In this video, Doctor Andrea Furlan explains why people with chronic pain do not like to exercise, why they feel pain after they exercise and therefore, they avoid exercising. This is a paradoxical effect of exercise, because, normally, exercise has an analgesic effect.
Dr. Furlan is a pain specialist in Toronto Canada. She is a physiatrist (specialist in Physical Medicine & Rehabilitation). She holds a medical degree from the University of Sao Paulo and a Ph.D. degree from the University of Toronto.     Fibromyalgia is a nightmare to manage and live some days I get so depressed and so mad at being in awful pain every second of everyday.😢

    I don’t have fibromyalgia but do empathize. For me it’s also 24/7 even w/chronic high dose opioids and mult surgeries on lower back/si joint for 20+ yrs plus endometriosis. Prob like you no pill, therapy or test I haven’t tried incl natural. But found a doctor of physical therapy. Shoulder surgery is soon but distracts mefrom my back 😉. jk. We’re not alone. Chronic pain now an epidemic. Im lucky where I live to have good heath care. Can only imagine living in some other places w/little to no access to healthcare or drugs. I’m thankful it’s not worse Reminds me could be worse nor do I want that lesson. Suki Baxter and Taro’s Feldenkrais method videos helped. Michael Sealy has very good hypnosis videos for pain and sleep. Be kind and patient w/yourself and body. When we relax (esp warm bath) our muscle tissues/joints relax and lesson pain.     Wow! Every person who has or has had back pain or chronic pain should be prescribed this video to watch. In fact all your videos should be made a requirement for all us chronic pain patients. I didn't think i could learn any more about chronic pain than i already have after 24 years and 11 back surgeries. In one video, I learned more than i have in years of self research. Thank you so much! Btw, it took me 5-6 months of physical therapy to even notice a significant difference in my pain since I'd be hurting more and just completely wiped out for 2 days. Now I know why and why this stubborn woman shouldn't overdo it every time I do my PT exercises at home. That euphoria makes me want to keep exercising till I can't move anymore. Then I'm hurting bad for a couple of days where i can't do hardly anything other than rest. Then back to exercise and trying to not overdo it. It turns into a vicious pain cycle if I'm not careful.

    I hurt my back at 25. Had to have 10 hrs surgery at L5S1 to remove the whole dish and scaffold the area using part of my hip. I never get the strength in my legs back. I can stand and walk super slowly for around three hours a day. That's been my life since then. I'm now 47 and very very bored of laying in this room.... I miss so many things. :( I try so hard to keep an even keen but its so hard. That bed feels like my death bed but I'm staying alive... It wouldn't be so bad if I had money... Things like virtual reality could do so much for my well being... but no money. Always... no money. I was a skilled worker... I put so much effort into shooting for a bright future....

    Hang in there. 3 hours is way better than nothing. Protein helps, especially collagen. My relative soaks chia seeds in lemon juice to break their hulls then adds water because chia stimulates the growth of connective tissue.@@bubblezovlove7213

    What a wonderful video. I can really relate to this. I used to go through a slump and depression after exercising which was weird. The doctor attributed it to my subclinical hypothyroidism. But ever since I took up dancing which I love and I was consistent, I feel I have made some gradual improvements in my health, pain management etc. The last part of your video gives a lot of hope. Neuroplasticity. Dr Andrea, you are doing a fabulous service to people going through pain with such wonderful educational videos.

    Dr Andrea this is a powerful video. I am learning the hard way that my approach to exercise is wrong. I am totally wiped out mentally and physically when I do too much.     Great video, very informative. My relationship with pain is one of many levels. The ultimate love hate relationship, yet many a time if severe pain did stop this stubborn woman from overdoing the limitations of her body's capabilities my life would be worse comprised. So today chronic pain and I have mutual respect. The best time for both of us is when we find distractions, in writing, music, movies, comedy ,extra walking in the stores for exercise, stretching.. Like a baby stretching before they get up in the morning so should we...in the bed...under the covers even.. if I don't I wake up shorter and can barely walk... Pain Meds are the last resort and the consequences are so harsh... Death someday will be my friend though not to day!

    Excellent explanations by dr andrea about paradoxical effects of exercises in chronic pain patients . And conditioned pain modulation .yes often i have noticed and heared this top down mechanism not working up due to improper working of drug cabinet inner pharmacy . So its a wounderful method how to inhibit and activate brainstem . Yes graded pacing exercises like aerobics ( walking ) yoga and isometrics will reverse these central sensitization .fibromyalgia patients are really invisible disabled needs our intervention education and reassurance as a team .but most drs unaware of this as pain system instead diagnosis trapeziitis as fibromyalgia .....excellent exercises plays a key role .amazing video dr andrea .....its with full stuff ...

