How will I know when to stop Finasteride ? Dr Tejinder Bhatti Explains in detail

How do you know when to stop taking Finasteride? No medicine should be taken if it is not needed and so this test is well worth it.
The baldness gene expresses differently in different people. In some it reveals itself in very young age and in others in middle age. Similarly there are phases when it becomes dormant or goes to sleep.
So when you are in the remission phase, you can take a Finasteride break. But how will you know when to go off the drug?
after I published my last video titled- “When will my baldness gene switch off!”
People keep taking medication and that is a good thing since you are sure that you are doing the best you can.
But there are some who are uncomfortable taking medications. For them they wish to know if there is a likelihood of their discontinuing if the baldness has slowed down, stabilised or as more eloquently or dramatically put- “switch off”!
However, going ‘cold turkey’ when the gene is still active can have ill effects.
As we all know Finasteride blocks only 73% DHT and hence hair fall will continue. Whether your baldness has stabilised or you are on Finasteride hair fall continues. Shedding is actually natural and the hair cycle will make you lose 100 hairs a day and 100 will grow back. This should not be cause for worry.
But when you start to notice that you have far less hair fall than before, it is time to take a test. To test if the baldness gene has switched off.
Like most of my patients, I am sure you too stop Finasteride for 15 days after every 3 months of taking it religiously. If that be the case, extend this period to 4 weeks and see if hair fall is excessive. If not, prolong this to 6 weeks and then if all is still well, to 8 weeks.
Finasteride is flushed out of the body in maximum 60 days or 8 weeks after stopping it.
If the hair fall is the same as before and you feel, stopping has not made a difference, it is time to take a break since your baldness gene is now hibernating.
I do recommend it.
But always remember that the gene may hibernate or go to sleep for a while, but never really switches off. But after the age of 35 till you are 55, there is in most patients a respite from the relentless onslaught of genetic baldness.
This window period is the time you can consider to be off medication if you must.

    Hello Dr, I’ve had a transplant in July 21 and been 1 mg finasteride for the last one year I have no significant hair fall . I had got all the tests mentioned in your video done before starting finasteride. I’m 39 years old no side effects as such. Should I continue taking finasteride or taper down the dosage or stop it.

    @@hairtransplantinindia Hello Dr, Ive been on 1mg finasteride last one year, i got all the tests done mentioned in your video before. the test results as on 11/7/22 is Testosterone 547.25 ng/dL, free testosterone 68 pg/ml(normal range is 1-28), SHBG is 41.1 nmol/L, ESTRADIOL/OESTROGEN(E2) is 27 pg/ml. Is high levels of free testosterone ok or should i be concerned about anything.

    @@amithaiyappa1 Tests are not relevant at this stage. You should stop the drug for 30 days and see if stopping has not started your hair to shed. That is the final test that your hair loss is now stable and you ought not to be on medication for it.

    Sounds great. I didn't quite get the last part of the video though: when you said "taking advantage of this period between 35 and 55 years of age", did you mean going off the drug for as long as possible because it may be temporarily not needed or actually continue in low dosage to get maxium benefits? I.e. better regrowth with low dosage thanks to sleeping balding gene or half-asleep balding gene.

    Hi Doctor, i see that you suggest to take a longer break to test of the gene has switched off, will we be loosing the time just chekcing if it has switched or now. or is just fine? and how will we know if DHT is still atttacking ? will we see hair thinning? we might have to wait for longer period to see the hair thinning again ang that might cause to loss the time period. thanks

    If hair does not bein to fall off suddenly in this period that you take a break, you can take longer periods off. The gene is getting dormant.

    Thanks for the info Dr. As you pointed out that Finastride can curb upto 73% DHT only, is there any way to further arrest it to higher degree? I am on Finax 1 mg twice a week alongwith Minoxidil 5%, even few Vitamins. Please suggest if DHT blocker can further be arrested l?

    Dutasteride blocks over 90% I believe, as the doctor has said taking dutasteride oncer or twice per week and finasteride on the other days seems to work very well.

    Thanks for your dedication and continuous insight. I think most of the sexual side effects from Finasteride is not real and similar to placebo! What do you think about Gyno from Finasteride? These are physical and hard to deny. Have any of your patients had it?

