Artificial sweeteners suggests a link to a higher risk of stroke, heart disease and death overall. Artificial sweeteners may be likelier to make you get hungry, eat more throughout the day and develop diabetes Now the new study, published this month in the peer-reviewed British Medical Journal, links consumption of artificial sweeteners to a 9 percent higher risk of cardiovascular disease - heart and blood vessel harm that's the leading cause of death in the U.S. and globally.

    Listening to you gives the impression that the elevated levels were due to consumption. Yet consumption was not tracked. Makes me wonder how accurate your interpretation of the other studies you mention are.

    One part of the study did measure elevated blood levels after consumption of 30 g of erythritol - blood levels increased to 1000x times baseline, well above the levels that were correlated with increased risk in the database studies, and post-consumption blood levels remained very high for 2 days.

    @@sophiekarnak3936 I’m willing to bet that consumption of that amount of real sugar would also cause that effect days later. Probably no one consumes that much erythritol at once, too much intestinal distress. A packet or two in coffee, or one sugar free monster drink?

    In Australia, they market products that are basically all erythritol as monk-fruit sweeteners. We're talking 99% erythritol and only 1% monk-fruit extract, yet they're still able to label them as monk-fruit sweeteners.

    @@Ramen4All Yes, I have seen this first-hand. I have pulled multiple products marked monk fruit sweetener off mainstream supermarket shelves they pretty much all contained 1% monk fruit extract. The only ones that didn't contained even less - about 0.3% monk fruit extract.

    I usually take stevia with xylitol in my coffee every morning. The brand of stevia has erythritol in it. Had a horrible though as I drink a soft drink called Nexba sweetened with stevia and Zylitol. Must see if it has eythritol in it. Might just have to just use Zylitol. I live in Brisbane Queensland.

    It the initial study was for unhealthy patients with conditions that already put them at higher risks of strokes, so how do you know if these people who were already at risk for strokes were taking erythritol because they were diabetic, and therefore there is correlation but not causation?

    This is most studies, especially diet/ingredient related. They never control for or can't control for the variables and so all you end up with is weak correlatives.

    The correlational studies did control for factors like diabetes and obesity. Also, erythritol has been proven to increase blood clotting in vitro (in human blood) and in mice, so there is a plausible mechanism how high blood levels of erythritol could cause increased risk for heart attack and stroke.

    What I would like to know about this latest erythritol study, was there an adjustment for overweight/obese and/or diabetes? These factors in themselves increase the likelihood of CVD. This population (overweight or diabetes) is more likely to heavily consume artificial sweeteners.

    @@kimgo4518 Relationship between intake of artificial sweeteners and body mass index in young non-diabetic adults: a cross-sectional study Background: Artificial sweeteners have skyrocketed the market in the last decade and there are unlimited products available today that contain them. They are often projected as one of the easy and effortless method of cutting down calories and have gained importance in the community. They are being used as one of the dietary tools to control or prevent weight gain not only in diabetic adults but also in young healthy adults and children. There is a need to examine any possible contribution of these substances on body weight and BMI of an individual. Methods: A cross sectional study was conducted among randomly selected healthy non-diabetic subjects of both sexes in the age group 18-35 years. Those with history of intake of artificial sweeteners in the last one year (in the form of pellets or diet soda) were considered as study group and equal no of non-users were taken as comparison group for study purpose. A pretested semi structured questionnaire prepared in English language was used to collect information. Variables such as height, weight and body mass index were noted. Data was analyzed using SPSS (version 20.0). Results: There was a statistically significant difference between the two groups in terms of weight and BMI (p=0.003 and 0.002 respectively). The mean weight and BMI was more in those subjects who had consumed more than one serving of diet can per day. Sweeteners in pellet form failed to show any statistical significance association between frequency of use and BMI. Conclusions: The study suggested of a possible association between the use of artificial sweeteners and BMI. Young adults need to be sensitized about various safe weight management techniques and healthy lifestyle, rather than resorting to methods like intake of artificial sweeteners.

