    I suffer from this problem of flat feet pain. Oddly enough minimalist shoes have seem to help me the most so far however I would love to see a good podiatrist one of these days. Hoping to see more helpful videos about this issue and possible solutions.

    I went to the podiatrist last year because I was having chronic foot and knee pain. She didn’t examine me, but instead just talked to me for 5 minutes and told me to buy some $300 orthotics. I’m blown away at how thorough you are with each of your patients exams.

    What's odd is that I was flat footed and supinate. All my shoes were worn on the outside. After about a year of stretching and foot and calf exercises I'm no longer flat footed and my shoe wear is neutral now. Barefoot shoes really helped with bio feedback so I knew if I was walking or running funny. Posterior tibial exercises were the smoking gun for me.

    @@ricardodb_3998 the biggest issue, actually was posture. I had posterior pelvic tilt and forward neck from sitting so much. Once I corrected that, I noticed my flat foot issue was gone. Hard to believe but it's true. I didn't need any foot exercises. Fixing your posture aligns your body in a neutral position with optimal weight distribution. My feet naturally went back to normal. I spent 10+ years with flat feet.

    doc....i'm obsessed with your videos. I am a busy lmt, and foot pain is getting in my way of working/walking/wearing shoes.     Hi, great video. After failed bunion surgery I have short first metatarsal and frozen sesamoids. I don’t understand if my pain comes from short metatarsal, or because of sesamoids. I i cannot find information about frozen sesamoids. Could you please make a video. Thank you.

    Thank you very much for sharing this information. It's super helpful! Much appreciate it. I understand the function of orthotics, and I have been using them for years, but in my opinion they only make sense in phases with acute pain. Your feet feel stiff with orthotics and they miss the contact to the ground. It's not a big layer, but it takes away a whole lot. It can be a temporary aid, but it's not the solution. At least, that is what I think after some years of going through life with aching knees.

    The massage balls with the little bumps on them are so painful to me! Even if I do it gently. I just wanted to say that. 😊 my ankle has been swollen for a couple weeks. I went to the podiatrist yesterday got x-rays and he pointed out my bad arthritis on the top of my foot and other places I think. I have a bone spur on my heel as well, and Achilles tendinitis . He gave me a boot to wear and prescribed an anti-inflammatory . I’m not looking forward to wearing the boot and I’m waiting for my order to be delivered -(The device you put on your other shoe to make the height more equal with the boot on) I have rheumatoid arthritis and osteoarthritis . The osteoarthritis is wrecking havoc on my lower body, including both knees. I need to get knee replacement surgery long story, but I’ve got to get the right doctor first. In addition to all these problems, I have to walk my dog three times a day, and I’ve been using a walker to do that for the past year because of my knee problems - And now my ankle/foot problem Any advice would be appreciated. Did I mention my feet are flat and was born knock kneed , and my feet point inward according to the Podiatrist

    On minute 12 .... definetly believe you, because my feet does that heavy pronation and anytime I lift, I have to wear orthodics or good shoes

    Hello Dr. I have poor ankle dorsiflexion and it cause my foot to overpronation. I try to stretch the calf muscle but I cant feel my calf muscle being stretch at all. Does myofascia release with foam roller will help?

    Thanks Dr. Bernacki! You are not making this up. I purchased one of your recommended orthotics for oversupination-worked great! Also purchased some Brooks sneakers. The combination helped my torn peroneal tendon to heal and not re-injure! 👍🏻 I have very high arches per my podiatrist.

    I have fallen arches. I have arch supports and now they are in my uniform shoes. Agonizing. I had a fall, and my kne has been painful for a couple of months. If I could come to Michigan I would love to visit and be diagnosed. I have osteoarthritis, and that doesn’t help.