    Most cases with gynecomazia while patient is on Finasteride occur in those who have obesity or are overweight. Their estradiol level is already high and when they commence on Finasteride, it makes matters worse. However, most people have pre existing genetic predilection to gynecomastia. But if you have gynecomazia and other side effetcs attributable to Finastreide, do stop the drug. Health is more important than hair.

I have a query here that I have thinning of hair and on a regular day when I comb my hair I don't see much hair falling off but when I apply oil on to my hair before taking a shower I can see close to 10 or 15 strands in my palm.. it is something common?     hair loss is normal- that is due to the hair cycle.Only when we lose more than 90-110 hairs a day is it a cause for alarm.

    No I do not prescribe it since topical Finasteride formulations have not yet achieved the capability to reach the follicle directly without being absorbed into the blood stream

    hi, i am taking fin from 2017, and 4 years was great!, but when i combined ssri so my sex drive was gone. i always thought is was the ssri so i stopper before 5 months ago and still has no sex drive. so i did a blood test and found high progesterone. is it can happen that side effects on finasteride oocurs just after 4,5 years?

    Hello Doctor! I recently started watching your videos and they are helpful! I want to mention that I have aggressive hair loss, which started from the age of 16. I am now 21. I am 20 months into oral Finasteride and I am using minoxidil as well. The thing is that my dht blood levels reversed back to my baseline levels. When I started taking Finasteride, my internal DHT levels were being reduced constantly about 44%, but now as I said, they are back into the baseline. Also, it’s been 5-6 months that I am losing my density again which Finasteride gave me. So, definitely I started seeing a decline into my results. I am thinking of adding Dutasteride once a week. Could it be helpful? Thank you very much.

    @@cloudstrife2396 Firstly, there is no correlation of DHT levels with baldness. Baldness of anrogenetic origin is not due to mounting DHT levels but due to DHT receptors' sensitivity to DHT. You should continue with Tab Finastreide 1 mg a day. I will not advocate Dutasteride at this age. Are you taking whey protein? Is your lifestyle healthy?

Could my body formed a situation of resistance phenomenon, or my aggressive genes are counter attacking me? No whey protein. Diet is clean, and I am trying to fix some deficiencies, like Vit D3.     Hi Doctor, I was on propecia for 16 months and stopped my since mid Sept 2020. I was on 1mg daily for a year then 4 times a week the last four month. The reason I stopped was because my hair was getting wispy after being on it. Since then, I think my crown has lost density about 4 months ago. And the last week I’ve been dropping more than usual. Should I get back on it?

    Am I correct in understanding your recommendation as follows: Should individuals who tolerate Finasteride well consider staying on it even between the ages of 35-55 were the genes could potentially be dormant? Alternatively, are you suggesting that if one no longer notices any hair loss, they should gradually discontinue the medication and assess hair loss after a 6-week period? If significant hair loss occurs during this time, does it indicate that their genetic predisposition for hair loss is still active, necessitating a return to Finasteride?

    Hi Doctor ... very Thankful for your videos as always but i must say ... how the heck are we going to know if Gene is dormant ... by the time one realises its not , one has lost a significant amount of hair or has worsened on the miniaturisation ... this is very hard to know Doc .. is there a test or blood work that one can diagnose if the bald gene has gone to bed lol ???     I do not think that if your hair loss is not stable yet, you should go for a hair transplant without the cover of Oral Finasteride. Doctors fear prescribing it so they do not lose a client.

    Yes there are some people who do not respond to Finasteride. Either their baldness is too aggressive or they have been using it for along time.

    Hello Doctor. I am turning 60 soon. My hair has been slowly thinning for some years. What is your opinion in regards to older men taking finasteride. Is it still effective? Would you recommend starting treatment at a reduced dosage? Notably, my erections are not as rock hard as they were when was young so I'm concerned that this may predispose me to sexual side effects.

    Sexual side effects are not common with Finasteride. You have the option of stopping it if that happens, Yes it will work in your case if what you have is male pattern baldness.

