A Complete Guide to Finasteride in Hair Loss & Hair Transplantation | 100 FACTS

This talk is a Complete Finasteride Guide and contains all the questions about Finasteride and its use in hair loss, balding and hair transplant I have ever been asked. To be clear at the outset, tailor-cut dosages of Finasteride are for hair loss treatment and not after a hair transplant when a dose of 1mg a day is mandatory in most cases. For other indications of hair loss, there is no one dosage that fits all groups of patients with hair loss. It is interesting how crowd behavior has evolved dosaging of Finasteride over a period of time in various permutations and combinations. Every patient is different with differing metabolism, differing risk tolerance and differing hardiness of the mind since the mind rules the body. This is important to understand since compliance is crucial while recommending Finasteride and the patient has to be comfortable using it. Clinics follow dosage regimens that best suit their patients and practice. Many a times this advice is without scientific basis or clinical propriety but has a salubrious effect on the patient’s hair loss.
The National Center for Biotechnology Information stated that lowering Finasteride dose to 0.2 mg a day eliminates side effects while remaining useful to fight MPB. But no hair transplant surgeon would have experienced such low doses giving salubrious effects in hair loss. Certain doctors prescribing miniscule apologetic doses of Finasteride 0.5 mg on Monday-Wednesday-Friday merely pander to the underlying fears of the patient who is wary of Finasteride. Any doctor who gives respite from his worst fears, is music to the patient’s ears! This dosing cannot do justice since average 0.2 mg a day is much below the effective dose 0.8 mg per day if we refer to the dose-response curve. Moreover, it does not cover the weekends when testosterone levels are the highest in the week due to outdoor activities. However, it has been seen that a daily dose varying from 0.5 mg to 1 mg is where the sweet spot of dosing of Finasteride lies. According to dosing studies, lesser than 0.5 mg a day does not work. So any dose that is sufficient to stabilize hair loss, maintain and regrow hair with no side effects is the optimal customized does for that patient.

    @@rajan2291 No it is of no use. It will just be absorbed by the blood but not reach the roots since dosage absorbed is low.

    @@rajan2291 You should take 1 mg a day for 6 months. But follow your doctor's advice- he may not have felt you are eligible for Finasteride due to some reason.

    Doctor, you are truly one of the best source of information and knowledge on the web for us young men with hair loss. I could not be more thankful for stumbling on your channel. My question to you is what the maximum level of estradiol and minimum level of testosterone one should have in order to start using finasteride. And secondly, I am a 21 year old men with a Norwood 2 balding and I was prescribed a dose of 0,25mg finasteride every day in order to minimize sides. Do you think this will be enough for my relatively low balding scale? Thank you in andvance!

    Thanks. Hormonal levels are no indication for dosing of Finasteride. They only help rule out pre existing hypogonadism or provlivity to get sides like ED and gynecomastia. I would be hesitant in starting a patient with total testosterone level less than 400 ng/dL on long term oral Finasteride.     @@hairtransplantinindia Thank you for your answer, as I had slightly higher progesterone (2.79 ng/mL), do you think it is going to be an issue if I use fin? My test level was 6.49 ng/mL and my E2 was 24.95 pg/mL. PS. I am lean built and I am working out for years.

    Thanks for the precise and informative snippets that you have compiled for finasteride , Dr Bhatti .You are doing a wonderful job . I would not expect a detailed answer from you for a youtube comment , but ill go ahead anyways. there seem to be a group of people including me who been using finasteride for the past 1 year and having good maintenance , only to see , not only shedding but also miniaturization to a certain degree since the psat 2 months. Due to this I have resorted to combining fin 1mg daily with dut 0.5mg once a week. As you have mentioned that the mind could govern the body in funny ways ,i started "noticing" that my facial hair growth has reduced minimally and my face has become a bit more rounder since starting Dut for the past 2 months. Any sort of anecdotal evidence from your patient care experince is welcome. Have a good day!

    Thank you for your kind words and for watching the video on my channel. Dutasteride blocks 94% DHT which causes facial hair and which maintains it. No this is never a concern with Finasteride since it blocks just 70%. The round appearance of the face could be since you are losing beard hair or they are thinning and this is changing the profile of your face. But it can also be due to higher levels of estrogen consequent to the DHT combination you are using. Time to get these levels checked.

