അബുമാലിക്കു(റ) പറഞ്ഞു, അല്ലാഹുവിന്‍റെ ദൂതന്‍(സ) പറഞ്ഞു: ശുദ്ധി, വിശ്വാസത്തിന്‍റെ നേര്‍പകുതിയാകുന്നു. (മുസ്ലിം). പുരുഷലിംഗത്തിന് മേലുള്ള അയഞ്ഞ ചർമ്മം (foreskin) പൂർണ്ണമായോ ഭാഗികമായോ നീക്കം ചെയ്യുന്നതിനാണ് ചേലാകർമ്മം എന്നു പറയുന്നത്. മതപരമായ ഒരു കർമ്മമെന്നതിലുപരി ആരോഗ്യ കാരണങ്ങൾക്കായി ലോകാടിസ്ഥാനത്തിൽ തന്നെ ചേലാകർമ്മം ചെയ്തു വരുന്നതായി നമുക്ക് കാണാം. ചേലാകർമ്മം ചെയ്യാത്തവരുടെ ലിംഗത്തിന്റെ അഗ്രഭാഗം (glans) തൊലി കൊണ്ട് (foreskin) മൂടിയ നിലയിലായിരിക്കും. ശരിയായ നിലയിൽ ശുദ്ധീകരിക്കാൻ സാധിക്കാത്ത glans ന്റെ വിടവുകളിൽ smegma, യൂറിൻ, ബീജം (sperm) മറ്റ് വേസ്റ്റുകളും അടിഞ്ഞ് കൂടും. ഇത് balanitis, balano prostatis എന്നീ അസുഖങ്ങൾക്ക് കാരണമാവുന്നു. ഈ അസുഖങ്ങൾ തുടർന്ന് പോയാൽ പിന്നീടത് ലിംഗ കാൻസർ പോലുള്ള മാരക അസുഖങ്ങൾക്ക് കാരണമാവും. അത് പോലത്തന്നെ ചില സാഹചര്യങ്ങളിൽ foreskin തുറക്കാൻ സാധിക്കാത്ത നിലയിൽ Glans ന്റെ മുകളിൽ ടൈറ്റ് ആവുകയും para phimosis എന്ന അസുഖത്തിന് കാരണമാവുകയും ചെയ്യുന്നു. അത് മൂത്ര ദ്വാരം അടഞ്ഞ് പോകാനും സുഖമമായുള്ള മൂത്രവിസർജനം നടത്താൻ സാധിക്കാതെ വരികയും ചെയ്യുന്നു. ഈ അസുഖത്തിന് meatal stenosis എന്നാണ് പറയപ്പെടുന്നത്. ചേലാകർമ്മം ചെയ്യുന്നതിലൂടെ മാത്രമേ ഇത്തരം അസുഖങ്ങളിൽ നിന്ന് മുൻകരുതലാവുകയുള്ളുവെന്ന് പുതിയ കാല മെഡിക്കൽ പഠനങ്ങൾ സ്ഥിരപ്പെടുത്തുന്നു. 1990 ൽ ഇറങ്ങിയ New England of journal of Medicine ൽ ഡോ: ഷാൻ എഴുതുന്നു: “ശിശുക്കളിൽ ചെറുപ്രായത്തിലെ നടത്തുന്ന പരിഛേദന കർമ്മം, ലിംഗത്തിലെ ശുദ്ധീകരണം എളുപ്പമാക്കുകയും foreskin ന്റെ ഇടയിൽ അണുക്കൾ ഒരുമിച്ച് കൂടുന്നത് തടയുകയും ചെയ്യുന്നു”. (schoen :new England journal of medicine 1990-322) മൂന്ന്: മൂത്രനാളത്തിലെ അണുബാധയിൽ നിന്ന് (urinary tract infection) ചേലാകർമ്മം സുരക്ഷയൊരുക്കുന്നു. Glans ൽ നിന്ന് foreskin നീക്കം ചെയ്യപ്പെടാത്ത സാഹചര്യത്തിൽ, glans ന്റെ അരികുകളിൽ അഴുക്കുകൾ അടിഞ്ഞ് കൂടുന്ന വിഷയം മുൻപ് സൂചിപ്പിച്ചുവല്ലോ, ഇത്തരം ഘട്ടത്തിൽ escherchiacoli, protes mirabliis, serratia, pseudomonas, klebsiella തുടങ്ങിയ ബാക്ടീരിയകളുടെ വളർച്ചക്ക് കാരണമാവുകയും, അത് മൂത്രനാളത്തിലും, മൂത്ര സഞ്ചിയിലും കിഡ്‌നിയിലും അണുബാധയക്ക് കാരണമാവുകയും ചെയ്യുന്നു. ലിംഗത്തിലവശേഷിക്കുന്ന ഈ ബാക്ടീരിയകൾ ലൈംഗിക ബന്ധത്തിലൂടെ ഭാര്യയിലെത്താനും കാരണമാവുന്നു.

Meatitis is commonly a self-limited problem but can be treated by application of an antibiotic ointment and keeping the area dry. Meatal stenosis has been postulated to be a result of ligation of the frenular artery or from ammoniacal meatitis and account for 26% of the late complications .While the meatus may appear small, it can open adequately during voiding; this explains why most children with meatal stenosis do not present until after toilet training. The foreskin has four major roles in penile health: protection, sensation, to allow ‘glide’ during sexual intercourse provided by additional skin, and finally immune system functions with the presence of immune cells in the inner mucosal layer. When the penis is flaccid, the foreskin is designed to offer protection to the head of the penis, the glans. It is believed that this increases the sensitivity of the glans and its many sensitive nerve endings. As the penis becomes erect, the skin of the foreskin layers form a longer tube, enabling comfort with the erection and an additional gliding of the shaft skin. This is thought to reduce friction with penetration and increase sexual pleasure.