    I get calus on the outside of my big toes ... always been lable flat footed , always done extreme sports like motocross and bmxing skating . Have experienced huge shocks to my anckles for years .my dad likened it to me laying on the floor and hiting a sledge hammering into the bottom of the feet . Probably on a 4 year injury(23) and huge clicking of the anckles but going to physio again now so here hoping the knowledge from here helps Thanks for the videos

    my issue with orthotics or arch support with flat feet (not fallen arches) is it makes my foot very sore where the arch is supposed to be and causes almost a blister

    I am a supinatior runner at national level and have medial ankle pain that hasn’t recovered in over 12weeks. I’ve really come to my final low with running and am on the blink of throwing in the towel. Mri/X-ray and physio and still no results. Just lingering pain. 6 years of no pain and then a past year it has all come crumbling for me. (15min 5k runner)

    Strengthening glute muscles lead to strong ankles and strong feet as well as using toe separators if you have bunion problems

    Right now I am injured with a few tears in my tendons(shown in an mri), I have somewhat flat feet, and I pronate my ankle when I walk and run. What do you suggest i do? I went to physio and they just did laser treatment and got me custom orthotics, but i feel not much is improving. Im also struggling because I need to walk my dog everyday. I wear a compression sock on that foot and it hurts if i dont. I think i may have become reliable on it. Should i be doing these exercises while i have the tears? is resting my only option?

    I saw a podiatrist. All he did was palpate my foot said I had PTTD. Told me to get new shoes and gave me insoles (which hurt worse) and told me to ice he wanted to give me a shot or meds. But I refused both. Told me to come back in two months. Never gave me any exercises or stretches. I am still in so much pain, trying online to find things that might work. I will not be going back as it was a waste of $250.00 and 15 min of my time. I had non weight bearing X-Ray done earlier and he just looked at those. I did get a pair if under armour Hovr turbulence. I have always had calluses on both big toes as long as I can remember. I just want the pain to go away, it is 24 7

    I need some help from you I was in a pool and I jumped on water and thought the pool was deeper but it was not so I hurt my left leg so now around the ankle is hurting and I can't walk with the left leg it's been a week it's peerty mutch the same it's not getting better

    Hello first time viewer. I have severe flat foot that has thrown my body out of alignment. I have grade 4 knees and issues with hip and lower back? Is there a video on this? I was told to get hoka shoes and surgery. I am 47 and really no one who will be there for me to help mewhen I have surgery plus I was exercising on my treadmill hurt my ankle and pulled a hamstring in my left leg. Any advice?

    Flat feet since birth. Arthritis in my knees. Podiatrist said try certain brand of rigid orthotic. Arthritis in my big toe. The foot with the arthritis hurts now on the outside of the foot when walking with new orthotic. Said I have some of the flatest feet shes ever seen. My big arthritic toe hurts in closed in shoes. I wear slide in shoes a lot due to my big toe hurting being crammed into a shoe. Im running out of ideas.

    Gotta love the American healthcare system lol I’m flat footed, pretty bad overpronation, a little overweight. Finally went to the doctor because I stand on concrete all day as a technician. Finally get to the pediatrist, and he just wants to sell me orthotics. I ask him about exercises or something. He says “yeah I guess do some stretches” doesn’t tell me what kind, or how many.. I’ve learned more about foot health and foot mechanics in the last couple days than I have in many many doctor visits..

    Sir i had triamcilone acetonide 40 injection in my upper tibia tendon to reduce inflammation and i have read that it inhibit rejuvenation and leads to cell atrophy... So can one time steroid injection on ligament/tendon cause irreversible serious damage?? Pls reply🙏🙏🙏🙏🙏🙏🙏

    Is it possible to have outer heel wear pattern on your shoe heels and pronate will flat arches at the same time? My take from the video is that if your suprinating then you have a high arch, I know I'm misunderstanding but I don't know where exactly. Thx, great video, great work.

    You mentioned that you follow the literature, but did you miss those papers that consistently show that custom orthotics are in no way more effective than OTC orthotics? Also if the orthotics and shoes you suggest help so much, why do you not show the before/after shot on the overpronating runner using the orthotics to show the effect?