    my personal experience, quit table sugar altogether. after few weeks of self control, tongue sensitivity increases and then natural fruits taste more sweet, even simple sugar taste more sweet than before. in fact even small amount of sweetness makes us satiated. we start enjoying the delicate mild flavours.

    no sweeteners and minimal fruit is best, in fact unless on rare occasions stay away from pineapple, appe, grapes and oranges! Strawberries, black berries etc are best. Bananas bad also though need to find good potassium. im willing to give a pass on bananas but still limit them. Oranges apples and grapes are reaaly bad as is pineapple! im not a fan of either grapes or oranges and have also not liked the, other then a flavoring at times in gum. I also don’t like bananas unless its in way unhealthy baked goods or pancakes. i love green apple and i love pineapple thats hard but gotta give it up!

    Dr Hansen, have you actually read the study? It doesn’t link “consumption of artificial sweeteners to a 9 percent higher risk of cardiovascular disease” because the subjects who had cardiovascular disease were not monitored at all for erythritol consumption (not even by diet recall). Others have pointed out that unhealthy bodies produce more erythritol, and that there could be actual reverse causality going on.

    Maltitol SHOCKED me the first time I ever had it in a Whole Foods brownie that I didn't know had sugar alcohol in it. It blew my stomach up like I was 9 months pregnant. I had no idea what was going on. It was horrifying. They took the product off the shelf but returned it several months later. A certain percentage of people do not process sugar alcohols well. Clearly I am one of them.

    I have ulcerative colitis and Maltitol is a laxative for me. Last summer, I ate one small bag of sugar-free gummy bears with Maltitol and I spent the next 8 hours running to the toilet.

    It’s not Stevia that’s the culprit. It’s the erythritol that is used to bulk up natural sweeteners. Stevia is completely safe and so is Monkfruit. Very little stevia is added to natural sweeteners. Most of it is erythritol as it looks very much like sugar. I have been taking stevia mixed with erythritol for the past ten years and I have not died yet and I have not had any adverse health effects yet. I may test my blood to see if my clotting factor is high. They are going to do more research on erythritol. I am looking forward to it. Meanwhile, I am going to still consume stevia+erythritol in moderation.

    @@TomGrubbe I just bought monk fruit drops and it’s not sweet at all. It has been a rip off. The stevia liquid I bought from iherb is good and it’s very sweet. I feel like sending the monk fruit drops back to the manufacturer. It’s a rip off. Stevia is nearly 400 to 500 times sweeter than sugar and it really keeps me going.

    Your body makes erythritol. If you already have heart disease, obesity, hyper insulinemia, you'll have high erythritol anyway. The study followed people with previous heart attacks.

    They've been trying to demonize Aspertame for 40 years, with little luck. The National Diabetes Association says that it doesn't increase insulin levels - which is to be expected since there's so little of it. Plus it's not a sugar molecule biochemically, so it wouldn't be expected to generate a response. If there are slight weight changes, it's probably due to other factors - not easily characterized. Since you'll probably die of something else, I wouldn't lose much sleep over it.

    I can't deal with Aspertame because it is a single molecule amino acid and it messes with my brain if I use it. It might be perfectly fine for others but it doesn't agree with my particular brain chemistry.

    My Father drank coffee with Equal even though I warned him not to. Found out he had dementia when he died. I think a drug company makes Aspartame. Read a book about it by a woman that said the sugar made her thyroid go crazy and her doctor wanted to remove it. She also had other bad reactions. Wish I could recall the name of her book, When the writer stopped all the artificial sugar her health came back to normal.

    This is discouraging. I'm a 59 year old runner. I drink a lot of water. But I also drink diet soda. Water gets old - especially when I drink it before, during and after my runs of an hour or longer. I've known runners who had cardiac events during workouts. Now, I'm wondering if it's the sweeteners.