    Wow! I'm so glad I stumbled onto your videos. Some of the most informative and relevant videos on the tube for pain and pain management - they've really provided me some much needed relief right away. Only recently diagnosed with fibromyalgia - while dealing with intense physiotherapy for 2 herniated discs. Starting to get a good grip on things with low dose of nortriptylene and multi-modalities approach. I'm wondering if you've heard of sitting as an immediate trigger for myofascial right sided back spasms? Haven't been able to sit for 4 months - getting by, but driving poses issues. I'm not exactly sure how to describe it to my doctor. Anyhow; loving your channel north of you here in Thunder Bay. It's been so helpful already. Thank you! :)

    Very well explained. I work in home health physical therapy and one of the most difficult things I encounter is trying to convince someone with chronic pain that the way to get better is to exercise more. If someone has been a couch potato for the last 80 years, it's not easy for them to start a new habit that is completely different than anything else they have been doing. The word "exercise" scares them, as they automatically think of doing something super strenuous that will cause them enormous pain. I will share some of the information you mentioned with my patients, their family members and their caregivers in hopes it will make sense to them.

    Very interesting Dr. Furlan! I've never understood when people say how great they feel after yoga. It leaves me done for the day lol! Same with chiropractic adjustments, Fascinated by the inner pharmacy, mine must be empty, ( especially after having had Covid).     I have lupus, psoriatic arthritis, and fibromyalgia. I, too, found yoga exhausting. Even though I did yoga every day before I became ill. I have found that short Qigong sessions are helpful. Nothing strenuous, very gentle. And I *try* to take a 5 minute walk every day, but sometimes I just cannot. Some Qigong instructors are gentler than others. I recommend Qigong with Kseny on KZread. It has really made a difference for me. I'm not left drained and heading straight for the bed or hot bath (or both) afterwards. I hope this has been helpful to you, if for no other reason than I see you and I understand. Best of luck to you!

    With most of my chronic pain exercise definitely makes it worse. One type of pain though, arthritis in my knees, I do light walking for 5 minutes several times a day because I know they'll stiffen up and get worse faster if I don't

    I have walked distance with a backpack to keep my endurance since the beginning of the pandemic. I have done this for strength, endurance, depression, insanity, resilience and absorbing sunshine or daylight. Most of my pain I can walk off. With a good diet most issues disappear on their own.

    Exercise ramps up my pain. I do it anyway because I have pain either way! Any recommendations for no tolerance of drugs given for central sensitization. I have tried all the anti depressants, lyrica etc but am not able to continue due to bad side effects even at a low dose

    Hi, I'm Disha, from Chile. I apologize in advance for my english. I suffer chronic pain I asume from an accident I had 35 years ago where I broke my back. One of my chronic pains is "Lumbociática" ciatic lumbar nerve ...sorry, don't know how to say it better...but it makes exercise really hard since it hurts so much that I walk with fear... Sit with fear...and I also have Alodinia all over my right hip and lower back... Is there another way of getting some relief, maybe another kind of exercises? Thank you so much for your videos. 💜

    On my way to the gym now, the blood flow numbs my pain during the rush I feel normal for a short period but I'm very mindful of how much I do to wreak as less consequences as possible later...

    Having fibromyalgia for me was finding something that was gentle and relaxing. Tai chi qigong helps. Just the noise of a gym upsets me anything strenuous was no good.

    @@DrAndreaFurlan when I was diagnosed 22 years ago not a lot was known about Fibromyalgia. In groups I went to every one complained that told to exercise it’s good for you. There was only gym or swimming mentioned. It was about 10/12 years later I used to teach tai chi but had the understanding that fibro clients shouldn’t do whole class do some sitting as though it’s gentle it can be quite strong. Best to build up slowly. To be honest with the experience I gained I would say Qigong is really the best to start with. From uk

    Hello dr .iam following you quite a long time .found ur exercises and advises beneficial .thank u .now i wanted ur advice .three weeks before i had a clevecle fracture .first week was very painful .now is better .bone is overlaped .iam with a sling only .what i wanted to know is.does this heal and fix naturally or i must have surgery at the end .dr says it is up to me .but i have no clue that without surgery it heals or not .should i wait or not

    I thought I was just crazy in my head because when I do therapy for my neck and back pain it feels worse than before! I do walk because I have bad knees also so I know I need to keep walking.

