In collaboration with Trillium Health Partners, we will be improving how patients leave hospital to complete their recovery or continue their care at home by providing Seamless Transitions: Hospital to Home. Through this new initiative we will redesign and improve this transition that better meets the needs of our patients.
And you will meet Pamela Read, one of our Share Care Council members. Launched earlier this year, the Council is a partnership with our patients and families to give them a direct voice in shaping our programs and services. Their insights and recommendations are informing the design of Seamless Transitions: Hospital to Home initiative.
