Why is Shoulder Pain Worse at Night?

Shoulder pain often gets worse at night because of the chemical changes happening in your body. Your immune system is increasing inflammation to remove damaged tissue and initiate healing. You have slowed down from the day and you aren't moving as much. The pain and stiffness is settling in and without gentle range of motion or other activities to manage the inflammation the intensity of the associated pain increases.
In a second video I will share some recommendations to reduce shoulder pain and promote healing.

    It also hurts more because you're less distracted. Your body's mental defenses mechanisms decline at night, so you're a lot more aware of the pain.

    Don't think this is true. Cuz I'm literally lying in the bed doing nothing lying on my opposite side and I feel no pain. However, last night I lied this same way and I was in so much pain. I got about 2-3 broken hours of sleep last night

    Thanks for this video. I injured my shoulder last April during a incline dumbbell press . My physical therapist gave me some exercises to do and all that did help . Now what I’ve been dealing with is exactly what you are describing . Feeling ok to good during the day but waking up in the morning with horrible shoulder pain. Now I understand why. Thanks

    Or in the middle of the night waking up with horrific pain unable to move the entire arm/shoulder and not being able to get back to sleep. Doctor could not seem to understand why I was having many times worse pain at night that my pain med took care of during the day, but couldn't come close to working on during the night.

    Some great tips...thank you! I recently had rotator cuff repair (about 8wks out) and sleeping just totally sucks. In fact as the days go on I feel some of that pain I experienced prior to my surgery seems to be returning, through my range of motion is improving. I'm trying to stay positive, but the struggle is def. here. However, Thank you as well for describing more of the chemistry behind healing, ice really helps, so does a bit of heat after work. Some PT folks don't explain these issues, I'm happy you addressed this issue!

    Same here, Deborah. I had the exact same surgery as you 11 days ago. I’m so anxious about overdoing the PT that I’m being super cautious. Very often my shoulder aches badly after gentle exercise. I’m constantly wondering if I’m moving it too much or not enough!!

    I had a shoulder replacement on the left side 2 years ago. It has never been the same. Like you said pain worse at night. Several months ago the other side is very painful. A therapist has me doing several gentle exercises that were done in the beginning like I’m back to square one. I hope this gets better in time.

    Hello Tony-M, I am on day-4 of the LT shoulder arthroscopic surgery----Three Arthrex screws at three different locations on the humeral head for re-attaching the tendons of various muscles using criss-cross bio-synthetic threads, Push-lock repair of SLAP and labrum and the OPUS for the rotator cuff. The Brachial plexus block came off after 18 hours and pain started appearing. I started with 400 mg of Ibuprofen, used ice and finished the first night. The sling that the surgical center provided was only an arm sling and NOT sling & swathe variety where the forearm stays tucked on the belly and does not allow any free motion of the arm like a pendulum. So, I noticed that as I get up, my arm was moving like a pendulum and I instantly realized that this was a WRONG sling. I asked my family to quickly switch me into Sling & Swathe that I had from the dislocation a week ago. The next day, the Hydra-Chiller unit came in and I have been using it 30-60 min sessions, 2-3 times a day. But the most bothersome question I have is the type of sling the surgery center gave me. I thought that my sling would be slight abduction type with a foam-block or wedge in the arm-pit to keep that abduction and I am going to place the same question to my ortho-office. Day and night are all mixed. I sleep when I get sleepy. In-between, I am trying to remain active with the rest of the body and RT arm/forearm/hand/wrist/fingers.

    Thank you for this explanation. I"ve had both shoulders repaired. Full thickness tears both times. Night pains were the worse part of each ordeal. I remember I actually looked foward to my second shoulder surgery because I knew the night pains were going to go away.

    I’m in exactly the same position. 4 weeks ago had covid jab, next day, arm and shoulder pain. Been for remedial massage and it’s helping.

    I’m in the same situation 11 months ago my right shoulder and 2 months my left shoulder,sleep time was the worse and I feel like this recovery is more slow and painful a certain point 🥲sometimes feel going backwards

    I've had rotator cuff and Biescep securgy, working on 6weeks. I do most of my therapy in my pool ,it feels good and I bleave it's helping me progress TP is tough he said I'm doing great.

