What is diastolic dysfunction?

In this video, Dr Sanjay Gupta discuses diastolic dysfunction.
The heart is a pump. It has to relax and fill with blood and then contract to push that blood out. The time interval during which the heart contracts is called systole and the period during which it relaxes to fill with blood is called diastole.
For many years, the ability of the heart is measured by its ability to contract and this is best ascertained with echocardiography. Those patients who have hearts which are not able to push blood out of the heart as well as expected are termed as having systolic dysfunction or heart failure and it is well recognised that these patients have a reduced quality of life and life expectancy compared to patients who are able to push blood out as well.
In the search for identifying people at risk of developing systolic dysfunction as early in the disease process as possible, doctors started realising that how much blood the heart pumps out during systole is dependent on how much blood had filled the heart during diastole. In conditions like high blood pressure which often led to systolic dysfunction eventually, it was observed that the heart did not relax as easily and therefore filled with less blood to pump out. These relaxation abnormalities were termed as diastolic dysfunction.
The truth is that there is no such thing as isolated diastolic dysfunction although many medics seem to think there is. Systole and diastole are both phases of the same cardiac cycle and therefore abnormality in one phase will affect the other phase. This is a fundamental law in cardiology called starling’s law. The heart is a 3-dimensional structure which contracts radially, longitudinally and also twists. The test we use to assess the systolic function is echocardiography which looks at the heart in 2 dimensions and doesn’t routinely assess for longitudinal or twisting function and therefore the systolic function may look normal. It seldom is normal.
WHen we use blood tests like BNP which give us an assessment of how much pressure is within the heart, we find BNP levels are raised in patients with diastolic dysfunction and echocardiographically ‘normal’ systolic function.
Diastolic dysfunction is often caused by ageing, high blood pressure, diabetes, and excessive weight. If left unaddressed it may progress to echocardiographically detected systolic dysfunction. There are no particular tablets as such which will reverse it. The more important thing is to control the risk factors that may have caused it in the first place. This includes weight loss, lifestyle modification and regular exercise and control of blood pressure, diabetes and sleep apnoea.

    I have seen now a couple of VDO of you recently and I find them very interesting and all very well explained. Thanks for this! One question though: I could not find one VDO of you discussing the topic of Coronary Artery Calcium scoring. Which to my knowledge is one of the best methods to determine someone's risk level of getting a heart attack in the future. Any particular reason why you have not discussed this topic in one of your VDO's so far? Thanks a lot!!

    Thank you doctor for such a good explanation. I m a mother of 7 month old baby and recently found out that my diastolic pressure is high it is always around 85-90 . And I also feel chest pain sometimes on left side . Is this a serious issue and Could we please tell me which test i need to do to find out any ill effects on the heart.

    Can you give info on left ventricle deviation? I’ve had this for years now. Had several Cardiologist, that have never made it clear to me what this is. I also have irregular heart beats rather often. EKG is always abnormal.

    @@karimah3462 Hi, Unfortunately, I didn’t get a response. My blood pressure does all sorts of strange things that I’ve asked numerous cardiologist about, but I have yet to get an answer. Nobody seems to know which seems strange to me because you would think cardiologists would know or at least try and find out. Anyway, it’s frustrating. Take care 😊

    When you lying down, return of the blood increase to your heart, we call it increase preload. Now the heart has got more load to work on, and it has to generate systolic pressure to push the blood into your aorta and that is how your systolic pressure up.

    Best video that give us the language to pressure our US doctors to do the BNP, EKG tests on pre-surgery patients who are clearly showing early symptoms. Assistants take BP on arrival for appts, and have no idea what they are listening to. Pt has been recording his BP for 3 years, the sys is high during the day, dias is very low in pm, as is pulse. Pulse pressure high. Why are US doctors so uneducated on this?

    I'm an athlete/mountaineer so get regular exercise - hiking a minimum of 5 miles a day and up to 15-20 miles a day. I'm muscular at 165 and 5'7", don't smoke, eat a grass fed/organic diet. I occasionally have a glass of wine. My BP is 115/55. It seems there's nothing I can do to address my diastolic BP and now I'm symptomatic (periodic dizziness). Suggestions?

    @@visakeswaran3620 Found out my dizziness is Chronic Vestibular Migraine. Doing Vestibular therapy and that's helped a lot. Still a little dizzy in the morning but most of the day I'm good now. I'm also hydrating better. It's something I've always been bad at. My diastolic is now around 60 so made some progress. Based on my reading there's not a lot you can do other than monitor it. If your systolic is slow (below 110) you might consider more salt in your diet.

