nicothevet

Hello, my name is Nico Maritz. I have been a vet for 30 years and I love helping people care for their dogs and cats. This channel has been created to give dog and cat lovers a place to find more information about the medical and psychological problems that may affect cats and dogs in the course of their lives.

I aim to provide user-friendly bite-sized chunks of information in easily digestible, thorough discussions about any and all conditions that I am asked about on this channel. Over time, hopefully, we will amass a sort of reference library as we accumulate more and more questions and answers.

This channel is not intended to offer a one-on-one veterinary consultation service. Rather, it is an opportunity for me to explain the condition your pet has been diagnosed with in a way that will hopefully be clear and informative so that you are able to better understand the condition and the pros and cons of all the various treatment options

    Dr. Thank you. My 11 yr old Bichon,Maltese, poodle and ? Was born into a horrible backyard situation. I adopted him at 8 wks. I am a RN who leans toward holistic but always has him cared by vets. I always educate myself before immediately treating. It’s been approximately 18mo he was losing hair on the tail tip. The skin eventually became ugly, like a rats tail. Since we have been on the diagnosis trail. It’s been a trek. $$$ . We did it all starting with diet. I have always provided a diverse diet of the best researched kibble and canned food incorporating, broccoli, kale, lettuce, carrots, potatoes, ancient grains, sardines, proteins of cooked plain chicken,beef, fish and eggs. I also supplement with probiotics and healthy fat oils. Then allergy testings, numerous blood and chem labs. All perfect. Two weeks ago, I had an X-ray which showed an enlarged liver. He developed the thirst, urination and seemed to be famished about 6 months ago. The skin along his back is extremely flaky with with ketosis bumps. But he has no signs of itch or discomfort. Yesterday, we did the low dose prednisone test and he is positive for Cushing. Pituitary. I am going to start the Trilostane. What recommendations do you have for holistic herbs to support this treatment?

    My cat is is 4 year old in 4 days and he has been doing these wheezing attacks for 1 year now we went to two different vets and they always tell me that he probably has and inflammation and they always give me a anti inflammatory medication but both of the times it didn’t work. Idk what to do I’m worried that one day it’ll be fatal

    Hi Samia Thanks for getting in touch. I am sorry to hear about your cat's health problems. I think the best advice in this case is to ask your vet to refer you to a specialist who can give you a precise diagnosis and then find the best treatment for that diagnosis for your cat. Hope the helps Best wishes Nico

    VERY helpful information, thank you sir. I've been treating a young outdoor kitty for what I'm now certain is THIS milliary dermatitis. I thought it 'might' be mange when he first showed up - he was a stray but became friendly rather quick, so I was able to washcloth bathe him with a "borax/3% h2O2/water" mix, which I did repeatedly, 2-3 times weekly - he went from his entire body covered in raw and patchy areas to SIGNIFICANT improvement. Raw skin healed up nicely, fur grew back, appetite picked up and he became playful with the other outdoor kitties in his new pack family. BUT, the little bumps keep coming back and he scratches. Took him to vet, she told me "flea allergy" - put him on antibiotics & gave me a foamy shampoo that does nothing. I gave him a topical flea treatment and will continue with the baths to manage it as that seems to give the best result and relief for him. After bathing, I have a mixture of coconut oil (liquid form) with a dropper of licorice root essential oil (anti-inflammatory & antimicrobial) - I massage a little onto any areas of his skin that might need soothing. So far, this bath and skin treatment seem to be the only thing working, but I see this will be a lifetime maintenance routine for him. At least he doesn't try as hard to escape his baths now like he did in the beginning, I have far less scratches from him trying to climb me. 😂

  • @spearageddon3279
    @spearageddon327920 күн бұрын

    PS - regarding the flea allergy diagnosis, I have flea combed him during every bath and he surprisingly does NOT have fleas. Of course being outdoors he can get one or two, but he's never had an issue of concern with fleas. He clearly just has allergies, or very sensitive skin.

