What is the post-finasteride syndrome?

Episode 18. In this episode, Dr. Vikram Jayaprakash and Dr. Russell Knudsen discuss the concept of post-finasteride syndrome. What happens if you get side effects from finasteride? Are there long-lasting side effects? In this video, we unpack what has come to be quite a controversial topic.

    The problem with most studies is it doesn't show how side effects could be caused by other markers or environmental effects. I've been taking finasteride for 10 years, and about a year back I started feeling really depressed. My doc said he thinks I should stop the medication, and I asked before that, could I have a full panel of bloodwork done just to see if something else is causing it. Turns out I had extremely low levels of vitamin D. I started supplementing and getting outside more and now I feel great. Correlation does not always equal causation.

    This is what happened to me. I was taking 1mg per day as that is what I was told to take. First 3 months I didn’t notice a single side effect. I thought this is great. I’ll get a hair transplant carry on taking this. Anyway, month 4 I had a complete breakdown. Now just to be clear on this, I have never been depressed in my life. Never suffered from anxiety either. Month 4 on 1mg per day. I started noticing I would get disturbed sleep every night. Thought nothing of it. Within the space of 2 weeks I completely broke down. Felt depressed as hell. Had to take myself out of work and just starting to feel a bit better now a week off the tablet. There is no question it is the tablet because I was on nothing else and I’m absolutely gutted that it is the tablet as I don’t want anymore hair falling out. After finding this video tho I will take your advice. I will leave it for a month then try lower dosage and hopefully without side effects. Thanks Edit: just an update to this and I feel very embarrassed saying this but I did in fact get a hair transplant and went back to 1mg per day and I have had absolutely no side effects at all. I don’t know why I initially had them but I’ve been on finesterade for probably 18 months now and I’ve had zero effects. Yeah so there you go, sorry if I put people off and I questioned whether I should put this but you have to be honest. Cheers

    Have you guys ever spoken about how delayed onset symptoms work for dutasteride? Dutasteride has a much longer half like compared to finasteride, 6-8 hours for the latter and about a month for the former, so a 120 times difference. If it takes 3 months to begin experiencing recovery for fin would you have to wait 120 times as long for dutasteride (30 years!)?

    Hello doctors, I have a finasteride question that has not been addressed: what happens when the 5mg is cut in advance? Does the medication deteriorate over time if cut into quarters? My pharmacist gave me the 5mg pills but actually pre-cut 100 pills into quarters. Is that okay? I heard that this may risk the medication losing its potency if pre-cut so far in advance. If someone could please respond, I would greatly appreciate this as I am losing my hair quite fast.

  • @TheHairLossShow

    @TheHairLossShow

    4 жыл бұрын

    I don't fully know the answer to that. If I prescribe the 5mg tab to patients, I get them to cut it up themselves, 1 tab per week. I couldn't tell you what the effect of pre-cutting them would be. Also, with that amount, you would have over a year supply which I suspect would be beyond the expiry date. But cutting the tablets is fine in principle.

    I have a feeling the numbers are wrong when it comes to side effects from Finasteride. They say only 2-3% of people get sides from Fin, that BS. How is it that myself, and a good friend of mine both suffered similar side effects when taking Fin? My sides came a bit later, and his sides earlier. That to me doesn't sound very rare.

    Started on Finestride, then dutasteride. Had progressively worsening effects...however not very long later I had discovered I had low T from a few other, completely unrelated medical problems and am now on TRT for life. I think dutasteride seven days a week is complete overkill as my DHT is permanently crushed to almost zero, but even with no DHT my side effects are much less on TRT than before. To a large extent, if you have sufficient overall T levels you can survive with severely depressed DHT levels for a long time. People with ‘post finestride syndrome’ are most likely with an advancing low testosterone problem that don’t have enough T any longer to cover for the missing DHT... Also, DHT levels when you have low T seem to pegged more as a ratio of estrogen than testosterone...people who ate higher than average estrogen converters, with low T and a high ratio of DHT to T seem to people vulnerable to baldness anyway... If you are thinking about starting on a DHT blocker, get a full hormone panel done first!

