As The Blade Turns: Filipp Shvetsky - Eteri Tutberidze's Doctor & Kamila Valieva Doping Hearing

Спорт

Why is Dr. Filipp Shvetsky the favorite doctor of Eteri Tutberidze and the Russian Figure Skating Federation? Why is an anesthesiologist always present at the boards when Russian figure skaters are achieving their greatest moments? Why did he want to patent giving athletes xenon gas? And what will the Court of Arbitration for Sport (CAS) decide in the Kamila Valieva doping case at the 2022 Olympic Games? I break it all down in the latest edition of 'As The Blade Turns'.

I have been reviewing the 2000 case against gymnast Andreea Raducan who was stripped of her all-around gold after testing positive for cold medicine. Her team physician was banned for two Olympic cycles. While I don't think the med in the cold medicine is as bad as what Valieva was taking, I think there is precedent. A snippet I thought was interesting in that case and relevant here: " Răducan's case was brought before the Court of Arbitration for Sport in late September 2000. While the arbitration panel did concede that Răducan had not gained any advantage by taking the pseudoephedrine, and that she was an underage athlete who had followed her team physician's instructions, they also upheld the IOC's decision. The basis for their decision was the belief that the Anti-Doping Code of the Olympics had to be enforced "without compromise", regardless of the intentions or age of the athlete"

@@karolemorehead8382 Because she's 15 and Russian, so there are both moral and political problems that supposedly didn't exist with Raducan (to be fair, I would argue that Raducan was also a minor, but not in the eyes of the IOC and WADA, apparently).

Glad you brought that as IOC's and WADA's dilemma's how to "enforce" the rule for an athlete form Russia and one from Romania, big power vs no power. Tthey have clear double standards. Andreea took a Nurofen (that's an over the counter medication for cold and flu in Romania), without the knowledge of the doctor (she went into his kit and took one without thelling him) in the night of all around competition, because she got congested (she said the strong A/C in the gymnastic pavillion made her caught a cold during the team competition). Needless to say the the Russian federation insisted that Romanian was stripped of the Gold medal in the team competition, too, which took place before the all around, after which she tested negative. The court of arbitration didn't take in acoount anything (that she was a minor, that she took the Nurofen from the Team Dr's aid kit, that the substance was not helping her with anything) and they enforced the rule "without compromise". Few years later, after the pseudoefedrine was taken off from WADA's banned substances, Andreea submitted an inquiry with the IOC to get her gold medal back and she was refused, AGAIN. One wonders if the "without compromise" is applied just to small countries?

My take (& it doesn't matter, I get that) is that this whole situation would happen sooner or later. It's magnified exponentially since it's at the Olympics. I'm grateful b/c there may actually be world-wide viewers holding the federations accountable. While I am sorry for K, whom I do believe to be an incredibly gifted athlete *I mean, c'mon, those extensions, although sometimes overused DaniG style are from her & not as a result of doping*-her results should be nullified since the playing field was altered. I would imagine that if her results do stand, there will be huge negative ramifications for the $$$ that the Olympics garner. At any rate, I am not at all surprised this happened. I'm sorry for K but I sure hope that this changes my beloved sport for the better. I miss those days. Thx again, Dave.

Anaesthesiologists are also trained in acute and chronic pain management/analgesia (in addition to airway management and provision of different types of anaesthesia). Some of them may even choose to do fellowships after graduating from anaesthetic training which allows them to subspecialise in chronic pain. Anaesthetists are also well versed in cardiovascular and respiratory haemodynamics. My only guess is that this anaesthetist transitioned into acute and chronic pain management and became a medical skating advisor through that route? It does seem rather atypical as I would have expected a specific sports physician to be involved, especially when we are talking about Olympic tier athletes. Edit: Anaesthetists are also well trained in giving local anaesthetic blocks and steroid injection (often under ultrasound guidance) to reduce local inflammation in inflamed joints, rotator cuff and prolapsed lumbar disc injuries.

Ortho isn’t as great at pain management- they’re really focused on surgical repair. A sports doc or someone trained in physical medicine and rehab would actually be my first choice for a “team doctor” Having a pain specialist actually is scary- we have pain as a tool to prevent worsening injuries - so over treating pain actually will lead to more injury, more rapid body breakdown etc Which is 100% what we’re seeing in sambo 70. 🤯🤬🤯 I would call it child abuse. And all of the adults in that group need to face criminal charges on top of lifetime bans from sport.