    @Amy D yup, that's what i was diagnosed with. Had Chronic Vestibular Migraine. Did Vestibular therapy, upped my Magnesium intake, and homeopathy all helped to alleviate the symptoms. My BP is still weird (110/60) but doesn't seem to be causing any issues.

    Hello doctor is it normal when my systolic goes up over 150 my diastolic goes to normal range and when systolic go’s down under 150 my diastolic goes up 101-105 ? Also no meds all natural I’ve been on a struck diet and working out for one week already lost 7LBS and BP down little bit but just can’t get the dialostic down

    Thank you! Guess if not enough blood is going in, it will weaken the heart? And weaken other organs that need oxygenated blood. Is there a way to find out the reason for less blood coming in when the heart is relaxed? If not a valve problem, damaged muscle? Not enough bone marrow to make blood, not enough oxygenated blood, meds? Have RA too. Started B12, iron and folic acid today for my anemia. On a diuretic but left leg still swollen...all bad signs! My diastolic is often in 40s and its worrying. Finding help in NHS is next to impossible. What can you possibly say in a 10 min GP online appointment, if you get one! A waste of time. I need private treatment. A cardiologist like you but not sure how to go about it in the condition (almost non mobile) and circumstances (no help) I am in currently. Going private is the way, but how. Am in London. Wish I was in York! Any advice will be welcome.

    Suppose the hypothetical diastolic dysfunction is very intermittent, or very seldom? I'm referring to what may be an exercised-induced heart block, where normal systolic-diastolic readings might be 140/80, 80 bpm during lite exercise, but suddenly drop to 140/60, 42 bpm. In the past these events occurred only when taking Lisinopril, or Flomax double-dose, but at no other time. Having been on Losartan for 6 months, they are occurring again. Btw, the episodes resolve in 3-4 minutes, dizziness but no fainting, with bpm returning to normal 65-80 heart rate. Events have also occurred with postural change, after eating large meal.

    I have this exact thing, except don't take medications. Did you ever get an answer from your doctor as to what causes it? My cardiologist says she has no idea. For me it happens suddenly and at any point in the day or night. It resolves on its own too.

    @@princessmari5063 ...my post is now 2 years old, and have since discovered the actual cause, and treatment of the heart condition. I discovered 1 1/2 yrs into the significant palpitations the entire condition was a result of a mild, but undiagnosed Covid infection. The inflammation caused by it was causing everything. After much research I began taking Ibuprofen twice a day, and cured it. I now continue to experience other, milder Long Covid symptoms, but no heart symptoms.

    My blood pressure is always around 130/92-96. I am confused as my diet is good i exercise a fair amount and I don’t smoke. Why is the systolic good but the diastolic always high. Any ideas please as it’s really upsetting me. Thank you.

    My pharmacist and my Dr are concerned about my diastolic. I suddenly developed high blood pressure do to a huge amount of stress. I had a crisis that should have had me in emergency 205/101, but i could not leave home because i have my mom here now who has dementia. Then about a week ago i think i had what is called a tia, a few days after that i was in my pharmacy and checked my pressure and it was 110/47. The pharmacy said the meds im on are not enough to have caused this. So i go see a specialist next week. My Dr must be concerned enough he got the app rushed. I also just found out i may have lung cancer, the found 6 concerning nodules in both lungs. Every get the feeling your days are numbered?

    I have "high bl.preasure" and atrial fibrilation.Must take flecainide and beta blocker.What I notice,but no budy cares, is that my diastolic bl. preasure is always to low(50-60, never up 80) even my systolic BP is high.Difference beetween sist.and dyast.BP is always more than 60!? Kan you explain it,please.?!(I have operation 30 years ago-Patch plastica ASD and I have live long without any problem).Thank You verry much!Great video's!

    It would be helpful to advise that Grade I diastolic dysfunction is present in the vast majority of 60's and therefore normal. The atrial kick compensates for the lack of relaxation as the heart ages. I am not a Dictator but this is my understanding, and therefore no need to worry by the over 60's. E/A ration

    Question. Is there a way to tell if someone had a previous years ago heart attack? I went through something one night for about 2 hours, i thought was food poisoning. It was much like my gallbladder attack. (gallbladder removed) The pain was excruciating and i kept wanting to throw up and i was soaking wet with cold sweat. No stupid me did not go to the hospital, i really thought at the time it was food poison. It has been bothering me ever since that it may have been something more. My kids ate the same thing and did not get sick.