    Thank you for your informative video Dr. Bhatti! I'd like to ask for your advice. I'm on a 1 month break from propecia (I previously heard that you can take a 1 month break after the 5th/6th month. I started taking finasteride (propecia) 3 times per week about 5 months ago). After the break, I'm thinking about switching to proscar, due to the price difference. On question 35, you stated that switching brands may cause intense shedding (In my case, both the propecia and proscar have the same licencee company written on the box). After the 1 month break, if I continue with proscar instead, will I still be taking a risk of shedding or other side effects? I would appreciate your feedback.

    Shedding is not likely since the company Merck has the same isomorphic form of Finasteride for propecia and proscar. But I am curious to know how you will be splitting the 5 mg proscar tablet into 0.5 mg pieces!

    Hey Doc, Why do you not prefer using topical finasteride over oral finasteride for young patients with thinning , since topical finasteride has same % of scalp DHT reduction and lesser % serum DHT reduction as compared to oral finasteride ,thereby reducing the risk of side effects...

    Hi Doctor, thanks for the valuable information. Can you pls clarify the below- 1. Exactly after how many days of hair transplant, the Finasteride and Minoxidil dosage should start? 2.     Minoxidil to be started after crusts come off around 14 days post procedure. Finasteride after 5 days of hair transplant once antibiotics are over. The recommendation is to take them daily.

    Hi Doc. Incredible series of videos, so much great info provided, it's much appreciated. One question. Creatine is one of the best supplements for increased gym performance, but there is one study out there that found it significantly increased DHT levels in men, which in turn causes increased hair loss. Is it okay to supplement Creatine as long as you take finasteride, as the finasteride would block the conversion to free testosterone DHT, or does the Creatine increase DHT through another mechanism and should be avoided completely?     Thank you for this mega compilation of finasteride related questions and their clarifications. I sometimes feel that there is a deliberate effort to suppress any cure for hair loss or treatment for hair growth as it would lead to massive losses in revenues from sales of finasteride, minoxidil, hair vitamins, shampoos, wigs/patches/fibres, prp, hair transplants etc...

    Great video. One of the original purposes of Proscar was to reduce the size of the prostate which can have various health benefits like less frequent urination. In addition to blocking DHT which causes hair loss does Finasteride in 1 mg dosage decrease the size of the prostate?

    I hope he was the doctor I met 10 years ago. The clinic In Ottawa mind you, that I went to quickly went to look at my donor area, although I had a hair full of hair. When I resisted the idea of Finasteride, they didn't address it and said that hair transplant will "cure" my baldness. Now after 10 years I am watching this video, shiny bald and I am trying topical finasteride. This thing works. Parts that were all gone are coming back. maybe I am a super responder, but I am seeing good results. Will get transplant later to fill in the rest of the baldness.

    Hello Sir, Thanks for this amazing video. I have been taking finasteride since 2017 and doing pretty well on it. I am 31 years old. Since 2020 I have been suffering from depression and was taking low dose antidepressants for it prescribed by the Dr. Recently since middle of 2021 my libido plummeted and my erections are rare and soft. I initially attributed it to my antidepressant, since I never thought about finasteride. However when I stopped finasteride this last month, my erections are hard and my libido has improved. I am still on my SSRI's. What should I do now? My hair growth has been great on finasteride and I do not want to stop it but am concerned about my sexual side effects. ( I workout regularly, non smoker, abstain from alcohol ). Could you please suggest what should I do? Thank You!

    Doctor, i have a question with regards to the donor area: In one of your videos you said that those grafts which have been extracted during the anagen phase are best. So, as a patient would it be best to shave one´s head, say 3-4 days prior to the surgery? as this would make sure that only those would be visible for extraction which are in fact in this favourable anagen phase?

    Yes, this is a good way to do so. The only thing that limits it's use is that patients take consultation mostly online and then come direct for a hair transplant. if they come shaven, a lot of information about their balding will be lost to the doctor.

    Doctor, I'd like to follow your switch-off-protocol to prevent the build up of dutestaride, however, I'm reading the half life of dutestaride lasts 6 months.     Its much less as explained in the video. 15 days is enough for Finasteride. For Dutasteride you can stop for 4 weeks every 3 months if you have sides. Dutasteride is a stonger DHT blocker so you will need to observe what suits you best. Consult your prescribing doctor.

