TMJD Signs & Symptoms in Relation to TMJ Anatomy - Priya Mistry, DDS (the TMJ doc)

Hello Friends! In this video, I discuss the various signs and symptoms that come along with TMJD. These can be vast and varied and this is why TMJD is often called "the Great Imposter Disease". Because the symptoms are so varied, it can be very difficult to diagnose TMJD. I do my best in this video to explain how these signs and symptoms relate to the anatomy of the temporomandibular joint and it's various structures.
If you're unfamiliar with TMJ Disorders, common signs and symptoms include but are not limited to: headaches, neck pain, jaw pain, ear-related concerns, shoulder pain, specific tooth pain, jaw joint noises, and/or episodes of the jaw locking or catching.

    I'm SO glad I've found your channel. You're a wonderful educator, and you're giving me great insight into what might be going on with me. In fact, what you describe at 16:26 is EXACTLY what I'm going through right now. In the last week, I saw both a neurologist and an otolaryngologist.     Tmj has been by far the worst/ scariest/ most painful experience I have ever gone through. So hard to find a qualified dentist to treat it. Very expensive as well. Had symptoms for years that kept getting worse with stress. The stabbing ear pain is horrible. Original Dentist sped up my tmj issues with doing an impression that turned into slipped discs and teeth misaligned, and horrible jaw pain. Haven’t been able to eat solid food for 3 weeks after that dentist apt. Reg dentists are so unqualified when it comes to tmj symptoms. Praying on my tmj doc to fix this next week 🙏🏻 this is horrible to deal with.

    Thanks so much, I’m terrible at science and wouldn’t even know where to Google to find out the right muscles are even involved, the visuals are really helpful to understand why I’m doing a particular exercise. Im more likely to do something I understand the logic why.

    Hi Dr.Mistry. About 4 months ago, I developed TMJD very acutely during dinner one night on the right side of my face with a sudden sharp facial/jaw pain during during chewing. Most of the noises when I chew are crepitus. I had never had TMJD before. About 6 months prior to TMJD episode, I had dental work done on my right very last upper and lower molars to close the space between them and the molars next to them. Since then I had started experiencing sudden focal pain on the distal molars when biting on hard foods such as cereal or nuts. Over the months several adjustments were attempted by my dentist to provide relief with no success. During that time, I ate mostly on my left side to avoid this pain. Images done by my dentist did not show any apparent tooth issues. Per his initial recommendation, I saw a TMJD physical therapy specialist. She identified a displaced disk on my left side of my face and "manually reduced it back to proper position". She prescribed several facial therapy exercises. After several weeks, still no relief from TMJD was obtained. I also felt like that I can no longer bring my teeth together for chewing on my right side . My dentist tried a using a frontal teeth spacer to deprogram my bite by using it 24 hours x1 week and at night only x 6 weeks. During this time I tried to use anti-inflammatories, hot and cold applications on the muscles of my face. Still no relief was achieved. He sent me to a neuromuscular TMJD specialist dentist. He did several images including x rays, tomography, and CT scans. He recommended dental orthotics to be worn over 24 hours around the clock including eating meals etc. for the rest of my life along TENS, Thor Laser Therapy, K7 therapy during the first weeks. I had never heard of 24 hour for life orthotics. I teach classes. I am not sure I can tolerate a 24 hour orthotic. Do you have any suggestions. Thank You, Brian Guvenir RN BSN

    Thanks for another great video. I feel I know more about TMD than my dentist. He did, recognize my clenching and need for a hard mouth guard, but I think I need to move on to a TMJ specialist before my symptoms get to the debilitating pain I keep reading about. Any recommendations in the Los Angeles area would be great.

    Hi thank you taking the time to make these videos. Having never suffered from ear infections I've found it so strange to suddenly get them. This week I thought I had ear infection but a friend brought it to my attention that I may be grinding my teeth. Long story short... I know I grind and I now have a sore swollen tongue, and my top and bottom back molars are extremely swollen and sore as well as the painful and stuffy feeling ear. Are these possible signs of tmj? (Just finished a course of meds so technically symptoms should have cleared up now). Oh and lastly I've often become aware of how I clench my jaw very tight at times. When I realise it I try to relax it... And I also have a habit of chewing my inner cheeks when anxious.

    Thank you so much for the information!! My medical journey has been very a mess!! My dentist said I have RA in my jaw. I asked for x-rays but never received. Now I have the Crepitus sound. You mentioned an orthotic. What type of Dr. can help me with that? Thank you again for your videos, I don't feel as lost anymore!!😁

    Hi! Thank you so much for your videos, so incredibly helpful. My sound is more like a clunking on the right side. I have to close the right side of my jaw before my left side will close. Would that be DD w/o reduction?