    Date sugar is my sweetener of choice. Organic date sugar. It's made from powdered dried dates, and 2 tsp.(6 g) has 1 g of fiber per 4 g of total sugars. It is probably one of the most minimally processed sweeteners you can find. Perfect for a whole food plant based diet. OR any diet really, unless allergic to dates. Some people say they don't find date sugar quite sweet enough, but luckily I don't have much of a sweet tooth. so I don't find that to be the case. It has worked well for all my baking needs so far. It is a little pricey compared to some other options. But date farmers don't have the mega-million federal subsidies.

    In the 1990's carbonation in carbonated water of soft drinks began to come from ADM rather than traditional sources. It was cheaper as by-product of alcohol fuel production. This source of carbonation has tremendous Carbon Monoxide contamination. The normal gut bacteria Pepto Streptococcus will quadruple in as little as 3 hours when exposed to CO. The exponential growth uses-up all the bodies protein groceries before it wanes. As amino acids and fatty acids are circulated to the lymph nodes from the gut, fats and carbs in the gut are then stored close to where they are absorbed. Beer belly is close to the same thing only worse. All CO2 from fermentation has some CO. Older CO2 sources had none.

    Thank you for this. I was in the habit of drinking several lemon perfect waters daily (8g erythritol/beverage) and as a person who has the Prothrombin G20210A mutation, your information here just gave me a major wake up call. Thank you. I’m in the danger zone years (50+) to boot so I just canceled my lemon perfect subscription and will only enjoy one occasionally rather than sucking down 3-5/day. Yikes.

    Grew my own stevia plants. Nothing special needed, just a stake and medium pot as plant grew bushy. Died like any herb, by hanging. Put leaves in baggies. Float or crunble dry leaves into your drink. Or use a strainer. Did stain water slightly green.

    From my experience of drinking lots of Monster (I'm in med school, it's brutal lol) and from class, I can tell you that Erythritol kept my hunger down while Niacin did wonder by breaking down all the fats (through activation of Hormone sensitive lipase). I lost 40 lbs in 6 months, and I didn't change anything beside Monster and strict restrictions on added sugar (I drink, eat all the fried and fast foods, cause we med students barely have time to breath, let alone cook). It works wonder

    Hahaha I feel you man. The caffeine and forgetting to even eat might be contributing a tad too. I now work Cardiology at night in a big teaching hospital and I drink a lot of Alani Nu (of course I’m a PA, so bless you guys your hours are likely MUCH worse than mine ever were and for longer, hang in there!).

    When you hit your 40s this wont work. Everything changes in your 40s. Once youre through this rough period of your life w med school, develop healthy habits so you wont die by 50. Im 47 and have learned the hard way (watching friends).

    @@itsme_shanti true, it is an appetite suppressant, definitely contribute to the decrease in hunger and calorie deficit. I'm not sure if the effect with niacin is additive or compounded, but it definitely worth looking into...after I finish Step 1. 200k on my head is also unhealthy too, I gotta pick between the lesser of 2 evils 🥲

    I put a wee amount of sweet n low in my ice coffee occasionally. My 67y woman friend has a bad UTI but she eats and cooks with lots of sugar. The sugar makes the UTI worse.

    Glycine - the amino acid is a safe sweetener. It's actually healthier to supplement with extra glycine. Large amounts taken over a life time will extend lifespan - if you're a mouse(probably works on humans too). I found a Cola extract for sale that has no sweetener included. I'm planning on trying it with glycine and carbonated water.

    You said something interesting. If erythritol occurs naturally in fruits like watermelon, grapes and peaches, do we know how much their diet consisted of these variables, in addition to giving them derived erythritol? Or better yet, how much erythritol values are in people who eat fruit alone or in a SAD diet? 🤔

    Very informative video. I hope to see one about allulose one day. My own experience so far is that it hasn’t caused much digestive discomfort, gas, or bloating etc., if any (though studies show it too can cause this if the consumption is high enough). It also tastes MUCH better than other “non caloric “ sweeteners and easily dissolves. BUT . . . It often happens that there is no free lunch, right?

