    Hey Derek, I have a question? Is it possibly a good idea to do monotherapy after pct? “My friend” was running Test/Deca/Tren/Mast for 10 weeks, switched over to TRT for 4 weeks, then cleared out for two weeks before starting pct. Currently dosing Clomid/Nolva/Proviron and Cialis. After pct, would it be a good idea to then switch over to Enclomiphene monotherapy for maybe 8 weeks, coming off for two to let levels balance back out, then hop back on cycle? Thanks dude!

    Hi derek. I know this has nothing to do with this video but i have a question. Im currently using propecia and want to start naturally boosting testosterone. My plan is to start lifting weights to put on muscle and lose fat, eat steak and eggs more often, and start taking magnesium and vitamin d3 supplements. So if those things naturally increase my testosterone, then will my finasteride then be less effective because my testosterone is high so my dht is higher?

    What a conceited argument: Attia basically claims ApoB is nature / God’s mistake, akin to damage from smoking, and should be corrected by a drug which crudely interferes with the body’s biochemistry. “If you don’t diagnose somebody with FH until they’re 50, they might not live a totally normal lifespan.” Case closed, God! Big Pharma won!

    He isn’t saying high ldl-c is ok, he’s actually very cautious to tell people to work with their doctors. He’s saying we do not know if the comparison with FH is valid, there’s a study in progress that will produce novel findings in this particular subgroup of people on low carb with normal bmi, high ldl-c, low triglycerides, and high hdl-c. Until we have long term studies on this triad, we can’t definitively say that it’s equivalent to the cardiovascular risk seen in FH cases, it’s not apples to apples. So let’s get more studies on this.

    Listening to David Diamond and reading Malcom Kendrick’s book, it is only a small cohort of familial hypercholesterolemia, who have a blood clotting issue that are at risk. As an N of one, I’m 60 years old, my CAC score is zero, my trig/hdl ratio 0.4 My LDL cholesterol in American units 406mg/dl I do not have familial hypercholesteraemia either the genetic testing has been done on me. I am carnivore have been 7 years. I have recently been to one of the top lipid specialists in my area U.K., he is indoctrinated with LDL and statins. After an hours conversation. He decided that I am at little risk. I am interested in real world outcomes, particularly my own, never been healthier on any metric measured since I’ve been carnivore apart from the panic over my LDL, and yet I have no arteriosclerosis.

The lipid specialist in question has decided that my case is so interesting that he is going to get me to have a CT angiogram with contrast and look at detailed plaque formation (or not) including soft plaque, so we will see.

    This was an absolute nothing burger of a clip. This guy said nothing of substance while using massive doctoral language just to confuse everybody in the audience watching. Essentially, what he's saying is that people are getting into the carnivore diet haphazardly and LDL cholesterol is a motive for heart disease, without qualifying or explaining statistics that talk about LDL as a motive that proves heart disease or cardiovascular issues. I want him to post even one study that focuses on peers long enough to make the claim that he's making. I highly doubt he is a single peer study that even comes close to diagnosing the fact of a higher level LDL cholesterol. Also, this guy completely ignores the fact that there are two types of LDL cholesterol, fat and small particles, and having an elevated LDL over 190, usually means you're eating a lot of junk food and processed food because you're stacking up large particle LDL which sticks to your arteries, and when you have a slightly lower LDL, that You're building lower particle LDL, which are breaking up plaque in your system. For some reason, doctors and experts on this one ignore this fact, but you don't have one type of LDL cholesterol that determines everything.

    I think I might have FH(genetically high LDL), but I started statin therapy (crestor 20mg) since I was in my early 20s and I'm almost 41 now. Is there anything else I can be doing supplements or exercise wise that can reduce my risk of atherosclerosis? I take Curcumin to reduce inflammation throughout the body, but you said reducing inflammation in the circulatory system doesn't help in this case? I also just started taking a combo supplemeny containing Pycnogenol and Centellicum (got kola) that's been shown to help stabilize existing plaques and reduce there size over time. Thank you. Oh and ive also been proactive about my elevated BP since my 20s with primarily Telmisartan. I've tried to be proactive since my 20s with treatment medications. Also forgot to mention I take Metformin for the weight loss and potential anti-aging benefits and im not diabetic.

