Is DHT the ONLY cause of male pattern baldness ? | What is the DHT Paradox?

Is DHT the only cause of baldness?-Well, evidence suggests that DHT may not be the only cause of male pattern baldness!
Questions & Skepticism abound regarding the exact role of DHT.
In this video I discuss the many Paradoxes of Male Pattern Alopecia and of the role of DHT in causing androgenetic alopecia.
Male pattern baldness or androgenetic alopecia besides being a scourge for male vanity, is often an insurmountable challenge to the treating surgeon. Despite this and after decades of research we still do not know all about why some people get bald and others do not!
Once affected by male pattern baldness, hair follicles begin to shrink with each passing cycle. This miniaturization, or conversion of terminal hairs to vellus, and reduction of anagen to telogen ratio leads to excessive hair loss and in time balding. Eventually bald areas appear which slowly become slick bald and then progress to cover larger areas which outstrip availability of donor grafts.
Despite the fact that its prevalence is 80% of men and 50% of women throughout a lifetime, and that it has been extensively studied and researched over many decades, we have still not built a coherent pathology model that would logically and scientifically prove its precursors, biological step-processes, and physiological responses.
Though theories about male pattern baldness and counter arguments of pathophysiology abound, these 4 facts are unchallenged:
• 5-alpha reductase enzyme activity increases in the scalp of the patient with androgenetic alopecia. Males with a genetic deficiency of 5-αR2 never develop baldness
• Increased levels of DHT are detected in the scalp
• There is an increase in DHT receptors in the scalp
• Inhibiting the conversion of testosterone to DHT helps in reigning in androgenetic alopecia
Consensus supports a genetic, and androgen-mediated causation mechanism as responsible for male pattern baldness. It is today widely accepted that DHT, which is the strongest form of the androgen testosterone is the reason for miniaturization or shrinkage of hair follicles in male pattern baldness.
But this Theory is riddled with valid counter-arguments and abound.
Those who support this Genetic Theory that hovers around the primary role of DHT, can help answering these 7 questions that continue to confound:
1. What causes DHT to increase in tissues prone to baldness?
2. How does DHT miniaturize baldness-prone hair follicles?
3. Why is DHT also responsible for hair growth as part of the development of secondary sexual characteristics in males?
4. Pre-pubertal castrates undergo a permanent 95% reduction in endogenous androgen production and never develop male pattern baldness later in life. But why does castration not reverse male pattern baldness in post-pubertal castrates. Why does it merely stop it’s progression?
5. Is there a correlation between DHT and tissue remodeling that goes hand in hand alongside onset of male pattern baldness?
6. Why do identical twins have dissimilar balding patterns and speed of baldness. Does epigenetics also play a role?
7. If the incidence of baldness increases with aging, how do you explain this in the setting of lower androgen production in this age group?

    Does cupping (hijjama) have any benificial role with regarding concentration of dht in scalp,does this therapy extracts it, and help to regrow hairs?

    Will natural supplements like saw palmetto and pumpkin seed oil capsules for blocking of dht work along with ketoconazole and minoxidil as i faced a lot of sides on finasteride mainly sexual and mental side effects. Would you prefer some additional medication that could be used off label apart from ketoconazole and minoxidil incase the patient is susceptible to side effects from finasteride? Would taking finasteride topically reduce the sides or would taking it 2-3 times a week at low doses help. Of course i would also take the medication after talking with my dermatologist, just that it would be nice to hear the opinion of somebody else.

    I was just prescribed finasteride and minoxidil for my androgenic alopecia, I told my Dr that I didn't want too really take minoxidil. But he recommended I use it for a few months until the finasteride actually starts to enter my system. Would it be alright if I took minoxidil for 2-4 months and stopped? Or would I be okay just waiting for the finasteride to kick in? I'm worried about the shedding phase with minoxidil and also experiencing more hair loss after getting off minoxidil. I'm about a norwood 2-3 with thinning on the crown

    Each has a different mechanism of action. Hair maintained by Minoxidil will shed once you stop taking it.

    What is your views on' gravity theory" that says on top of head due to gravity blood can not reach in proper amount so top portion looses hair while side portion not

    Perhaps it's not gravity, but circulation itself is to blame. Blood flow to the top regions is just weaker or constricted.

    Sir, I am a bit confused. I am experiencing hair fall from the last six months(My hairline is perfect but from top there is a lot of hair fall). I got some blood tests done and got to know that I have Sub-clinical Hyperthyroidism(TSH: 0.45) and Vitamin-D deficiency (14 Ng/mL). Adding to this, my father is bald. Now can you please help me to figure out, if I am having Androgenetic Alopecia or is it just due to Vitamin deficiency?

