    Good info! I had a slap tear repair and rotator cuff work also my ac joint .. that was two years ago. Coming this November I need to go back for a bicep tenodesis and ac joint work again!

    Thank you for sharing this. I really liked to hear your analysis on how the treatment of SLAP tears varies for different age groups. I would love if you could do an episode regarding treatment of posterior labral tears for different age groups. All the videos I've found on this matter are solely aimed at young athletes. So would be great to know if there's any current treatment available for people above age 40.

    One week out from bicep tenodesis and feeling great. I’m 42 and active. Range of motion is already better than before surgery and I have to remind myself not to lift… Of course it’s sore, but not really any pain. Hoping my success “sticks”

    I had a SLAP tear and had a SLAP repair on my right shoulder(dominant arm) in early July 2021. I was 38 at the time and on the age cusp of SLAP repair vs a biceps tenodesis. My surgeon said it was a clean tear (don’t throw yard debris over your fence, kids. Just open the stupid gate and walk around) and did the SLAP repair. Closing in on 10mo post surgery, I have most of my range of motion back, but I’m still having biceps tendon pain and now a possible impingement in the AC joint. My surgeon proposed doing a biceps tenodesis and a distal clavicle excision if I’m still having issues around the 1 year mark. He’s leaving it up to me to make the decision, based on how much of the pain I can tolerate and for how long. I have a history of instability in the right clavicle after a freak accident leading to a subluxation of the SC joint back in 1998. I’ve been watching a lot of your videos to understand the shoulder joint better and to ask more specific questions to my doctor. So thanks for the information! I’m going back at the end of May to discuss how I’m doing and make a decision on whether to wait or go ahead and do the second surgery.

    Caro, yes you were on the edge of the age range. Sorry you are considering having another surgery. Sometimes to help differentiate whether you are having pain in the AC joint the doc can inject the AC joint with lidocaine and see how much relief you get, even temporarily. If your pain doesn't improve, it may be just your biceps and not your AC joint. Hope this helps, good luck!

    Thank you for detailed break down. I have two questions please. 1. If someone has a bicep tenodesis done, the labrum is still torn correct? if so, what happens to the labrum? Is there an issue having it still torn after a bicep tenodesis? 2)I am 40 yr old male with slap tear (grade 2?). I dont have much pain, my only issue is I probably have about ~90% arm strength, is it worth having surgery for this? I like to go to the gym and do karate.

    Silo2000, great questions. Obviously you should discuss these questions with your doc to see what makes the most sense for you. If a tenodesis (or tenotomy) is done the pull on the labrum is removed. Some surgeons still repair the labrum even after releasing the biceps if the labral tear is unstable.

    Back in 2009 and 2010, I had two SLAP repairs, and bone anchors on my left shoulder. In 2022, I had to have my left shoulder worked on again with 280 degrees of labrum tear (first repaira failed and I further tore more). This past week, I had surgery on my right shoulder for a fall that dislocated it back in January. I had a large SLAP tear repaired, bone spurs from the impact fracture, they did a capsulorrhaphy to tighten it up, and they did a biceps tendonesis. Im a week post op and the funny think is, I have more pain from cramps due to my sling than I do from the surgery.

    @@rotatorcuffexpert What makes it worse is that I've also had back surgery on my L5 in 2018 and meniscus surgery on my right knee in 2022. I had my knee surgery only 4 months before my third surgery on my left shoulder.

    interesting. I'm 35, office worker, no sports, but I do hold children, move things, and throw the ball for the dog. My right shoulder is a bit unstable and my MRI showed a very tiny SLAP tear. i have a meeting with a PA-C on Tuesday to discuss further treatment(s); I'm hoping to completely avoid surgery and continue with PT and PRP and/or prolotherapy.

    @@rotatorcuffexpert thanks sir. appears I have a bucket handle tear that might be slipping beneath the biceps tendon. gonna see the surgeon in a month to discuss options. i'm hoping for a simple deridement? is that what's it called? shaving/cutting away of the injured labrum tissue and avoiding a tenodesis. any thoughts on that?

