Dystonia, Akathisia, Parkinsonism, and Tardive Dyskinesia - Antipsychotics Side Effects

Dystonia is a neurological movement disorder characterized by involuntary muscle contractions that cause repetitive or twisting movements and abnormal postures. These muscle contractions can be sustained or intermittent and may result in painful, awkward, or distorted movements. The condition can affect any part of the body and can range from mild to severe.
Dystonia is caused by a dysfunction in the brain's basal ganglia, which is responsible for coordinating muscle contractions. It can be classified into several types based on the affected body part or the underlying cause. Some common types of dystonia include:
1. Generalized Dystonia: This type affects multiple body parts and can lead to twisting and contorting movements that involve the limbs, trunk, and sometimes the neck and face.
2. Focal Dystonia: This type is confined to a specific body part. Examples include cervical dystonia (affecting the neck muscles, also known as spasmodic torticollis), blepharospasm (involving the eyelids), and writer's cramp (affecting the hand and forearm muscles).
3. Segmental Dystonia: In this type, adjacent body parts are affected. For instance, cranial-cervical dystonia involves the head and neck.
4. Hemidystonia: Only one side of the body is affected in this type, leading to abnormal movements and postures on that side.
5. Dystonia-plus Syndromes: These are conditions where dystonia is a prominent feature along with other neurological symptoms. Examples include Dopa-Responsive Dystonia (DRD) and Paroxysmal Nonkinesigenic Dyskinesia (PNKD).
6. Secondary Dystonia: Dystonia can also be caused by underlying factors such as brain injury, certain medications, infections, or other neurological conditions. This is known as secondary dystonia.
The exact cause of primary dystonia (dystonia without an underlying cause) is often unknown. However, genetic factors are believed to play a role in its development. Certain gene mutations have been associated with various forms of primary dystonia.
While there is no cure for dystonia, treatment options are available to manage its symptoms and improve the quality of life for individuals affected by the condition. Treatment approaches may include:
- Botulinum Toxin Injections: Botulinum toxin, commonly known as Botox, is injected into specific muscles to temporarily paralyze them and reduce abnormal movements.
- Oral Medications: Medications like anticholinergics, muscle relaxants, and dopamine-modulating drugs can help manage symptoms in some cases.
- Physical Therapy: Exercises and stretching routines can help improve muscle flexibility and coordination.
- Surgery: In severe cases where other treatments are ineffective, surgical procedures like deep brain stimulation (DBS) may be considered. DBS involves implanting electrodes into specific areas of the brain to modulate abnormal brain activity.
If you suspect you or someone you know is experiencing symptoms of dystonia, it's important to consult a medical professional, ideally a neurologist, for a proper diagnosis and treatment recommendations tailored to the individual's needs.

Weird... Benzodiazepine withdrawal causes Akathisia too. Worst thing about benzos is that you can develop a condition called tolerance withdrawal which means that even when you are taking your normal dosage, some switches in you and you go straight into withdrawal. Anyone can get this too no matter if you are taking daily or as needed. I had to learn this all the hard way, I spent 4 years recovering. Benzos is only recommended short term

Right but the tardive akathisia from SSRI is brutal, for me I'm on year 5 and I would not survive without taking a benzo intermittently. I do try to stick to every other day and occasionally take several days off. But nothing else has helped even a little. Even after intense workouts, I already feel agitated driving home.

THANK YOU!! The doctors don’t know how to treat withdrawal Akathisia and told me for 2 years that I had “rebound anxiety” and that I would get better if I was put on another medication for anxiety and attended psychotherapy. I was put on so many meds and nothing ever helped. Probably because I was ripped off like 5 drugs in the span of a few months. Had been taking some of them for two decades plus. The guy never gave a justification as to why he was just ripping me off all my meds and then putting me on another cocktail. Now I have symptoms of TD from coming off another antipsychotic after being on it for 5+ years. Doctor said to just stop at 20mg when I complained about worse Akathisia.

Thank you for this concise and insightful breakdown of antipsychotic side effects! Your explanation of acute dystonia, akathisia, parkinsonism, and tardive dyskinesia is incredibly helpful. It's reassuring to know that there are targeted treatments for each side effect, such as benzotropine or antihistamines for dystonia and benzodiazepines for akathisia. Your mention of managing parkinsonism without discontinuing antipsychotics with benzotropine or amantadine is enlightening. Tardive dyskinesia's potential development after months on antipsychotics is a crucial point, and your recommended treatments like velbanzin provide valuable options. This video serves as a valuable resource for anyone navigating antipsychotic medications. Looking forward to more informative content from you! 👍👏