    Most people lose 50% of their hair before they realize they are balding, if you see the hair do something now because the sooner you act the longer you will keep it. Ketoconazole 2% shampoo, Finasteride, and micro needling are a good regimen for early hair loss. Collagen peptide protein powder doesn't hurt either...

    Hi, sir. In the past one month, I’ve visited two dermatologist in two different cities; one of them said that the reason for my hair loss is Androgentic Alopecia, while the other is attributing it to Telogen Effluvium. When I told the him that the other doctor had said that I was suffering from Androgentic Alopecia and I also insisted that I’ve Alopecia, he simply questioned whether he’s a doctor or I’m. I’m very confused and in dilemma as he had asked me to stop taking finasteride which had been working well as I don’t witness a single hair in my comb anymore.

    If you don't have side effects then I'd keep taking it. Been on it for years myself, it's got a long halflife so you can actually take it every other day and get just about the same results.

    Hi sir. I can see hair thinning on my crown area, it could be dht or something else. But consuming finasteride can have side effect as suggested by a doctor. So is there any other medication i can take without any side effects. And how effective is minoxidil in treating it?

    Hi I am a plastic surgeon ho deals only with androgenetic alopecia. I believe that diet does not play a role in hair loss due to genetic causes. If it does it is only supplements and other medications that increase DHT.

    Doctor i heard that minoxidil not only makes the thinning hair addictive to the drug but also the good hairs as well . As a result when we stop using minoxidil the good hairs also fall of . Is it true ?

    The hair that are not balding will not be affected. Only the thinning hairs that are so maintained by Minoxidil will revert to the condition in which they would have been at that time without Minoxidil.

    Hello sir, I have been on dutasteride for 1.5 years.I am 23 years old now. Although, I haven't found any hair regrowth till now but it certainly helped me in stopping the further hair loss. However, during this 1.5 years, I have sometimes experienced significant hairfall for short periods of time. This short span comes and go on its own. But each time this happens, it makes me worry if the effect of dutasteride is fading away and it is no longer working for me. So my question is, am I experiencing telogen effluvium along with androgenetic alopecia or the dutasteride is no longer working for me? Should I start a treatment for telogen effluvium if that's the case?

    You could simply be having sheds where many hairs have the same cycle life and shed and regrow at the same time. Take monthly pics and save them and you can compare after a shed to 3 months later and see if they came back.

    I was also on dutasteride for 1.5 years and produced no sperm just 1 drop in the end. And it thinned my hair in the crown rather fast or kept thinning at normal speed. Dutasteride is the Devil.

    They need to be on immunosuppresants for a few months after so the organs do not get rejected and need to be hospitalised. I do not know anyone who will be prepared to do so.

    You will benefit by hair loss medications in consultation with a local doctor. these are all prescription medicines and cannot be prescribed online. I can only give information bu cannot ascertain if they will be good for you since I cannot examine or consult with you.

    Doc... Looks like eating eggs can cause baldness, now I don't eat eggs for more than a week and any other foods that has eggs in it.. I notice that there are some hair starting to regrow in the top of my forehead..

    Hi Dr Bhati, I have been on Finastride (2.5mg/day) for over an year now after the transplant and no improvement so far, the hair in my transplanted area grew but the density is still very very less and infact my other area (front top) where i had thick hair before transplant started thinning now. Not sure if all the transplanted hair really grew or just a few percentage did. But the real question here is that me being a vegetarian (lactose intolerant) what is it that i need to eat while exercising. Heard that Whey protein could increase the rate of baldness, so started using pea protein but no difference. How about Creatine? Can i use it for strength training. What is it that i should really do to stop hair loss in addition to taking finastride and multi vitamins?

    Your mistake has been you figured Minoxidil is an alternative to Finasteride. It isn't! Minoxidil is not a DHT blocker so how can you expect it will retain hair in androgenetic alopecia.

    Sorry for the error. Any unnatural protein pack should be resisted when you have the AA gene. Also your Finasteride dosage is high. Have you heard of reflex hyperandrogenecity? See my video.

    Sir as u mentioned 5 alpha reductase activity is main reason for baldness, which is also a reason for chronic prostate inflammation and Benign prostatic hyperplasia in future.So if a person who just begun receding hair line, if he do dietary and life style change like a bph patient, does he get benefited for hair regrowth too.?     Oiliness is an inherent quality. It can also increase with increase in androgen levels. Not due to miniaturisation.

