Central sensitization is a condition that affects many people with chronic pain. The person feels like brain fog, difficulty to concentrate, poor memory, low mood, low energy, fatigue and has pain in many parts of their body. This is common in people with fibromyalgia and complex regional pain syndrome. Central sensitization is also present in people who have opioid-induced hyperalgesia.
The treatments involve mind-body therapies and movement (exercises).

    Dr. Furlan has told the story of my life with fibromyalgia, central sensitization, and now, after a three level lumbar fusion, I am fearful that my tendency to simply develop more pain pathways will mean that I get no relief from this fusion, but just more new places for my brain to over focus ! When I first discovered the concepts of neuroplasticity it was such a relief to have clear information that could explain the WHYs around chronic pain syndromes. Now, back to doing the work of retraining my 74 year old brain, to hopefully have better overall control over my symptoms.

    I didn't even have words to describe my pain to my doctor, I guessed that's why he was unable to help me. I have been praying to find a way to heal my tingling and numbness which goes from my toes to my brain. I don't care how many years it takes to heal, but at least now I have hope. This doctor is saving so many souls from so much suffering and despair. Gratitude, gratitude, gratitude. I feel better already.

    I have head n face tingling it's so ugly pain on favevfor few sec but not on one side it changes then it stops and upon that terrible ringing ear in 1 right ear , so I have been lots of test n last torching mri , ct scan much better , I hate mri n all report came normal, now no diagnosis they says it like sycodomatic disorder, in 3 months every 15 days I changed different medication it eorks to less symptoms then its back now vslium added too. So how can we accept n. Live normal life, any solution pls share

    It’s extremely difficult to suffer such pain when looking so normal. Never able to sleep, POTS, migraines, smells, light and everything else this wonderful doctor has talked about. I’m desperately trying to retrain my brain.

    @@DrAndreaFurlan​​⁠ - I was diagnosed with central sensitization in 2010 at Vanderbilt by a highly decorated PhD pain physician. He did not tell me how to fix it, but I assume there was less information about it back then. He put me on a fentanyl patch. Two years later, a geneticist at Vanderbilt diagnosed me with Ehlers Danlos Syndrome. I know EDS causes a lot of pain by itself due to mechanical faults and damage in the body tissues. But I believe the central sensitization diagnosis was also correct due to my sensitivity to noise, lights, tastes, smells, crowds, etc., etc.. Over the years, I asked for reductions in my pain medication because I knew my tolerance was increasing and it was harder to control my pain. But when I had an illness or injury and requested an increase in medication, it was never granted. I am now fully disabled, not on any pain medication, and I spend my days living alone lying on my couch. I suffer from terrible anxiety, depression, insomnia, fatigue, and of most of all, pain. Doctors do not take me seriously when I tell them how much I am suffering. The best way I can control my symptoms is by laying still, in the dark, quietly listening to KZread videos. I know opioids are not recommended in most situations, but with pain medication, I am able to function. My pain is reduced, my mood is better, and I am able to sleep at night. Unfortunately, due to my numerous trips to the ER due to uncontrollable pain, no doctor will prescribe controlled substances. I have a broken tooth that needs to be pulled, but I am avoiding it because I know I will not be able to handle the pain without medication. I just want doctors to know that people who suffer from severe, chronic pain are not drug-seekers. I have lost my career, husband, family, and friends - not due to drugs - due to pain.

    I was diagnosed a few months ago. It took about 14 years to find an amazing Doctor. She believed me and ran all the tests possible, sent me to great specialists, and they all have agreed that it's CPS. I also suffer from PTSD, anxiety and depression, PCOS, and myofascial syndrome. I've been taking duloxetine, amitriptyline, b12, metformin... the side effects were awful at first, but once my body got used to the meds, I started feeling better. The pain is never completely gone, but at least I'm not bedridden anymore.

