanks

    I have been diagnosed with partial tear of supraspinstus & subscapularis.. my shoulder range of movements aren’t restricted but I constantly feel pain at rest. I have stopped all physical/gym activity since 1 month now. When can I resume gym activity & when will my injury completely heal so that I can return to routine gym work?? Thank you

    I am a gynaecologist by profession, my Ortho doctor has asked me to avoid all overhead activities as long as possible because healing will cause fibrosis which will weaken the muscle and heavy workout in future can cause tear again!

    @@anishsharma7772 it’s still somewhat the same! Started to take collagen tablets since a week now.. doctor said take it for 2 months, if not improved then we’ll take inj platelets at the injury site! I’m still expecting minimum 3 months to recover :/

    i have also torn my rotator cuff at gym its been 2 months now ,at first i thought its a normal impingement later i figured out its something major going on with my shoulder now its not healing at all its the same range of motion is not limited though Can u guide me through it what mistakes should i avoid I think i should start some workout now i have taken quite a long gap of 1.5 month now ,how about skipping chest and shoulder day and hitting all other parts is it safe?

    @@dapinderpandher7791are u from india? i had surgery of full thickness tear of shoulder ligaments on 29th feb 2020. i had two days severe pain, but from third day it decreased. i don't know about partial tear treatment. i am having pain in other shoulder and i think it has partial tear. so i will ask doctor about this.

    @ TRUTH BROWSER hi buddy , hope you are healing great . I am a amateur calisthenics athlete. I injured my supraspinatus too . I have a 3.1mm partial thickness intratendinous tear of my supraspinatus tendon. Doing rehab but no dramatic improvement yet . Brother just wanted your advice, what would be better for me ? Stem cells? Surgery? I can’t even perform a push-up. I can do all my basic daily activities, but cannot do pushing movements without pain . Please reply if possible.     @@rockybalboa61 hi bro, i had surgery on 29th feb. after two months it started healing. i can feel my shoulder bone cracking sound but no pain. now i can do lot more movements with my hand. but i am unable to lift my arm upwards fully. i think surgery is best. stem cell is waste of money. i visited stem cell people in hyderabad, but they too suggested surgery. just bear the pain for 1 month. after that u can feel the pain going off.

Buddy but my only point is I do not have a full thickness tear and I can even perform pull ups ( but not push ups) , I can even climb a rope but coming down is pain , so I can do all basic activities without much discomfort. That’s why I asked for your opinion what would be better for me ? Still confused.

    Hello Doc, I have just been diagnosed with a 7mm tear of my supraspinatus tendon I'm a 62year old very active and fit semi retired person. This initial problem happened about 5mth ago... should I just take it easy and see if this will get better? the pain is manageable, thanks in advance!

    @@paladindanse98 considering I'm always doing something and using my arms all the time it's getting better I don't think I will have an operation, mind you it takes about 6 month for any improvement, try not lifting your arm too high use a ladder instead to keep arm lower and sleep with the arm by your side, I now have little pain and just dug a 40m shallow trench with a pic for my water irrigation without any problem also try some of the exercises there are many videos on the net...

    Doctor Howard, I don't understand why Doctors as yourself don't discuss the Cause! please talk about the most important aspect of this! THE SOURCE OF THE TEAR... Specifically if the tear is from a Hooked acromion Bone Spur.. If a patient has a significant acromion bone spur that is causing the tearing, it's simply NOT going to heal on it's own but often degrade to a full thickness tear. I speak from experience.

    30 y/o male, weight lifter, partial supraspinatus tear, doctor doesn't want to perform surgery.. problem has been going on for 4-5 months now.. it improved somewhat but hasn't gotten to 100% and flares up sometimes. Recent MRI 2 weeks ago conducted shows tear. It HAS gotten somewhat better but still isn't 100% and I can't return to full activity or even close because this will just set it back.Do you think eventually it will return to normal or heal? I'm rehabbing it now, no heavy lifting.. using therapeutic ultrasound as well.

    Jonathan Smith I have the same issue. Did MRI and Shows High grade partial tear from lifting. Doing physical therapy 3x a week for almost 2 months now. It’s getting better but i can’t lift weights and it sucks. Considering to get surgery since the tear is not gonna heal with just PT.

    Almost 2 months is still too early to give up and say it's not progressing enough for surgery... My issues started 6 months ago, BUT, I was still lifting heavy as recently as 3 months ago so any time it started to get better I would just reset it by lifting heavy.. It isn't until the last 2-3 months I stopped lifting heavy and I have to say I'm making good progress, but still not lifting heavy or pushing it, but day to day life no issues.

    Jonathan Smith Jonathan Smith I injured my self May of last year. I didnt know it was that bad so im still lifting weights and just avoiding heavy shoulder workout. The pain keep getting worse as the day goes by. So i started with getting Xray. No bone issue. I was referred to go do PT and after 3 weeks, pain got worse that both of my shoulders are now in bad pain. Then I decided to stop the PT and get MRI. After seeing my orthopedic Dr. he told me about the high grade tear. My option is surgery or keep doing PT. So i went ahead and do more PT(6 weeks program) next week will be my last and going to see my Dr on friday for follow up. My PT explained to me, the tear is never going to heal. With my lifestyle and loves weightlifting. PT is not going to be enough to get me back to where i was before. My tear is barely attacked. Even now without working out. Its Been 2 months. I can hardly sleep comfortably with my shoulder. I cant live like this. To young to life like an old lady lol. I have two young boys and 2 puppies to take care off 🥰 Good luck to yours!

    Today I am pain free, I don't know if It healed or what, but the problem was going on for a long time but finally is passed me. #1) I started doing more external rotation work in my workouts, to build the supporting muscles and help fix any imbalances... So I'm doing things like reverse dumbbell flies... #2) I'm cutting back on the volume of how much over head movements I do. I used to do ALOT of shoulder press movements, now I'll do only 2 sets over head presses with a barbell and maybe 1 set dumbbell press. On the bench press, I still have been able to bench heavy and do it pain free, but I'm not doing as many heavy sets. I'll do moderate sets, then maybe 1 heavy set, then go back down to light weight. #3) With covid going on and gyms closed, I'm doing ALOT of pushups every 2-3 days.. and I'm only lifting weights once every 2 weeks, but I'm still pretty strong in the weights.For you, I'd say do less over head movements, less heavy pressing movements in general for now.... Do external work, like reverse dumbbell flies, get a resistance band and do outward movements, (you can youtube some rotator cuff resistance band movements) do this for about 3-4 weeks.. after that, slowly get back into lifting... once you're lifting again, you CAN begin heavy pressing movements, but I would do more than 2 heavy press sets per workout, the rest do moderate weight. Overhead movements, limit that too... You CAN still overhead press heavy, and pain free, but don't do as many heavy overhead sets, or overhead sets in general. Also, I start EVERY WORKOUT with reverse dumbell flies, to warm up my rotator cuff and supporting shoulder muscles, before I go into any lifting movements.When you do external rotation exercises, so LIGHT WEIGHT, perfect form, 10-15 reps, for 3 sets. When I'm doing reverse dumbell flies, I never go heavier than 12 pounds, and I do them nice and slow, perfect reps, for 12-15 reps. When doing rotator cuff exercises/external movements, you want to do LIGHT WEIGHT HIGH REPS PERFECT FORM.This is what worked for me. MRI showed high grade partial thickness tear 13 months ago, never had another MRI since, so cant say if it healed, but the pain is gone, and I've been able to workout pain free since the fall 2019. External rotation, and doing a few less heavy pressing sets and less over head movements was key for me.