    Finasteride is prescribed to only those women who have attained menopause as testosterone levels increase after menopause. A higher dose of finasteride is used for such women as they usually do not report any side effects.

    Hello Doctor , I've been taking 0.5 Alternate days finasteride as advised by you ( through online consultation) since march. I got covid in april and after 2 months recovery i started losing excessive hair from all over the scalp. I started taking 1mg on alternate days from july and in August i started full dose of 1mg per day. Is 1mg too much? I'm loosing less hair from top of scalp and more from back area. I think shedding has slowed down somewhat (like less tha 100 hair per day). Should i continue this doseage? I'm also apply minoxidil 5% at night as advised by you. I'm 28 and NW2.5-NW 3.

    No, there is no regrowth of lost hair. However, progression of baldness stops and by having oral finasteride or by applying topical minoxidil the hair becomes thicker.

    Not they do not in the majority. But if it does you can either increase to 1 mg a day or add Dutasteride once a week in consultation with your treating doctor.

    Here he mentions "falling prey to its'(finasteride) many side effects" at the end, and we should take advantage of when the gene is "dormant". He was criticizing in a video to people who take half a dose everyday to avoid side effects, saying side effect is not real, or very few people have it. So which one is it doc?

    Side effects in a very small minority are real. I never say that they aren't. In these people I suggest they go off the drug since health is more important than health. However I keep exhorting people to keep their faith in the drug since most cases are psychologically high strung when they start Finasteride due to negative press. There is no alternative to oral Fiansterde yet and I therefore wish well for my audiences to explain to the the nuances while indulging in DHT blockers. Thanks for watching.

    Most likely it is androgenetic alopecia. Finasteride is not recommended for use for those below 18 years. I do not recommend it before 21. Since in the development phase of your growth, DHT has an imortant role to play. Just use 5% Minoxidil and take your mind off hair loss and live a healthy lifestyle.

    Initially I was taking finastride regularly, but I experienced erectile dysfunction and premature ejaculation. Then I started taking it on alternate days almost for a year and I did not get side effects as mentioned above . Now I m again taking finastride 1 mg regularly and I don't experience any side effect now , except adult type acne which I think it is bcz of finastride.

    Hi Doctor, I recently came across your very informative channel. I am 21 and I started taking finasteride 1 year back 1 mg per day after 3-4 months my hair loss was controlled and I switched the dosage to 4 days a week. After several months, I was experiencing no hair loss that is why I took a break form it for about 1 month and after 2 weeks the hair fall started again.     Here's the one thing I wanted to comment on. When you talk about hair shedding and to know when the switching off is taking place. Finasteride will work in different ways and it will grow non-terminal hairs but they're not velus (peach fuzz) either. They are somewhere in between and they only grow to a certain length. Maybe like an inch and a half. But my question is are we talking about those types of hairs when they start disappearing that we know that we should be back on finasteride or or are we talking about the terminal hairs that grow indefinitely in length? What I am saying, is that one of the medications, minoxidil or Finasteride, have indeed woke up some of the miniaturized hairs along the entire perimeter. (Personal experience) And while they do "not" grow past an inch and a half and the diameter doesn't quite get to the terminal hair diameter, there "is" some cosmetic benefit. And having said that, I was wondering if you are familiar with this and if you are, which medication do you give the credit to? Thank you!

    @@rickr2029 Yes this hapens in all. But since this hair is not combable, patients do not give much importance to it. Since these hairs are unkempt I have seen many patients shaving them.

    The very fact you grew hair back means you have anrogenetic alopecia. To confirm, you can stop medications for 60 days. The effect lasts as long as you keep taking the medications.

    @@abhijeetkatya1377 No hair that are lost can be regained by any treatment. Treatments only thicken thinning hairs and that too as long as you are taking the treatment.

    In my case it is not genetic or inherited. Because not a single person in my family or ancestors have any kind of baldness. That's why I am curious to know.

    I’ve been on 1mg finasteride daily for 18 years. Hair loss stabilized since first year. Now find it hard to lose weight in chest and belly areas even with exercise. Would switching to 1mg 3x a week help with that without affecting hair?

