Dr Jordan Shallow DC, at Muscle Doc: Integrative Performance Therapy talks about the three important keys to reducing the likelihood of getting shoulder impingement.
1) Decreasing approximation of the glenohumeral joint by stretching prime movers.
2) Decrease static internal rotation
3) Decrease scapular elevation and protraction.

    Nice information! I currently am working with the Olympics weightlifting team as their sports medicine provider (ATC) and can concur with you! It's also nice to watch your videos as review and help some of the Chinese doctors learn as well. Keep it up!

    Thanks for the video man. I wish I could find a doctor like yourself around the New York City area. I've been a weight lifter for over 25 years I am constantly struggling with shoulder issues in spite of doing a lot of rotator cuff strengthening work.

    Hey doc I noticed they're a lot of exercises to retract the shoulder blade. Could you make a video on good exercises for shoulder depression. Maybe scapular pull-ups?

    What about anterior impingment of the LHB tendon? My tendonosis has been there for 3 months and it was due over-straining or stretching the tendon doing an elbow body-weight move on the pole. The movement rotates the shoulder inwards. What is your take on hanging passively from a bar in that case?

    Do you have any examples of the stretches for prime movers? Good video explaining everything but you don't show how to actually fix the problem.

    Does this all still apply if you only experience symptoms with over head movements, mostly in the external rotation position?

    Thanks for the video. Question: should you shrug at the top of overhead presses or keep traps depressed throughout? I've been reading conflicting advice on this issue concerning the risk of shoulder impingement from OHP exercises. Thanks!

    Dont shrug. Currently working as an athletic trainer with the Olympic weightlifting team for China. Dont shrug, it can cause disruption with the scapular rhythm and movement pattern. Shoulder flexion utilizes the lower trapezius in order to help fight against scapular tilting and aids in the ROM lifting the arms up

    Great info! I have a quick question for ya... If I lie face down on a flat bench and try to press over head(I have been using a 5lb plate) I feel a severe weakness at full extension and a slight pain in the shoulder. Where do you think this is coming from and what is a good exercise to fix? Thanks in advance!

    Where is the pain located ? If you're face down and pressing over head then your isometrically activating externally rotation of the shoulder . If the pain is felt around the back of the shoulder like a "pinching " feeling it could be posterior capsule tension aka posterior entrapment of the shoulder . OR you could have some scapular tracking/ stability issues. Try the rotator cuff progression in my videos as well as the lacross ball myofascial release and see if that helps!

    +The Muscle Doc when I reach full extension, it comes from the area of the AC joint, but seems to be deeper in the tissue so I don't think it's the AC joint itself.

    The problem is that i feel the impingement and the nerve pinch when i retract and depress the scapula. what to do?

    Try stretching your rhomboids. Clasp your hands together on front of your body while pulling forward. You can move your arms up and down changing the angle of pull and sensation of stretch. Also, try using a lacross ball to help mobilize your thoracic joints

    Chris Duffin believes that mace swinging was crucial for him to heal his shoulders. Do you think that club or mace swinging could help some people with shoulder impingement issues?

    hangonsnoop I anticipate that a guy who sells a Mace like product would advocate its use . It depends on what's causing the impingement. Is it primary due to bad posture/ improper loading . Or Is it secondary to some structural pathology?

    heat! bec it allows more blood flow. Ice is just something you apply at the very beginning to prevent swelling.