    I would like to know if I had the correct surgery depending on the diagnosis. Please find below journey of mine. Injury : I had an injury in 2009 while playing cricket where I swung my left shoulder with great power and it took one complete rotation and felt sever pain after that. Went to couple of doctors and they recommended to just do Physio therapy . However in June I baited another doctor and they did an mri to my left shoulder. My MRI report:(from June 2022) IMPRESSION: I. SLAP 3 tear; superior labral tear extends anterior and posterior between the glenoid articular cartilage and superior labrum aud between the superior labrum and biceps without biceps retraction. Low-grade tendinosis involving the proximal segment ot the intra-articular biceps distal to the biceps labral tear. 2. Low-grade tendinosis involving the distal subscapularis and infiaspinatus tendons without tear. Reactive subcortical cyst formation within the posterior-supertor humeral head adjacent to tendonopathic intraspinatus fibers. Treatment: I had steroid injections: one in June 2022 and other in august 2022 Surgery : I had my left shoulder SLAP 3 tear surgery on November 15 2022, 10 days ago. My surgery included Arthroscopy for : 1) repair of slap lesion 2) decompression of subacromial space with partial acromioplasty with or without coracoacromial release 3) biceps tenodesis Post OP: Now I will be having a sling and breg polar care cube system ice box for icing the shoulder for a month. After day 3 of removal of my dressing , Now i am doing 4 exercises for 10 min for 4 times a day until a month. After one month they will remove my sling and have to go to rehab for 4 months. So recovery period is atleast a 6 month journey from day of the surgery Currently I am doing well. Will update every week about my status

    Hello Dr Daniel, When comparing intra-articular biceps Tenodesis (with the tendon in The Groove) compared to Biceps Sub-pectoral Tenodesis in patients with Mechanical Symptoms. would you consider Intra-articular Biceps Tenodesis if the patient not suffering Anterior shoulder pain or go strain to Subpectoral Tenodesis ? Would you consider BAG in Older Patients suffering Superior Rotator cuff tears instead of Biceps Tenotomy (Pros /Cons) ? Thanks

    Good questions. Most people with rct and biceps symptoms I use a suprapec tenodesis. I am not sure what BAG is sorry.

    My surgeon wants to perform both- relocating the biceps tendon & slap repair along with shaving the clavicle at the AC joint. Is this not typical?

    thank you for the informative video! I'm 26 and currently facing the decision between surgery and conservative therapy due to a type 2 SLAP lesion. In addition to the discussions with my treating doctor, I'm trying to gain more perspectives and opinions on the matter. In this regard, I would be particularly interested in the following four questions: 1) what functional limitations (regarding movement, maximal strength, stability, etc.) can be expected after the full recovery from a SLAP repair compared to tenodesis? 2) are there significant differences in the duration of the healing process? 3) did the study also address the causes of the 1% higher recurrence rate? And this question concerns me the most: Currently, I only experience mild to no pain in everyday life. I only feel slight to moderate pulling when bending my arm behind my back (for example, when tucking a shirt into pants or applying lotion to my back), and a noticeable but not completely unbearable pulling-like pain during strength training (pain scale approximately 8/15). 4) Is surgery advisable in this case? And in the case of choosing conservative therapy with physiotherapy and no surgery, can I continue participating in (strength) sports as before? Isn't there a risk of further damaging the tendon/labrum?

    Hey Hans, those are a lot of great questions and without examining you and looking at your MRI films it is impossible to give you those kind of detailed answers. Oftentimes in younger people we may do a SLAP repair and in older do a biceps tenodesis because the general condition of the shoulder and biceps tendon. I would love to know what you decide.

    I chose physiotherapy and it did not help at all. I kept exercising with (what i thought) moderate weights and now my other shoulder is also teared (same feeling). I am really desperate right now... it is not possible to exercise anymore and because of my job i am currently not in the position to get surgery for the next 6 months, which would include staying at home for a too long period of time.

    Also i would need surgery for both shoulders, which would add to another 6 months to the regeneration time, making it a whole year without training and not being able to do my current job.