    Thanks for the very informative video. I noticed that my 8yo girl whippet had dropped a step or two, stopped zooming, was dragging behind on walks and sometimes tended to lethargy. I also noticed she was drinking and peeing a lot, was panting at night and was more hungry than usual. The vet ran a bunch of tests including the LDDS test and feels confident with a clinical diagnosis of Cushings (pituitary version). With the budget smashed I still haven’t committed to the ACTH stim test based medical treatment program. BUT, she is now on a low fat Hills digestive diet, with low purines treats, plus a multi vitamin supplement with high vitamin E, a probiotic and a tea spoon of omega 3 6 9 oil in her food, and her symptoms have reduced, in some cases significantly. The biggest turnaround was that she now has a spring back in her step, is keeping up with my other whippet again during leashed walks, and can get to about 3/4 of her quick trot when off leash zooming. She rarely pants at night now and the excessive drinking / peeing has reduced a little as well, but she is still very hungry. She scavenges food any where she can. For now, this will have to do. But I will in the months ahead go with the medical treatment program (I want her to have the best quality life she can, and also hopefully lose that whoppet of a belly), just need to adjust the budget. The costs are not trivial!

    We’re in the US and have been feeding our 17yo kidney cat a natural kidney prescription food by Darwin’s pet food brand. The kidney formula is called Intelligent Design for cats. It’s a raw meat food but our cat prefers it lightly steamed with a bit of filtered water added. We also give him a few drops of a pure fish oil from Nordic Naturals pet line. And we give a few drops of a kidney support called Pet Wellbeing kidney Gold formula. He’s been on this food regimen for the past two years and is still doing okay. His skin tent tests are still looking as not dehydrated so we haven’t started any sub q fluids yet but we keep an eye on that. Thank you for this informative video! Sorry to all of the cats and pet caregivers suffering this awful diagnosis. Pets are truly the best part of life :)

    How long before i should she a change in my dogs symptoms after she has started treatment on Vetoryl? Also my vet is requesting that i do the first atch test at 3 weeks after she began treatment instead of the 10 to 14 day suggested time as per the Dachra website instructions. Do you see any problems with what she is adviding? Will the results have the same findings if its done at 2 weeks vs. 3 weeks? Lastly,other then seeing no reduction in her symptoms during the past 2.5 weeks,I have noticed an increase in a caughing issue which she struggled with many years ago which is called "a reverse sneeze". Prior to the Vetoryl she was not clearing her throat and making the repetative deep caughing noise which she has now begun doing on a daily basis since beginning the medication. I was wondering if there is any connection between cushing or Vetoryl itself and an increase in this treachia related breathing condition that you know of. Thx very much.

  • @nicothevet7851
    @nicothevet7851Ай бұрын

    Hi BabyBear Thanks for getting in touch. Usually we would expect to see a benefit from the Vetoryl treatment within 4 weeks. Regarding the sneezing/coughing/snorting - one possible explanation is that the high cortisol levels caused by Cushings Disease may have actually been producing enough cortisol to treat and allergic or inflammatory respiratory condition like chronic bronchitis and now that you are treating the Cushings there is less cortisol in her body and therefore the chronic bronchitis is no longer under control. This is just one possibility - have a chat with your vet about it Hope this helps Nico