    You might have had acute pancreatitis. Look that up. Happened to me, it's like a band around your chest esp. the back. However mine was accompanied by extreme vom/diah, so it was food poisoning. I thought I was having a heart attack too but soon realized pancreatitis. The cold sweat tho is more like heart attack, tho pain can do that to you. How to tell? Scar tissue on heart. EKG's will show if you did, if your doc is worth his degree. You could also have MORE gallstones, even with it removed.

    Dr. It would be very helpful to point out the level of this disease. For example, diastolic dysfunction grade 1 in patients with otherwise normal heart and echo result is not a concern. With some life style changes, i,e., dieting, exercise, lower blood pressure, etc..will eventually dissapeare. If the diastolic dysfunction does get worse and the level goes up to grade 2 or 3 at this stage it will began to effect the systolic function as well. I love your video. Please can you talk more about the grade of this disease. How someone can improve it, how to differentiate level 1 from 2, and if this could be an isolated finding on echo due to new machine and doctor analyzing the pressure to much while over thinking..

    @Autumn Rose This is my case. 2 years ago one of my echo showed grade one. The last two echo's showed no diastolic dysfunction. I changed my diet and work on reducing my blood pressure. However, lately I m having some abnormalities with my left atria 😫😫 always something. So tired.

    Have you tried to look at patients' insulin levels, since insulin is a growth hormone that may contribute to left ventricular hypertrophy followed by diastolic dysfunction. I wonder what your perspective is on the impact of chronic hyperinsulinemia?

    Hi Dorothy, I am a retired echocardiographer who used to scan patients' hearts with ultrasound, not a physician. Normal aging would increase the heart muscle mass due to becoming less active. Those walking after age sixty for at least 20 minutes a day is claimed to preserve the diastolic function of the left ventricle (main pumping chamber) So I am wondering if the sedentary life does not gradually raise blood sugar, thereby insulin and cause the heart muscle to thicken and lose its elasticity and ability to relax in the filling phase to maintain its efficiency. That is an explanation that comes to my mind based on this video presenter's description of insulin's impact. Thank you. @@dorothybrown8859

    Was in the hospital .last year for 6 week with a infections ..I had kidney failure but went back to normal had fluid overload .was on 10 deferent antibiotics .I do have high blood but I don't understand whan. I Frist went to the hospital they did ECG test come back good said Avery thing was good 2 weeks later. I have DD told me I was grade 2 but that was last year .got ECG done today lady told me Avery thing looks normal. Soo question is can to much antibiotics cause this are infections are my blood pressure.and I have no symptoms or is there any symptoms and my heart valve look good.

    @@biggiecoalla14 yes. I walk everyday for at least 30mins. My biggest systom is shortness of breath. It scares me at times and makes me panicked. Walking really helps as I don't have as many shortness of breath like I did before walking. Hoped I answered your question.

    Yes and no. It depends what is the grade of the dysfunction. Typically, grade 1 is not a concern in otherwise patients with NO other finding.

    Hi sir, I'm a 17 year old male, quite fit, bmi 24.7... Whenever i check my blood pressure, it is always around 140/60 or 130/60...when I'm calm it goes down to normal systolic around 125... But the diastolic is always less than 70... This is really disturbing because I experience a bounding pulse in most parts of the body.. I do have anxiety about it. (health anxiety since lockdown) (I've been taking homeopathy for that and I'm better) .. But i have been controlling it...i also try to control diet..i do regular exercise too.. My questions are: 1.Why is my diastolic blood pressure not coming back to normal even if i cope with everything? 2.when someone has high bp... Usually both number go up for them... Why mine has gone up and down. (syst up amd diast. Down)? 3.i know it is mostly anxiety... But is mine different? I hope you read fully... :')

    Diastolic pressure shouldn't move much at all, even with exercise. That's a good thing. The systolic pressure and heart rate should jump around as you move or even drop when you eat. All your numbers seem good; your symptoms point to anxiety, and that will cause the beating feeling that can lead to more stress. I've been fighting this also and it's hard to move past the anxiety. Unfortunately my diastolic jumps about 17 points just from a short trip to the store. So more tests are needed. Relax and try not to be stressed as that can lead to issues many issues over time.

    Hello Doctor i am facing irregular Rythms of my heart beat 35 to 40 times every day and my blood pressure is 140/100 . I feel like my heart is struggling all the time .     Magnesium oil rub it on every night! take magnesium malate too. Increase your potassium too and make sure your electrolytes are on point. Cut out all processed sugars and refined carbs and start fasting 18-6. 6 hour eating window. Take hawthorn Berry extract and cayenne teas.

