I discuss what is a normal insulin level vs insulin resistance and how to interpret your insulin test results. I also share 2 studies: One of them looked at how insulin levels change throughout the day and the other one looked at how different diets affect insulin levels.

Before looking into insulin test results, what is insulin?
Insulin is a hormone responsible for regulating blood sugars. Each time you eat, you elevate your insulin. The more times you eat or snack something - the more insulin you will have in your system.
In normal conditions, when you are insulin sensitive, the amount of glucose and the amount of insulin would be perfectly balanced.
Having too much insulin over time, results in insulin resistance. Insulin resistance is a state where cells in your body refuse to let glucose in to be used as fuel. The amount of sugar or glucose in our blood should always be around 100 mg/dL. It is equivalent to around 1 teaspoon of glucose in your blood. 1 teaspoon in 5 litres of blood.
How do you test your insulin levels?
You can test either your fasting insulin levels, your C-peptide levels, or your HOMA-IR.
How to interpret your insulin results? And how to know if you are insulin resistant or not?
That’s a normal range:
FG (Fasting Glucose) 70-100 mg/dL or 3.9 to 5.6 mmol/L
A1c Less than 5.7
FI (Fasting Insulin) 2 - 5 mIU/ml
Less than 5 mIU/ml - normal
Less than 3 mIU/ml - ideal
HOMA-IR 0.5-1.5

How to interpret HOMA-IR scores?
Healthy Range: 1.0 (0.5-1.5)
More than 1.5 indicates early insulin resistance
More than 2.9 indicates significant insulin resistance
- HOMA-IR test measures how hard your body has to work to keep your blood sugars at normal levels.
How to interpret fasting insulin results?
Up to 5 is normal.
More than 5 - you are insulin resistant
If your result is 10 or more, you have a high level of insulin resistance
And how to interpret C-peptide test results?
The normal range for a c-peptide test is: 0.51 to 2.72 ng/mL
This may also be expressed as: 0.17-0.90 nmol/L

And here’s another study that looked at how different diets affect insulin. It compared the high carbohydrate diet, low in saturated fat, and the low carbohydrate diet, high in saturated fat. The low carbohydrate, high fat diet resulted in much lower insulin concentrations.

    7 months ago I had a TIA. I was diagnosed a 2 diabetic almost 10 years ago. I was taking glimiperide and 2,000 metformin a day and averaging a 175 glucose and a 7.5 A1c. In Febuary I went healthy keto and had a bunch of tests done last week. My A1C is now 5.6 off all diabetes meds and my insulin level was 3 after a 12 hour fast. My cortisol and CRP were also normal. I'm kicking myself for not doing a baseline in Feb but atleast I know things are good now. On a side note, since starting Keto my cardiologist took me off my heart and high blood pressure meds also. You are what you eat, eat healthy.

    A comment : HOMA-IR normal population reference value is 1.7 to 2.5 ( Varies for every population and ethnic group.) The figure 405 in denominator of HOMA-IR, which corresponds to normal insulin resistance value i.e. 1, was obtained by a multiplication product of lowest possible glucose (63 mg%) and lowest possible insulin (6.43 microU/ml). Insulin assay is a very sensitive test. Reported insulin level values depend upon various factors, such as method and kit used, time elapsed between sample collection and testing etc. Standard laboratory with good sample load is desirable. The present reference values ( 2 - 24 ) were determined long ago when specific insulin assays were not available. The range needs to to be redefined, may be 6 - 10 is more realistic.

    Thanks for your great video. Around 5 months ago I was suffering fatigue and pain in liver area, although I was not overweight and my diet wasn’t so bad I did an ultrasound and found I had some fatty liver. So in the last five months have done keto and got my bloods done. Do you think my results are good and given I am at the lower end of normal weight range, would you come off keto and have a low carb diet in order to put a few kg of weight on or just keep with keto and eat more fats (although this is making my LDL very very high)? 1). Triglycerides at 0.8mmol/L -or- 71mg/dL 2). Body fat % - 10-11% 3). Fasting insulin - 2.6mIU/L 4). Fasting glucose - 72mg/dl -or- 4.0mmol/L 5). HbA1c - 4.2% -or- 22mmol/mol 5). HOMA-IR - (72x2.6)/405 = 0.4622 6). Fasting Insulin Resistance Index = 0.4 7). HDL cholesterol: 2.5mmol/ -or- 97.67mg/dl 8). LDL cholesterol: 6.9mmol/L -or- 266mg/dl (must be due to the keto diet) Many thanks in advance and appreciate your kind help

    Hi Dr. On lchf diet, fasting serum reduces greatly. How low that can go to and still safe not to get into hypoglycemia? I am on moderarely low carb (60 gm per day, high fat 90 to 100 gm) diet for last 3 months to reverse diabetes under doctor supervision. After 3 months, recently, I got 0.4 of fasting insulin (fasting glucose 110). and fasting c-peptide came to 1.33. My HbA1c reduced from 6.8 to 6.1. 3 months ago my HomaA2IR was 1.24%, fasting insulin 2.4 and fasting glucose 125). I am still taking 500 mg metformin twice a day. How do I interpret this and is there anything to be concerned about?