  • @babybear4524
    @babybear4524Ай бұрын

    THANK YOU. Thats an interesting theory. It seems that her symptoms have actually gotten worse since taking the 10mg dose of Vetoryl the past 4 weeks. She is hungier and thirstier then before and urinates more frequently. Well its hard to say for sure but definitely no improvement. The panting is noticably increased thats for sure and her level of fatigue and overall lack of interest in anything other then food has not changed. Would this mean that the dosage isn't high enough to bring down the cortisol level in the blood? I am concerned that not following the exact Dechra Protocol for testing guidelines which my vet doesnt seem to care about could produre inaccurate results. Because I am administering her medication at 6pm due to her lack of appetite earlier in the day,my vet told me to bring her in for the ATCH test at 8am which would be 14 hrs past her last dose. The guidelines state that the atch test should be given 4 to 6 hrs after medication and I could find no online data to support that it is accepttable to deviate from these specific Vetoryl guidelines. Therefore I wanted to know if its acceptable to do the test at 14 hrs after the last dose or if you believe that this variation of the suggested protocol, could produce invalid results in terms of the measurements which the atch test aims to determine. I would gate to think this critical measurement of blood cortisol level would be inacurate and that the dosage of Vetoryl may be adjuested based on flawed results. I would appreciate your opinion on this subject as it relates to this particular test or any other variation in testing suggested by a vet which differs from that which the manufacturer has precisely outlined in the package insert. Thank you so much.@nicothevet7851

    @@babybear4524 Hi Baby Bear I always stick to the recommended protocol so have no experience in deviating from it. Perhaps chat to the manufacturer about implications of deviating from the protocol. Regarding response to treatment - if she is not responding to the treatment and it is being given at the correct dose then sometimes splitting the daily dose and giving half in the morning and half in the evening works better. It may also be worth considering asking your vet to refer you to a specialist to review her. Hope this helps Nico

  • @babybear4524
    @babybear4524Ай бұрын

    @nicothevet7851 Well I decided to switch Vets today and am glad I did because this place has its own online pharmacy and this Doctor advocates the use of Trilostane as an option other then the Vetoryl. The other vet I saw was opposed to Trilostane. So my current vet recommended I increase her to 12mg of Trilostane daily for a mth or 2 depending on how she is doing. He was nice enough not to require I do the Atch test yet which helps me financially and still allows me to gradually increase dosage by using the compounded 12 mg which isn't available by using Vetoryl. Have you found there to be any difference in the efficacy of Vetoryl vs. Trilostane? Does the medication have to be given with food such as a meal containing fat or is it OK to give with a spoon of peanut butter since my dog doesn't want to eat much until later afternoon and I would like her to take the meds earlier in the day so that when we do the atch test in 1 or 2 months she will be able to get it done 3 hrs after medication instead of having to deal with the current issue of her taking it at night and getting the test 14 hrs after. I knew that would be a problem and glad I have a new vet who confirmed with me that if the meds are not given 3 to 4 hrs before the test it could in fact yield false results. It's so important for pet owners to research and be thier own advocates for thier pet because had I not read the protocol and spoken to other owners online I never would have been aware that my vet was deviating from those guidelines. Actually when I tried to order the 12mg dose just now it's backordered and so I may have to split it into a 5mg and a 7 mg Trilostane pill. Do you think it matters which one I give at what time or maybe it's best to give both pills at once so when it's time for the atch test I will know exactly when she took it? Thx

    I used the spot local insecticide and the next day he grew his hair that were so shiny. Now after 4 weeks I can see the patches again. I am washing the house regularly every two days. He wasnt scratching or licking for four weeks. I wonder if there's an option for outdoor shoe immersion when you get home such as when you go to swimming pools. That would likely help no fleas.

    Absolutely brilliant video. You went over everything I wanted to know and it was very reassuring. My cat, we think has mild asthma. He is 14 years old and we spoke to the vet about it about 6 months ago who has suggested to just watch him for now and see if it gets worse then possibly introduce an inhaler. I'd just like to add that treatments are expensive. We talked about having a chest xray at a cost of £600 which is really a lot of money for us. The inhaler will cost £200 and we will find the money for that. Thank you very much for sharing your video. I'm pretty sure my cat has asthma now and my vet is also quite confident of this. We were able to take a video of George (my cat) whilst he was having a coughing attack and show it to him. Although George is 14 he is very active. He goes out several times a day and is keen on a long walk in the evenings. We know because he has a tracker and its fun to see how far he goes and he loves to play with us indoors.

