New Patient: Knee Pain

(Shared with gracious consent from our patient!)
This is a typical new patient visit -- this time focusing on a recent-onset of knee pain.
You'll see our consult, exam, report of findings & a hands-on demo of his "rehab homework", which is the home exercise program (HEP) I've prescribed.
Note: after the exam, our memory card ran out, so you're missing about 8 minutes I spend using myofasical release/massage to work on his hips & quads.
For MSK providers: you'll notice I give explanations and describe mechanisms that I suspect are accurate, but that can't be verified by current research. My intent here is to be "directionally accurate" with my descriptions.
Meaning, while the actual mechanisms may differ (as determined by future research) if the "story" is followed, the patient can understand, implement and see the results they want.
I almost don't care what's happening on a cellular level, for example, as long as we see consistent and rapid advancement toward the patient's goals, and these results are reproducible by them, to help prevent future recurrences.

    First, from the deepest part of me, thank you Dr.Dan. To hear life being lived from the perspective of a person who may not have much time left has to be the absolute most important and amazing thing I have come across in my life. To hear you speak, to me, moves me more than any yogi or religious adept ever has. You are giving a very deep and unique gift to everyone who hears you speak. I can hear the vibrance and life in each word you speak. I do not know if the videos you record about your personal feelings and unique perspective take a toll on you mentally or physically, but please know they are felt as an almost divine experience from one human to another. I promise you the words you put out on this channel are received and felt to the depths of some us out here. Saying “Thank you” would insult the amount of gratitude I have for just being able to listen to a voice that is past the trivialities that have become daily life for a lot of people. Please just keep speaking for as long as you can because for some of us,hopefully a lot of us, we really do hear you.

    You're right, I'm so sorry! I owe you guys an update. Honestly, my last scan about three weeks ago wasn't great, tumor in my liver grew to an inch, three spots in my lungs via millimeter. This one hit me a little harder I think, because things have been stable for about six months before that. BUT! I've been mulling over my options, making some changes to diet, starting a new self treatment regimen that looks promising in research (I'll share in the next update!). I promise I'll post an update in the next week! Until then, you and everyone else in our situation are in my prayers and thoughts daily. Wishing you excellent health, speedy recovery and many years of happiness ahead!

    Great question, and one I don't have an answer for! Pain is famously hard to measure. Our perception of pain from a given stimulus can vary greatly within one individual, even over short periods of time. Thus, comparing an individuals pain tolerance is extremely difficult, not to mention comparing ones tolerance to that of others. Wish I could give you a better answer, but great question!

    @@philip5230Perhaps? There's a logic to that, because we know our body systems can adapt to our experiences. Why not pain, too? But… There are also cases where persistent pain can actually increase your sensitivity, and become more painful. Personally, my goal with patients is to reduce pain altogether, and equip people with the tools they need to keep it from coming back. If we achieve that goal, our tolerance to pain kind of doesn't matter anymore 😊 Again, none of this is well understood in the literature.

    @@DrDanSays i dont know if it makes a difference but i dont have constant pain im just trying to get my pain tolerance up because i do boxing.
