Carter does genuinely care about him, but the stuff in his head makes him challenge it. He thinks his feelings for John Paul are a 'test' from the devil because that's what his father indoctrinated him to believe from childhood. He second-guesses every thought and feeling that he has because everything in his life is viewed through the prism of the belief system he was brainwashed into.

Its odd when Carter says "Yes" at the end of the scene. It almost sounds dubbed in and a strange response at that. A nod of the head or "okay" seems more appropriate. Or I could be over thinking. lOL.