I have tourettism naturally.. I sustained it from birth complications, I was born almost a month late and was cut off from oxygen. It makes me have akathisia sometimes, especially when I don't eat and I get stressed out and anxious.. They put me on antipysychotics in my teens and it made my akathisia go full blown tardive, and it would happen almost all day every day... It was awful, it went on for years and I wound up having to get on methadone to make it stop, cause nothing else would do it.. I actually found that out on my own after a dental surgery to remove all 4 of my wisdom teeth lead to a hydrocodone prescription... I wound up having to buy pills from people without a prescription so I could pee dirty to get in to the methadone program as an "addict," because my own doctor, and all the doctors I saw trying to get the akathisia to go away wouldn't touch the subject.. They didn't even notify me that they worked to stop akathisia if everything else fails.. They're actually the most powerful anti-akathisia agent there is.. The universe just had a way of letting me know because I was on the verge of taking myself out, probably.. Yeesh. Before that, I had seen 5 different doctors and took a whole list of medications, like 10 different things, trying to make the akathisia go away so I could sleep.. I was seriously on the verge of taking my own life... It was also no surprise that the mental health office I was going to that was treating autistic kids with comorbid behavioral and neurological issues shut down and "lost" everyones medical records in the move. The pharmaceutical and medical industry are shameful.

@Cosmicgrl___ I used to have tics too, akathisia, all sorts of crazy stuff related to my tourettism, turns out what I was missing was dopamine. It even balanced out my endogenous opioid imbalance or whatever misfire was happening, once I got on a low dose of adderall I was able to get off methadone, which was what I was using to stop the tics after the antipsychotics made it worse.. Ironically, when I was on methyphenidate, which is ritilin and kind of like adderall but weaker and closer in structure to cocaine, that got rid of my tics too, but after D.A.R.E. started coming to schools and painted it as legal cocaine for kids, parents started questioning doctors, and doctors stopped giving it to kids with ADHD in a lot of cases, which turns out is comorbid and commonly goes hand in hand with tourettes and tourettism.. That's when the antipsychotics started coming in to my life.. Lol.. Running away from those, and doctors, is how I wound up on drugs, (can you believe over half the people in prison have ADHD/related issues, and are there for drug issues, and the behaviors that got them there are related to frontal lobe imbalances?) and now like 15 years later I'm finally stabilzing because doctors are finally facing the truth about dopamine and frontal lobe issues. A good example.. Putin has Parkinsonism, a dopaminergic imbalance, and look at how crazy he acts. Dude needs some drugs. The right ones. He can't even keep his feet still. Look up "putin twitching" or "putin feet" lol

I am effected by the symptoms of my anti-psychotic medication (my involuntary movements) unfortunately, I'm literally only here because I went down a rabbit hole of studying the medication I am taking, it all makes SO much sense now, unfortunately my medication being stopped due to monetary reasons made it somewhat worse. I have been taking this medication for 2 years now, sometimes I thank every deity that it helps, and sometimes I pray that the psychological pain goes away. I hate how you have to make a deal with yourself on what is more important, because on or off my medication, I still find myself struggling. I am very skeptical of psychiatrists who don't understand their patients or don't think thoroughly enough about which treatment-medication combination might work best.

@@MindfulSapient HIII THANK YOU FOR NOTICING MY COMMENT!! Every antipsychotic acts different for people, I am taking Latuda, after around 6 months to a year I started getting involuntary facial movements, my nose twitches and so do my eyes, sometimes my mouth. As time progressed my legs would jolt and it would trigger my facial twitching and viceversa. If you try the drug for a month and come off of it nothing would really happen. I suggest being on it for 2 months at a reasonable dose. It takes a few weeks for it to have its full effect, varying on the medication you might have to eat atleast 350 calories. For some people they would rather stay on a medication that has side effects if it means their mental health is improving. When I first started taking it Id get nausea, cramps, fatigue and really bad anxiety, but it gets easier after at least a week.

@@NeoCapableTechnology Thanks for replying Ferriera! I hope it gets better for you soon and you get to heal and live your life without any of these side effects. You must be a strong person to have kept going in spite of having them. I would've given up a long time ago. My experience with psychiatry is nothing less than terrible. The main problem with me is that my nervous system has become hyper-sensitized due to the use of benzo. The reason i asked if i could get off of the anti-psychotic after a month is because i couldnt do the same with the benzo. After taking it for 15 days i stopped and felt extremely sick, not just mentally but physically too with symptoms like extreme burning pain in muscles, extreme fatigue, brain fog, memory problems, all kinds of digestive issues etc. I couldnt continue past the 3rd day and got back on it. I have tried a couple of SSRI's since then but they all made me extremely foggy, depressed and fatigued. I didnt have any problem with SSRI's prior to taking the benzo. Also i was recently prescribed an atypical anti-psychotic 'levosupiride', primarily for my digestive symptoms but after taking it for a week, my muscles started getting really stiff and painful. So i stopped and it has been 5 days but my brain fog and pain hasn't alleviated yet. It feels like something burnt in my brain. And now the doc wants to put me on risperdal (Risperidone) and i dont know what to do anymore. I feel like i am done with psychiatry. Sorry for the long post. I just feel completely lost at the moment and i dont trust doctors anymore. Just needed to vent i guess!