His past kidney tests have been normal until 4 months ago when tests done for a totally unrelated issue revealed early kidney failure with an elevated SDMA, dilute urine and elevated phosphorus. Theoretically , while actively ill with the FHV , the reaction to the virus may aggravate the asthma but it most cases in my experience this does not seem to happen Hope this helps Nico

    Thank you doctor for this information. I was encouraged by the way you said that people who choose not to spend the money or dont have it to spend or choose not to pursue years of testings and pills and all that goes with it including the commitment which is needed to take the path of medically supervised vetranarian treatment,shouldnt feel guilty. That if we choose to allow our dog to live with the disease its not a terrible option because as you know so much of this involves how we as owners feel about ourselves and the guilt we live with should are dog die sooner then we believe it could have lived if we had chosen medical intervention. Its about them,the dog or cat but so too its about us and our own emotionals. I am trying to come to terms with and grasp the very sudden and extreme changes in my dog and figure out which of these paths to take. Money is a big issue for me and although not diagnosed by a vet,I am 100 percent certain this is what she has. Another you tube vet gave a presentation similiar to yours in every respect except he spoke of very specific natural supplements which do show improvement in the dogs symptoms when taken. So i was hoping that you would also discuss this topic but you didnt. Then i saw a comment of yours in which you said that natural suppliments dont work. I was surprised you said this and it confused me. Now i dont know what to think or believe. If I can not help her with these alternative treatments I will feel terrible and if i cant afford yrs of vet treatment which may lead to having to stop the trilostane anyway,then what can i do? I need a way to control the urgency to urinate as she wakes me up every night. I can clean up the pee and poop on doggie pads but i need sleep to take care of her. She put on 4 lbs in a month or 2 and idk how much to allow her to eat because she is never full unless she eats too much. She is insatiable with water.Last night i was so scared as she was panting and i wanted to help her but felt helpless. She sleeps alot and her personality went from wanting to be held to hating to be held,from relaxed to aggitated,she also became deaf this past 6 mths which is odd. She is my service dog and I cant take her with me now due to her condition. She doesnt bark at all and i have tovwatch her every minute as she has to watch me since this is our only form of communication now. I love her so much i will do anything but wasting 1000s of dollars to be told what i already can see is obvious seems stupid. Sure i wish i could try the meds but i am low income and have many health conditions to worry about and pay for like cateract surgery. I just want to stay healthy enough to care for her. There must be some suggestions you have on how to improve there condition. I watched a video on how to lower cortisol levels in humans and wondered if the same principles apply to dogs. He spoke about sleep and vitamin D3 and sunlight as being key factors. Many suppliments gor cushings say that melatonin is effective as a suppliment. There are do many cortisol related suppliments on Amazon and reviews of those with cushings disease dogs who claim these things work. Do they work or do these suppliments just give false hope to those of us searching for a magic cure? Thanks

    Our kitty had his first major dermatitis flair up last year and we finally got it under control with Prednisolone. Kitty was fine for a few months and is now having a breakout. Our vet talked us into ordering Atopica but the price tag isn’t manageable for us. I’m going to reach out to another vet and see if they would be willing to treat kitty’s flair ups with steroids as our current vet is opposed to that form of treatment. Thank you for the great advice and insight!

  • @nicothevet7851
    @nicothevet78513 ай бұрын

    Hi Suzy Thanks for getting in touch. Yes , the cost of Atopica is pretty steep and unsustainable for Ost of use. I use steroids routinely and frequently and without significant problems. The most common problem is that cats may get fatin which case we give them a break from treatment. Using steroids according to the rules of lowest effective dose and shortest effective courses is associated with allow risk of significant problems and restores happiness and quality of life to cats suffering from military dermatitis. Hope this helps Nico

    I’ve heard that the cortisone can affect their kidney and organs? That’s the reason I believed it can’t work long term, please can you advise as my vet wants to do a lot of organ tests before prescribing cortisone.