    Doc m using cotton hanky to cover my head while going out but whenever I remove it I saw few hair on it and it's been 2 months 10 days after hair transplant , so m confused whether it is shedding phase or jst static current which is pulling my hair? And those hair will come back?

    Everyone sheds 100 hairs each day and 100 grow back if you are not balding. The irony is that we cannot see hair that grow!

    Great video thank you. Subbed! I’m 37 and been on 1.25mg of Fin for 19 days now. Noticed slight thinning for 6 months before starting. Now I think I’m starting shedding (around 150/200 hairs per day this week). Im wondering why blocking DHT tends to promote a shed? I understand blocking DHT will stop ‘polluting’ the growth of current and new hairs, but why does it promote the shedding? Is it because now the new hair “seeds” are no longer poisoned by DHT and are able to grow so the body sheds old hairs to make way for the new upcoming hairs?

    Its because the DHT blockade allows the root to get healthier and robust. The root grows a stronger hair and this detaches from the previous. It leads to shedding.

    @@hairtransplantinindia Thank you so much for answering :) I really hope this means my drastic sudden increase in falling hairs so early into taking Fin is a good sign. It never fell like this before Fin. If I weren’t taking Fin, I assume these hairs would have slowly fallen out eventually anyway and just not had a chance to be replaced. May I ask a final question, am I right to assume the percentage of shedded hairs that will successfully regrow is genetic. I guess some people get little to no regrowth where some may see 100% and even extra hairs growing back?

    @@44daggers44 Shedding is a good sign when it happens after starting Finasteride. It is also a good sign after a hair transplant. It denotes life within- within the root that is active. Just as the tree that sheds and then after the cruel winter grows new bright green leaves.

    hello doctor, i am part of the group that has just reached 20 (around september) but has been experiencing a slow decline in the corners of my hairline for the past 2-3 years (i started noticing the slightest hint of hair loss back in 2020 even though i believe i was around a norwood 1). I am now a norwood 2-2.5 if im correct. in my country the medicine is obtainable without a prescription. therefore i've already made the purchase of a 30-day supply of generic 5mg finasteride (planning to cut them into 4 1.25mg pieces as you have mentioned in the video). also because i have never heard of the advice not to take it before the age of 21 by anyone at all. since you advised not to take finasteride before the age of 21, what do you suggest i do instead? thanks.

    Merck recommends 18 years as minimum age. However, companies are known to push their drugs for maximum profit. They do not take cognizance of the fact that DHT has a function till 21.

    Doctor, I have been taking 0.5 mg of finasteride for a month. I've had some effects like trouble breathing and hunger, but they have subsided. In the last couple days, I've got a new troubling side effect, which is brain fog and sleepiness. It's very difficult for me to concentrate for most of the day. In your experience, does brain fog eventually subside? I'm willing to fight through this if so, but other wise I guess I have to stop. Any suggestions, would be appreciated. (Also, I have OCD and Aspergers.)

    I work out daily and take creatine, plus ashwagandha (300mg) at night, and I've heard both increase testosterone/DHT - so if I'm taking finasteride to block DHT, should I avoid creatine and ashwagandha?

    Donor hair are not fully DHT resistant. No hair is. During the growing phase, DHT can do harm and some percentage of grafts will never grow.

I have clearly observed of chest enlargement and tenderness in the chest. But still I am using Fenustride. I worried about chest enlargement. I am not suffering from obesity.     hi dr. in relation to the hairline matters, at the age of 35, after restoring fue hair, is there any chances for the patient to gradually reduce then stop consuming the finasteride? in my case, i may suffer iron deficiency with an uneven receding hairline which may be in the norwood 2 or 2.5 scale. i am planning to get face to face consultation and may proceed with hair transplant to reverse to norwood 1.

    Yes, after 35 years of age progression of baldness usually stops, however it is best to take it for at least 6 months after the procedure and then stop finasteride for 15 days and assess whether you have hairfall or not. If you do, it is advised to continue for another 3 months and again stop for 15 days to reassess progression of baldness. Dose can also be reduced but it is advised to show your condition to a doctor for appropriate assessment, lab investigations and prescription.     I too have people who have not taken my advice but have had a good result. But what you need to know is that these patients are few, and you will never find a person who has extensive progressive baldness and is young who will report a good result without Finasteride. Good results without Finasteride happen in those who have stable baldness.