    I'm having a slap repair, and I also have a paralabral cyst taken care of. I'm 51 years old I been doing physiotherapy for the past 2 plus years. I've also had rotter cuff surgery on my left shoulder and developed post-operative capsulitis in my left shoulder that surgery was 12 months ago and still having problems with pain, and now the problem with a labral slap tear due to over use of the dominant right arm my work history is in truck parts warehousing so heavy repetitive lifting for past 20 years my specialist has recommended surgery to repair the labral tear and cyst I've had pain in my right shoulder for about 15 months now with physio so my question would be would having the bicep realignment be a better option than fixing the slap tear Cheers col

    Colin, wow that is a lot of shoulder issues! Typically in patients over the age of 40 or so a biceps tenodesis or tenotomy is performed instead of a repair of the SLAP.     Hi there, I found the video very informative. I am 43 and have a type-3 SLAP tear in my right shoulder. Despite my age, could a SLAP repair be a reasonable option since my biceps tendon is fully intact? Also, what are the chances in general of a bucket handle tear healing without surgery?

    Hi Daniel, thanks for the comment. In general a bucket hande tear of the labrum is very rare and is very unlikely to heal. A SLAP repair is always an option, it just might not be the best option.     Thanks for the info I'm 50yr old male I have mild to moderate pain with Mild degenerative arthropathy at acromion clavicular joint with mild subacromial/subdeltoid bursitis. Tendinosis of supraspinatus tendon with low grade articular surface partial tear of supraspinatus. superior labral SLAP tear with probable early formation of ganglion cyst in the suprascapular and spinoglenoid notch what options do I have. I also have early extension into anchor of biceps tendon.with underlying sub-labral sulcus and sub-labral foraman. THANKS

    Ronald, I guess you copied and pasted your MRI report? That description can only come from a radiologists reading. There is a lot to unpack in that MRI reading. You need a good shoulder surgeon to evaluate you and to actually look at the images to match your symptoms to your MRI findings and then create a plan with you. Good Luck!

    I have chronic hill sachs lesion and bankart lesion the mid and inferior anterior glenoid labrum and small tear in biceps tendon and subluxation. I'm 35 but still active and want to throw and swim hard. Would just the tenodesis be enough to get me there or is the labrum repair also needed? Would the labrum repair be useless?

    Brian, It is not that easy. Some of it is surgeon dependent. Some repair the labrum some don't Some just do the tenotomy or tenodesis. It is debatable whether fixing the labrum after the biceps has been released off the labrum. A lot depends on you, your doctor and your exam. I hope this helps some.

    Hi Doctor, I’m 41 y old I had Slap surgery having bicep pain during night and doctors not recommended for bicep tenodises for my questions due to good biceps anchors in mri. As just 4 weeks post operation having pain only in night will my biceps pain go away

    Sir i had a slap tear type IV and lhbt instability + superspinatus pasta 2, and have had the slap debriment + tenodesis + sst reapir, does it mean that they should have repaired the labrum? im a 22 y.o overhead athelete, and now im 4 months post arthroscopy, rehab goin pretty well still having some pain with overhead pressing, but flat bench press for example is pain free now, still hace some weird clicking in my shoulder when externally rotating it but i guess it will get better over time, since it was way worse 2 monts ago.     Wow! that is a bad injury and a lot of surgery. Once the biceps is tenodesed it no longer pulls on the labrum. Debridement of the labrum is often appropriate. Good Luck! Sounds like you are recovering nicely.

    if there is biceps instability then the biceps needs to be addressed. The type of procedure depends on where the instability is. Sorry to be vague but the best understanding will be by talking with your surgeon.

    @@rotatorcuffexpert not a problem, im really thankful for the answer doctor. I was just curious if the lhbt is instable is the treatment always tenodesis/tenotomy, becouse from what i have read on pubmed and from talking to my surgeon these are the only 2 options.

    @@rotatorcuffexpert for instance my surgeon told my, after the artroscopy that in my case the lhbt was totally unstable and the tenodesis was the best way to adress it.