    I think your right because my shoulder pain starts after I watch tv for awhile. Then before bed I get active while and fall asleep ok then 4 to 6 . hours later a ache comes on bad.

    I go in for rotator cuff surgery, Dr described my tear as …”massive tear, significant atrophy”. Can you elaborate more and also, the back of my trap really bothers me when I’m at the keyboard after 10 seconds or so or just having my arm up slightly with little or no resistance.? Can you shed some light on that please? Thank you!

    I’ve been down the rabbit holes trying to find out WHATS actually wrong so I have a plan. I had my doctor give me a referral on feb 16 of this year, it is now march 3d and the orthopedics never got my referral. I never got my mri results back. It’s frustrating.

    I’ve had a reverse shoulder replacement and in the morning I’m in agony I do nothing but try and rest use the whole of my double bed and then get up aching , I’m going to try light exercises as you have said and hope this takes away the pain first thing in the morning and throughout the night ps I’m 4 weeks after my op and the 12 hour MR tablets don’t seem to do anything I’m really getting so stressed I’ll try anything it’s midday I’ve just got up still in pain I’m going to do some light exercises and do them tomorrow at 8am and hope this relieves most of the pain thanks in advance.     It can be many times worse than what you experience during the day. It's my understanding cortisol levels drop drastically at night and that's what causes the pain to increase. I don't know why doctors don't understand this. My doctor acted like I was nuts when I told them how much worse it was at night.

    It’s common among adhesive capsulitis. The pain does not ease much during the day either.

    I didn't wear the abduction sling very long during first six weeks I had a second surgery on a bad injury where surgeon removed the hardware I'm at week seven and not doing great I'm going to PT twice a week I'm hoping I didn't damage any repair Should I try to push harder or slow it down I'm 63 Thanks Donna

    I had a misplaced Covid jab over 6 months ago and have had intense pain in my shoulder ever since. After having a scan and speaking to a qualified physiotherapist he tells me l have impingement and rotator cuff tendinitis. I am now waiting for an appointment for exercises. Will this actually help with my problem?

    I fractured my shoulder when I was 35 and was told nothing can be done. Now, 72, at night I cry. Thank goodness just a few nights a week or sometimes everynight. What exercise?

    I am Sachin De from India.. I slipped and fell on my right shoulder in the month of Oct'21. I had an MRI done and it confirmed that it was an rotator cuff tear. On 4th of Feb'22 I went through the rotator cuff tear surgery. I was advised that until six weeks I will have have my hand in the sling and post that will get started with the physiotherapy sessions. However, I am in my fourth week post surgery but since last couple of days I am feeling more pain in my right shoulder and specially in the morning after I wakeup. I really want to understand how would I know if everything is still intact I mean the attached muscle

    Based on the most current clinical research, neither heat nor ice can penetrate deep enough into the joint to actually create a change in the physiology. So warmth is often considered soothing and relaxing to the patient. Ice has a chilling, numbing sensation on the skin. The best clinical advice is to first trust in what your surgeon recommends and second, look at the clinical research which suggests neither treatment makes a difference and you should use the one that feels best to you.

    Thanks for the advise, Sir, can you say something, that my right shoulder was over striched when I was doing exercise lying on the bench with the dumbles, weight is about 14 pounds each, at the moment I am 70 with a good health, but the pain is still their from the last more than six months, doing light exercise and isometric but invane, also I am not taking any medicines, in the day time no pain, but in the sleeping time I feels which disturbing my sleep.     I wish I would have run across this information 4 nights ago. Post op day 9 rotater cuff surgery with other stuff done. 13 holes in my shoulder. Not in too much pain during day but it starts getting intense around dusk, and I have spent 3 of the past 5 nights up all night, crying and pacing floors. Pain meds didn't touch pain. Shoulder blades and neck muscles feel hugely strained. What the doc said makes perfect sense. I wish my own surgeon would have been as concerned or as knowledgeable. He prescribed a set daily amount of OTC tylenol, etc. I'm really mad about it too. I applaud this doctor's advice and information.
