    When I was 17 (11 years ago) I had a major shoulder repair surgery with my rotator cuff. All I remember afterwards is my surgeon/Dr telling me that he wished he had done the surgery sooner and it was way worse than he had ever expected. I had 7 anchors put in then and 1.5lbs of scar tissue taken out. It still to this day isn’t the same

    Thanks Doc, more info than my DR told me. What about a patch installed for the scar tissue to attach? My Dr mentioned some sort of patch installed for the scar tissue. I had a complete tear of the Supraspinatus with retraction. I'm 22 days post op, and doing pretty good

    I'm guessing cape coral... Given the heavy machinery? If not west side.. East side... HOLLYWOOD? I had full rc repair in May.. 3 full thickness tears... 1 partial.. Frayed bicept... I'm doing great! Now back to work.. Couldn't get disability.. N it's getting sore.. Praying it holds! Ty for your info! You sound like a great Dr!

    How strong are the anchors and the Medical sutures? I have 3 anchors holding my pectoral major but I'm 6'4 255 and I feel like when I yawn or move my am by accident (or on purpose because I'm washing my hands or the like) that its gonna pop off. Im 10 days out of surgery but yesterday I used non non-surgery arm a TON which flexed the surgery pec every once in a while.

    Robert, The anchors as sutures are very strong. The real question is how good is your tendon quality and your repair. Becareful using the other as you may be stressing that repair.     Can you give me your honest thoughts on PRP injections? More specifically do you think it would repair a full thickness tear of the biceps tendon or a torn labrum? Perhaps a video explaining PRP injections and what they can and can't do for the viewers

    Dr, first thanks for the videos. It help me understand what I am going through with my right shoulder. Don’t want most my information out there but I went to the er finally because after two months the pain would not go away. I have a labrum tear with a bicep tendon and anchor tear as well. Rotator cuff is fine besides tendinosis. For being 35 going on 36, is this something that would need surgery or can surgery be avoided?

    Hey Jeansonnea, It is really hard to say. It would be best to find an ortho to look at your MRI images as well as do a physical exam. Then you can have a good discussion about what your unique options are. Hope this helps.

    So as you reconstruct or replace after two failed repairs what happens to all the anchors from the previous rotator cuff surgeries? Do you take them out?

    I know you said metal anchors aren’t used much anymore, but I was wondering if they were used, would that then make you not eligible for a MRI anymore since you have metal in your body or is it not enough to cause a problem?

    Excellent question Jennifer! Most anchors (used in the US within the last 20 yrs ) metal or otherwise are MRI safe!

    50 yrs old. I have massive 3 tendon tear in dominant arm. Surgeon says will need open repair and deltoid take down. I am worried about it re tearing due to weightlifting in my spare time. I now have the same issues with my left shoulder and can now not lift either arm. I am told the repair has a high likelihood of re tear and am wondering if I should go through this major surgery and rehab if it has such a high risk of not working?

    Hi Jani, Thanks for your comment. Make sure you have the best surgeon with lots of experience fixing rotator cuffs. Talk to them about your options. SCR, Supplemental grafts, tendon transfers, InSpace Balloon. These are all options your surgeon should have at their disposal, just in case. Hope this helps and let us know how you are doing.

    I am 8 months post op from arthroscopic knotless double row repair to supraspinatus. 2x PEEK Healicoil anchors internal row, and 2x PEEK Multifix anchors external row. Also had arthroscopic single row repair on Subscapularis using Q-Fix All Suture anchors X2. My question is, are all types of anchors equal or are some superior? Does the patient's pathology also determine anchor selection? Thank you for your videos.

    Good question. Those anchors are all made by Smith-Nephew. They are different as they do different jobs and are in different areas of bone. For example the Q-fix is better in the area of bone where the subscapularis attaches as its hard cortical bone and minimal cancellous bone. The healicoil anchor holds better in more cancellous bone which is found in the area where the supraspinatus attaches. I Hope the way I explained it makes some sense!

    Thank you. Yes that makes sense. Also, just wondering if you could make a video about the different types of sutures in relation to rotator cuff surgery. Perhaps touching on issues such as why the sutures are permanent, what they are generally made of, the difference between suture tapes and regular suture, knots vs knotless repair, biocompatabilty and the long term fate of the implanted suture. That would be helpful. My supraspinatus was stitched with Ultratape suture, again by Smith & Nephew. Thank you once again all the way from Australia. Many kind regards.

    Okay, had a full thickness supraspinatus tear and it required 4 anchors. Is this normal for a full thickness tear. I saw the tear on the MRI and it looked pretty bad. I believe there was a partial tear near it that was repaired as well.

    Yes! It was a full tear & appeared like a "torn ragged edge of a curtain" to me. And the other tear that was not full thickness was repaired at the same time. When I asked my doc about the 4 anchors being used he said that was how many he prefers on a full thickness tear of that size. He cleaned up the edge of the tear & reattached. I am at 13 weeks, but about 7-8 weeks in I swear I felt like a puppet in that the shoulder joint made crinkly creeky type audible sounds when I raised & lowered it! And when me or my husband put his hand on it while I did it you could feel this!!! So strange. No pain associated. It went away in about a week and a half. It was everyone's opinion that it would diminish as my muscles got stronger around the area & the scar tissue broke up a bit. Dunno but that was very wierd for sure. So yes; during that time I was super careful not to overdo it. But kept up with PT exercises.