    Are there some papers that show that? Sure some basic studies that are not standardized in any way (Which is the critique of those papers: What orthotics? How did they make them? What were the conditions they made them for? Did they randomly make custom orthotics for what reason? What techniques and how aggressive did they make them? - No Answers to these important questions?) Also, you will notice that those few papers you reference are written by surgeons who normally write about how effective their surgery is (just like Purdue pharma published papers on how opioids are not addictive). Getting someone into a good pair of custom orthotics can give instant, and I mean maybe like, 5 minutes to see their pain going away. I personally don't need to spend hours making a video of before and after pronation pictures because it doesn't matter to me, only that someone is pain-free. If you are flat-footed, I would love to meet you and make you a pair of custom orthotics if you ever swing by, don't even worry about the cost because I feel confident they will help.

    I've been labeled flat footed basically my whole life. Recently I have been to a podiatrist due to ankle/ foot pain. I've a hard a time standing and walking long periods of time. He diagnosed me with tendonitis and the exercise he suggested haven't given any relief.

    My flat feet CAUSED my overpronation....nerve nlock before surgry 6 weeks ago, zapped my nerves...result: constant pain, heaviness, tingles, etc. What can i do, if anything?

    I have a issue with my right foot that I have a lot of trouble to diagnose. I try many different pairs of shoes and boots and they all feel uncomfortable on my right foot, my ankle (right one only) push inward creating pressure on the shoes and making my foot hurt a lot the more I walk. I have difficulty and lot of pain even walking only 1 km. Im seeing a podiatrist already and the orthotic that he gave me just make the pain worst. I had a x ray and ultrasound as well but it didn’t find the issue. Im getting a bit desperate because I don’t know what’s the problem and how to fix it. My new pair of shoes that my podiatrist recommended me hurt me too much I can’t wear them. Also I’m seeing a physio and I’m doing stretches and exercises everyday but my foot still doesn’t get bettter. :/ so I’m thinking I might have a foot deformity because what feels okay on my left foot doesn’t feel right on my right one. Also I have nerve pain, tangling and needle pain. What could my problem be?

    Yeah I’m in the same boat. I have knee pain, lower back pain, ankle and the worst is my right foot. Outside of my right foot underneath pinky toe area, I get sharp pains when I stand walk/too long. I can’t wear boots anymore without being in a shit ton of pain after couple hours. I definitely over pronate and my heels will wear down in an angle on certain boots or shoes. I roll my foot all the time and lose balance when I’m golfing. I’m 34 and athletic. I feel like I’m 55+ with all these problems that stem from Over Pronating I guess? Idk how to fix it cuz nothing helps. Stretchy running shoes help a lot. Like the slip on Adidas have some stretch, stretchy Nikes too, those are shoes I can wear for a while. I need help too don’t know what to do. I’ve tried the arch supports and they don’t work. My foot seemingly keeps getting wider (hobbit feet)

    wellcome to the club "curse of the right foot". I have the same prolblem as you guys. Im 23yo. Dosed physical activity, individual orthotics and proper shoes (this is the most difficult, a lot of money is spent on bad shoes, which turn out to be bad for the foot, but it can not be returned to the store, because to evaluate them you need to walk in them). Very much influenced by weight. Even a few extra pounds gained over the winter greatly affect the number of possible pain-free kilometers. The best way to get your weight in order is to diet and exercise bikes or ellipticals. The load on the foot is much lower than running. In emergencies I take nonsteroidal anti-inflammatory drugs (ibuprofen). But the most important thing is physical activity - every day. My brother is an orthopedist, and he tells me that so far in our country there are no trained surgeons to do quality surgery. Maybe by the time I'm 30, I'll go to Western Europe and have the surgery, because I have no idea what the quality of life in old age will be, if I have such problems at 23...

    Hahd Fhjr, thanks for the great response! You may try to use orthotics - they work like shock absorbers, removing pressure and stress from painful areas in the foot and ankle. They can also prevent foot deformity or keep it from getting worse, better position the foot inside the shoe and improve the overall way that the foot and lower leg move.     Good shoes with good insoles should help manage your problem and within days and weeks your should feel better. However, If for some reason there is no improvement, than you should visit with podiatrist to find out if additional issues are in play. I wish you all the best.

