    I just have a doubt. Which is more common in ALS “ pseudo bulbar” or “bulbar” palsy? I saw a patient recently with tongue fasciculations and unable to swallow and assumed bulbar is more common in ALS.

    Bulbar is more common in motor neuron diseases. Fasciculation with bulbar palsy in your patient goes in favour of MND.

    Rickets is a disease of growing bones . So, it is seen in children. In adults, osteomalacia/ osteoporosis is seen ( similar to rickets but clinical features vary).

    My baby boy is now around 22 months old. When he started walking I feel his legs are bow. But Unfortunately I have no idea of this discease. In feb 2024 a doctor suggested me lab test of alkine phosphate and some other. After their result I Found that he is suffering from rickets. Well. I started his medication but giving him d max drops calcium p and D×3 injections. After the treatment of 2 and half month when I revisit doctor I found his vitamin D level is at 120. But there is no change on his legs shape. The medication did nothing.... Doctor asked me to stop giving vitamin D immediately. But after 3 to 4 months. My baby is at same condition. His legs are bending outside under knee to foot joint. Please help me what I can do for my child....

    You should consult your doctor and get follow up X-rays to know if bone mineralisation is happening or not. If yes, then you shouldn’t worry for the shape as it takes time to completely recover. Otherwise get consultation from an orthopedist.

    Jolt? I've had Adenosine and there was no jolt. They administer it and everyone holds their breath waiting for the reset or having to hit you with the paddles. There was a moment where I could feel death and then my heart reset.

    This is a case of portal hypertension. In IVC obstruction, all the collateral veins ( above and below umbilicus ) fill up in upward direction . In this video, veins above umbilicus were fulfilling this criteria only.

    Thanks for pointing it out. You can say either Extension ( with respect to plantar surface ) or Dorsi-Flexion of the great toe.

    There are multiple causes for this disease. You need to consult your nearby doctor ( preferably an orthopaedic doctor) . This will help you .

    But the second (assumed dead) patient's doll eye reflex is positive. Observe the Right eye.

    Right eye appears to move a bit. But it isn’t significant. So, reflex is negative . ( for positive reflex, both eyes should move in opposite direction to head movement).

    Thank you for the great video. I find that while this examination technique is very easy to perform, interpreting it can be quite difficult. I mainly use it to gather evidence of upper motor neuron signs in the thoracic segment from patients I suspect to have amyotrophic lateral sclerosis. However, it's often confusing to determine whether the loss of SAR is pathological or simply absent in normal individuals.

    Same problem .Can stress and anxiety also cause mild AR ? Bcz my test showed mild AR and my doc said it is normal .and i do have severe anxiety problems

    Your doctor is right. Sometimes test can show minor abnormalities without any clinical symptoms. You don’t have to worry about it.

    @user-eg8rj8bh7z Dancing carotid pulse is an extreme phenomenon of severe AR. So, mild AR won’t cause these symptoms.

    This video is so amazing. I had PSVT for 5 years through puberty and would have tachycardic episodes. My BPM at its highest was 210. I was also given Adenosine in the ER to bring back normal cardiac rhythm. Amazing stuff. Thanks for sharing this video so I could see what my heart rhythm looked like. Cheers.
