    Hi everyone, I had Olccranon fracture back in June and needed surgery. I had plate and screw around me. Then I got my stitches removed, and I started PT after that. After my follow ups my surgeon I wasn't getting any range going. Then my surgeon recommend to see an elbow contractor. I was able to reach only 100s with PT before seeing him. I went to see the elbow contractor with a CT. I had Heterotopic ossification from my CT scan, and needed second surgery to release. As for now I am able to start PT and starting straighten now. I hope everyone has better results soon.

    @@ns7n357 Hopefully you have a good physical therapist. By 9 months you should have most of your movement back. I had a Jas Splint after the first couple months that really extended my range of motion. After my hardware removal I got pretty much all my extension back. I know that it is possible to have a procedure where they can force your arm back into place under anesthesia. Having the hardware removed and yoga really helped!

    @@starfish7890 the plate is there to hold the broken bits of bone in the right places while new bone grows to knit them together. Typically, the new bone ‘glues’ the pieces together in a week or two and from then on, it gradually hardens to maybe 75% strength in 8-10 weeks and full strength over a year. As the bone heals, the plates are no longer needed. These are very rough numbers - I am not a doctor but I have just had my olecranon fracture fixed and this is what I’ve been told.

    How soon can I begin ROM after olecranon hardware removal? I am terrified of losing as much ROM as I did with the oroginal hardware placement. My elbow gets stiff within an hour of not moving it now (9 months post implant placement of open comminuted fracture with plate/screws). It took 5.5 weeks to get my ROM back but I have never been able to get full extension back and I am currently unable to do anything that involves use of my triceps to any extend due to severe triceps pain when trying to push on anything (such as trying to tuck my comforter in to my bed). • @crazy4cocoapuff

    @crazy4cocoapuff

    4 жыл бұрын

    @@annawiemer5929 hi! So I will start by saying that my hardware was placed by the surgeon on call, who specialized in spinal surgeries and was known to be very technical about his work. When I compared the OR report with the Depuy Synthesis hardware instructions, they were performed identical to what the surgeon did. However, I was very thin and actually lost tremendous muscle and all around body weight after struggling with inability to use my arm normally for a year and being told there was nothing wrong with the hardware. My triceps was excruciating and you could feel that with extension of the arm, the top curved edge of the olecranon plate was cutting into the triceps. The 1st 2 surgeons did not palpate my elbow and hardware on extension and I believe the 2nd did not even read the OR report either. When I saw the 3rd surgeon for consultation, he had clearly read the OR report and he completely evaluated my ROM while palpating the elbow joint and hardware as it moved with extension. He explained that while technically the hardware was placed correctly, the plate was placed on top of the triceps and the plate was cutting into my triceps each time I extended my arm. This was apparent also in the OR report, with the top hook of the plate stabbed through the triceps and into the olecranon. I could physically tell this was happening as well as palpate it happening, but this 3rd surgeon is the only one who also actually assessed the issue completely by doing an actual evaluation. I had pointed this situation out to 3 different physical therapists, the 1st one whom blamed me on not being able to do the exercises she prescribed. The 2nd 2 both felt that if I was having so much pain that I should not be pushing myself further. Also, there were 2 bone fragments that healed on to the back of my olecranon/ulna and the 2nd one was too large to glide past the other bone that forms the elbow joint. Regardless, I had a total of 3 surgeries. The 2nd was to remove the hardware. I am going to tell you that after the hardware removal I had ZERO, and I mean ZERO pain with extension the very next day!! I was able to utilize my triceps muscle in all aspects with NONE of the excruciating and prohibitive pain I had the day before. I could make my bed, cut pizza etc, close my hatchback... everything. But I did not gain hardly any ROM because the bone fragments were still there ! So the 3rd surgery was to go in and smooth those out and supposedly the surgeon said there was a ton of tissue and stuff that he dug out. I now have what would be full ROM, BUT I can not hyper extend my joint like I can on the elbow that was not broke. I am very flexible and most of my joints can bend backwards etc, so this may not apply to most others. Also, there were also physiological dynamics that actually blocked ROM due to how the fracture healed, along with how the hardware was placed. I will say that having the hardware removed was the best thing I ever did (even though I had been told prior by a different physical therapist and the first 2 surgeons that there was nothing wrong with the hardware other than it being too big for me and that now days they leave it in). If I had been confident the hardware removal would have resulted in eradication of the intense triceps pain I had been in and disability of my arm , than I would have pursued its removal with vigor, and had it taken out asap!!! I could have prevented all the weakness and loss of muscle that had occurred over the course of the year.

    @@crazy4cocoapuff, I thank you from the bottom of my heart that you took the time to answer with so much detail my question. It's been nearly 4 weeks since I got surgery and I am trying as hard as I can to extend my arm as I used to before. I also have naturally hyper extension in my arms, and I got so excited to read that you do too! I am still not able to hyper extend as I used to, but I am already at 0° like a normal person. But still, I want to reach hyper extension so that my arms' movement look symmetrical again. Were you able to move you arm the next day after the first and second operations? I was told to move it the very next day (1st operation). A physiotherapist came and helped me flex my arm but not extend it. I am flexing my arm past 90° but still cannot touch my shoulder yet. But I am told I shouldn't worry about it, because flextion always comes back with time. What I should worry about is the extension. After reading your answer, I am convinced I want the hardware to be removed asap too. So 9-12 months after 1st surgery. I was told that the hardware removal is a much more easy and simple procedure and the surgery is more simple. The recovery after the second op is much faster too. Is all of this true?

    @@annawiemer5929 hi, yes, I honestly had no pain the day after hardware remival, and as I said, I was able to use my arm in ways that I had not been able to for a year, due to the painful hardware. For the last 2 surgeries I did not have a splint, and I began moving with gentle ROM the day of surgery. The 3rd surgery was a little more difficult than the hardware removal but that is because the joint capsule had to be disrupted. Regardless, it was all worth it.

    @@annawiemer5929 To be honest I wish i could extend it to 0 degree although i have slight hyper extension in my other arm like 5 degrees, its been 1 year for me and i still lack around 20 degree even to get that i went through extremely painfull physio and still have extreme tricep pain similar to what cocoapuff mentioned, i hope it goes away after hardware removal .. i have read online hardware removal sometimes increases ROM so im keeping my hopes high :) Can i ask what type of hardware was placed for you? its tension band wiring for me...