    Sir I had arthroscopy in 2019 having degenerative changes in AC joint And subacromial subdeltoid bursa after that I felt fine but in recent days after 2 years my pain got worse and I spoke to doctor about then he recommended to do MRI of my right shoulder now the reports have come it shows 1.Mild AC joint arthrosis with lateral downsloping of acromian.2. Bursa 3. Slap II tear 4. Subchondral cyst involving the inferior glenoid With oestochndral fragment Within joint. After the report doctor said we will do another arthroscopy by bankart repair and we will clean the ac joint and several areas in which the problem consists. But for me personally when I move my hand upwards it is fine when I move it down it pops and hurts a little. We r going with the doctor's diagnosis on this. Sir what do u recommend should I tell doctor to do Mumford procedure or what they are doing is great. This week I will having my arthroscopy For this only.     Thanks Pankaj for question. It is difficult to have a surgery and then need another surgery 2 yrs later. Obviously that is not our goal as patient or surgeon. I cannot give you advice about what surgery should be done as that can only be determined by a good physical exam and a good mri. That being said your doctor stated "clean the AC joint". I would interpret that as a distal clavicle excision which is equivalent to an arthroscopic mumford. Talk it over with your surgeon so that you understand what exactly he/she is going to do and why.     I had a slap repair 3 weeks ago. The doc anchored the labrum but also performed a bicep tenotomy. I wonder how this will affect my gym performance in the future. I am 43 and fully athletic.

    That is a great question. Unfortunately you won't really know until the healing process and recovery is over. For a biceps tenotomy and shoulder scope it varies from person to person but a typical time to return to sport with out restrictions can be 3 - 6. Good luck and let us know how you are feeling.     @@rotatorcuffexpert Thanks for your reply. I just sent the email. At the moment I really worry more about my bicep than my shoulder. A have terrible pain on the shoulder area but this was expected before performing the surgery. My bicep feels a bit strange though. I notice a very mild bump which is the popeye bicep you've mentioned in your videos but nothing too noticable. What worries me most is the possibility of the bicep descending more in the future and making the bump look more intense. Do you think that this is possible?

    ​ @Rotator Cuff Repair Expert it's been almost 7 weeks after my surgery. My shoulder is getting better but my bicep is getting worse. I am having cramps out of the blue and really strong burning sensations on my arm, shoulder to elbow. The bicep deformity has also become a bit more intense. I think that tenotomy was not for me. Any suggestions?

    @@pargyropoulos my doctor performed a tenodesis which i wasn't aware that is an option prior to surgery. I have not yet (after10yrs) gained range of motion even I followed the physiotherapies. Your shoulder will never be 100% again and you wont change the result of your surgery. Finding the right surgeon is difficult one and you will never be sure. Let us know of your progress. Have you seen any improvement?

    Hey Doctor what is a person is 29yrs old such in my case I was diagnosed with a slap tear. My doctor recommended a buco tendinosis surgery but I’m kind of on the fence to get it. I’m doing PT and it’s getting a lot better. Will it hinder my workout?

    Edgar, There are many different kind of SLAP tears. Each has its own treatment. So it depends, also there is a risk of completely tearing your biceps. This would lead to a deformity and by the looks of your biceps you would notice it for sure!

    I just had surgery last week, partial supraspinatus tear, partial biceps tear & SLAP tear haven’t got to speak to the surgeon yet, but we had already planned on doing the biceps tenodesis going off the MRI, but the SLAP Tear wasn’t showing. From the looks of the notes the SLAP tear was just debridement. Is that what you would normally do if you were doing a biceps tenodesis or would both get fixed?