@@etherealtb6021 I agree with Karen Burgner... As surgeons, Orthopaedic doctors are not well versed in the intricacies of pain management. Typically the two most common routes to becoming a Pain Specialist is either through Anaesthetic training or through Intensivist training (ICU doctors). Other routes (less common) are through Neurosurgery or Neurology. That is because many types of analgesia (especially narcotics) can also be respiratory depressants as well as addictive. It requires someone well versed in the management of cardiorespiratory haemodynamics to select and titrate the appropriately safe dosages. Orthopaedic surgeons, like most other surgeons, have a decent idea about pain management for their specific operations etc. But anything more complicated beyond that they will refer the patient to a specific Pain specialist. For Olympic athletes, a specially trained Sports & Rehab Physician would be the most ideal choice.

Looking deeply into this scenario has become more important than who's getting the team event gold medal or who will be able to compete in women's event. The fact is a banned substance has been found in a skater's test and everything points to the evidence that she herself didn't decide to take the drug or was even aware of this being part of the stuff she's been given. Dave is absolutely right when he questions why this doctor continues working so close in competitive sports. If any professional fails so badly as he apparently did in the past, why any ethical sports staff would hire him ever again? it's time to raise this kind of question instead of narrowing the scope of what everyone should be looking into. The worst of this situation would be if it's like it's officals, journalists, sports institutions X Kamila. They have to reach behind her and make it very clear that this is an aopportunity to save generations of children from being experimented with.

If everybody but Kamilla was done in 10 days then One has to ask was Kamillas done too and they held it back till after Russia put Kamilla in the team event. Or was it even Kamilla's sample that tested positive. We all know Anna and Sasha would have won short and long as well. Would love to know the answer to that question.

In all sports, the performance on the day is what counts, so it seems illogical at first that athletes testing negative at the Olympics would test positive months before. However, as Dave has said, figure skating is unique in that it is so complex and has so many different elements, each of which have to be trained with high numbers of repetitions to gain consistency. Recovery time from training is also an important factor. Because figure skating has frequent competitions (and the margin for error in ladies skating is slim because not many women can jump quads and triple axels), being consistent is absolutely vital to the success of a female figure skater. If a drug allows you to do even 1 more run through per day, or 10 more jumps, this adds up tremendously over time. So, if a skater were able to take a banned substance that would allow them to do more runthroughs and repetitions, and perhaps decrease recovery time, its easy to see that while the drug wouldnt change much in a one-off situation, it would give a considerable unfair advantage if used to assist the training process over time. I hope this makes sense- basically taking drugs to enhance performance during training when skaters arent at competitions allows them to reap the benefits by gaining consistency without getting caught by doping tests. Notice how remarkably consistent the Russians have been this season (and for the last several- I am not accusing the girls of knowingly cheating, I put all the blame on the adults around them). I doubt they are the only ones doing it, but knowing Russia's history for doping it is certainly more prevalent. I also dont doubt the girls' work ethic and talent- no drug can make you feel the music like Kamila Valieva or Alyona Kostornaia can (if there were, perhaps Daniil G would be better at choreography). But, its definitely been something thats been suspected for years, and Kamila will undoubtedly not be the only person being taken advantage of in this system. I'm also not in any way hating on the Russians- I adore watching them perform. Kamila's short program this year (in memory of her grandmother) took me back to when I was watching the Pyeongchang Olympics immediately after my own grandmother's passing. As a Brit I dont appreciate Daniil G's choreography to Bolero at all, but I do appreciate that Kamila does her best to sell it. Equally, I was rendered speechless watching Kostornaia when she performed her Angel program at the 2019 GPF. Medvedeva and Zagitova also drew me into the sport in the first place when I was a young teenager, Alexandra Trusova and Anna Shcherbakova are pioneers in the sport, and I will never tire of watching Liza Tuktamysheva entertain a crowd. Point is, I love watching these talented athletes. I just hate that Eteri Tutberidze has left so many girls broken, and the lengths her team will go to get the maximum results. It needs to stop before figure skating and the Olympics as a whole entirely lose the little credibility they have left. Sorry for the essay!