    Hi....i hv 5.8 on my Hb1AC, 87 fasting glucose ....i am curious about my insulin condition..if its low, fine, or IR....will blood test of fasting insulin and antibody enough? Because the lab here are confused when i ask about HOMA IR.. Thank you very much

    It was a nightmare just getting an insulin level. You gave me the numbers I needed to begin to figure out how bad it is. The insulin is 7. C-Peptide is next. Intermittent fasting and fewer carbs will allow me to finally have something my mother never had: a chance to be free from the pills, the 20 years of watching patients having complications. What you have done is give me the knowledge I need to have a life most don't understand. Medicine has failed us. Thanks for this video. After 20 years of Respiratory Therapy, and seeing all the failure, people like you are what real medicine should be. Blessings

    On 12/9/21 Blood sugar fasting 112. AIC 6.1. Insulin 2.9. 67 years old. Pre Diabetic range. Was on Metformin/GB for many year. Went off it last March. Went on low carb diet. Lost 30 lbs. Now weigh 222 I am 6 foot 4. I had a pituitary tumor removed 24 years ago no long term return of tumor. Morning blood sugar typically 115 average. I seem to urinate a lot at times. I use to use a spray at night for Diabetes Insipidus due to pituitary tumor removal. . Thoughts?

    Hi, this sounds like type 1 diabetes, definitely get checked. Glucose 8.5 is pretty high.

    ​Hi Greta! How would you interpret this? When I was first diagnosed with PCOS, my fasting insulin was 12.1 mIU/mL and fasting glucose was 96. NOW, my fasting insulin is 6.3 μU/mL. Are these units the same? I checked online conversions but I'm still confused (milli vs micro). AND I STILL have all the symptoms of insulin resistance, hypoglycemia, and high testosterone so I am really surprised to see the insulin number go down. It's so confusing. Am I improving or am I worse as I see no change in symptoms? My current A1C is 5.2% and fasting glucose is 89.

    Hi doc I had a fasting insulin of less than 2, a1c 5.2, and fasting blood sugar of 96. Could the insulin level be problematic? My doc said that was a great thing to have it below normal limits, I’ve read conflicting information

    One of the best videos I have watched on the topic of insulin resistance. Here in the US, our primary care doctors NEVER check the insulin level and we have to argue with them a lot to get it tested. Next time I will ask for c-peptide also. I have subscribed and liked also.

    Thanks for this educational content. I been battling pre-diabetes as it's genetic in my family. Due tonthe womderful facts given by health care professionals regarding IF and keto etc. Blood results: last year homa IR: 13.1 (online calculator) Today, it's 3.3 , big improvement, but still on borderline spectrum with a1c of 5.7. Good to know all these facts to monitor your health .

    @AdvancedNutritionandHealth I'm following low carb, so probably 50g-75g from whole foods. I am not doing strict keto. I am in my late 30s and moderately active. I eat squash, Turnips, quinoa at odd times, but no potatoes nor rice( high GI). I been getting good results. I would rather implement strict keto in a later phase of weight loss. As for fruits I do eat Berries and 🍐, clementine,. Veggies, yes, and have been increasing quantities. I make try to do fruits early in the day not at night but lower Glycemic fruits.

    @charllyons9261 interesting, I would say this is more 120-150g of carbs per day. My diet is low carb keto, around 50-75g carbs, and I stay away from foods like quinoa or clementines when on keto. Try once entering everything you eat to an app to see 😊

    Hello! I did my OGTT during 3 hours few weeks ago, and my doctor only told me to go on diet and repeat the results for a month. My glucose was (fasting - 4,2; 8,5 | 4,2 | 2,9) My insulin levels (fasting - 5,6; 70,6 | 28,6 | 5,2 uIU/ml) Can you please help me out 😭

    Hi, you can read this article about interpreting your results. www.racgp.org.au/afp/2012/june/oral-glucose-tolerance-testing . Fasting glucose below 5.6 and insulin below 5 is considered good.