    I have fibrimyalgi and my physiotherapist recommend hydrogym. Afterthat i became better. My pains reduced about 50 percent and my mind-fatigue went. But he said i should continue to be as stronger as before.your explenation was very useful. Thanks alot ❤❤

    You covered half of the symptoms. I had 22 out of 34 CSS symptoms and 16 occuring simultaneously for nearly 6 years. I have been suffering with it for 26 years and disabled by it for 16 years. I began “doing the work” to accept and commit to using the tools and have improved significantly. I am down to mostly 4-6 symptoms constantly but still feel run over by a truck most mornings. But I was an extreme case I think and undiagnosed until 6 years ago. It takes a lot of work to calm this monster down. Much of the work and therapy seems counterintuitive at first. Don’t give up.

    @@Truerealism747 So the last four years has been a journey, combination of isolation and sedentary living plus a big problem with the medication caused me to gain quite a bit of weight. I made the effort to lose that weight and I started walking. It was tough because my body (brain) rebelled against the walking at first. So I started desensitizing by going out a little bit further each day and each time and then I decided to just be really easy with myself and so I just kept walking. In time I started making breakthroughs, and I bought a Fitbit watch to help me motivate. Then I got some dumbbells and some of those exercise bands. Also got one of those exercise balls that you can sit on to begin getting some balance and put some variety into it. The whole process is kind of doing the meditation, the deep breathing, practicing relaxing every muscle from head to toe, walking, doing some weights, and just being patient with yourself. I’ve made a lot of progress. I’ve gotten a lot of improvement and function back. But it’s not for the faint hearted, that is for sure. And 3 years to see big improvement is not a long time if you have it pretty bad to start with. God bless you and I wish you great success.

    Thank you for the explanation; I found your channel because I was recently diagnosed with CS after a year of searches . It all started from a TMJ issue which, if left alone, would have gone away on its own . It was retraumatized by different manipulations and now I feel like my brain is attached to my symptoms. I fear any kind of movement, jaw exercises ( done mildly mind you - one repetition more specifically) fires up my symptoms, cervical and back exercises made symptoms worse and so I developed anxiety around working out. I only do walks now. I feel like my body is against me and whatever I try to do just makes things worse. I was recommended behavioral therapy which I will start today and I will focus on the "mental" side of things for a while since anything else just makes things worse, which gives me more anxiety and depression. I used to be highly active, I still am very healthy but for these troubling symptoms, and so depression is something that overtook me. The thing that baffles me is that I am even scared of getting better. I am really messed up psychologically because of what is happening and I don't know what normal feels like anymore.

    Luv to watch your videos, I have learned so much. I was in a car accident and suffered whiplash and lower back pain. I was seen in the ER and sent home with a week of rest and anti-inflammatory prescription. After a few days I thought I was better(no sick days =no pay)and went back to work (welder) .After the second day back I started having back pain and some dissines but just took more pills and kept working 💪. After three weeks I could barely Walk and finally decided to see my doctor, he was shocked to see my condition and prescribed pre-gambalin twice a day with a muscle relaxant. Well after a month I was still having problems standing or sitting for more than a half hour. He then prescribed x-rays and MRI and added some Cymbalta. I can now walk and drive my car (for up to an hour). It's been six months since the accident and dealing with pain is the only thing I think of. Just to say thank you for helping me understand the different things that are related to pain, your way of explaining things is a true blessing 🙌

    Wow this information is great made me understand what is going on in my body I’m definitely central sensitization I’m so happy I cam across this video thank you Dr. A lot of things you mentioned about pain is what I’m feeling with the pain. I took control of a lot of things to make me feel better on it works a whole bunch. But there are days the pain is just so excruciating I have to stay in bed and sleep. But I’m a fighter and I push myself again unless the pain is so excruciating. My diet is good very good there are a few things I have to stick to but nothing I can’t do just have to put my mind to it. Thanks again a whole bunch

    Thank you so much. My doctor has never referred to Central Sensitization, although it has been referred to as Fibromyalgia. Also, just to say, thank you so very much for not having music on throughout your presentation. I appreciate it so much as, due to my symptoms, I'm unable to process information when there is too much going on (it becomes very stressful).     I've had 10 surgeries , 6 of them were orthopedic . There is no avoiding pain . My first one was 1980 . The reconstruction of my knee has never stopped hurting . 2008 distal bicep rupture and reattachment . 2017 full hip replacement still aches . 2021 long head bicep rupture and rotator cuff torn and repaired . All constant pain plus others , and I just live with it . I promise it is not all in my head .