I doubt it. Nassar was in the US. This is international. I mean I can’t imagine they would be taken to court in Russia. Plus Nassar was a person who was overlooked by the system. Here this is probably bigger than Tutberidze et al, and is likely state sponsored. So there is no court or public reckoning that could even happen. So it would make sense to have a doctor present that has experience in emergency medicine as a first resource in case of injury (especially serious injury). This could be done by any doctor with this additional qualification (just it is anesthesiologist most of the time bc of experince with intubation ect.). Obviously you would still need an orthopedic doctor on your team for further treatment.

People will question the validity of her landing on the podium and the integrity of the Olympics will be lost. Can you imagine the energy that Kamila would be receiving during warm ups? No matter how much someone tells you that they’re just “haters” I can’t see that being good for a 15 year old’s mental health. I am wondering what legal standard is being used to assess whether a “protected person” should be suspended from a competition. For example, to be found guilty in a criminal proceeding, prosecution has to prove guilt beyond reasonable doubt; in a civil proceeding - the standard is preponderance of evidence. What are the regulations that the arbitrators will follow in deciding this case? Is there a precedent when a similar case was already decided - with a protected person with positive out of competition testing for a micro-dose of a banned substance. Dave is just reading articles…where’s the research? It’s like when the government tries to pass a complicated tax bill - the media take one or three sentences out of the bill and spin it depending on their political leanings…in the meantime, the public has no idea what are the actual contents of a 1,000 page complex bill? What’s the law governing this whole procedure?

@@emmotions09 mistake how? False negative tests can happen because it’s not detected. But by the time you identify a positive that sample has been tested many many times independently. Because they can’t disclose information we don’t know whether they’re able to test a different sample from that time. I heard something about an A sample but i’m not even sure if there’s a B sample. But if there has been a doubt about it false positive she would not have been provisional banned and all this process started. I can see the argument about allowing her to compete but I think against it. It will send the wrong message. It also messes with the psychology of the other competitors. If this had not been announced at all, fine you could let her compete and then deal with it afterwards like what happened with the team event. But now the other athletes would know that they’re competing against someone who doped. I have seen countless comments who said that they felt depressed about even watching the sport. At the very least some skaters might feel that way about the sport that they spent their entire life’s towards at the Olympics! It could be very demoralizing. Will the crowd be cheering or booing? How is Kamila gonna feel? I don’t think it’s as easy as it sounds.

Hey, I‘m also an anaesthetist and I also sometimes work as a doctor at sports events! And not just me! First of all, I‘m a doctor. One important thing: in many countries, including Russia or Germany (where I live) anaethetists have a very different training and very different „duties“. We not only hang around in the OT and watch people sleeping 😴, 😅. We are also trained as emergency doctors and work on ambulances or helicopters. I know many colleagues who work for soccer teams etc. We are trained for all kinds of emergencies, including fractures, head injuries etc. So it’s not unusual that an anaesthetist is a team doctor, it does make sense. But I know that in the USA the training at medical school is very different. In most countries in Europe we don‘t have specialists who only work as emergency doctors. Most doctors on the ambulance helicopter 🚁 for example are anaesthetists, some are surgeons, etc. But of course you are absolutely right, this doctor you are talking about is an absolute disgrace to our profession 🤬

Love your reporting !! I just read on Reuters that the CAS hearing was conducted not by the doping panel but by the legal unit. Here’s the quote: “Initially it was thought the case would be heard by CAS's six-member doping unit, but the court later said its legal disputes unit would be handling it.” Do you think there is significance to that change? If I remember correctly, the IOC (and the Russian media) was using the phrase “it’s a legal issue” when reporting on the delay of the team medal ceremony when the story first broke.

I agree that the Olympics are better off with top competitors in play, but I don’t think that carving out new rules for Russia will do anything to change this. Whatever rules you give them - they’ll find a way to work around them, there’s almost a national pride element to “outsmarting” the rules. I think only a full ban can force them to realize that this isn’t a game.

