When patients receive a diagnosis of sciatica, they commonly question why sciatica pain is worse at night. The sciatic nerve is a mixed nerve containing information from your brain through your spinal cords to your muscles and organs, telling them what to do, as well as transferring the sensory information of your body back into the brain. The sciatic nerves comes out of the low back and extends down into the leg and foot.
Sciatica is a set of symptoms that can be felt anywhere down the sciatic nerve's pathway. It is typically caused when the nerves become pinched, compressed, stretched, or otherwise affected in some negative way. The pain it produces can vary in relation to how much of the nerve is involved. It can range from mild pain and discomfort to very chronic. It can also lead to muscle weakness, loss of function and movement in the lower body, decrease in reflexes.
Sciatic pain tends to be the most debilitating when it is a nerve-related pain causing a sharp stabbing pain that prevents the patient from even standing on the leg.
The most common causes of sciatica typically involve the alignment of the lumbar spine. A herniated disc is when the inner nucleus pushes through, causing pressure or loss of space within the area where the sciatic nerve root is coming out, leading to a pinched nerve.
Sciatic pain is worse at night because when you try to relax, you have nothing to distract you from the pain you are experiencing in the body, and as your body tries to fall asleep, your muscles and organs slow down, but your nerve system becomes more sensitive, and hyper-aware which can make it very difficult to find a comfortable position to sleep since the things you didn't notice were hurting while distracted will be more noticeable when you're trying to sleep. Another factor that can stretch the sciatic nerve even more is the pressure points from lying down, which can lead to more pain.
To reduce sciatic pain, you have a few options:
Try to find positions that don't stretch the nerves.
Determine if you have scoliosis, which can worsen the causation of sciatica.
Warm baths can help relax the muscles as well as soothe inflamed areas to help decompress the nerves.
The best way to address sciatica is to deal with a proactive treatment. The causal factors are commonly related to spinal alignment. Addressing these issues can improve the sciatic pain you are feeling.

Thank you Dr I have been following you for many years now. I only wish there were more doctors that knew as much as I have learn from you. Many doctors and physical therapist say they can treat or help with scoliosis only for me to find out I am with the wrong or PT. I have to listen and watch your videos and then ask my PT about certain exercises or the Stroth method and more often they have heard of anything.