    Do you have patients who don't take breaks from Finasteride for 2 weeks after 3 months of use? I have been on 1mg for 2.5 years, I have not taken any breaks from Finasteride. Zero side effects(23 year old male). Also I feel it is too risky for male patients who are in early 20s with strong family history of grade 7 pattern on both maternal and paternal sides, to take experiment taking breaks on this drug despite stabilization while taking finasteride. What do you think about this doctor??

    A fortnight off Finasteride every 3 months has not been seen to be detrimental even in the most aggressive cases who are young. But if you do not fear or have side effects you need not take a break The break is only for the weak-hearted and for those with genuine sides!

    Sir I am taking finasteride 1mg since 70days(as I have family history of hairloss) still seeing hair shedding how long it takes to work ,I am 23 now and in grade 2-3 will it stop my existing hair from falling off? Planning of transplant within 2years should I continue it till 2 long years? Haven't seen any side affects till yet!!

    Yes Finasteride is the right way forward when you have thinning and have a strong family history. A stitch in time saves nine! Once you are 25, a hair transplant can be done to restore the hairline.

    Sir,.. Propronolol induced hair loss, is it perment or temporary,,, actually I hv been using mixidil and finax past 7yrs... Recently I hv taken propronolol for my anxiety, after taking that, I observed heavy loss, so I stopped it on fear of hair loss,. Please, guide me regarding this issue... Could I continue propronolol, what measures I should take to prevent loss

    It just reduces the length of the anagen phase. Hair loss is not permanent with Propranolol.

    Hi doctor You say that topical finastride isn't effective but atleast does it work 50% or the least 30% on scalp? Because I have seen many topical minoxidil mixed with finastride

    Those are commercial peparations to make money. hair loss industry is full of such treatments. Treatments come and go. No, topical DHT blockers do not work even 2 %

    Doc , there are clear benefits of FUT apparently. For example - 1. More grafts over a lifetime ,2. More likely to be able to extract hair follicles which have more likelihood of immunity to DHT etc. With all due respect why don't you provide FUT service? Genuinely curious.

    Hello doctor. I'm a 32 year old male. 12 years on finax + minoxidil+follihair. Diffuse pattern grade ~3.5. I'm having serious urinary side effects on finasteride now- burning sensation during urination, weak erection, abdominal tightness+ persistent pain. These side effects go away within a week of stopping finasteride. I've tried various dosages as low as 0.25 mg/day but side effects slowly creepy back within a couple of weeks. Most doctors I've consulted are unaware of finasteride causing these side-effects and refuse a transplant because hair are too close. I have fine brown less dense Caucasian type hair. I also have extensive familial history with both my parents having high grades of hairloss and I've inherented patterns of both male and female pattern hairloss i.e. my sides are thinning as well as my crown. I'm scared of completely going off finasteride because I know I'll lose a lot of hair very fast but fhe the side effects have become intolerable now. Kindly advise 🙏🏻

    Nicotine touching the grafts is not deleterious. However prolonged passive smoking is. Stay away till 6 months are over.

    Initially there will be shedding if you have androgenic alopecia. Shedding finishes by 4-6 weeks and then results start to kick in by 6 months mark.

    Hi, I took the 1mg finasteride tablet for two days, which makes it 2mg taken in total. Then I stopped taking it on the third day due to decreased libido ( the last tablet was taken five days ago) However, I am a healthy 22 year-old male and I’m sure that I did not have any sexual dysfunctions the day before I took the first tablet. Therefore, I’m not sure if it is the nocebo effect that causes me sexual side effects or it is that the drug actually brings me side effects. If it IS because of the drug, how long does it take for my body to wash out the amount ( 2mg in total ) of finasteride I took and when should the side effects go away? Extremely anxious right now

    This is unbelievable- ED after 2 doses. You should stay away from the drug if you are so anxious that the drug did that to you.

    Yes, the mind really affects the physical performance. It is well documented in many psych literature (l.e. there are E.D. patients that undergo tests when physically they are normal, meanwhile the doctors realize that there are fights in the relationship.) Also, you said that you are OCD or close to that. I would advise you to really be careful with anything you are taking and always consult a good doctor, no matter if he/she is expensive or not. I am sure the doctor in this video will agree with everything I said.