There are good reasons why these teenagers are considered "protected persons" and why they are not considered "adult", hence responsible like an adult. On the other hand: if Kamila is prooven positive with doping substance(s) then all responsible adults around her should be banned for a lifetime. It would be such a devastating betrayal of her - without any doubt outstanding - talent.

I’m just appalled at everything they’ve done to all of the poor girls under this coach. The diuretic is just one part of it, they didn’t let the 2014 Olympic competitors drink any water on the day of competition! And the poor 15 yr old whose hip literally broke on her in the middle of competition - that degree of osteoporosis in someone so young is only possible if she’s severely disrupted her puberty hormones and is chronically under-nourished and over-trained. This whole thing is horrific child abuse and needs to be stopped.

All I’m gonna say is that, none of this surprising. My mom was a doctor in Moscow during the ‘80s and she saw first hand how much doping was happening in Russian sports. Athletes would be taken to the emergency room and then whisked away by govt officials to a special hospital so they could hide the doping. My mom worked closely with a nurse who was the sister of a VERY famous Russian pairs figure skater. She told my mom all about the doping that was going on with her brother and the top Russian figure skaters. This is a systematic issue that’s been normalized for a long time and won’t end until Russian is properly punished.

After doing research and I watched Icarus in a college class, I refuse to believe a 15 year old girl would intentionally be taking these drugs. There's no way she would have even got them herself. I think for sure they told her they were "vitamins" or they coerced her into taking them. I followed Kamila about a month ago on Instagram and she's an angel.

Do they and the ISU all have the same IMG agents? (LOL the conflicts of interest regarding IMG in the past 20 years make me pose what would otherwise be a ridiculous question). You mention the possibility that she might be "okay" after this, noting the scars of this happening will be permanent in some way. How about the other girls in this event? Will it effect judging bias? On principle alone, let alone biased judges, they cannot get a fair shake in this experience....right? I believe they knew the results a long time ago. If it came out before she even left Russia or during Europeans it wouldn't have been as politically effective as coming out after she competed in the Team competition. The west wants to go to war with Russia and they are always demonizing them bc that's what you have to do to get your population to support it. We have that country surrounded by military; we have the world surrounded so of course they are going to be nationalistic. If I didn't follow every competition the past 4 yrs I wouldn't even be able to believe the drug test at all because I would assume it was another lie to get me to hate Russia. But, I watched so that's like 90% true in my head; I never have certainties. When the world is always against your country you better believe they are going to unify. Thank god she isn't American bc Americans would rip her apart bc they love tearing each other down and don't really believe in redemption. At least she can go home to Russia and be supported and I want her to feel loved. If Russian military ever has us surrounded you will see how fast we can unify and wave the flag. Our country gave Tamyra Mensah-Stock no celebration being the first black female to win Gold in wrestling because she was patriotic. She wasn't the kind of black person that most Americans like so they ignored her acting like she didn't exist. She didn't fit the narrative so you don't see her in TV advertisements and probably never will. I don't blame Russians for sticking together especially when they are the ones surrounded while western journalists ask Putin why he is so aggressive. I don't think Americans have the right to call him aggressive when he isn't the one with all the military bases. I never watch clips of any politicians interviews or speech bc it has mislead me in the past. I watch it all or don't watch it. Usually I do and most of the time Putin seems like the only rational person in the room who clearly doesn't want war. I was at a Methadone Clinic for my past addiction to Methadone, it's the only drug I was addicted to the whole time and the Methadone clinic can drug test and find anything in just 2 days and I don't believe it really took this long which is another reason it's timing is what I question.

@@amethyst5512 if an agency is supposed to drug test and they decide on their own to hold back results and knowingly let an athlete compete then they are as corrupt as these coaches and team doctor. It's not for them to decide to wait because they want it to get more attention. I think if they held back results it wasn't bc they actually cared about the athletes. I hope we have major changes but if you are corrupt enough to sit on test results then you are too easily corrupted to be trusted. However, we can only speculate because we just just know what they want us to know and I can suspect all I want that they sat on results because a powerful west wanted to make Russians look bad because when you want to get your citizens on board and support military action against a country it's important to demonize the country you want to go to war against but it will always just be speculation because I only know what information they let me know. I never claim to be certain about most things because certainty requires knowing all the details and we only know what they let us know so speculation is all I can do.