  • @antonio7066

    You need a full testosterone panel, including DHT and estrogen. If you get a liver function test done as well, then an endo can calculate your free androgen index based on SHBG and albumin levels. My free androgen index was absolutely appalling. Even worse than my free T...it was basically 50% of the minimum level at which you can be treated in the U.K. There are some people who take estrogen as a hairloss measure as it apparently reduces 5AR activity somehow and thus lowers DHT. Since going on TRT I’ve cut my Dutasteride usage from every day to twice weekly and added in topical fin 0.25 and increased minoxidil from 5% topical to 15%... So far hair seems to have got thicker, not thinner...

    Does finasteride cause sleep disorders? I have taken it only for 9 days, with a slight weakening of erections and a significant decrease in libido. However, the strangest symptom is waking up too early. It is not a matter of anxiety: when I am nervous, I usually have a hard time falling asleep. During these days, instead, I have fallen asleep very quickly, but I have always woken up early and become unable to fall asleep afterwards. This has never occured to me in my life, regardless of the level of stress. I discontinued the drug 3 days ago, with an improvement of the sexual symptoms (which had never been severe anyway), but the sleep disorders persist.

One more question Doctor, I have a very large head and I have read (www.hairscience.com/scalp-tension-related-hair-loss/www.hairscience.com/scalp-tension-related-hair-loss/) that large skull -> more scalp tension -> less blood flow -> lack nutrition for hair -> miniaturize. My Dad skull is small and he only start losing 20% of his vertex region hair at 60, his hairline and other regions are still perfect. So it seems there is a correlation between skull size and AGA. My Father family have perfect hair but my Mom, Uncles and cousins on her side have thinning allover but no receding hairline. Is this hypothesis plausible? If yes can we "shave" our skull safely so the scalp has less tension? I'm sorry for the long question and thank you for the informative podcasts so far!

Hasn't reversed hair loss, just held onto what I have. No one ever told me that it builds up in your system, or that a lower dose would be ok. No ED, but as a 50+ yr old, I put an increasing reduction in libido down to reduction in testosterone. I'm very interested to learn more about PFS after seeing the video. So basicaly there is no underlying variable that we yet know of, with witch we can predict that a person will develop PFS caused by finasteride usage (because of potential existence of that variable in a given person). And the statistics are showing that the incidence of PFS like symptoms in the "normal" lets say "age equated" group of people is higher than in the population of finasteride users. Witch goes on to say that there is absolutely no statisticaly sound evidence that anyone will ever develop PFS symptoms *because of finasteride usage... But since we use our intellect we can also use it and say that there *might be something that we dont yet know of that could be predictive of developing PFS so its good to acknowledge that and label PFS as having side effect symptoms 6 months after usage, so we could then see what happens there. But the numbers are now showing that its highly unlikely that PFS happens because of finasteride. And also your experience of never having any clients with the "PFS" is encouraging. Anyways, its just my interpretation of the information you are sharing. Thank you for doing this, it is very informative, makes me able to make good decisions about my incipient androgenetic alopecia.

    josip vran Exactly. That’s what I tell people who push the post finastaride syndrome theory. I say that if the studies done on the drug were incorrect, we would see a higher sample size of patients who have these side effects with doctors. But no matter where you go, if you ask hair doctors what percentage of their patients get side effects while on the drug, they’ll say it’s in line with the sample size of subjects who had possible side effects in the studies, and in many cases, much less. And the drug doesn’t block all DHT, only 60-70%. So there is still enough DHT for the few important functions. There are populations that, for genetic reasons, don’t make high amounts of DHT and they’re just fine and healthy as well. So it really does seem like it’s mainly a sexual maturation hormone and is used in small amounts for a few other things and once we are adults, we only need small amounts of it. Testosterone seems to be the most important.

To update, the last few days have actually been really good. My libido has actually came back, not super strong, but enough that i felt as normal as ive felt since December. I felt good yesterday and today so far. I think MAYBE I'm starting to get to the tail end of this?

    I had an effect after 2 days of taking 0.25mg for 2 days to see if it would agree with me. I was 18 had a high T, got a script from a pharmacist. After 2 days my dick did not work for a week and took another near 10 days to recover, I'm 32 and my hair is the same as before, allways had a strange hairline even as a kid. I would see a endocrinologist maybe TRT would help alot Could be something else. Full bloods and check for hornoan levels and just everything full blooodwork. Maybe take some supplements multivitamins and viatmins, eat betterz sleep longer get some excerzise. Try to avoid anti depressants as they could make you feel even more strange. Working out could help. In my mind finesteride and dut is just poison that works for some but it it's that toxic to a fetus god knows what else it must do. I was really lucky I only took a wel half dose over the course of 2 days crazy lucky I did not take dut. These guys sound like pros but it is how they make a living and less treatment = less money. A new and safer hair growth drug is being tailed the company does not have the best reputation but anything is possible. I'm on ablowndose of testosterone gel due to the pain meds in given lowering my T level. Has not effected my sex drive but just felt tired and not right. On the gel feel great but don'tess around with blackmarket workout T. Hope you feel better. How do you make a hormone?..... .... Don't pay her !