    Doctor, I watched the entire video here my question is can one take Minoxidil and Tab Fin without consulting a Dr and without doing any tests( or only free and total testosterone tests for future assessment) with alternate dosing of finesteride and a positive mindset, as u said only 0.5% will see possible side effects.Does this help in diffuse pattern alopecia?

    Fiansteride does not promote dementia. can you share the link where you read this! Prostate has no link with Finasteride other than it being used for treating BPH.

    DHT has many important functions. That s why it circulates in your blood stream. The development of secondary sexual characteristics is a function of DHT. Development in males finishes at age 21.

    Very informative. Would like to share my experience with others. I started .5mg on alternative days after a gap of 6 months. I experienced testicular pain and stopped after 10 days. I got motivated by your videos and made up mind to use again at .5mg alternative days. Thankfully no testicular pain, but pain in right calf for 2-4 days straight which now has reduced.... secondly, don't feel fresh when I get up in the morning ..as if I have a hangover.... thirdly, get blackouts when i get up from sitting positions. I am not scared or anything to use it. I wanna give it a try again for 3 months after using it for 4 years straight 1mg daily! I want to actually but all this I am experiencing wish could be avoided. I have high metabolism , eat clean, don't drink or smoke. Thanks again and it feels good to read other people's experiences .the comment section feels like a hair loss forum 😄

    The blackouts are because of low sodium in blood, the way you mentioned switching positions causing blackouts is exactly what happens when you are on low sodium diet. Check out The Salt Fix by Dr. James DiNicolantonio.

I thought my hair thinning is stable now so I started using Finastride on alternate days and only with in 45 days I realise that hair are thinner than before and i had no stress as such responsible for that thinning .     Stop Finasteride 60 days before planning a pregnancy. No 2 cases can be given the same specific answer of til when they need to take Finasteride. But it is certain that till such time your baldenss is not stable you should take it in low dosage or the dosage customised for you by your doctor.

    The gene switches off at 35-45 age. After this it is safe to stop the drug without much loss.

    The peak testosterone levels happen around the time you wake up! The half life of Finasteride is 6-8 hours. So taking it at 11 PM, just before you go to bed, will do you good. Thanks for watching.

In last February 2022 I noticed lost hair in my temple and overall thinning hair all over my scalp suddenly. Then I consulted a dermatologist and he suggested me to go for PRP treatment and I had gone through the procedure. But, unfortunately I have lost more hair.     Doctor my dermatologist prescribed me DHT blocker Can I take Finast (by Dr Reddy's finestride 5 mg) and cut it in 5 ??? When I visit my dermatologist medical store he only gives me dutasteride he don't sell finestride.. I took duta steride daily for 1 months and then on alternate days ..I faced no side effects ?     Yes you can use 5 mg and cut it in smaller pieces using the pill splitter as explained in this video.

    Sir i m 19 year i am suffering from mbp and have little crown and receding hairline. My doctor suggested me multivitamin, dutamax lotion ( topical dutasteried 0.025% w/h), and 5% minoxidil. So, how much year i can delay male pattern baldness?     Once you are past 18, Merck recommends you can go onto Oral Finasteride. If you do not your present medication will not hold the hair for long.

So Can I take a break of 10 days (or no of days you suggest) after ever single month of finasteride1mg everyday? Will this be even more safer and reduce the side effects more?

    I may be seasonal or Minoxidil may not be sufficient. You may therefore need to take Tab Finasteride.

    If you have an aversion to Finasteride, that is all you can use; but mind you if you lose the hairs they will not come back if you start Finasteride later. Many people do not understand this important fact of DHT blockers for young patients who are thinning.

    @@prasoonshakya350 No break is required after topical Finasteride since it does not get absorbed through skin in enough quantities to even act as a DHT blocker.

    No, you can use Finasteride/Dutasteride in consultation with your treating GE physician but be careful of the dosage since metabolism is affected and drug will pile up in the body with higher likelihood of side effects.