    @@DrAndreaFurlan I have had 22 surgeries since a bad car accident years ago and most recent was 10 months ago and this time I’m really having a hard time healing with the pain and I have been to so many specialists and no relief. I have had 5 foot reconstruction surgeries and facial reconstruction surgeries, just a few of the 22. My right side of my face is mostly wire and metal plates and now my hands are so painful, I can hardly use at times. I have tried exercise, physio, resting good didn’t and pain just gets worse each year.

    Subscribed. Your videos are always a great help to me. This video in particular resonates so strongly with me. I feel like I am in your clinic and you are addressing my problems personally. I certainly have Central Sensitization. I think of my pain as an orchestra. Once a certain part of my body starts to twitch or act up, slowly the string section will join or the wind section will come in, and soon the who orchestra is in full bloom. Such is the state of my pain problem. It reached such a hyper state, that even if I have a memory of some traumatic incident, the pain level shoots up. I am seeing a pain specialist. She prescribed tramadol and gabapentin. After a second or third visit, the tramadol and gabapentin dosage is drastically reduced, citing kidney, liver, and what else side effects. I went for kidney, liver scans, and tests, all came back negative. She is now talking about meditation etc. I totally see where she is coming from, but the advice doesn't do much to ease my pain and improve my life. I know my condition is chronic and I'm trying to get through one day at a time.

    Thank you so much Dr. Andrea for giving me hope. I have exactly what you've described. My dr. says it's fybromyalgy, but he never got me diagnosed, since we don't have a real diagnose in my country. I never gave up to get rid of the pain. I had all sorts of therapie and I'm practising yoga.

    Thank you for your videos! They haven been very helpful after I received my diagnoses. I've been 1 year in now of chronic pain. Been walking alot and focusing mainly on movement. Only recently did a surgeon confirm central sensitization. Since then I've been exploring mind body practices. The pain had reduce 50% in the last 6 months. I'm keen to see how the combination of mind body practices & exercises will help me. Im a 33 year old male. Its been a tough journey so far. Hoping to get back to my old self (but with better mind set and fitness)

    Yoga helps. My central desensitization shows up as sciatica. The surgical trauma response was the root cause I think; multiple surgeries for breast cancer destroyed and damaged miles of nerve activity mid chest that intervertebral interference. Everything went haywire! But long walks when I can, measured deep breathing, and intense floor yoga that counter poses the spine helps tremendously. Intermittent fasting too for some reason. Knowing this is somehow more comforting in ways that another surgery, I.e. hip replacement Could never be. Thank you 🙏🏾

    I was just diagnosed with Fibromyalgia and Neuropathy on hands and feet. It will be 1 year in April of 2022. I have recently changed my eating style. I make sure to have fruits, vegetables, chicken, fish, seafood. I am only eating brown rice. I have whole wheat bread once a day. I they to get a lot of protein in my body. I have cut out Pepsi, junk food, fried foods. I dont eat pasta. I try to incorporate walking daily inside the school I work for early in the morning before students arrive. The halls are very long around the bldg. I try to get in 4 rounds. Sometimes I only do 3, if I am very sick with the pain and exhaustion. Everyday is a different sensation. I will have some good days where I will have pain but I don't feel extremely sick. I enjoy learning with your videos.

    Thank you for this very informative explanation of central sensitization. I would like to suggest that, with certain traumas, limbic system impairment may require additional consideration in that CBT therapy and the other mindfulness practices you mention are extremely helpful as stand alone therapies, they may not be entirely enough because they do not address any underlying subconscious triggers linked to inflammatory trauma responses in the brain. Brain re-patterning exercises, such as found in a brain rewiring program like DNRS, could be best suited in that it encompasses all of the supportive modalities and works on all of them simultaneously and incrementally retrains the brain without triggering further inflammatory responses.

    You have just described me. I’m astonished. I feel understood. I don’t go to the doctor anymore because no one has an answer except ‘ well you know you have arthritis’. They tell me to take more Tylenol. I feel like I’m having terrible pain in my legs and when I get relief it seems like I have terrible pain in another part of my body. I thought maybe fibromyalgia but I don’t know. It’s very confusing. I’ve had 3 1/2 parathyroids removed after 4 years of suffering with kidney stones. I was so surprised to hear you mention sensitivity to scents. I can’t be around any scents. It make me nauseous. Thank you so much. I hope at some point I can be helped. I can’t believe how you are identifying how even if a toothpick touched me it hurts. God bless you.

