Finasteride Microdosing Vs. Customised dosing | What are the disadvantages?

What is the dose of Finasteride? Is Microdosing or customised dosing the better way forward. Or should we adhere to the FDA recommended regulation dose of Finasteride?
So when we speak of microdosing, can we get away with 0.2 mg a day and get a good result as far as maintenance of hair is concerned, without so much as even an inkling of a side effect -even in those vulnerable?
So coming back to microdosing, it has been shown by a particular study that the effect of taking 0.2 mg Finasteride daily or 0.5 mg Finasteride 3 days a week is not much different from taking 1 mg dose. You may support these findings by endless studies and theoretically this may even be true; but the body does not respond in all cases like we intend it to. In my practice, I have been using similar dosing - 0.5 mg on alternate days and found that not much benefit accrued in the vast majority of clients. However in those fit for the medication but otherwise fearful are advised 0.5 mg and then gradually I build up on it more to allay the patient’s fears and have him as an ally in his own interest so he develops confidence in the medication and the doctor and gradually overcomes his phobias. I do not exhibit this dose routinely since I am not convinced about its benefits as alluded by some studies. To me my clinical judgement and instinct borne out of years of practice and prescribing Finasteride in various doses is more important than one sided research.
So I do not think 0.2 mg taken daily or 0.5 mg taken 3 times a week and omitted on weekends is a plausible option in the face of aggressive baldness which is so common these days. This approach merely wastes precious time especially since patients agree to taking Finasteride very late in their hair loss timeline. It is gross injustice to advise them a miniscule dosage and waste another few months when we fully know that when the hair shaft diameter decreases to less than 50%, Finasteride fails to have any effect. It just reminds me of Nero and how he watched Rome burn as he played his fiddle.
So this is the rationale of customized dosing. A belief out of clinical experience that there is no one dose best suited for all. Every person is different, his metabolism and mind are different. We have to seek out the lowest dose best suited for him to maintain his hair and at the same time save him from side effects.

    Hello Doctor, first of all I want to say that your Videos are really good and informative, keep it going. I have been on 1 mg of Finasteride every other day for about 4 months and since one month I am on 1mg every day. I am now starting to panic a little because I haven't seen any results and my hair loss hasn't stabilised yet, after the 5 months of treatment. Is that unnormal because my dermatologist hadn't any answer for me? What shall I do? Continue 1mg Finasteride? Start Dutasteride? When can you expect to see results? I am 19 years old with receeding hairline and haven't had any side effects. Thank you.

    Thanks. It seems your hair loss is more aggressive than usual. Since hair loss continues despite Finasteride. However, let me assure you it would have been worse without it. Do not stop. You are too young for Finasteride though, but your dermatologists must have assessed your baldness and found you fit to initiate it for your hair loss. However I would be hesitant in recommending Dutasteride to a 19 year old. Are you applying 5% Minoxidil as well?

    Thank you for the quick response. I am not using Minixidil because I have sensitive skin and am bit too lazy. Would you recommend it though? I dont know if my hairloss is extremely fast because I am suffering since 4 years and lost only a little ground each year. But like you said I will continue Finasteride. Is it possible that I will still see results in the next months? I read that it takes 3 to 6 months to see any results and I am in since 5 months.

    @@denniskroner167 It can take upto a year. Your hair loss seems not alarming. I feel 0.5 mg on alternate days taken regularly is best for you. Minoxidil is not required more so because you have sensitive skin.     It’s reassuring to know there are professional researchers talking about these important topics. Just watched your DHT paradox video, it’s fantastic. I hope science is able to convey more answers sooner than later. It’s a shame available knowledge and treatments isn’t satisfying for premenopausal, under 30s women like me suffering with pattern hair loss.