People just tend to look for excuses so they can take the drug and tell themselves it cannot happen and everyone who tells otherwise is delusional. But once it happens there is no return to normal and you will regret that day when you decided to take it for the rest of your life and damn yourself for calling it fearmongering. You wish yourself badly back to the point when the decision was still to be made. If it is on you yet, don't take it. Not even a small dosage. @@canawg My best advice, that was also given to me by people who know, is to stop reading or thinking about finasteride for the next 3 months (May 1). After that, you can do whatever you like. But right now, your job is to break the downward spiral because it can determine what your final mental condition will be. No browsing or even thinking related to this topic, just look up how to control anxiety, spend your time in nature or in some other "good" place, learn to meditate (!). If you want to do more, pick up physical exercise. From what I read you are improving extremely well compared to the typical victim, and you have virtually no finasteride in your system, so don't fuck it up now.

  • @Concojone5

    @@canawg It's scary to go through this, I know, but it's normal. Expect slow progress. Accept it may not fully resolve, give it 3 months using the techniques I mentioned above, and you'll know. Only then assess and see if you need to seek mental help (people have success doing that). Meanwhile, if at any point, you have suicidal thoughts, seek professional help. Let's talk again in 3 months. Stop the panic spiral and trust the process. Pause the minoxidil for now. If others can do this, so can you. You're a hero.

    @@canawg I'm not a doctor, just a fellow victim who has heard/researched around a thousand stories, enough to have a good idea of the range of possibilities. I'm pretty sure you'll improve given your low dose, but let's be humble here. Typically there is partial or complete recovery, often slow, without medical help, and if you're not happy where you are in 3 months, medical help usually improves things (mental symptoms) from there. But let's not assume a bad outcome right now, let's give you 3 months of mental peace first. You'll be in a better place by then, I promise. Please take my earlier advice. I know it sucks, but you're a hero. We're heroes. Be grateful you quit in time, and took a tiny dose. I wish I had been that smart. The next step is to make the best our of your situation. You'll have to be hero. P.S. yes it's FDA-approved, but what you're experiencing is a side effect mentioned on the label in most countries around the world. They'll take finasteride off the market, but it will take some time because the medical world is just beginning to grasp the risks of finasteride. It will go like Vioxx and all other FDA-aproved drugs that were later pulled off the market. It is not unusual for medications to have permanent side effects, certain antidepressants are known for this, through a similar action as finasteride, so read up on this if you ever get prescribed one in your life)

This would need to be done for a number of months. The HCG will stimulate neurosteroid synthesis in the brain (verified in multiple TBI studies), Progesterone will further make more Allopregnanolone, and HDAC inhibitors help reverse some of the methylation (verified in cancer research) that occurred on both the enzymes and the receptors. If you're someone who got really screwed up by 5ARIS, you may need to run additional "cycles" of DHT based compounds to downregulate and restore signaling to the Androgen Receptors after you reboot the neurosteroids using the above.

    Is it possible that there weren't complaints previously with men treating their prostates simply because men in that age range "expect" to not feel like balls of fire sexually, physically, or even in terms of mental acuity? Maybe if they were affected, it was just chalked up to just "life." Whereas when a younger man's life is diminished in those areas, the difference is much more stark and noticeable.

And I don't understand why hair surgeons keep denying postfinasteride syndrome, when all other doctors are starting to acknowledge it and when even the official labeling/packaging is now warning about it. We go to them for advice because they are supposed to know, and we end up with this terrible condition while they just keep saying how great finasteride is without even questioning themselves. “It's difficult to get a man to understand something when his salary depends on not understanding it."