    This was one of my favorite channels... sad to see it's dead. I thought he enjoyed being a KZreadr and creating content to help his community that he had built, but I guess he has more important things to focus on now, such as Marek Health, Gorilla and other commitments. I don't blame him. But just one video a week would have gone a long way. I hope he eventually comes back, but I won't get my hopes up. I wish him all the luck and success in the world. They were doing the polar opposite of what my Doctors have me do to get healthy, lose weight, increase my activity, and start working out. That includes getting off of 3-5 shots of insulin daily having a A1c of 480 and having to eat small meals constantly to keep from dropping out due to low blood glucose. After 10 years of Dying and going blind from following the Doctors orders, I lost 100 lbs in a year and have kept it off. I listened to the youtube Guru's about Carnivore.I lost 100 lbs in a year and have kept it off. Then I started to get fit. I can now run, vertical jump 36" , deadlift 300 lbs, perform a proper ATG back squat and perform many moves on Rings. The medical industry lied/failed me at every turn. MY ldl is 72 and HDL of 185 at last test. Everything the doctor tested is right in the middle of the good range. Now add to that after watching Dave feldman's video's on Cholesterol testing flaws, why would you even believe the testing. I went for blood work after a 3 day water fast and the doctor wanted to put me on a Statin "LDL was 195 and HDL was under 100". Correct me if I'm wrong, but the first side effect is increased chance of diabetes. Hell no, I'm not going back there. I told him I could change it naturally and he argued it's not possible. 3 days later I tested at my cost with a diet of grass fed beef fat and Eggs. LDL has under 60 and HDL was over 150. He flipped out when I told him what I ate. After all that Who would you trust??? I have a lot of faith in You Derrick, along with Huberman, and Attia. But found you, and them after getting healed. Trust takes time, and you have years of negative results to try to overcome from the lying medical industry. "Think Covid" while the carnivore crowd does not. That said, I am slowly adjusting my diet and testing quarterly. I now have to pay for tests as my insurance only pays for 1 test annually since I'm Healthy". What would you have done with the facts presented and who would you trust. This needs to be fixed!!! P.S. I am 65 years old, 5'11" 32" waist, 48" chest, 180 lbs and can out run my grand kids.

    Peter should read the Citizen Science Foundation study published recently. It basically compares two very similar healthy middle aged groups with one having a "normal" diet and LDL at 123 vs another group having a Keto diet and LDL at 272 for 4+ years Results: No statistical difference in arteriosclerosis. And the Keto group was trending better. Yeah... There you go: the [fill the blank diet] that offsets LDL harm seems to be the Keto diet

    This is a challenging topic because keto or carnivore diets have shown weight loss results and positive effects for specific conditions. I cannot seem to find a good study that looks at individuals with these diet models and the percentage of atherosclerosis development compared to others with a standard diet. I would also want to see if there was indeed any clinical significance between people with FH and the risk of developing atherosclerosis with the low-carb diet models. If i understand Dr. Attias discussion correctly, one aspect of FH that is key to longevity and risk reduction is early identification and treatment with statin therapy for optimal outcomes. I'd really be interested to know about case studies where people following a keto or carnivore diet show atherosclerosis in the blood vessels. I keep seeing things regarding labwork, but not cases involving actual plaque accumulation. Also to be clear, I'm not pro keto or carnivore, I'm just genuinely curious about this topic.

    I have FH and statins+plant based diet brought my LDL from 290 to 70. I have 60% blockage. My brother had FH and unfortunately had his symptoms overlooked and died from 95% blockage.

    @Shankar-Bhaskar coronary calcium score is a good way to track potential build up. It won't show soft plaque, so it's not full proof. If score is zero risk very low. If it starts showing presence low dose statin worth trying. I don't have side effects at 20mg/day.

    Confusing conversation: first he said it is defined by the phenotype and not the genotype which I believe means if LDL > 190. But then he says if you can get diagnosed via a genetic test early enough you can watch out for it. So are we saying FH (familia hyper chosterolemia) is a genetic thing and you should get a genetic test and if you have it, then maybe carnivore isn't the right diet for you? However, if you don't have it via a genetic test then carnivore is fine? What is the point they are trying to make?

    No, FH can be and often is diagnosed a priori to genetics via biomarkers and family history. Our polygenic risk score banks are created a posteriori via accumulation from lots of people diagnosed. That bank is then fed back into genetic testing to assess an average genetic risk score for FH expression that must include bloodwork and biomarkers for final diagnosis. With regards to the carnivore diet point, he's saying that carnivore in general puts you at such an elevated ApoB threshold that its risk for ASCVD (atherosclerotic cardiovascular disease) is massively increased. Some people online then dogmatically insist on whataboutism while pointing to things like inflammation, insulin sensitivity, metabolic health, weight reduction while on carnivore, etc. while ignoring the fact that we have done multiple studies on entire populations of FH-individuals and found that these other things do not slow down their progression of ASCVD almost whatsoever. Only medications that directly or indirectly impact their atherogenic particles significantly slow down their progression of ASCVD. That's why Attia brings up the "grandmom smoking 50 years no cancer" metaphor/analogy because people aren't statistically literate.

    That's true! I recommend to take a look at Dr. Robert Lustig's lectures on cholesterol & sugar, especially on the triglyceride to HDL ratio. After going through med school, I can absolutely confirm that the average MD got about 1 hour lecture on nutrition, meaning they have no clue about nutrition. I've been on carnivore for a year and I've never felt better my whole life.

Damaged OxLDL has higher ApoB and can enter the intima if the glycocalyx is damaged (by sugar or inflammatory particles). This is what you're referring to... but I'm sure you already knew all that.

    are they supposed to make a conclusion of your personal anecdote? There have been meta analysis combining every single major study done on cholesterol, the correlation with heart disease is almost not funny because it is so well understood. No offense I just feel like people try denying evidence because they themselves have found success in something. You can also feel relatively good while having high LDL levels