    I was wondering, My hair fall stated around 22 years of age, now I'm 37. As my hair fell and grew back again I became less worried and didn't pay much attention until recently when I noticed thinning hair shafts and lots of tiny hairs with uncontrolled shedding., I stsrted taking 2mg finestroid, 3 time a week for 5 weeks now. My massive shedding shopped. I don't know if its the hair fall cycle at play or not. I don't think I'm seeing any side effects yet. I was wondering if I should keep the current dosage or minimize it, since I think maybe less would do for me and minimize the chances of future potential side effects.

    @@hairtransplantinindia 2mg 3 times/week. Before this I stared darma rolling 1s a week with topical monoxidil daily1s Right after the massive shedding started last year. After 6 months nothing changed, and had another massive shedding and I quit minoxidil especially because I find it annoying. Now I wonder, if finastroid works for me as it seems to b working. Should I start minoxidil again in hopes of reversing the hair shift thinning even if it would cause a very minor improvement? I hate how committed you have to be with minoxidil. Or should I be on finasteride for a year or a half a year longer to find out if an finastroid is actually working for me?

    thank you for the great knowledge doctor, I have seen many videos for doctors talking regarding this subject and I have also asked many Finasteride users who agreed that 1 mg 3 times a week is the optimal dose which I believe its equivalent to 0.5 mg per day. also, another side effect that worries me is that it might affect your facial hair "beard density and growth rate" due to the direct impact of the Finasteride on DHT levels on the body which as you know facial hair is mostly influenced by the DHT levels in the body, and I have seen crossed the internet many people have reported decreased facial hair growth rate and density.

    Thank you for the comment, Mr Osamah. DHT has more effect on facial and bodily hairs around puberty and adolescence. In later age it is more anecdotal and not the uausall norm.

    @@hairtransplantinindia How to increase facial hair in your 40s then? Tried facial Minox, by applying though it didn't work for 1.5 years of doing such. I Derma rolled and applied it twice aday 5 or 10 %. Its true that after I started Fin recently, that my facial hair is even less growing. I have find blond hair and only beard on chin and moustache as you see in Avatar. Other men in my family have beards to their waistline and quite powerful. I am a Viking man and its strange since my T levels have been very high in my youth with a lot of aggression and natural muscle building. I am 105 kg naturally and 183 cm only in height. Been bigger, I can build very fast and my arms are 50 cm without much work, I can starve and always over 45 cm. Arms been 56 cm when I trained in the gym regulary. Now I just do a little at home. Its just strange the beard thing when otherwise manly genes. Any way to get it out? I taken 80 days of 1 mg Fin a day and still await more results. Some regrowth left side and forehead coming slightly down again the line and some formation of baby hairs but looks a bit thin with Minox topical on scalp also with both initial sheddings combined. Great channel just came about it. Many Northern Regards.

    hair transplant has nothing to do with the drug in the system. If you are on the medication, you should take quarterly breaks.

    Hey gents, I’ve been taking around 0.05mg ( like a grain of sand ) for a few weeks now because 1mg and 0.25mg flared my Gyno up considerably. So far the 0.05+- has been showing good signs, I don’t get flare ups and my hair isn’t falling out when I get out the shower anymore so it’s looking good so far . Just posting this incase anyone else has Gyno troubles.

    hello friend, i'm from russia. I started taking finasteride 1.25 mg every other day, in the first two weeks I had testicles, but I knew that this should pass after a while, everything was fine with libido, there was itching in the nipples, the nipples were swollen ... quit ... sorry, so I hoped to bring my hair back to life... I don't want to stop fighting baldness, I plan to restore the shape of my nipples and try to mix Fina tablets in minoxidil and apply topically. Tell me, how are you doing now?

    Thats kinda crazy. How are things going so far? Cuz i cant seem to tackle off sides no matter how low i go. Some people say it subside overtime but the discomfort in mu chest was too much to handle let alone the sexual sides

    I take a low dose...1/8 of 1mg. Maybe around 2 a week. I also use ru58841, shampoo, and minoxidil. My hair has definitely improved. I went through an intense shed. However, my hair is getting better. I can see new hairs growing on my hairline. My crown is definitely improving as well. I was amazed.