    It could be. It could also be that you're able to go outside of the boundary of the joint. The clunking is more consistent with hyper-extending the joint and going outside of the boundary of the joint. Please watch this video: kzread.info/dash/bejne/qoqqscGtnJPdj9o.html

    Have you seen cases where the muscle in the temple region and jaw area have atrophied prior to disc displacement w/o reduction? My masseter and temple muscle atrophied then I lost the ability to shift my jaw to one side and can only open two fingers. These vids are really helpful in understanding but I have yet to hear anyone talk about muscle loss.

    I'm a very nervous person so I have to learn to let my jaw hang loose & breathe through my nose & avoid being overstressed by life situations at all costs that means avoiding my husband's gemini brother lol

    Will the orthotic help all the pain associated with all the sounds the jaw makes? I mainly have the crackling and clicking… my orthotic comes next month, so I was wondering.

    It can help with clicking. Crackling is more difficult because it’s due to problems within the joint

    Will constant ear fullness from TMD ever completely go away? I’ve been getting massages once a week for about two months and it hasn’t completely resolved. The fullness is worse on the side where my TMD is worse.

    I've been dealing with tooth and earache issues and thought it was an issue with my teeth as I had a couple that needed attention, I finally had a dentist remove 2 teeth that I was sure was Causing the problem and did some bone grafts. Well after the extractions and when the pain medication wore off, I started to have the same pain every time I would eat even the soft diet during the healing and then noticed I couldn't open my mouth more than maybe 3/4 of an inch without extreme pain and the earache an headaches became unbearable. My dentist wanted me to come in for impressions to continue my dental procedures but I explained my issue and he suggested warm compresses to help it and maybe set a PT appointment for to treat TMD, my primary doc ordered a neck CT to rule out a problem with my submandibular gland which is slightly enlarged, however also suggests treatment for TMD. Still confused and concerned with how all the excruciating pain in my jaw, head and ear could be attributed to TMD I found your channel and this video finally put it altogether for me, I understand now that I have to address the TMD with a professional, I only hope I can find someone with your expertise in my area.     Hey, doc i am dealing with pain in my right ear, been through several ENT's but didn't found any infection. My left jaw clicks and whenever i open my mouth my upper jaw acts stiff, also I am sensitive to loud sounds is this TMJ? My maxillofacial has prescribed Durajoint GM for two months. Need your help!

    I forgot to ask could you make a video showing us exactly what a orthotic looks like? Which teeth it covers if worn top or bottom teeth is it worn daytime only or night too and do you eat with it. How long its worn for 12weeks?

    If you follow me on TikTok, I've made some videos that shows what an orthotic looks like. It's really not helpful to know what it looks like, though, because it looks similar to a dental nightguard. Sort of like all eye glasses look similar but it's the prescription built into them that's unique to each individual that makes all the difference.     I normally get TMD pain in the morning or sometimes when I speak. My posture isnt very good and have very very tight trapezius muscles when I wake up. It actually is relieved by eating strangely. Mines not debilitating and more a minor annoyance, but it's the thought that it could get to the point I cant speak or eat that worries me and I struggle not to think about that. I wear a nightguard btw

    @@bennybenny7382 Well, a nightguard can work for a few people with TMJD but it is not appropriate for treating this condition. A TMJ orthotic is far superior. Having said that, if your discomfort is mild then it may fine for you to continue wearing your nightguard. As for progression, your symptoms could stay as they are OR they could progress - it's impossible to say.

    Dear Dr Priya thank you for excellent videos and information. I have very strange case, I have tension headache in my left side and according to MRI images I do not have any problem with the join and straight in my spine at the neck. I tried many treatment scenarios such as relaxation medicine, 300 physiotherapy sessions, and trigger point injection (7 session each session is about 80 injections for muscles knots) in trapezius muscles but still my headache is still and the face and scull muscles still very tide. when I press in my face muscles I hear the Tinnitus in my head and ears. still I feel there are some muscles knots in my neck and the spasm muscles in my face and head. please advice I can answer any question for more clarification ...

    I have been dealing with tmjd for years now. I wear an orthotic, but it seems I clench so hard that sometimes it's painful in the morning. I did arthrocentesis but that did not help. I've also noticed that my jaw slides sideways whenever I open my mouth. I am from Trinidad. What do you recommend I do next.

    Hi Doctor! The only time I experience a pop noise is when I am chewing on one side of my mouth. When I am not chewing and I open and close my mouth, I do not hear or experience a pop or click noise. I only hear and experience a pop noise when I am chewing on one side of my mouth and it’s when I bite down on the food with my back teeth on one side. Would that be the same thing as Disc displacement without reduction?