    I'm curious how to interpret mine. I was first diagnosed with type 2 DM in July 2018 with 10.8 A1C and fasting glucose of 285. Was on seven medications for about two weeks including glimepiride and 3 metformin 500 a day. I was determined after those two weeks that I don't want to live with tons of meds. Switched to low carb through trial and error...and lost about 45 lbs in two years. Have been maintaining A1C of 5.1-5.4 since early 2019 till now with 5.3 as my latest A1c back in April 2021. All that with just eliminating white breads, added sugar, 90% fruits, pastas, potatoes and with a bit of help of one metformin 500 (extended release). I have only done fasting insulin test once...which was 7.1 (and fasting glucose at that time was 102). This was done in a lab vs my meter at home usually ranges around 80-90. Now by homa IR standard looks like I'm in the 'higher' part of early insulin resistance? The lab report says 'normal' range of insulin is 5-20....although another report says 3-20...I know I'm not on keto but if I understand this correctly...7.1 insulin is still "normal "as my body doesn't produce too much, but it's also not too little? (according to my doctor my body produces too little for my overweight body at that time when i was 45 lbs heavier). Maybe under different circumstances 7.1 is acceptable after eating, but maybe I should shoot for less than that for fasting? Hope I can do it!

    You're having some great progress. Well done. You could further improve your insulin by going fully keto. Conventional doctors would count 7 as ok. However, this level can create health damaging effects. 2-5 is the true level you should be aiming.     @@AdvancedNutritionandHealth Thanks Greta for your reply! Will try to reduce it! I do agree with what you said about conventional doctors standard...for example, I was told a1c of 5.7 is 'acceptable' especially being 'diabetic' I feel offended by that like because I'm diabetic I can't have a normal a1c?? So I am proud to maintain my 5.3! I guess I was doing 'too well' with losing weight on low carb (not keto) at about 70-80 grams/day and after a while I had to just revise my meal plan cause I didn't want to lose weight anymore (I'm now 169cm and 69 kg weight (with just my underwear). I thought that 71-73kg was my 'perfect' weight but unfortunately i had hemmorhoid surgery in dec 2019 that made me lost weight to around 67kg since I couldn't eat much for about 2-3 months. Trying to add maybe about 1-2 kg of muscle so my morning weight would be around 71 but it's definitely hard to gain weight now!

    What you said at 8'40'' is very important to me. If I understand correctly, reducing carbs or other measures such as intermittent fasting for example will reduce the level of insulin including the fasting insulin but while the body slowly becomes more insulin sensitive you can have some transient increases in blood sugar because you have lower level of insulin. And, after some healing happens, one should end up with improved glucose numbers as well. Did I get this right? Because many folks may think that low carb makes things worse

    Cpeptide of 0.7ng/ml says just low on old labs and a vid scares me (mastering diabetis) saying below1 or 0.8 is bad. Feel much less stressed after watching your vid. My last fasting insulin was 4 but went up to 50 when ate a carb breakfast(unusual for me to eat that). Dont think have diabetis , am slender and active, but sometimes low BG symptoms.

    Great information but iam little confused , Glucose 82 fasting insulin 3.1 A1c 5.3 So I calculated 82 x 3.1 divided by 405 = 0.62 Am I insulin resistance ??

    My lab results posted today and I am frantically looking for explanation when I found this video. Very clear and detailed. So helpful. My A1C was 5.7 and fasting glucose in low 100s and anytime I ate any carbs glucose went up to even as high as high 200s. Insulin was not tested at the time. That was 6months ago. I was so scared and cut out all my carbs. Today's results shows my fasting insulin 2.1, range 2.6 to 24.9 and my fasting glucose is 84 and A1C 5.4. Is this good? Now I'm worried about pancreas issues because my insulin too low. Can you please reply and share your thoughts and experiences? Labs are done after 12hrs fast. Thank you so much🙏

    Hi, this all sounds great. Your insulin is still within limits (2 minimum). Make sure to eat some low sugar/low carb veggies and fruits like salad, avocado, tomatoes, berries, nuts, etc.

    You get a cortisol spike in the morning, to wake you up and keep you going 👌 this spikes the glucose also.

    @AdvancedNutritionandHealth my insulin level was 13 µU/mL and my PCP said it was normal since below 24 normal range. I wasn't even aware that this is a problem and I need to take preventative measures... Glucose was 100, A1C 5.2%. My hair was thinning and I gained a lot of weight in a short time, and my doctor just told me that everything is within normal range and she doesn't know what causing it. Why most doctors do not do anything when insulin levels are elevated? Do they wait until it's exreme and above 24? I wish she advised me that these levels are risky and early signs of future complications...