If you have ever experienced this. You would you know. I have experienced it around 4 or 5 times in my life (no more then an hour or so each time) I’ve likely never experienced anything else as unpleasant or unbearable. I would describe it as an intense physical ‘Whole body’ Anxiety, including but not limited to sensations and emotions of panic, terror and even paranoia. Basically the practically continuous state of Fight and Flight / experienced at the same time. Deep and slow breathing and mindfulness techniques provides some basic relief, until typically the sensations / symptoms and associated panic begin to pass within around 45 to 90 minutes.

Can you reverse supersensitivity by giving a dopamine agonist? It should be able to reverse it by downregulating the receptor and return the sensitivity to the baseline level? Although it would make the psychotic symptoms worse initally. Is that beeing tested in animal models?

I had what they called acute dystonia from 3 days of seroquil a few years back but it seems different to these videos. I’d get constant movements to the extreme in my neck, back and mouth. Yesterday I was taken by ambulance for something similar for an increase in amitriptyline dosage 3 days prior and for hours I had my arms throwing up in the air, involuntary screams, tongue poking out and clicking my tongue, making kissing sounds and blowing raspberries. I have ehlers danlos syndrome which often comes with extreme side effects to these drugs but it didn’t seem like dyskinesia nor dystonia so now I’m confused

  • @USMLEpass

    @USMLEpass

Antipsychotics don't affect the muscles at all. They affect the nerves which innervate the muscles. Here's my hypothesis. When the nerve's function gets altered by antipsychotics, the initial response would be to resist this alteration by increasing action potential firing. Which is why we see increased muscle movements first. However, with prolonged use of antipsychotics a certain level of tolerance develops (as we see in all cells). In other word, the nerve finally gives up trying to resist this inhibition. Which is why we see decrease movements later. I'm not terribly sure of this, but that's my thoughts. This would be a great research idea though!

I have been on Olanzapine for 4 months now, and to me, it is a god send. I never knew what was wrong with me, or that I was mentally ill, but when I told others I was diagnosed their response was "What, you didn't know already?". For years I suffered with something similar to Akathisia without antipsychotics, it felt like terror all over but I kept being told it was anxiety. The first night I took my olanzapine, around an hour later, I felt calm. I had never felt calm before, and I honestly do not know how I was even keeping myself alive with how I felt before. My brain shut up for once, and I realised that I did indeed hear voices, but I thought it was a normal internal monologue. Now my thoughts are actually my own. I pray that on 7.5MG of olanzapine as it isn't a high dose I don't get any of the bad side effects listed here, and am hoping if I was going too I already would have got them. Please, if you are very ill then they can be a godsend. I was diagnosed with Bipolar spectrum disorder, Emotionally unstable personality disorder, and complex PTSD.

I just had to add a comment that I've placed on-line before about a treatment possibility. I took my wife to a psychiatrist for depression. She was placed on Abilify (which is approved by the FDA for schizophrenia but is marketed for depression). She took Abilify for 13 days and then stopped it because she didn't like the effect it had on her. About two weeks later she started to have uncontrollable movements of her arms and legs and her head which scared the heck out of both of us. I took her to a neurologist the next day and was told that she had tardive dyskinesia and that it might be permanent and that there was no treatment. I am a physician so I did an on-line search for tardive dyskinesia and found a note from some source I don't remember now, but which said that propanalol 10 mg twice a day has helped some people with tardive dyskinesia. I got her the medication and started her on it. Within a few days this awful tardive dyskinesia disappeared and she has remained on propanalol since then since she is terrified that the symptoms might reappear. I just want to let people know about the possibility that there might be a standard medication available that may be able to help with this terrifying medication side effect. She has had no side effects with the propanalol which she has been taking now for 4 years.

There are very, very, very few doctors that know how to taper a patient off of benzos. I have been on the damn drug for 28 years. I average about 4 hours of light sleep. The drug ruins the brain's natural gaba ability to sleep. Most all of my problems began with very dumb doctors over prescribing Xanax. Within about 12 days on Xanax my brain was dependent on it, and I could not sleep. The rebound was horrendous. People need education about psychiatric drugs. Most all of them are toxic.