    Did you see my workout video on my channel? I started that program at 10 minutes, then 15, then 20 an dnow 35-35 minutes, in the swimming pool, 3-4 times a week. However, my pain levels have never decreased. I do get some pain relief for a few hours after, but not consistently.

    I get worse after exercise....i have fibromyalgia n sciatica....3disc bulge but doesn't pinch nerve.... but I have tremendous pain.... can't stand for half an hour.... your videos are helpful

    Dear Dr. Furlan, how would you start with people who even are not able to walk a little? Have you heard from Fluoroquinolone-associated Disability (FQAD)? Some susceptible patients have horrendous reactions to this group of antibiotic, which a) has a dreadful impact on collagen-regeneration, tendons and ligaments and b) blocks GABA-A receptors and leads to small fiber neuropathy in the nervous system. I see the benefit of excercise, even for people with diagnosed issues (like a collagen disorder), but how to implement if even the lowest paced activity (e.g. walking) is not possible to do? There's a huge amount of people with FQAD ending up in wheelchairs, from perfectly healthy to not able to move at all a couple of days in treatment. The second group I have issues with it is patients with long-covid/CFS, as they tend to crash even after a little bit of excercise with limitations of walking to a couple of steps per day.     My whole body throbs from arthritis in every joint in my body of course I exercise to keep the joints strong but I don't know what to do about the pain opioids don't work pain management doesn't have anything near as good as kratom

    Would taking painkillers and other medication eg (gabapentin) just so I can exercise through the pain, be bad in the long run because its dulling or prohibiting the synapses process? Thank you so much for this explanation. Bedridden for decades (spinal problems), I'm only mid-age with a fair way to go and wanting to get my life back, thank you again.

    It is really important that you see a physiotherapist. The physiotherapist will examine your specific situation and tell you what you can and cannot do. These videos are for educational purposes only. Now that you are more knowledgeable about your condition and the types of exercises you could be doing, it will be much easier for you to discuss with your physiotherapist. You will understand what they are saying, and why they are recommending one exercise and not the other one. I think that patients who are more knowledgeable about their options are much better to adhere to the treatment recommendations. That is the ultimate goal of these videos, that you can discuss with your health care professional and be a partner in the decisions.

    Namaskar Mam. 🙏 When I do yoga(ordinary streching) or weight lifting(mild) or intense swimming or running,i start feeling weird in brain,anger arousal, confusion, weaker cognitive function and sleep issues. I went to psycatterist she gave me "Pregabalin+Duloxetune" and it helped me a lot,though not completely,i stopped the medication due to side effects after 6 months, benefits also stopped after that. Then I changed my psycatterist and she told me that I have illnesses anxiety disorder and gave SSRI,it did no good and I stopped them. Everytime I do intense exercise i feel those symptoms,i have been to so many doctors in which only "Pregabalin+Duloxetine" helped,but i want to know the reason why it happens with me, specially when neck is involved symptoms are more intense. Its more than 14 years of thus problem. Pls pls give your opinion 🙏 I am from India,if i lived in Canada,i would have visited you in your clinic, because you have acknowledged so many issues,which our Indian doctors don't believe,they say it's in your mind.

    @@ARPIT-gm3lw I also have a confused mind after intense exercise. Therefore I settled on light slow exercises and stretching, it brings only pleasant sensations. I experiment a lot at home with different exercise equipment and observe the reaction of the body. For example I found that slow tilting the body back and forth while lying with the thoracic region on a large roller brings me relaxation and relief from spasms in the entire spine (for a while). After the roller I do light exercises to strengthen my back. You need to look for some suitable light exercises and do them for at least 15 minutes every day or every other day, exercises can be alternated.

    My inner pharmacy has me addicted to exercise. I can start getting high just by thinking about exercising. At daybreak, each day, I strap a step counter on my left ankle and move into my busy day. Before I know it, my feet can move me over have 15,000 steps each day. How. This (my) ankle step counter creates a biofeedback loop. The better my brain monitors, and guides, my daily steps, the more my brain stem pharmacy pumps feel good drugs. Thus the loop. - I accidentally learned this method way back in in 1980's, as a youngster. I stumbled onto an old paperback book, Silva Mild Control. It featured a biofeedback finger applied heart rate monitor. The purpose, back then, was to teach meditation; to consciously slow your heart rate down. In a weird way, I decided to use it for the opposite purpose. As a result, I haven't consulted a doctor in over 30 years. My brain stem gives me everything that I need. -