    This on it's own doesn't show anything. Insulin must go up after eating. You need to know your fasting glucose and fasting insulin, HbA1c is also useful.

    Wow I'm surprised to see that basmati rice actually has a higher glycemic index than spaghetti or fries? That is so confusing. I've had basmati rice quite a few times (cooked about 40-50 grams mixed in with a fiber dense red rice), while it did bring up my glucose a bit...definitely not as high as (regular) spaghetti or potatoes. Maybe different food affect different people?

    Hi, different product brands can have different nutrient content. Also, the cooking method, and what you eat it with affect blood glucose response. Rice mixes with some wild or other type of rice that contain more fibre would be healthier. And yes, everyone can have a different blood glucose response. However, if you have Insulin resistance or type 2 diabetes, these foods would only make it worse.

    You should do a HOMA-IR test to see if you're insulin resistant. Sometimes too much insulin can push your glucose too low. Also, checking glucose and insulin at your doctor's office at a certain time won't give you the full picture of what's going on. Using a CGM device would be more accurate.     Hi! Great info and easy to understand. My husband's glucose was high but yet his insulin level is normal and his resistance test came back negative. Can he have diabetes with normal insulin levels? I'm so confused about that. His symptom was dropping alot of weight all of a sudden. Also, what would you recommend for his diet since he cant afford to loose more weight. He cant get in to see an endocrinologist for 4 months so I'm hoping you can help with those 2 questions. Seems like the keto and low carbs will just make him loose more weight. 🙁

    Hi, it could still be type 2 diabetes or prediabetes. At later stages of diabetes you might have too little insulin. What was his HbA1c level? Just get a blood glucose monitor and check glucose readings in the morning and after meals. Is he very thirsty? Does he have any other diabetes symptoms? Continue limiting high sugar and high starch meals. Is he still in the normal weight range for his height, or is it getting abnormally low? Including bulletproof keto coffees and extra fat in a form of fat bombs or chocolate could help to keep the weight stable.

    Recent labs for fasting BG 113, Insulin 7.9, C-Peptide 1.58, A1C 5.0, HOMA-IR 2.2 - I don't know how to gauge these results in light of being on a carnivore diet for 5 months. BG & A1C were lower on Keto prior ( 90 & 4.8), insulin about the same.

    @@alyygirl122 I was able to bring my labs back into my prior Keto range by lowering the amount of protein and increasing the fat. My BG stablized to under 100 and my ketones picked up right away. So no, I have not had any hypoglycemic events and my last insulin was at 3. Still waiting on results from a recent test. My weight loss however is still in a stall but I haven't gained any more.

    Hi, what's your fasting glucose? Did you do the HOMA-IR test? From current results, your glucose is normal, but insulin is elevated. Meaning, your body has to work harder to maintain normal glucose.

    @alyygirl122 you don't have it anywhere written? Their classification is wrong, so it would be useful to know the numbers. What symptoms are you having? It can be many different things, not only hypoglycaemia 👌 also, knowing that your insulin is quite high, it might push glucose too low sometimes.

    Hi, your fasting insulin is low. The optimal range is between 2 and 5. Your fasting blood sugar is good, but on the low end, and HbA1c is still good but on the higher end.     You should work on lowering your glucose and insulin slightly. It is not bad, but could be improved with less carbs (sugary and starchy), some intermittent fasting, more walks, less stress, and better sleep 😊

    Hi, it seems that your insulin levels are too low. Insulin levels that are too low might indicate that there's something else going on in your body. Definitely get further tests done. Studies show that insulin levels below normal are linked with dementia.

    Hello dr 3 months back i was eating too much carbs and sweets and i was doing frequent urination and i lost weight then i went for check up and my fasting glucose level was 252 mg/dl. Den now its been 3 months since i started keto diet and now my fasting glucose level varies between 90 to 120 mg/dl I did hba1c and it came 8.19% and my fasting insulin test came 2.3 Micro international unit /milli litter So am i doing good? Its not type 1 right?