    Its not equal parts, but it is approximate. I use a cutter. Low dose fin is much better. Wish they offered lower doses this is a pain to be quite honest. Its like a small tiny crumb.

    Stopping Minoxidil is not as much needed as Finasteride. However you can stop it too for simplicity of compliance.

    Studies show that 0.5 mg stops 60% of DHT conversion vs 1 mg stopping 70% stoppage, and 1.5mg to 5mg only stops 75%. Its about getting the most out of the least chemical exposure, especially since we're taking this for life, and will have to up the dosage or move to Dutasteride.

    You may be right till the 0.8-1 mg point, but any further increase will not cause additional benefits. Also DHT Blockade is not dose dependent.

    Its funny they claim Saw Palmetto will stop around 50% DHT. I always wondered if typical side effects will go up on 90% DHT blocker with Dutasteride intake shift from Fin.

    The information you share and the crisp vocabulary you use, sir, is second to none. Coming to my issue, I am 32 years old and its been 6 months that I have started losing hair aggressively, around 250 strands everyday. Balding out really fast; primarily in the crown area. Incidentally i have also been diagnosed with Inflammatory Bowel Syndrome 2yrs back. I believe there is a correlation between the two. I would highly appreciate your expert opinion om it. I have started minoxidil + finasteride topical two week ago. After watching your videos I have decided to embark on Finasteride 1mg HS dose from tonight itself. A reply from you would be highly encouraging for me. Much obliged sir.

    Tahnk you for being on my Channel. Irritable Bowel syndrome is known to cause telogen effluvium. However it is not the root cause of what you are experiencing. It is exeggerating symptoms of male pattern baldness. Do you have baldness? Please continue Finasteride but get a total testosterone level done as baseline.

    @@hairtransplantinindia yes my crown is almost bald, which was filled with thick lush hair less than a year ago. The whole scap is visible now. Its distressing. I got my DHT levels checked last month, which was well within the levels. I guess the receptors have been activated irrespective of the total DHT level in the body. I would get a total testosterone level checked and get back to you. Thank you for your quick reply.

    @@afuckingnameplz it was working fine, i mean hair loss has drastically reduced. But eventually i stopped oral finasteride due to side effects. (Decrease in libido and other stuff) Now im using topical minoxidil and finasteride combined with rudensyl. Hair loss is still there but lesser than usual. Im actually not sure of my next move. Maybe will try oral saw palmetto.

    When you stop the drug for 15 days every 3 months, if you think there i lesser hair loss, extend the Finasteride-free period to 30 days. You will know for sure if your baldnes is getting stable and it is the time to lower the dosage.

    I'm 48 and have been slowly losing my hair over the past 13 years, I have recently started micro needling once a week, applying rosemary oil and changed my shampoo to Nizoral. I will start taking Finestride from July (after graduation pi tures). What dose do u recommend I start with?

    Hello Dr. Bhatti, thanks so much for all the valuable information around the subject and science of hair loss. I'm a father of a 19-year-old who's recently showed signs of diffuse thinning. He's been on 0.25mg of Finasteride for 3 months. We just upped his dosage to 0.5mg daily. He's shown zero signs of side effects to this point. I have a few questions I was hoping your could answer: 1) Should we move him to 1mg daily or is 0.5mg the sweet spot, or should we do 1mg every other day? 2) What time should he take the medication if he sleeps from 2am to 11am? 3) He's physically active and lifts weights, is this negative in the hair loss battle? 4) Is additional protein supplementation a negative? I really appreciate your help as we're just starting the process.

    I have a question. I started Finasteride at 0.5mg dosage daily, I had no side effects except my semen was watery (water like thin), I stopped Finasteride and the issue resolved within 24 hours. Now I'm taking 0.5mg every other day and no such issues what so ever. Do you think that watery semen issue resolves overtime? Thank you!