  • @nicothevet7851
    @nicothevet78514 ай бұрын

    Hi Tracey I cannot comment on specific cases but in my experience there is no significant risk of long term damage when using cortisone/steroids according to the rules for using these drugs ie use the shortest courses possible and lowest effective dose to meet the needs of the patient. If an individual must be on very long courses then taper the dose to the minimum effective dose , ideally tapering down to alternate day ( every other day) low dose dosing. Once a cat or dog or human being has been of cortisone some of their liver enzymes( liver blood tests) may be increased by the cortisone - this is almost never significant but knowing that these levels where normal before starting the treatment my make us more confident ascribing the blood level increases to the cortisone. A similar thing happens to some people( like me) on long term statins for high cholesterol and as long as the elevations are within the predictable range , your doctor will ignore them Hope this helps Nico

    My sweet tabby has been diagnosed with flea dermatitis but she has not had fleas for over 5 years !! I am getting so frustrated not knowing how to treat her. My other cat has no issues at all 😫 can’t work it out

  • @nicothevet7851
    @nicothevet78514 ай бұрын

    Hi Christie I am sorry to hear how frustrated you and your cat are. I think the confusion stems from terminology - many vets call the condition " flea allergy" when in fact we should call it "skin allergy or miliary dermatitis". By naming it correctly it becomes clear that, although fleas may be contributing to the problem and so it's imperative we get rid of them, the problem is that the cat is likely allergic to several triggers and most of them are things in the air ad environment that we cannot get rid of so we have to treat the allergy with cortisone or ciclosporin. Perhaps watch the video to see a longer explanation. Hope this helps Best wishes Nico

  • @nicothevet7851
    @nicothevet78514 ай бұрын

    Hi Travis Thanks for getting in touch - it's possible that the vaccine triggered the allergy but more likely something else triggered the allergy. ask the vet to treat the allergy and if it goes away and only recurs when she has her next vaccine then that would suggest the vaccine as the culprit but if it goes away and comes back at some point in the future unrelated to recent vaccination then it is likely fleas and/or environmental allergens that trigger the allergy Hope this helps Best wishes Nico

    If your cat is doing this just take them to the vet. It may not be asthma. My cat was doing this EXACT same thing. Only to take them to the vet and find out it wasn't asthma. Was given AntiBiotics called, "Azithromycin for oral suspension USP 200mg per 5ml"... After a few doses my cat stopped doing this. Hope this helps some of yall...

    Excellent summary. I'm on my second senior cat with CKD and on SQ for 7mos now twice daily. I balance the nausea and appetite stimulants with the meds recommended here. She won't eat k/d, but phosphate numbers are still good. I'll try phosphate binders again to see if I can't back down on the SQs. Realistically, it's about quality of life now and keeping her eating what doesn't flare up the IBD. Her numbers put her in the high 2's but the fact she cannot live without at least 70+70ml SQ per day puts her higher. It's a balancing act. I've been at this for several years now, consider myself somewhat experienced... I recommend to everyone, Tanya's cat CKD website/book and SQ's when needed and until the end - use a digital scale that weighs in grams to hang your bag from to be consistent. My last cat was 4.7LBs and needed 60ml every 8hrs, hadn't drank water for over 6mos... at least this cat is still drinking and I'm not having to struggle with sleep and SQ every 8hrs. Pets... we love em and will do everything for them... xo

    We switched our 13 year old cat to Merrick grain free wet food Rabbit or Duck recipe and she has made alot of improvement. She prefers the rabbit over anything. Rabbit is a novel protein and easily digestible for kitties with IBD or Kidney disease. It's a bit expensive but worth it and no food is going to waste. I was feeding Fancy Feast for years until she got really sick in November.