    No, it’s not the monkfruit. You may have misunderstood the article. It’s the erythritol that they were talking about. Erythritol is mixed with monkfruit and stevia to bulk up the product and give it a sugar like consistency. That’s why monkfruit was mentioned. These natural sweeteners have very little stevia and monkfruit in them. Erythritol is the main ingredient. Monkfruit is absolutely safe.

    Guys this study was done with 30g of erythritol. Most americans do not consume this much unless you're drinking energy drinks. You should be fine if you add a packet or two to your coffee. This substance is not evil.

    In a small pint of ice cream you will find around 30g of erythritol. I'd say most Americans now are consuming this much if they drink energy drinks, eat keto-style ice cream or baked goods, sweeten their coffee with sugar substitutes containing erythritol, and so on. I wouldn't say the substance is evil, but I would say the substance is potentially lethal and harmful to our health at low doses

    @@kennixox262 I use 90-100 gram for small basque cheesecake. That would be 9-12 portions, max 10g per portion. 1/3 of what they checked. I haven't seen any problems for last 3 years.

    30 g of erythritol increased blood levels to more than 1000x baseline, which is vastly higher than the amount that was correlated with increased risk. Maybe 10 g will only increase it 300x from baseline, who knows, but that may still be over the level that increases risk.

    Thanks good review! Foot note to the whole fruit. We seem to be able now to eat any fruit year arround and that is a problem! Fruit was in the 'old' days a dessert eating it after your meal which by chance triggers then less insuline. Also fruits were eaten in season and they were real, not like today these blown up larger sugar bomb apples you can buy in the supermarket, the smaller the better the older the variety the better. I got gout attacks which later I discovered to come from eating too many of my own grown grapes and peaches. Eating fruit should be managed, not necessarily every day not those with high sugar content and preferably complementary after dinner or lunch. Fruit does trigger insuline!

    Great video ! Please follow up on Stevia, I drink loads of Zevia soda !! I chew 3 table spoons of flaxseed a day with a pinch of salt to make it taste good for gut health and regularity ( It has soluble, insoluble fiber and vitamins )

    I bought the now brand Monk fruit sweetener based upon smoothie recommendation list and it has 11% cane alcohol definitely doesn’t have anything that says erythrital in it - there’s so much mislabeling out there! is this a completely different product or just a convincing label change?

    It’s erythritol. I have been consuming stevia mixed with erythritol in my coffee for the last ten years and I have not had any adverse heath effects. I guess I should do a blood test to find out if my clotting factor is high. They are going to do more studies on erythritol and I waiting for that to happen. Meanwhile, I am going to continue taking it in moderation.

    The NOW brand monk fruit liquid does not have any erythritol, or any other sugar alcohol in it. The "cane alcohol" in it is literally alcohol, and it's just in there as a preservative to prevent bacterial growth. It's not a "sugar alcohol" any more than rum is!

    oh man, maybe this is why my body starts to reject anything baked with erythritol. I tolerate it really well at first however I've noticed after a few days my body starts to reject it, eating a single bite of erythritol-sweetened cakes make me instantly ill despite no gastro-intestinal upset.

    Then why didn't he tell you that this study didn't look at what people were eating ? they have no idea if they ate erythritol or if their body's produced it and its known that the more diabetic you are the more erythritol you produce, so was the erythritol the cause or just another symptom of sickness that was at the crime scene ? and the "study" with 8 people well thats no study at all how can you have a sample size of 8 and make any conclusions?

    Is there any research to suggest humans shouldn’t mix fats and sugars? It doesn’t occur naturally in nature. It seems like humans evolved a bad uricase enzyme to turn fructose into fat, but also evolved the ability to live on high fat diets too. Seems weird that we evolved to eat both but both make us sick. Maybe it’s just one or the other

    I drink a protein shake daily that has 5g per serving of erythritol, and nearly every product I see for protein contains it or stevia. How should I read the 30g per day criteria mentioned for this study? Is my 5g a day putting me at high risk? What is a protein option that doesn't contain sweeteners i.e. is safest?