    Hello sir. 5 years before i had used minoxidil for 1 year. And stopped it because minoxidil hast given any results for me.Subsequently my hair loss gone worse.. Now i had done a hair transplant last week... Can i use finasteride 1 mg ALONE ..will this be effective for my existing hair and transplanted hair.

    When you get a hair transplant you should use Minoxidil alongwith Finasteride for 6 months. fter that, Minoxidil may be omitted and if you have progressive baldness, only Finasteride can be continued if you have skin issues with Minoxidil.

    I’m on TRT and about to start finasteride I’m nervous and excited not sure if I will need more estrogen control or not will have to see. I feel well informed thanks to research and a couple lectures from this awesome doctor and I also get regular blood work. I just hope if I do get sides that they will go away with discontinue of use. It’s the “post finasteride syndrome” that is the most worrisome but this doc doesn’t believe in it and I trust him for better or worse.

    In those who re physically fit, and on TRT and Finasteride, side effects seldom exist. Unless ofcourse you are young and have hypogonadism.

    It does not help. It gets absorbed into the blood stream just like oral. You waste precious time while falling prey to hearsay. We all seek the easier way out from our predicament It is mostly the wrong path. Companies benefit by our insecurities since there is so much money to be made selling topicals.

    @@klartext4682 Why rub it into skin when the chief purpose to use it is so it does not get systemic (into the blood stream) and cause ED! Cost is not a factor to the clientele I treat.

    Finasteride gets metabolised in the liver. If your liver is healthy, your liver enzymes SGOT, SGPT, etc. are normal, it is safe to take.

    Hiii sir..need your guidance...I know ki minoxidil lagane se initial days me hair fall hota hai...which is the sign of minoxidil is working...lekin me 13 days se apply kar raha hu.... Lekin hair fall nai ho raha....does it mean minoxidil isn't working?.... I'm applying lotion form (5%)...1ml...once a day....and my dermatologist told me to apply only once a day....should I appl once a day or twice?

    Applying minoxidil once a day is enough. Hair shedding is observed only 3 months after applying minoxidil, not within 13 days. You won't be able to appreciate its positive effects of making hair denser before 6 months.

    Your breast glands are removed during gynecomastia surgery. When there is no beast tissue, there will be no sides with Finasteride. Even then they are very rare.

    Reflex HyperAndrogenety. Dr. Can you please talk about this. Is it common? Can Finasteride cause it? Im ok with a " normal shed ". But a hardcore shed that can accelerate your hair loss its too much for me. Im thinking in jumping on Finasteride but im really apreensive because of this.

    These biological products that are available in nature are more of placebos as compared to pharmacological grade medications. The blockage if at all will be minimal and not what those on Finasteride would expect. When you have hair loss, time is against your plans to meddle with these.

    Ketoconazole is a very mild DHT blocker. Finasteride is effective in androgenic alopecia only.

    My hair fall started suddenly and lot of hair started coming out. I was dieting and had become anorexic so my my hair fell out. My crown has diffused thinning and a little bit on the side of hair. No hair loss on temples as you can see in profile picture. I have gained weight again and my hair fall has stabilized but Is it male pattern baldness?

    Yes if you see thinning that follows a pattern, it is androgenetic alopecia. A consultation will be a good thing.

    hello doctor , 3 yrs ago i was taking oral finasteride 1mg (after lunch) with topical morrf (5%minoxidil and 0.1%finasteride) . Daily topical morrf applying for twice a day was impossible for me so i switched to oral minoxidil 2.5 mg (lonitab 2.5 mg). now i am taking oral finasteride 1mg (after lunch) my lunch time is after 2 pm and taking om 2.5mg at night after dinner my dinner time is 11.30 pm. Am i taking oral finasteride at wrong time ? when should i take oral finasteride? Can i take oral finastride 1mg with om 2.5 mg at night after dinner ? pls suggest me doctor i will grateful to you.

    The peak blood concentration of Finasteride occurs 6-8 hours after ingestion. The testosterone surge that happens around 7 AM needs to be combated intelligently so even lower doses of Finasteride work. So late night is the best time to consume. Please take it at 11.30 Pm alongwith your Oral Minoxidil as recommended by your doctor. Needless to state always keep your doctor in picture.