  • @nicothevet7851
    @nicothevet78515 ай бұрын

    Hi Justerre Thanks for getting inn touch . Yes , if fleas are the cause of the allergy then getting rid of the fleas will resolve the allergy but for most cats this is not the case and in these individuals the allergy is triggered by environmental factors that we have little control over so are left with having to treat with medications like steroids and cyclosporin and dietary trials. Dietary manipulation sometimes give some improvement but very rarely means that no medications are required. I feel your pain , I would prefer to not use medications and I think to "cure" allergic individuals will come one day in the form of stem cell gene splicing therapy to correct fault DNA sequence causing allergies but that technology is somewhere in the future. I am a severe allergy sufferer myself so also wish modern medicine would come up with better treatment and management protocols. Best wishes Nico

    Hi Christal Thanks for getting I touch. Yes, I am not a fan of just using a cone because this stops him damaging his skin but he is still living with the itch ad irritation. Discuss using prednisolone, depomendrone and/or cyclosporin with your vets - the dose and frequency of use is tailored to each individual. Also think about changing food to RCW feline anallergenic diet of hills z/d diet - trialling these foods is much much less important that giving the medication but if you can I would suggest doing both. Hope this helps Best wishes Nico

    My cat is 15 and has had severe allergies since he turned 5. He used to get a depo injection ever 4-5 weeks but the vet scared me with the possible diabetes side effect so I stopped taking him. I limit his diet to very high quality wet food and of course let him get a few munches of his limited ingredient turkey kibble (because he asks for it daily). Wish I could get him off kibble for good, because the more he eats it the more he itches and rips out his fur. I’ve had him allergy tested numerous times and he is allergic to everything in cat food including the most popular proteins. I tried making him food at home and it made him itchy. I wash his blankets and beds and my floors with non toxic products. I give him the best filtered water… this allergy nightmare has had me troubleshooting a solution for 10 years and I still have had no luck. I don’t mean any disrespect because I thought your video was very informative and helpful to those starting out on this tough journey. However, what is the root cause of an overactive immune system? If cats are like us it appears that we need to dig deeper and look at the gut and treat the root cause. It’s maddening to go to the vet constantly to get a prescription filled or go for shots. I give up. 😢

    Hi Talin Thanks for getting in touch - yes I agree agree, modern medicine is still rubbish at treating any sort of allergy or immune system problem. I am very allergic myself and spent some time in a coma in 2019 with autoimmune encephalitis. We have medications that can address the symptoms but I agree that we need to find the root cause and correct this . I suspect the root cause and the "cure " for allergies will come in my lifetime. It is likely to take the form of stem cell gene splicing therapy to correct the faulty DNA coding in allergic individuals. Until then all we can do is what you have been doing - focus on gut heart , find a best suited diet and treat with steroids and cyclosporin as required. Regarding diet - my choice for gut health and skin allergies is RCW hypoallergenic or hills z/d diet. Hope this helps Best wishes Nico

    I have 5 cats and this year did get fleas. Evidently they were up nearly 50%. My one year old M.Coon was overgrooming and the locum vet said flea allergy and proceeded to give steroid shot, antibiotics plus sent away with Bravecto costing £44.50. I did not want this but was just wanting to get her home. I do not want this B. Flea stuff, now stuck with it. Very annoying and as many people are saying now vets a, overcharge with out asking and b, throw the first thing at you with much consultation. At least that one did.

    My cat was diagnosed with asthma a month or so ago. She is long haired and likes to groom herself a lot, and from that she would always vomit hairballs. I was always worried if that would damage her pipes somehow. I don't know if that is related or not, but she has a full blown asthma now. She was given two steroids injections two weeks apart to start with. Then she was given steroid pills to be reduced slowly. She also has inhalers, Albuterol HFA 90mcg. I have to use the inhalers at least three times a day to control her wheezing and really heavy breathing. I wanted to ask her vet to give her the inhaler steroids, since those seem to be more effective option with less side effects I hope??? What would you recommend? Are inhaler steroids a better option than the pills? Honestly, I don't think the pills are doing much for her. They might work for couple of hours, but the heavy breathing comes back again. Thank you sir!