    Hi, I found your video very interesting. Lately there has a been a theory going around called “Scalp tension theory” that scalp tension mediated by scalp perimeter muscles around the galea aponeurotica can cause hair follicles to miniaturize and exacerbate Dht in the scalp. The reasoning behind this is that the “pattern” of hairloss correlates with the insertion of these perimeter mucles namely the occipito- frontalis, and temporalis muscles. I want to know what your opinion on this is and if it is a viable alterantive explanation. Thank you so much doctor.

    Also regarding this DHT paradox, do you think it is possible that hypoxia plays a role in baldness and could it be that some environmental factor like hypoxia can cause DHT to elicit a different reaction in scalps than from beard follicles? I say this because minoxidil seems to grow more hair than finasteride which makes me wonder if blood flow and oxygen could be the root cause.

    Minoxidil and Finasteride have different mechanisms of action. Though Minoxidil can grow hair very well in the right hair loss sufferer, unless DHT is taken the purpose of starting on hair loss medications is defeated if the baldness is progressive. The exact science behind hair loss is still not 100% discoled to us.

    Sir one trichologist said as we age strain in scalp occurs and hair follicles get injured by this strain, and to fill this injury dht act like antiseptic and bind to that to fill injury As a result follicles miniturize Back and side of head does not have such strain so they do not mintuirized

    Best is to give it a try- using only Finasteride. Omitting Finasteride is not advised since you know the answer- most hair so sustained will fall off and never grow back.

    Stress triggers the HPA axis. Stress can surely caise hair loss but all of it will recover if the gene of baldness is lacking. However those who carry the gene will not find the hair lost growing back completely. Stress has an effect on all biological systems and the hair follicle is not an exception- in fact it is the first to show!

    DHI is better than generic FUE due to several reasons. I myself use a modification of this method- the Anagen Q+ technique.

    Great vidoe! I'm going crazy when I don't have answers to these questions but all we can do is hope for research to come out and answer some of them. Btw I had another question. When trying to grow a beard using minoxidil, will your growth be negatively impacted if you're on oral finasteride? I do know that Type 1 5AR which is mostly present on the skin is not blocked by finasteride so does that mean finasteride doesn't harm beardgrowth? Or does the beard hair follicle still contain type 2 5AR which are blocked by finasteride and so it would be affected?

    Beard hair is determined by DHT at a young ae before you reach the age of 21. After that no medication will grow beard hair. Only thin hair can be made thicker by using Minoxidil which is a vasodilator and works by getting more blood flow to the beard area. Finasteride taken after 21 has no role in beard hair growth.

    Topical Finasteride is of no use. If you apply Minoxidil 12 hours after the previous application of any greasy product after washing it off, there is no interference whatsoever.

    Maybe because heat mainly escape from the top of the head. Therefore there can be a decrease in certain nutrition through sweat or other things.

    Sir, i had lost many hairs from 16-20. 6 months back my doctor prescribed me minoxidil and finasteride. I have been taking it from past 6 months but now again my hairs are falling out. It seems finasteride and minoxidil stopped working for me so my doctor prescribed me dutasteride. Is it ok to take it?

    Sir,, I am loosing healthy hairs during head wash,,, If DHT would have been the reason for their fall then they would have miniaturised first and then fallen off,, but that is not the case,, sir, can that be a case also where healthy thick hairs fall due to DHT?

    No they will not fall like that. This could be breakage of hair or telogen effluvium.

    hey doc, I am 34 and I took first pill of finasteride, but I missed the second day as I got so scared watching all the videos. I want someone to tell me no major side effects will happen as I want to make use of finasteride. I love my beard as well, if not make it better I cannot afford to lose my beard as if someday i plan to shave my head, at least i can rock my beard

    side effects are rare, by now you probably already know... And if you have them cut the dosage and see if they go away, Finasteride has a long half life, Dutasteride even longer so you can get away with 1mg every other day and even less has been shown to be just about as effective as the 1.25 quarter tab of Proscar.

    Sir fantastic knowledge . Sir please help me out i am on minoxidil prp and derma roller more ove i have got a hair transplant done 3 years back i am trying to save my existing hairs but with these treatments i have not stopped my hair fall now i want to switch to finasteride now my wife is pregnant it is her ist month pregnancy shoul i go for finasteride or not as it causes problems in male foetus but the conception has been done please guide me sir i am extremely confused

    You have taken the absolute right decision to start Finasteride at the appropriate time after your wife became pregnant. There is no harm at all in starting the drug now but keep the tablets away so she does not handle them by mistake.

    The build up of DHT in those areas is probably the body’s response in an attempt to desperately hold onto the hair! There has to be another mechanism. Especially when you see thinning and balding from the nape of the neck which nobody miraculously has an answer for.