    Not necessarily. It is a judgment call by your surgeon. You don’t want to hear this but…Worst case, they can always go back and do a biceps tenodesis. Good luck Sam and let us know how you are doing in your recovery

    I'm in the age group that you didn't really discuss. 36 yo former collegiate pole vaulter and javelin thrower. I'm nearing my 2 yr post op date of bicep tenodesis, capsulorraphy, supraspinatus debridement, and bankart repair. I will be scheduled for surgery again soon for tenodesis revision as I had an acute rupture of the tenodesis with popeye deformity. I have had severe cramping and pain that persists after conservative treatment and struggle with opening jars due to loss of strength. Dr. will also do a subacromial decompression. I also feel like my shoulder is unstable again. This will be my 7th surgery on this shoulder. What is the chance that a bicep tenodesis doesn't fix the SLAP tear? Would I still be a candidate for SLAP repair at my age or is that even possible since having had a tenodesis?

    Cassandra WOW 7 surgeries on one shoulder! I am so sorry. So, you do have many things that make your situation unique. Your age, your constellation of injuries and the number of surgeries. SLAP repair, biceps tenodesis, and capsulorraphy is an unusual combination. Probably due to your college track career. Typically we don't have to do a SLAP repair if we are doing tenodesis. Unless the labrum is really unstable. Good luck and let us know how you are doing in your recovery

    @@rotatorcuffexpert Is there an increased risk of rupture with sub pectoral (open) tenodesis, compared to the other options for tenodesis? I had the sub pectoral (open) and that is what I will be having a revision on coming up. Or is there an increased risk of mechanical failure? I must have had a pretty diseased tendon to begin with, and am concerned with that fact that I may not have much viable tendon to work with which will leave me with permanent deformity and pain.

    Hello sir, did that study just look at revision rates? Are functional outcomes for very active, high functioning 50+ year olds similar s/p tenodesis vs SLAP repair?

    Go'n, there are several studies to look at, but the general trend is that people greater than 50 yo have better function and lower revision rates with biceps tenodesis. Hope this helps and good luck on your off roading adventures!

    Hey Brian, it can be confusing. If you have an unstable SLAP tear and are young it is typically fixed. If you have an unnstable SLAP tear and you are old the biceps is released off the tear (to take the tension off the repair) and then the biceps is secured outside the shoulder joint.     I just had a type 2 slap, impingement, clavicle excision and biceps tenodesis. 3 months out i have zero ROM.. PT 3 times a week for 3 mos

    Sounds like you need to have a conversation with your surgeon. If you are really stiff it might be time for a manipulation. Good luck, hope you get motion back soon!

    Dang I was diagnosed slap tear with a cyst on the spinoglenoid notch and a bone spur at the ac joint I'm 46 and I have 98 percent ROM it's been almost a year now since the injury was reported I'm leaning towards not getting the bicep tenodesis done.

    Hello sir .. am suffering from my shoulder not from pain in the general meaning but from annoying feeling it comes and go from time to another now i have it back again and i’ve don all of the tests for impingement and tear and the results were negative but now i doubt the i have SLAP so how can i be sure of that !? BTW i had an MRI few months ago but it saye all good and to be honest i dont think the report of that MRI is precise

    Ali, thanks for your question. Often times labral tears including SLAP tears are hard to see on MRI if dye is not injected first. Its called an MRI arthrogram of the shoulder. Good luck and let us how you are doing.

    Allon, Grades of tears are typically describes as full thickness 100% torn, high grade partial tear 61% to 99% torn, Moderate grade tear 40-60% torn. Low grade partial 1% to 39% torn. Hope this helps!

    What would you recommend for a 38 yo firefighter with a type 2 SLAP tear and longitudinal split in the biceps tendon. Positive O'Brien's and Speeds tests. Thank you!

    Everyone is different and needs a formal evaluation, and a discussion of the pros and cons of the options with their surgeon. That being said it would be very difficult to repair the biceps and the SLAP tear. Hope this helps at least a little

    @@rotatorcuffexpert So, that surgeon's plan was to do a biceps tenodesis and debride the labrum. I got a second opinion and doing a few diagnostic cortisone injections was suggested. I've read that those can negatively impact surgical outcomes? Not sure what to do as I've been dealing with this since Jan and have already tried PT and Stem Cell injections. Would greatly appreciate your advice! Thank you.