    You should really update this video as it now comes across like you are not just downplaying the potential adverse effects but are deliberately mischaracterising them along with their history of reporting. Saying they essentially appeared from nowhere 10 years ago in a ‘strange round-about way’ is simply not true. There have been many animal studies, going back to, at least, the early 2000’s showing permanent genital alterations. Court documents were released in 2019 showing the safety profile from the manufacturer to be extremely misleading when it came to logging adverse events. It has since been confirmed the persistent adverse symptoms are likely due to gene deregulation (epigenetics), therefore claims of a nocebo effect are really just a tactic to discredit and downplay reports of legitimate harm.

    But that doesn’t change the fact that all the studies on pfs so far are poor quality I personally do believe in pfs but I actually believe it’s even rarer than portrayed online i think a lot of the cases are either psychosomatic or down to other health issues

    @etonmess6247

    @@RH-eb6tm Without wanting this to appear like a personal attack, you say you ‘believe in PFS’ yet follow up with the previously mentioned dismissive claims. Denial of PFS/persistent symptoms typically unfolds as follows: First it’s claimed it’s all make-believe or ‘psychosomatic’. When informed about evidence of biological abnormalities it’s said sufferers must have another underlying illness or have abnormal results due to ‘lifestyle factors’. So now it’s not imaginary? Interesting. This is elaborated with further claims that biopsy results may indeed be indicative of disease, just unrelated to Fin use. At this point some vehement naysayers may claim of these men - and these are young men - must have, coincidentally, had these problems prior to using Fin but somehow they, collectively, only became aware them during or after Fin use, leading them to falsely believe Fin is responsible. Or, they were aware of them prior to Fin use but want a legal payout or something to blame their pre-existing symptoms on because they are dishonest. When it’s pointed out the biological abnormalities match those abnormalities found in previously healthy animals given 5ARI, responses are either thin on the ground, or it’s claimed that drug trials and FDA safety reports speak for themselves. Typically, there is no direct response to the fact Merck’s redacted court papers prove they lied about the drug’s safety record because there is no way to justify that course of action. It’s all just a big coincidence, right? If you’ve read the few focused PFS studies (those specifically investigating PFS patients and experimental PFS animal models) then have you reviewed the associated research references? How are you personally defining PFS? It’s simply lasting and varying symptoms after 5ARI use, and not necessarily limited to ‘sexual dysfunction’ or ‘depression’ that was later added to the warning leaflet. Not all evidence of harm is listed or searchable under ‘PFS’ and direct research on PFS men is still relatively new. So if you haven’t done so already, consider widening your investigations to include general and specific studies on 5ARI human and animal subjects with a focus on adverse outcomes, also androgen and androgen receptor-related diseases. Look at a wide range of neurosteroid and neuropsychiatric papers. Consider reviewing other post-drug syndrome research, you’ll note epigenetics is a common theme, so review genetic/epigenetic literature too. Don’t look in one place for answers.

Although, I disagree that there isn’t already solid proof that 5ARI are potentially harmful to some users. I think what you might mean is the current evidence (those focused PFS studies) isn’t quite strong enough to conclude a direct causation with Fin in some men who report persistent symptoms. There is, however, strong evidence that these men have enduring abnormalities and there is strong evidence that 5ARI have caused similar enduring tissue changes in rodents.

    Is it possible that post-finasteride syndrome is confused with the long time the drug remains on the skin? It is said that a simple dose of 1mg can remain on the skin for at least 30 days, so daily intake would lead to an over-accumulation in the body. In addition, it takes 3-6 months for the body to rid itself of the drug.

  • @natechenry

    @natechenry

    3 жыл бұрын

    It takes 30 days for the body to rid of the drug in its cells. That’s why 3x a week is more than enough for basically anyone who decides to take finasteride. “Remain on the skin” finasteride doesn’t remain on the skin it remains in our blood for about 30 days or just a bit more. Which is also why you can and should take a month or two off every 5-6 months fin to cleanse your body of the drug so it doesn’t build up over a long period of time which is a large reason why (SOME) people who take fin for so many years have long affects because the minority of people who do get affects didn’t take a break, these doctors explained it pretty well how you could take 1-2 months off fin and not lose any ground that you made from recovering or regrowing hair. I’m about to order finasteride and I’m starting off with the standard dose of 1mg fin 3x a week. I’m not going to take it every day seeing as 3 mg is more than enough or perhaps even too much. And if that’s the case I’ll just lower the dose to 0.5 mg 3 times a week.
