Dr. Freedman also has an in depth article on Exocrine Pancreatic Insufficiency written with Dr. Struyvenberg that anyone with EPI should read. Another really helpful resource was the book Primal Pancreas.

    I was diagnosed with EPI after a low fecal elastase test (72) last year, and I have been on PERT since then. I had been suffering from chronic IBS, diarrhea, weight loss, bloating, abdominal pain, and the PERT (Zenpep) seems to be helping with these symptoms, although I still have trouble maintaining weight and have occasional abdominal pain in the right side of my abdomen, below the rib cage and above the navel. I recently had my elastase retested, and my numbers are now above 400. My question is: was I misdiagnosed due to the poor stool quality of the first test, or can I still have EPI even with elastase numbers in the normal range? I've had multiple CTs, an abdominal ultrasound, PET scans and an MRI. No abnormalities were found. I plan to stay on PERT, but I'd sure like to know the hows and whys of my condition. My PHP was unaware of EPI until my case.

    Salam! I am from Lahore, Pakistan. I have been a patient of chronic pancreatitis for the past 16 years due to pancreatic divisum. Multiple endoscopic procedures like EUS, Gastroscopy, ERCP with sphincterotomy and stenting has been done. My gallbladder is surgically removed through laproscopy in 2017 but still there is no pain relief. Now almost 5 months ago the celiac block was done in PKLI hospital, Lahore. In this duration of 5 months i am feeling better because of pain relief but now my pain started again from last 10 days. I am totally exhausted and frustrated from life due to the constant severe abdominal pain. I am using strong opioids painkillers such as toradol, tramadol, nelbuphine and even morphine but there is no pain relief even for a micro second. I am always screaming and crying due to the pain severity. Sometimes, i thought about suicide because i have no more potential to bear this severe abdominal pain of chronic pancreatitis. Doctors advised me to go for a Whipple Surgery with Total Pancreatectomy but they said there is a lot of death risk and we cannot guarantee you the pain relief even after Whipple Surgery with Total Pancreatectomy due to this i am confused about the Whipple Surgery with Total Pancreatectomy.     Thanks for your lecture. I’m amazed how few doctors understand this disease, but most are happy to push drugs that they have limited experience with and not sure of the efficacy. Seems like the cure is available, but there’s no money to be made from it. You have low views so you’re on to something!

    Thanks so much for uploading/making this! I got out of hospital today, diagnosed with acute pancreatitis a couple months back and now its chronic with some necrosis. This has helped me feel reassured and your website is amazing, so much great information! I learned way more in this video than over 2 weeks in hospital, really appreciate it.     ​@@MissionCure i have chronic pancreatitis and epi and my weight is constantly decreasing.in april 2024 iwas 39 kg..in 2022 i was 53. my doctor gave ct scan and bllod test that appears normal but have stone and mpd in head. He said i will not intervene it right now.. he said i think u have bacterial overgrowth which is quite common in chronic pancreatitis patients. So he gave me antibiotic (rifagut) and normaxin..and creon 25k three times a day..my epi level is 4..i gained weight in this two month but only 1 kg..can you please help me how can i gain weight and is this a severe problem?

    Can e.p.i cause dry skin like eczema i have floating oily stools and do local gp test for epi pluss ive realised enzymes over the counter are not strong enough which ones would u recommend?

Additionally, blood tests for vitamin deficiencies and imaging studies like CT or MRI scans of the pancreas might be needed. A specialist can help determine if your loose stools are due to EPI or another underlying cause.     @@chetnajaiswal75 I benefitted a lot from fasting. It seems counterintuitive when you are losing weight, but it really helped to restore my gut microbiome. After that I started to gain weight and symptoms like bloating disappeared. Now I fast every time I feel it is necessary. I guess that, because of malfunctioning pancreas, bad bacteria (and bacteria in the wrong place in your gut) get a chance to develop. Despite supplementing pancreatic enzymes. Fasting may restore balance. Consult your doctor to see if this might be an option for you.

    What if I have tried so many pancreatic porcine enzymes and I have side effects from them and the only options are now OTC pancreatic vegan enzymes ? Also is this normal ( I have severe EPI lab 86 and chronic pancreatitis ) that before diagnosis I did not have runny stools but just constipation won’t enzymes constipate me even more ?

    22:32 has it occured to anyone that possibly because people are following low fat diets that its creating this problem? I thought we were supposed to eat fatty fish and butter on a regular basis? Olive oil and lamb? Isn't that what the Mediterranean diet is? If our gallbladder isn't being used often enough, it seems like it gets clogged, lazy and malfunctions.

    My daughter was inpatient and they determined acute pancreatitis. My prideful MIL kept from us the fact that her mother died from pancreatic cancer. It’s truly sad when family cares more about pride than their own grandchild.

    I got it from immunotherapy! I don’t see much on this topic. The pain was awful. It involves pancreas only and comes back when I wean off prednisone. No elevated igg4.

    I developed pancreatitis from checkpoint inhibitors. Adding digestive enzymes really has helped reduce prednisone. I will hopefully get off prednisone all together!

    Was your daughter a carrier of CFTR or did she have CFTR? I am a carrier of CFTR, but don't have CFTR. I have Pancreatitis and am on creon. Within the last two years I lost two brothers to pancreatic cancer! Brothers age were 55 and 57. Way too young to die! Doctors never did figure out the cause of the disease!

    I am in unbearable pain. Been in 4x. They put me on oxys 10 instead of 7.5 I would just love my pain to let up. It's horrible

    Hey I hear you, here in Michigan I've been to hospital a few times now been back and forth to the doctor for years. Not once given pain killers. 😢 yet I've also never been a opioid or even a drug user! I have no history of drug abuse yet my doctor never talks about using even a low dose painkiller like a hydro or something. I'm just surprised it depends where you go in the country for sure! Good luck with your struggle keep going don't give up

    Is morning bloating of stomach normal for this disease? I always wake up with a swollen sore abdomen in the morning and so much inflammation in the upper part of the stomach.

    Bloating can be a symptom of pancreatitis and EPI. However, something else may be going on. We recommend speaking with your doctor about any symptoms to better understand the underlying cause.

    What about absorbing medications like paracetamol, fexofenadine, antidepressants, gabapentin etc. Do these absorbs like normal or do these not work as effectively without the enzymes?

    Chronic pancreatitis and EPI can affect how well other medications are absorbed by your body. We recommend talking to your doctor about the specific medications you take and whether they should be taken with enzymes during meal time.

    I'm Muslim, and my doctor says other options are ineffective. There's a fatwa allowing porcine products for medications, and Allah knows best.