    I am eight weeks post surgery. I cannot actively elevate my arm, laterally, although I can do it passively with a pulley I can lift it in front with no pain, and behind me with no pain. I was not ideally compliant in the first few weeks after surgery. Do you think I tore the supraspinatous off of the anchor?

    @jackknifegyp, Good Morning! I assume you mean the rotator cuff tendons? In a RCR we are trying to preserve them yes. In a reverse we do not remove the tendons, but the reason why a reverse is performed is because at least one (usually two or more) of the rotator cuff tendons are irreparable.     Also am told the longhead bicep tendon has come out of the grove and will now become a destructive entity so I will need a tenotomy and a tenodisis for this. I am worried about the post op deformity of my arm and also the deltoid due to the previous comment made.

    Would a thin rotator cuff be harder to suture? Confirming the quality of the tendon or tissue can only be done arthroscopically right or mri reliable? Have you ever sutured a muscle tear? How many anchors would that approx take?

    Yes Batman, a thin rotator cuff is more difficult because the sutures can tear through it. You can get an idea of the quality of the tendon on the MRI, however during surgery is the only real way to know for sure. Hope this helps. As far as suturing the muscle, that is typically not done.

    @@rotatorcuffexpert Thank you. I am guessing if it is difficut to suture, the next step would be to try augmentation or would it be labeled irreperable? Regarding muscles, how much of the muscle belly for the posteior rotator cuff infraspinatus is visible on mri. That muscle runs all the way to end of scapula so just wondering how much of the muscle belly shows up on shoulder mri, like where is the cutoff. Probably up until the mid muscle? And does the teres major muscle show at all?

    We can typically see most if not all of the muscle including the teres minor. (teres major is not a rotator cuff muscle). The muscle of the rotator cuff is typically not the problem, it is the tendon of the rotator cuff that is torn typically.

    Hi, Is a surgery needed for this shoulder MRI results: Mild area of depression with increase bone signal attributed to contusion / edema is seen at the posterolateral margin of the humeral head. Impression: Consider anterior glenolabral (Perthes lesion) injury Consider Hill-Sachs lesion. Thank you.

    Hey Dan, (nice first name btw). Unfortunately the MRI report is only part of the decision for surgery. It should be determined by your physical exam, conversation , and your MRI. Good Luck . Make sure you discuss this with your surgeon.

    Sorry for the confusion. I was looking for a clever title and it has become more of a confusing title. The anchor is necessary if you have a RCR and it will not slow you down. I apologize I'm still trying to get better with my videos! Also thanks for subscribing Kelley!

    Hi I’m about 2 weeks out of surgery for a labrum tear. However my surgeon informed me 2 days after surgery an anchor kept braking during the procedure. Its a wait and see if additional surgery is needed after recovery. Is this typical? What questions should I be asking to be the most informed? Appreciate any guidance. Thanks!

    Hi Justin, I guess I am not sure what the surgeon meant. Did he mean the anchor he was putting in? Or sometimes we call the attachment of the biceps to the labrum the "biceps anchor" of the labrum. The clearer he can be the better. I would ask why, how does this effect my recovery, when will we know if I need further surgery. Good luck Justin.     Going in for a second surgery as the first failed, thinning tendons. First time i had full thickness tear of the suprspanatuis and partial of the other two, Terry’s minor was of, this time all three a full thickness tears. What happens to the anchors in the second surgery?

    You are in Miami right? I am still having severe pain in my right shoulder from having rotor cuff surgery and breaking my collarbone. Is this normal is it going to take a long time to heal? I don't have full mobility I probably have about 85% mobility back and it hurts like crazy from time to time. Can you give me some enlightenment as to what's going on. I do have anchors and my tendons were shredded

    I am so sorry you are having pain and stiffness! It is unusually to have a collar bone fracture and a rotator cuff tear at the same time. Each one of those are tough to get through by themselves. Be patient and kind to yourself. No not Miami.

    Thank... I used to live in Seagrove Beach. I just had total Shoulder replacement (bone saving)surgery 8 weeks ago. So far my range of motion is not so great, but you're videos Before and After my surgery have been very helpful (beach ball for shower) for me. I didn't realize you were in Atlanta until after my surgery was already scheduled or I might have come to see you instead!! Have a great day!

    Can you give me your honest thoughts on PRP injections? More specifically do you think it would repair a full thickness tear of the biceps tendon or a torn labrum? Perhaps a video explaining PRP injections and what they can and can't do for the viewers

    Mike without examining you and seeing your MRI images it is impossible to predict. It is a little confusing because you say Full thickness tear and then you say it is small. I'm not sure I fully understand your tear. Please go see an ortho so you can talk through your unique situation. Good Luck Mike!

    @@rotatorcuffexpert thank you dr. On my MRI it says "small perforating tear at the proximal extra-articular segment" I dont have the time right now to see my dr again and lack of insurance. I cant find anything describing if this is a full thickness tear or now. Thank you Dr.