T2D for over 19 years started strict keto and than to low carbs didn't take a baseline before i started in April 2019.( Off all diabetic medications ) 10 Dec 2019 Fasting data Insulin -4.2 , BG 6.08 , Homa-IR -1.138 Hba1c -5.5 18 Mar 2020 Insulin -5.7 , BG 5.3 Homa-IR-1.346 Ha1c- 5.3 May 27 2021 Insulin 7.2 , BG 4.7 Homa-IR - 1.5080 Hba1c -5.4 Did a C Peptide 1.13ng Why is my fasting BG getting lower and my Fasting Insulin going higher and I am getting more Insulin resistance ? Can it because I switched from strict keto to low carbs diet by increasing carbs and protein and reducing fats ? ( BMI 19.4) Ps The lowest Hba1c i went down to was 4.9% - 22/July 2020 ; before I moved from strict keto to low carbs .( didn't test for the test ) Thanks

    Hi, it sounds that introducing more carbs is causing more insulin and insulin resistance. have you tried healthy keto lifestyle where you get approximately 50 g of carbs a day. Going high fat, low carb, moderate protein is very important for reversing insulin resistance. Fat is the most neutral when it comes to blood glucose and insulin response 😊

When my weight was 6 kg below my "ideal weight " , I switched to low carbs . ( I didn't want to loose anymore weight ). From my test results you can see that my fasting blood glucose was much higher but lower insulin - when doing keto but even though i switched to low carb , ie increased carbs :- My fasting blood glucose is much lower but insulin is higher , this is what i don't understand ? ( I thought if my fasting blood glucose is lower shouldn't the insulin should be lower too ?)

    Hi, your insulin shows that your body struggles with too much carbs. Your glucose might be normal, but your body is making a lot of insulin to keep it low. You need to cut down on carbs, avoid snacking, and reduce your stress levels. Also, eat clean, all calories have different effects on your hormones.     Hello! My fasting glucose level is 97 (reference value 74-109) But my insulin level is way too low, it's 0.4 (reference value 2.6-24.9) and HbA1c is 4% I don't know how to interpret this. I'm 24 and I eat extremely healthy and I exercise regularly. I'm not everweight. I've been on intermittent fasting for 3 months. Could you help me understand this correctly, please?

    Your insulin is elevated. Try to cut down on snacking and reduce carbs, go for walks daily, eat more veggies. It will get better with time. For example, my insulin is 5 and my HOMA-IR is 0.9.

    Hi, you're welcome 😊🧡 it's all good. You're insulin sensitive and your glucose is normal. Check your glucose after the meals too, to make sure you're eating right and won't progress into prediabetes and diabetes later on in the future 👌

    So I have higher than the normal morning sugars, sit at about 5.7, never after meals and they always go down after breakfast My fasting insulin is 22IU/mL and a1c was 5.3 My doctor refuses to diagnose me sith type 2.. as I have a psychiatric disorder that causes irrational fear and anger, im very high strung. He claims that can drive insulin up.. Thoughts?

    Hi, what is your diet? What do you eat? What medicine are you taking? Is your fasting blood sugar 5.7 mmol/l all the time? 5.3% HbA1c indicates that you're not diabetic. But very high insulin shows you're insulin resistant/prediabetic. Your body works very hard to keep blood glucose low.

    @@AdvancedNutritionandHealth my diet is Blueberries & raspberries, low carb protein powder, cacao powder, avocado oil and flaxseed oil with some chia seeds made into a smoothie for breakfast. For lunch I have 200g of beef strips made into a burrito bowl, this consists of 125g brown rice, 175g lettuce, 75g of mixed beans & corn, 50g of avocado & 15ml Light Sour cream. My dinner is 125g of high protein tortellini, capsicum and sundried tomatoes with some mushrooms & pesto, the proteins for this meal are 20g anchovies in evoo & 150g shrimp W/ 10ml Avocado Oil (extra Virgin). Snacks are 100g of Apple & 1 pack of Crackers and Cheese (30g) Totalling 2100 calories - 5 ft 7 male, weight training 5 days a week, minimum 8000 steps My sleep is trash, I have insomnia and very high stress, my cortisol is often quite high due to the mental health aspect.

    Sugars are only usually above 5.5 when I eat things like junk food, which is seldom or I don't sleep well.

    Hi, it's OK, not too bad. Aim for 4-5 for insulin, and 80-85 for glucose. Eat less frequently, 1-3 meals a day with no snacking, and reduce carbs, eat more non-starchy veggies, and less rice, pasta, potatoes, sugar, etc. Also, avoid vegetable oils and any highly processed foods.

    Thanks for quickl reply..I m 40 now Started test for sugar in 2019 at the age of 37+ That time my BSF was 94 And PP was 132 After 6 months my BSF was 99 And this time i went for Hb1ac and it was 5.6.. After this i used to check my hb1ac in every 6 months and it never cross 5.6 Only in this July My BSF was 105 But in same day test Hb1ac was 5.1 Now my hb1ac is 5% .... So am i progressing diabetes or not Because my father was diagnosed diabetic in his late 50

    At the moment your numbers are alright 👍😊 my father had diabetes too. We are more likely to get it, but we can prevent it if we eat less carbs