    I also have 24/7 dizziness, but I am missing rear molars on one side, Oral surgeon told me this is messing up my bite & causing issues, maybe you might have the same or similar?

    Hi Doctor I have taken my right wisdom teeth out, ever since I have been having jaw pain and tooth pain, my Dentist took Xray and checked my tooth and no problem there. Does this mean I have Tmd.

    Hi Dr.can I ask each mouthguard is making my TMD worse like the bone and jaw sore. I can't take any medicine due to reflux issues and sensitive stomach and rebound headaches I've been in constant pain. Also this recent dentist I've been to told me I have an infection within the jaw. Does that make sense ? You can get infection from this? As I REALLY do not want to be on antibiotics if I don't need to but also need relief at this point. I think my main root is stress clenching and muscular... what is the best treatment or specialist to see for this ? Thank u!

    TMJD alone cannot cause an infection. Also a mouth guard is very different than a TMJ orthotic and often is not appropriate to treat TmJD

    Just wanted to update- I think it's my stomach he was referring too. I definitely have some inflammation and gut issues and that's when I started having jaw pain. I just got a splint but my jaw hurts more! It is so tight and I can't take any Nsaids- do you think the splint will help overtime? I just am worried if I don't get it to relax first the splint will make matters worst. not better. Thanks,

    @@daysfan221 well a splint is not necessarily a neuromuscular tmj orthotic. I really don’t know what kind of appliance you have so I don’t know how to answer your question. Sorry!

    I have some of these symptoms, ear pain, crackling noise, and pain that cause vertigo, but only on my left side, is that common?

    it includes tingling/numbness in cheeks. it could affect the gums and yes, tinnitus too.

    Hi mam, I got some clue from your video on and the incident happened to me 2 days before. I was biting on a raw mango which was in fridge for a day. It was crunchy bite of fresh one.. out of love toward that raw mango taste I had a bite of it after leaving some peices in fridge for 14 hr. It was kind od of leathery crunch bite.. suddenly my right tmj had severe pain and had 4 or 5 pops.. I guess it is because of improper bite. I am cautious for last 2 days hoping the problem will go of as there was no locking or cracking when jaw is opened and closed. But felling that wrong bite incidence when biting some thing not soft. It like some going out of the space towards right (right side out side not forward) this my observation .. it may be forward clicking if u say so.. thisnis happening when taking bite wrongly and hard bites... can u please suggest me on this

    @@theTMJdoc Did u close those gaps or did u get implants instead if you dont mind me asking? Also thanks for being so transparent, its kind of both comofrting and motivating to see that you look healthy and fine despite having extractions. Makes me hopeful that I too will be able to reverse the damage done by extractions in near future,. Btw ur vidoes have been so helpul in finding the right TMD ortho for my situation, so thankyou so so much for spreading awareness about this issue!!

    I lost my right tmj in an accident biut my left one is there and has ALWAYS popped.. im 41… no pain tho. Should i still get it looked at?

    My whole life has stopped since I had crowns made too tall on august 22, 2022 …. I’ve been off work in level ten pain and inflammation trying to navigate what doctors can help me ….. 😭the ER said my left TMJ is bone on bone

    From last few month I had clicking sound but suddenly my mouth get locked in closed position, mouth opening is only 2 finger width. I have deepbite and right side deflection of jaw while opening mouth . (In mri of TMJ both joints are normal.) But I can't open my mouth more than 2 finger width. clicking sound is gone now Mild pain in right TMJ Please can you tell me what is the reason of reduced mouth opening and what are treatment . I am using removable appliance bite blocks in upper teeth for the deep bite .

    @@yryfif hmmm. To me it sounds like you’re experiencing disc displacement without reduction - I’m unsure as to why the MRI report is not reflecting that.

    The pain is like being stabbed & burned in your ears and having a stabbing pain in one tooth at the same time my jaw is actually locked one one side so it's twisted with arthritis 🤪

    Mine has recently started and it’s strange, when I wake up in the morning the clicking and shifting of my lower jaw is gone but as the day goes on it progressively gets worse. I’m wondering if it’s the joint/jaw muscles tensing up throughout the day? I can physically see my bottom jaw shift to the left every bite down and it’s very annoying.

    It may be due to everything tensing/tightening throughout the day. Do you find yourself clenching or grinding your teeth during the day?

    @@theTMJdoc That's what I was thinking. Not sure if there's a way to stop that. I must be clenching my teeth without noticing it. I don't grind them, I do this weird thing without realizing but I end up pushing my tongue on my bottom teeth a lot, not sure if its related.
