In this episode of the Life After Diets podcast, we're diving into "Overcoming Binge Eating" by Dr. Christopher G. Fairbairn. It's a key book behind a lot of the binge eating treatment programs here in the UK, endorsed by major organizations like the eating disorder charity BEAT. But let's be honest, we both came into this with a bit of skepticism. This book is a big deal in the UK, but it's not something Stef was familiar with stateside, especially not in the day-to-day trenches of eating disorder treatment.
So, who's Dr. Fairbairn? He's the guy who, back in the late '70s or early '80s, started piecing together what we now understand about bulimia and binge eating. He worked out that what was being treated as a weird form of anorexia was actually something else entirely. Teaming up with Cosmopolitan magazine (of all platforms), he managed to get the word out and collect data on bulimia, basically putting binge eating disorders on the map.
Now, let's get into the meat of it. We both had our gripes with the book, mainly how it handles the treatment approach. It's very much steeped in Cognitive Behavioral Therapy (CBT), which, don't get me wrong, has its merits. But it feels like it's missing the emotional depth and complexity of eating disorders. Like, the book suggests that when you feel a binge coming on, you should try exercising or hitting up friends. It sounds a bit too simplistic for something as complicated as an eating disorder, right?
We also picked apart the book's views on regular eating, problem-solving, and its dieting advice. The suggestions seemed a bit out of touch with the reality of dealing with an eating disorder, veering dangerously close to reinforcing diet culture norms. It's like, we get the logic behind it, but it doesn't fully grasp the lived experience of someone battling binge eating.
Throughout our chat, we kept coming back to the need for a more empathetic, nuanced approach to treatment-one that really gets into the nitty-gritty of personal experience. It's not enough to just follow a set of instructions; dealing with binge eating is about tackling the emotional, psychological, and societal tangles that feed into it.
Wrapping up, we acknowledge Dr. Fairbairn's significant contributions to understanding binge eating disorders. Still, we're calling for a broader discussion on treatments that truly address the complexity of these issues.     Yes! This is the issue I have with CBT approaches usually. In my opinion it's clinical gaslighting. "The way to get past your emotional issue is to distract yourself or pretend you feel something other than you do feel. And if you can't do that, then you're not trying hard enough." It's *clearly* not a flaw with the process. It's YOUR fault. I hate to point it out (not really), but this is the same idea cults are built on. I appreciate your awareness of how these approaches hit the client--how it may activate the inner rebel or increase shame and exacerbate the problem. Thanks for having this conversation publicly. Oh--and the Weight Control Information Network?! Mind-blowing. Otherwise known as the Ministry for Eating Appropriate Portions (MEAP), I presume. ;)

    A psychiatrist at the Priory Group recommended this programme to me. I worked through the book with an AA sponsor who worked at the hospital. I was never told why an AA sponsor was chosen to oversee my programme but realise now it was likely because they viewed bulimia as a food addiction. He was lovely, but had no eating disorder experience. I found the programme hugely problematic and I complied only sporadically, driven by shame. His approach was to motivate me by being accountable to him through my food diary. The psychiatrist endorsed this! When it was clear the programme didn't work for me, I was asked if I would agree to a hospital admission for closer monitoring. I'd been an outpatient for two years and this was my cue to leave. For the next decade I continued to struggle, tried various programme s including OA, which I also found problematic. Since discovering intuitive eating and rejecting diet culture I've began my recovery journey. Heavy on the self compassion! I find your podcast and KZread channel really supportive. Thank you.

    This is part of the problem…the program can in theory be delivered by anyone 🙄 We are going to do an episode on the 12-step approach to eating so watch this space!

    Love this episode!! I’ve been in U.K. therapy with this book twice, once 15 years ago solo and most recently last year as a group effort. On the first occasion it left me feeling like a failure, and on the second it left everyone in the group feeling like failures. I’m So glad you’ve raised these points, I/we thought I/we was/were going mad! I’d actually been starting to self recover with resources like yours prior to the group attempt and this (book) made me so much worse very quickly (thanks to the shaming narrative and movement away from food intuition). By complete coincidence I got so annoyed with seeing it in my room that I put it on my fire 4 days ago and quite happily watched it burn 😂 so it was a joy today to listen to this episode xx

    This video is fantastic. I have benefitted enormously dealing with trauma with a more "traditional" therapist, but I don't think it is possible to overstate how important it is to recognize that not everyone--not all therapists--have the right training/skills/experience to help people with disordered eating! Life After Diets is an amazing podcast. Thank you!

I also think that it is important to note changes of key information in newer versions of any publication & to note that throughout.     Also for writing a book, I think had he done it collaboratively with experts by experience or something it might have felt more relatable too and then that mixes the expertise with the experience too

    Hi Sarah & Steph. I'm a regular follower/subscriber to your videos. I've especially enjoyed this episode & really agree that for a book still used in therapeutic settings, Dr. Fairburn's book supports many outdated & highly judgemental attitudes to therapy & treatment. His comments on the term addiction being debased by application to behaviours around food, sex, risk, etc. are insulting & downright old fashioned, rooted in ideas typical of Patriarchy & conservatism, not person centered inclusion. Like Sarah I am living in the UK, but did grow up in the US, so the opinions of each of you resonate with me & reflect the differing cultural influences either side of "the pond". Having tried to read this book, your comments have cemented my poor opinion of the efficacy of Dr. Fairburn's approach to this serious problem affecting so many of us who because of the addictive primal comfort of food as a substance to quell a range of emotions causing discomfort & often associated with shame & rebellion, in my experience, having struggled most of my life with weight issues & binge eating. Thanks for a very entertaining book review. 😅

    Parts of this book are reminiscent of what a nurse said to me on an acute ward. I’ll enlighten you: “if you don’t want to binge, just don’t binge.” 🤯🤯🤯🤯 I also struggle with over exercise and was told to go on a walk when I was struggling by the crisis team years back and I was like sure I’ll go back out then 😅 okay fine 😅

    Fairburn’s book was the 1st I found on the subject (about a year ago), and at that time, I was so grateful to have discovered something to explain what was happening to me. I was so amazed in the 1st part-I felt seen for the 1st time, and that this was a “thing” and there were others like me! A huge sigh of relief. However, as I read on, I was pretty horrified. It was condescending, pedantic, mis-informed, useless. It was very evident that the author did not have a Clue! As soon as I saw all the monitoring, I was out. 🙄 I worry that this book is used to inform clinicians on this subject, and the issues persist. I’ve learned more useful information from one of your videos than this entire book! Thanks so much for confirming what I thought, too!

    I'm trialing an app for a university research being done. They use DBT therapy. I find it ok but still struggle. I have read this book but not sure what I think about it?

    It's called dialectical behaviour therapy. It's focuses on black and white thinking and uses a form of parts work. If you google it you can read a bit more @@Od.3056

    @@Od.3056 DBT was a therapy Marsha lineham created. She had suffered a lot as a child and she created this form of therapy. It is used a lot these day instead of CBT. They use lots of different things, mindfulness,distress tolerance ,etc. I find some of it useful. I like how Marsha had had experiences she suffered as a child and she worked on developing this therapy over years. She had even spent time in psychiatric hospital herself.