    Yes this issue resolves with time. But remember t just takes one sperm to fertilise an egg. Get your semen analysis done now that you have stopped and then 3 months after restarting Finasteride. That will be an objetive study for yourself.

    Hello Dr. Bhatti. I find your channel very informative and I subscribed to it. I am 53 with severe male pattern baldness; just had a HT a week ago and received around 4500 grafts. The transplant clinic in Turkey didn’t recommend Fenasteride at all, but I am concerned about not blocking DHT in the longer term and loosing more hair despite having a HT. What would you recommend I should consider? Thank you sir!

    If your baldness is progressing, no useful purpose will be served by avoiding Oral Finasteride.

    Sir..I saw a graph in the forums that showed that one milligram of finasteride reduced the dht level by 68.7% and half a milligram of finasteride reduced the dht level by 67%, is this true? If the answer is yes, which one is better for hair loss. 0.5mg everyday or 1mg 3 times a week

    Hi doctor, I'm 26 ,I'm fall between NW2 and NW3 and I started fin 1mg alternate days and have been taking in from 8 months and taking a break 15 days for every 3 months. I haven't done my blood tests and as the area above my frontal hairline isn't filling so. I suppose that I should be upgrading it to 1mg daily.     Doctor, after hair transplant hair sheds out during initial months right. How do we know that we are not losing grafts ? I wash my head and apply minoxidil but I see a lot of hair falling out. I’m afraid if I’m losing actual grafts. What are the chances? I have not seen any blood though.

    Shedding is almost routine. You should sit back and relax since at this stage you can do nothing either way- but i you are on Finasteride, no graft or native thinning hair will be lost permanently.

    Why 50% is threshhold? If thickness is less than 50% but shaft length is more than 1", will finasteride not work on this hair? Do 50% lesser thick hair looks like vellus?

    Your doctor would know your baldness better- how stable it is and how likely it is that your hair transplanted were not miniaturised. He might also have evaluated your hormone levels and your psychology and felt you are not the right candidate for initiation on Finasteride. If he has not started you on Finasteride it must be for a reason. Most of my patients take Finastreide 1 mg a day for 6 months after the procedure and then I see if the drug has to be stopped, continued or scaled down. Each person is unique.

    Dutasteride blocs most of the DHT and has a long half life. I would not suggest Dutasteride unless the benefits of Finasteride (the first line of defense) are exhausted.

    Okay so let's say a patient has had a hair transplant and was taking finasteride at 1mg every day who eventually have sexual side effects 2 years later. The patient came off finasteride for several months to clear it out of their system because they did not know what else to do. In the other video you mentioned the protocol of 1mg of finasteride for 90 days followed by 15 days off and repeat. When coming back onto finasteride again would this be the appropriate time to follow this protocol or should they restabilize themselves with finasteride for 6 months before following the 90 days on 15 days off protocol?

    If you have baldness or signs of baldness at an early age, you have to block DHT and there is only one DHT blocker that is effective and safe- Finasteride. I have only one clinic and do not visit places outside Chandigarh.

    If you have miniaturization ie hair thinning even in the donor area, then one can do that. However, please remember to stop finasteride 1 month before your procedure as it becomes difficult to select best quality hair in the anagen phase which would last longer, during harvesting.

    I started taking 1mg daily but almost immediately noticed my breast area felt swollen. Went down to half a pill (.5mg) daily and it is much less bad, but still noticeable. Am I losing out on fighting my hair loss?? For what it’s worth I’m 38 and my hair loss is very minor. Thinning and slight beginnings of temple recession.

    Dear doctor, I am 25 and I am taking dutasteride 0.5 mg on alternate days from last 14 months as prescribed by my doctor. My hairs have become thicker and I am having almost zero hairfall.     Hello Dr, I have really important questions. 1. I start fin at 26 with norwood 3. Balding since 20. I tried fin for 7 months already 3 months 0.25 2 months alternate 0.25 & 0.5 1 months 0.5mg Now, the 7th months I wanted to up the dosage to 1.25mg eod.     2 weeks is too short a time for that. Palpitations are not so common even otherwise with Finasteride. Please get it checked from an internist.

