Professor Bloem is one of, if not the World's leading authorities on Parkinson's disease. He shares with us his research on this important topic and helps us understand how to best manage our symptoms through exercise.
The expression “citius, fortius, altius” (Latin for “faster, higher, stronger”) is the Olympic motto indicating what is required to achieve athletic excellence. Which of these skills is most important to reach an optimal personal performance was not specified by its creator, Pierre de Coubertin, but a combination is presumably optimal.
Similar issues are at play when trying to understand the beneficial outcomes of physical activity on general health and motor functioning in people with neurological disorders such as Parkinson's Disease.

Professor Bas Bloem is consultant neurologist at the Department of Neurology, Radboud University Medical Centre, The Netherlands. He is without a doubt one of the world’s top Parkinson’s specialists and has just received the prestigious Dutch "Stevinpremie" award.
Professor Bloem has published over 850 publications and has held too many positions and received too many awards to mention.
In 2002, he founded and became director of the Radboud UMC Centre of Expertise for Parkinson & Movement Disorders, which is recognised as a centre of excellence for PD.
He also developed ParkinsonNet, an innovative healthcare concept that consists of 70 professional networks for PD patients, covering all of The Netherlands. This initiative has received several awards.
Professor Bloem has two main research interests:
- cerebral compensatory mechanisms, especially in the field of gait and balance; and
- healthcare innovation.

    The Prof has an amazing ability to communicate in a very direct, clear and easy to understand manner. Impossible to argue against his presentation. I confess to doing less exercise than I am able. I am 76 and PD diagnosed 4 1/2 years ago. So, the Prof here in this video has convinced me to do more and kept track as a prod to keep going. I enjoyed the cartoon drawing at 37:58 as well.

    Love, love, love this. Diagnosed 8 months ago just after a hip replacement. I am already walking across the fields, cycling, boxing, Tai Chi, yoga and dancing. They all have different benefits. On walking/running outdoors vs treadmill, there are definitely additional benefits. Bing out in the country this includes navigating uneven surfaces. I am grateful you mentioned every day as sometimes people say I do too much and then fall into putting off until tomorrow - every day hahaha On vibration therapy, my gut feeling for me is it will help. I know playing and listening to music helps and part of that is the vibrations in the room from the music. On B12, vegans/vegetarians like me already take that and perhaps is something we need to make the medical profession aware of. I really notice when I forget to take mine, so will make more of an effort now. Thanks again for confirming I am on the right path .

    I have PD and would suggest that matcha green tea powder may be a better choice than coffee. For myself the caffeine in coffee and matcha (less caffeine) can aggravate my tremors and anxiety, but also, if I take it (the organic matcha powder mixed in water) it can allow me to exercise better, more vigorously, and longer. Green tea also has better phytochemicals, if I'm not mistaken.

    What a pleasant and optimistic man. The only problem is that despite continual exercise, my Parkinson's is swiftly deteriorating which makes exercising more and more difficult eg. because of more severe imbalance I can hardly ride my bicycle anymore.

    Have you looked at Dr Lauris Mishley's interview on this channel on her study of patients who do exceptionally well with PD? Wonder if there's anything else in their you might try / tweak. Good luck.

    Mine is progressing at a staggering rate, despite all the exercise. I find it interesting that the PD community can concede that we are all different and unique in our disease presentation but we all MUST exercise to slow the progression. And if you should be one of the unlucky ones like we seem to be, is it because we have failed at exercise? Something is very lopsided about this

    @kathrynaldrich5441 I still exercise daily because, even though I don't see it slowing progression, I believe it helps my posture. My neighbour who also had Parkinson's was severely stooped from early on.

    @@kathrynaldrich5441 I hear you, Kathryn. Very sorry to hear what's happening. Mine seems to still be progressing as well (lately with extreme anxiety) despite my best efforts to exercise. I will keep exercising anyway, as long as possible. My neurologist prescribed Lexapro which I'm reluctant to take because I don't want to use pharmaceuticals, but it seemed to work for a few days, but today seemed to not work, and it was rough (but maybe a little better than past anxiety attacks). Going out many times to walk briskly, and jogging a little up a hill to get a little out of breath. I try to pay attention to other factors as well that affect PD; nutrition (fish oil, etc), contact with nature, social contact. But when my symptoms are up, it's hard to be around people. I've had to let go of contact with people who's energy is not good for me. Bless us all who just want to feel better, more normal.

    I doubt if all this exercise will slow down the disease. I can't walk very far anymore. I was diagnosed 6 weeks ago. I never exercised much except for walking. Now walking is slow.

I am an athlete still riding mountain and road bike, ice skating, playing some of the best golf in my life 4 handicap, running, roller blading ping pong and any other sport you can throw at me.     This was a VERY informative and interesting presentation, Thank you for your presentation. You had mentioned that vitamin D and B12 levels seem to be affected by PD (or maybe the PD meds) I was diagnosed with PD over two years ago and about six months noticed a really bad tingling in my feet and occasionally my hands. I suggested that my doctor test my vitamin levels, which he thought would not provide any useful information. Several of my vitamin levels were low, but the B12 and D levels were extremely low. Do you think that the low levels are low due to PD meds or does the PD itself cause these vitamins to be depleted from my body? Is there any studies that you know of that have investigated this? Most of the doctors that I have seen don't think too much about vitamin levels.

    There was a question about how can you combine strength, balance, flexibility and exercise all in 30 minutes. I would highly suggest looking into a gentle yoga class to start, and then progress from there.

    Thank you for a wonderful presentation ! I am 79 yrs old. I was diagnosed with PD over 10 years ago. I was prescribed Metformin over 15 years ago for my diabetes. I can't help thinking that Metformin may be the cause of my PD and have read reports suggesting this. I can maintain a sugar level under 7 without taking Metformin. Should I discontinue taking Metformin? 1:37:16 1:37:16

    Walking a day you said 10 to 12 hours of walking a day! Nobody can do that I had a job where I had to work 10 miles a day! If you're a postal worker you might work for walk 14 miles a day but that's unrealistic

    I have discovered a curious paradox within the PD community. We seem to acknowledge that no two PD presentations are the same, yet the PD experts have the same oversimplified, one size fits all advice which is to exercise to both manage symptoms and slow down progression. I won't dispute the benefits of exercise because all people benefit from movement . But I resent being sold this one size fits all bill of goods that one can delay or even stave off symptoms. Using this logic, athletes the like of Davis Phinney and Brian Green should never have even developed PD in the first place when all studies to date only SUGGEST a possible correlation. It has not been proven and to say that it has, Dr. Bloem, is as egregious as you so eloquently point out in a DPF podcast "Mortality and Parkinson's ", we, in fact, do die from PD instead of with. Semantically, it's a small difference but in reality, the difference is huge in terms of disservice to our community members. We're a tough bunch. We prove this on a daily basis, but we deserve the truth however hard that may be.

    Unfortunatly, this is the case in a lot of groups... Like Diabetics, Heart-patients and the like... They seem to be in 'their bubble' and would even follow wrong guidance (from for example the Diabetes Liga (in Belgium) for example). It is like they are willfully blind... and keep eating a lot of carbs, drink soda drinks, etc. ... It is sickening. Sometimes I do one WHOLE day of research about something and also read peoples reactions under some video's and often already know more than ALL 'standard' doctors or 'experts'... Un-be-lie-vable !!