    Regarding the econometric trial you mention that proved sugar causes diabetes: 1 can of soda daily equated to a 1.1% risk of diabetes. Sugar was any sugar, including high fructose, which is the usual sugar in processed foods and drinks. Large amounts of fructose get converted into fats by the liver, and those fats cause insulin resistance, not sugar. Anyhow, you’d have to eat half a kilo of fructose a day to increase your diabetes risk by 10%. Whether even the 1% number is correct is another question. High fructose intake could correlate with any other number of bad habits that were not available in the country level data. GDP increase of 1% also caused a 1% increase in diabetes risk.     Try putting a bit of lemon juice in the water. It makes it so refreshing and lemons are good for you or put a tablespoon of BRAGGS to a glass of water. Try to drink only purified water as tap water has Fluoride in it. A couple of young boys after eating a tube of tooth paste. Don't buy the Brita brand jug as the filter does not get it out. Berkley filters are good.

    The body converts excess glucose into erythritol, so yes obese people and diabetics will have higher levels of erythritol in the blood and have higher rates of CVD. This study only looked at blood levels of erythritol and didn't account for the body converting glucose into erythritol

    Damnit, I thought you read the study on erithritol. 8n, 3 day study, the patients weren’t healthy (had DM2, HF, etc). And the platelet clotting was petri dish experimentation.

    I think what he meant was that the level of erythritol in the blood after consuming 30 g greatly exceeded the levels at which increased clotting was seen in vitro. But I agree it was phrased badly and made it sound like they actually measured clotting in the 8 subjects, which of course they didn't.

    That study is unreliable and precisely why it's the only one to signal such red flags in erythritol consumption. I suggest everyone watching this video or scared of erythritol to read the study and test parameters themselves

    Most Monk Fruit and Stevia sweeteners have added sugar alcohols. A main goto is erythritol. I use a healthy sweetener instead in my coffee, the amino acid glycine.

    You will not die from erethrytol people, though if you want to play it safe then use 100% Stevia to make sure in case of mild complications. Stevia does not induce a response. Been on it for more than 17 years, after doctor recommended it when no one en earth knew it existed. I had to order it from South America back then. [edith]: mobile autocorrect shenanigans.

    the amounts i'm hearing in these studies are pretty high, 30g of artificial sweetener is outrageously high. Especially since it is generally way sweeter than sugar. Drinking a diet soda every once and a while in a 12oz can or 16oz bottle isn't going to give you these side-effects. How about we do avoid overconsumption and promote portion control as the tried and true method? I'll have diet gingerale / diet coke a few times a week and i have 0 trouble losing weight and 0 side effects. I do believe many studies have shown if anything, it can help with hunger and cravings to maintain a deficit in calories. But all the data, good/bad for most sweeteners are seemingly safe in the amounts that should or should not be consumed.

    30g of erythritol is around 1/6th cup which really isn't that high if you're consuming it in sodas or sugar free desserts, even worse if you're consuming multiple sugar free sweets a day. That's why I'm going to quit erythritol, because I know on some days I was consuming far far more of that stuff than just 30g.

    The best sugar is no sugar, however I will stick with cane sugar when I do use sugar. I've been told for years to cut milk from my diet to lose weight and I've not and don't plan to . I'm 45 years 5'4" / 180lbs at my heaviest and about 170 when I work out regularly. I think the benefits of milk consumption outweigh the risks. My daughter is insulin resistant ( I had gestational diabetes and was not treated) but she's not on medication and does not like sweet, sugary foods. She's stopped eating many foods because of the sugars including donuts and iced cakes, she's 12 years. I think more research needs to be done on insulin resistance especially in children.