    Grades 6/7 mean you have lost all hairs from the typical areas where baldness occurs. Now the use of Finasteride can only be to better your scalp donor area awaiting a transplant if you are fit for a procedure.

    Yes it may be besides a host of other factors chief of which is evolutionary. Nature does away with an organ that we do not utilise for the purpose it was meant for Hairlines and a head full of hair was meant for controlling body temperature and not for esthesis. When we modulate our environment, God does away with the hair.sd

    Hey doctor I’m 21 and been on 0.5mg daily of Finasteride for 1 month. I’ve had no sexual side effects but I did notice a small lump grow on the left side of my chest. I’ve since stopped medicating and look to return at a lower dose. What dose do you think is most safe and effective? Or what do you recommend I do? I’m planning to clear my system and come back on 0.5mg 3x a week but would love your advice Thanks you

    You should have continued and not stopped. Minor problems abate in a month of two. The body is recalibrating itself in this period.

    0.2 mg is a very low dose. In fact it is ineffective mostly. I have never heard of anyone geting sides with it. You are thinking about this a lot!

    I am 26 years old and the only hair thinning I have is on my crown and it's not too aggressive. I took finasteride, 1mg/day for 12 days and experience sexual side effects and even woke up on the 12th day with fever sweats. Is this normal? I stopped taking finasteride because of this and maybe a day or two later I didn't have any sexual side effects except for a decrease in the amount of semen ejaculation. After about a week of not taking finasteride, everything went back to normal. Would it be wise for me to try to take finasteride 1mg 3 times/week?

    That is too early for a side effect to happen since tissue levels are not high enough. But if you are convinced yourself, please STOP!

    Sir, i am in my 30's. I have been using minoxidil & finasteride 1mg daily for the last 9 years. While i haven't had any regrowth, i have been able to maintain and considerably slow down baldness. However, in the last 6 months i have been taking finasteride 1mg only thrice a week on alternative days. A move which was promoted Owing to long term usage. I would like to know if i am better off with 1 mg daily or on alternative days considering i have been on it for a decade.

    You should start with 0.5 mg on alternate days and then build p in consultation with the doctor and if total testosterone is more than 400 ng/dL

    Doctor As we know that 1 mg dose blocks more or is more effective .... So If baldness started in early 20s then is it wise to start with 1 mg dose of fin. in 20s and early 30s because baldness in early age is dangerous.?? is it also wise for the same person to decrease the dose after mid 30s as baldness stabilises in this age generally ??? *OR WE should take chance and experiment start from lower dose 1st and then gradually increase the dose because our body GET USED TO the dose amount*

    Topical formulation is a bundle of lies. Finasteride does not penetrate the skin in sufficient amounts to cause an effect. It is only a placebo effect. Due to commercial interests many placebos are advertised by drug companies as the latest breakthrough- but you have to judge for yourself. Since you have been confused with so much information, you should try it on yourself for 3 months and see.

    my specific question will be to you that is it possible to maintain all the hair on the head despite of having 6 or 7grade genetic history if we start using finesteride at 22years when we have all hair and continue it for the rest of the life..     No it will not help you in retaining hair. 100% DHT is not sought to be blocked while using Finasteride/ Dutasteride.

    Sir i am using finpecia since 3 year before.. after that my hair loss is stable .. no further hair loss.. i am taking 1mg in alternate days(3mg in 7 days) it is work tremendouly till now but I have listen after 5-6 year it's effectiveness ger reduced because of tolerance.. is there any solution to again make it effective...?

    This does not happen in every case. Most do not need to take after 35-40 since the baldness stablises when the gene switches off.

    Yes, definitelyFinasteride does not stop hair loss compleetly, since ot does not block 100% DHT. No other medicine can do this.And it is not good to block all of the DHT the body makes, since DHT has important bodily functions. Yes hair loss accelerates when seasons change.