    I used dutasteride for 1.5 years hair kept balding as normally it didnt thicken up a single hair on my head. My sperm was reduced to 1 drop too. Tried finasteride alone for 4 months hair kept thinning and gave me retrograde ejaculation. I dont have aggressive baldness been balding for over 10 years and still got alot of hair left. I dont think dht is my main problem. I also tried oral minoxidill for 5 months it never worked.

    Yes you will have some hair fall but not that it will be alarming.But why would you not like to continue Minoxidil?

    Sir, I am 32 n have full head of hair like i had in my 20s but my hair is becoming thinner now in part of scalp where we say it's DHT sensative. I had thicker hair last year when i suffered from Corona in May. How do i know that it's bcz of dht or genetics. I don't suffer from hair loss, daily may be i am shedding max to max 20-25 hairs n i am saying this bcz i check pillow, in shower, while combing. I am observing it from last six months my scalp that if there is bald patches but it's same. My father started balding in his 20s. While my dada n bade papa had full head of hait What do you suggest?

    It is most likely genetic. If you are an otherwise healthy fit person, you can get a thyroid profile done. Are you taking any supplements?

    @@hairtransplantinindia thanks for your reply sir. One more thing that My mother side nana, mom and 3 mamas have full head of hair while three are bald. I lost alot of weight in Corona, i will get thyroid test done though i am slim and trim. Yes i was taking supplements from 2018 to 2019s end. Took supplements for two year basically whey protein but last year stopped as gym closed and since then i didnt take. I got checked a dermatologist in Bhopal he said you're suffering from telogen effilum as you he did pull test and told me you lost hair from all the sides of scalp after Corona. He examined my a year ago photos from May 2020 and i suffered corona July end and since the November i got shedding even before that. He suggested me minoxidil to take telogen hair into anagen face but i didn't take as it's lifetime commitment. It's been almost 1.5 month i am taking biotin and vitamins, have seen slightly improvement. He said it takes 6-12 month from recovering through TE. As doctor said healthy hair grows half inch a month and my hair is growing the same length, also i checked my hair didn't lose its pigment they are black and I have white hair too since the age of 16. I thought i should take your opinion as you're expert and i have dream to meet a great doctor like you.

    Sir it's purely genetics.. if your Genes will lead you to hairloss then lifestyle, food consumption, stress etc. Will lead to hairfall but will grow back for those who aren't inherited with bald Genes while others with that gene are bound to lose hair quickly

    I feel that genetically predisposed also called genetically inheritance of baldness is more responsible such as maternal and paternal sides of baldness pattern more responsible ,rather than DHT (androgenetic alopecia )being the only cause of baldness in men who have no genetically predisposed history of baldness and still getting bald ..….

    It has to be the receptors, top of the head follicles have them and sides don't. If you take hair from donor areas and put them in the high dht level area of the bald scalp they don't fall out so it's not the region it's the receptors or lack thereof in each hair follicle, at least that's where logic points until science can prove otherwise. The pubic areas don't have the receptors the balding areas do either, that's why when you transplant a beard hair onto the top of the scalp it doesn't miniaturize and fall out.

    Yes, the longevity hair depends on DHT receptors in hair follicles which are more sensitive to the action of DHT in the front than in the safe donor area. But balding does not depend on the DHT levels of a particular region of scalp. Instead, it depends on the sensitivity of the DHT receptors of hair follicles in a certain donor area for eg hair in the beard is more resistant to the action of DHT on the follicles.

    Because genetics is not predictable. One sibling is bald and the other is not. 80% of baldness comes from the males on your mother's side of the family tree!

    Hi, sir. In the past one month, I’ve visited two dermatologist in two different cities; one of them said that the reason for my hair loss is Androgentic Alopecia, while the other is attributing it to Telogen Effluvium. When I told the him that the other doctor had said that I was suffering from Androgentic Alopecia and I also insisted that I’ve Alopecia, he simply questioned whether he’s a doctor or I’m. I’m very confused and in dilemma as he had asked me to stop taking finasteride which had been working well as I don’t witness a single hair in my comb anymore.

Yes, a pattern like M is gradually forming which had started from the temples back in 2017; and relatively speaking, it’s still not as worse as I’ve seen in the cases of other people with Androgentic Alopecia, but yes M pattern is forming. As for the entire head, I don’t know I feel like my all hair has lost the shine it used to have. They’re not as black anymore as they were in the past, and are somewhat brownish. Both of the doctor are quite aged and thus experienced, I don’t know what i should do. Also, my maternal uncles all have Androgentic Alopecia as they’re fully bald from vertex, temples, and front.