    Because I am 46 anything you mention to do with pain is associated with pain. I have Essential Thrombocythemia for the past 13 years and I have chronic pain. I wish dr's actually explained things like this like you do in not a patronizing way like you making things up. Thank you for these videos.

    Thank you for this video, it has been by far the most informative video I have come across. I was diagnosed with CS 2yrs ago, (I have a number of the conditions you mentioned) I wasn’t given any information by the specialist and have had to research it myself.

    @@DrAndreaFurlan I do have a suggestion - I have Sacroiliitis and am currently waiting to have the sacral & iliac fused together, I’ve unfortunately had to cancel twice due to covid here in Australia, fingers crossed for Feb/March . All of this started when I was 32weeks with my first child (I have 2 miracle IVF boys) he will be 9 in two weeks. I had never had much pain before that except for two dislocated shoulders from teen sport that like to pop in and out.

    Dr., I thought I posted a lengthy comments to this video on my tablet. I don't see them here now. I can only Say: BINGO, so right on ... I suffer all of these symptoms. My symptoms gravitated to this CSS in the last 10 years or more. I suffer from a number Spinal Issues , too numerous to list here. I have no Reflexes, Chronic Pain in many areas / Chronic Anxiety with anxiety attacks for many years. I've been seeing a Pain Managament doctor for 11 years. Lumbar and cervical Moderate stenosis . DDD, Facet Joint Arthritis, Spondylesthesis L5 -S1, Disc Herniations L2. L3 (into the front and the rear !!), C4-C7 , Miltary Neck, Sacroilitis , arthritis in the Knees, Hips, and Fibromyalgia pains / tenderness Everywhere. This is the first Video I have seen with such total explanation. One of my Neurologists , 53 years in practice, said I have Peripheral Nerve Damage form the multiple levels of Spinal Compression. He recommended surgery, Consultation with 5 different Surgeons all said No Lumbar Surgery , maybe Cervical surgery if the pain becomes unbearable. Too much arthritis in the spine. I get Calf cramping, stabbing pains, tingling, in the arms and feet, on and off Saddle anethesia, and other "Strange Sensations". The list includes much of what you depicted in the beginning of the video. Interesting point, in 2011 , the 1st Cervical MRI I had ever conducted, the radiologist comments said that it appeared that I have a More Narrow than Normal Spinal canal.. I now wonder if that birth malady has contributed to the many ailments I face now. I am a 70 year old male, and a s an adult I was always over 200 lbs - to a max of 222 lbs (currently), but I always thought at 5 foot 11 inches in height I had a small neck : 15 to 16 inch for my size.... CSS is Hell on Earth. Affecting your Physicality and your Psyche at the same Time.

    Bóg tak cudownie stworzył człowieka i tylko uczyć się, ćwiczyć mózg aby mieć sukces na miarę swoich możliwości. Ból jest sygnałem i idę do lekarza tylko lekarze nie chcą albo nie mają wiedzy aby pomagać. Tabletka pomoże i tak pomagają aby nie bolało a następnie mówią ja już tu nic nie pomogę. Bóg dał internet i dziękuję Pani Doktor za wiedzę, którą Pani z nami się dzieli to dzięki temu wiem, gdzie sobie zaszkodziłam i jak mogę sobie pomóc. Mózg nie lubi stagnacji i to jest ARCYDZIEŁO. Podziwiałam małe dzieci jak bez słów uczą Matki ale Matki nic nie rozumieją ...(oczywiście te które się nie uczą). Niech Bóg obdarza Panią Błogosławieństwem. Dziękuję

    I am so grateful for your videos. The thrust towards balance is so useful to me -- movement with rest, mindfulness with activity, take your time. I started the exercises for stenosis+ and, though I can't even get to the "easy" of 8 repetitions yet, I notice a difference just adding in once a week for starters. Have started the chair cycle -- that one is a keeper. Swimming again now that pools have reopened in the US. Do you have a video for back pain exercises in the water?