    What happens when you support wny part of your body all the time? The structure of the body which has been supported will get weaker, thats simply adaptation of your body. You better should train your arch and should focus on walk the right way barefoot.

    Ofcause orthodics help, just like a crutch helps with a broken leg. Point it, it treats the symptoms rathervthan the root cause. Its never actually going to fix the problem. Maybe I'm wrong about this, but. I tend to believe this is a posture problem, and it can be fixed in the way you walk. I'm learning how to walk with a front foot stride, and I can really feel the muscles in my ankles and the base of my foot working like they never worked before. Surely, the answer must be to strengthen these mussles so they can do their job, rather than to support them to make them even weaker?

    @@jenniferonuoha3601 That is a difficult one, I am no doctor. Before you can fix a posture problem, the individual first need to recognise it as a problem and have a desire to address it. Not sure how you are going to do that with a 1 year old baby. I know the experts say its important for kids to walk barefoot a lot, so I would definitely do that. I can only say what I did and that it helped me. Flat foot is actually a misnomer, it is the symptoms not the root cause. The problem is the collapsed ankles. So when I stand, I made a point to consciously push my ankels out so they are straight. It feels weird in the beginning because you are distributing your weight differently on your feet, with more weigth on the edges of your feet, but in time, you get used to it and it begins to feel normal. Then, when it comes to walking, I've adopted forefoot walking. There are a lot of benefits to it, and there are videos on youtube about it. The important thing is you need to walk with your feet facing forward, not 45 degrees to the side. This is part of what causes the feet to fall flat. Fore foot walking helps to promote this. It also keeps your knees bent the whole time, allowing your feet and legs to absorb the shock rather than your hips. The idea is that you want to hit the ground with the little toe part of your foot first, and then put the rest of your foot down. If you do this, your knees will never be straight, while your stride will be slightly shorter. Not only does this help your feet absorb the impact, it also gives you more stability: you have much more control in your forefoot than in your ankle thats basically just a round bone. I would advise you though to get proffessional help. There are two physical therapists on KZread called Bob and Brad with a very corny introsong that has a video on forefoot walking. Perhaps check that out. The algoryth should do the rest to suggest more videos for you. I've been walking forefoot now for about 9 months, and its 90% second nature by now. It takes time and practice to get used to it, and it will also take time for the lazy muscles that is neglected in how modern people walk to become stronger, but the foot pain I developed duecto walking 5km everyday is now gone.

    This doesn't really fix the problem though, it covers it up. If we focused on strengthening our muscles and actively working to correct our gait, then that would actually fix the problem. I'd worn running shoes with arch support for almost 10 years because of overpronation, and while it did help with my ankle pain, it didn't fix my feet. As soon as I took the shoes off, my feet were back to overpronating. Now that I've switched to barefoot shoes, my toes are never mushed together and are free to move and splay properly, the muscles in my feet and lower body are being put to work, my achilles tendons aren't tight in the morning anymore, my balance is getting better, I've been able to focus on how I stand, walk, and run by making sure that my ankles are straight, and even in just a short time, if I stand properly, I have a bit of an arch. I always thought I had flat feet. I had looked up my options as a last ditch effort before giving in and getting orthotics, and I stumbled across barefoot shoes and I'm glad I did.

    The Orthotics in my opinion should be more flexible so the foot can move. If your foot don't move the foot muscles will get weaker and weaker.

    Although the first runner (1:37) has a neutral foot, I wouldn't call heel-strike a "reasonable running technique" no matter whether you're pronating or not.

    Absolutely correct Richard and great catch. The pain purpose here was to highlight the different between overpronation. Running gait could be a 10hr video on its own with a lot of critics on both sides! Great catch.

    Absolutely, flat feet is the shape of the foot, pronation is the motion of the foot. Theoretically with a rotated knee or hip a flat foot could more likely be supinated to walk.