    I guess my question is... is it not common to repair the SLAP & BICEP TENDONITIS at the same time? I have SLAP tear, Bicep tendonitis & minor tear in rotator cuff... I'm going in for surgery tomorrow... I was told they will determine what will be repaired once they're inside to look around.

    Gayl, that is a bit complicated. I tried to explain it in that video but I guess I didn't do a very good job. Often the SLAP and the biceps tendonitis are consider the same problem. Therefore when we get in the shoulder we determine what is the best option to treat what you have wrong. Hope this helps. Good Luck! Let us know how you do in your recover.

    Hi Gayl can you give me an update on what you had done? My surgeon basically said the same thing about having to look around to see what needs to be done. Based on the MRI he said labrum repair and biceps tenodesis. I'm hoping to only do the labrum repair to avoid bicep deformity. Thanks

    Hello sir, a 20 year old,about an year and half ago I started to do workouts, I guess not knowing the form correctly,I made mistakes overtime worsening of my shoulder. One morning,my shoulder swelled in the front.. with increased pain and tenderness in front of shoulder..earlier I was having clicking and grinding which has worsened now …also I feel like a weird band flicking just below my collarbone and pain in front and back of shoulder deep within the joint with certain movements…plz help did all types of rehab exercises but doesn’t seem to be working 😢…

    I’ve had 2 slap repairs and 1 bicep tenodesis surgery. 4 months post operative and I still feel the exact same pain if not worse to the point picking up a bottle water and getting a drink KILLS me.. seen the surgeon today.. he said it’s just bursitis push through the pain.. well in the last month he has gave me 2 steroid shots and the pain is still so bad it brings tears to my eyes…. Any ideas, thoughts? Is just “pushing through the pain” even a normal thing to do? I mean it feels like something is fixing to rip apart! Sometimes my whole shoulder feels as if it is being torn off… PLEASE HELP or some kind of answer!!!!

    1% is not a big deal.... get your patients between 30 and 50 in a good PT and Nutrition program. Also seriously consider PRP Therapy and Stem Cell therapy to regrow the cartilage, tendons, etc.

    @rotatorcuffexpert my wife is. But I am very familiar with the system and seriously disapprove of how Medical treatments are based on "prescribed charts and matrices" rather than analysis of the individual patient. From a liability perspective, it is preferred to push bicep tenodecis for all. Moreover most physical therapy is only covered for 6 to 12 weeks, with goal of regaining minimal former function. For example shoulder issues require therapy to the soft tissues all along the thoracic and cerival spine, as well as arms and chest. The rotator cuffs likely tore because of structural weakness and muscular imbalances. On top of that, a patient needs to eat enough micronutrients to regrow tissue and balance out his hormones. PRP, Stem Cells, peptides, and HGH-1 are all different types of therapies that can speed up recovery. Protocols are individual based. Almost no hospitals offer this to the general population since insurance rarely covers it. In the US, mostly private clinics do it.

    @rotatorcuffexpert correct Dr. Unfortunately, that is not common practice. Satistics show that there is a 1-5% higher probability of SLAP retear for patients over 35yo. So now all patients over that age are shoehorned into a bicep tenodecis.

    I had a SLAP repair at 40 from boxing. The tear was from 12 to 9 o'clock. The surgeon said my bicep tendon looked healthy and he didn't want to perform the tenodesis unless it was bothering me. I healed really fast (in 4 months I believe), but didn't return to any sort of heavier lifting until closer to the 1 year mark. It has been 3 years now, and I am starting to get pain when I throw even a football. The capsule feels loose as well. Would that be indicative of possibly needing a follow up and possibly a revision? Furthermore, would the tenodesis lead to more shoulder instability since the long head contributes to the stability of the glenohumeral head into the labrum? That would be my main hesistation. Your video was really helpful. Thanks!

    Hey Michael, that is a great question. In general a biceps tendon tear will not cause the sensation of looseness, it typically causes anterior shoulder pain. It is always good to get an eval by your doc. Good luck and let us know how you are doing. I don't think a biceps tenodesis typically causes instability at least not at our age!.