    I feel like I have some preload failure and some diastolic dysfunction with pots!.. Could be due to poor blood perfusion, would this be considered diastolic dysfunction? My Pulse pressure gets narrow, ie typical reading 103/87

    Not at all. Trivial MR is associated with MVP. The mitral valve oes not close properly, and there is leakage back into the left atrium during systole. There are number reason for the prolapse.

    No, it is nothing to do with water. This is because the heart muscle does not stretch to accommodate more blood. This is like a balloon where you put more water, and it stretches and accommodate more water,but If it is hard, does not stretch, then much water you force into the balloon,the balloon does not stretch. Similarly, when the heart muscle is hard , does not stretch to accommodate more blood during diastole, then it does not accommodate more blood And this is called diastolic dysfunction. Diastole is the filling phase of the heart, and systole is pumping phase phase of heart.

    @@sosweet4447 There are number causes which triggers diastolic dysfunction. You have to treat the underline cause. For example if the subject has a fast heart rate, then you need to give medication to slow down the heart rate. There are causes which has poor prognosis.

    @@sosweet4447 stress and anxiety cause diastolic dysfunction, and that could be the reason in your case. In stress heart rate is fast, whereas it does not allow the ventricle to fill properly due to fast beating.

    What about low blood pressure ? 98/64 -104/72 and sometimes lower ? Can this be dangerous as well ?? 💓 💓 💓 Can it be due to heart weakness ?? I would love to know the underlying cause !

    Lolita love , It is my understanding that you are fine. You can has a bit more salt if you wish. But read up. As long as you feel fine and are not dizzy, you are good. My I ask how old you are. I would not worry. It is not dangerous. I read the cut off is below 90/60.

    @@jameskantor0459 Thank you !! Yes, I do consume salt through sunflower seeds 🌻 and natural foods daily .. I have had blood pressure as low as 88/62!! And I don't feel dizzy !!🤯 I am a middle age female and always had very low blood pressure with heart palpitations .... This is my biggest concern . . .

    I used to worry about this and one of Dr Guptas video's about low bloodpressure has put my mind at peace. Look it up as I can't remember the title. My lowest BP was 78/43 and I'm fine

There was this one time I went for a routine check up and the nurse said I needed to lie down immediately and gave me orange juice until my blood pressure went up! I stayed in the hospital bed for an hour . . I believe my blood pressure was 86/59 not to sure ! And with heart palpitations . . I am so happy I found York Cardiology !! What a game changer ! ! I always feel safe just listening to his videos ! ! No other Cardiologist made me feel this way ! ! 💓

    I'm starting to notice that my systolic blood pressure is beginning to be in hypertension a lot while my diastolic blood pressure is beginning to be in hypotension a lot. I just got a reading tonight of 155/45 and it’s scary because what the heck happens when the diastolic gets any lower? Also my heart rate is almost always over 100 bpm, I have chest squeezing, lightheadedness, fatigue, and shortness of breath. I'm only 21 and I have family history of heart disease. I went to a GP got some tests done, echo and holter monitoring, but they called me up to say it was normal. I don't understand how everything is coming back normal, but I'm over here having symptoms everyday and I cannot even workout because just walking one day lowered my blood pressure to hypotension and I had really bad chest pains. But according to all the tests I'm fine. One day I was laying on the couch, almost unconscious because I'm so tired and then my chest started hurting. I had a really fast and irregular heart beat. My grandma was napping so I didn’t call for her, but I thought I was going to die that day. Hell, today I thought it was happening. I had major chest pains and I'm still having them as I type this. With all these test results coming back normal I feel like my GP thinks I'm lying or have some medical seeking attention illness and I don't. It's very frustrating.

    Try breathing exercise , called anulom vilom , do r&d and do it regularly , your bp will come to normal in maximum fortnight

    Have the same experience. My echo shows mild hypokinesis(segmental wall motion abnormality). Treated with metoprolol and aspirin got worse so treatment change to diltiazem and getting better. Had another test: Dobu stress echo and result normal ( grade 1 LV diastolic dysfunction). My cardio said to stop taking diltiazem, but i insisted to continue bec. If i dont symptoms will be worse again. Bottomline from 1st diagnosis of ischemic heart disease to none (cardio defer the prev. Diagnosis due to normal stress echo- but i am symptomatic and worse with no diltiazem).

    Hi i am 36 my doctor did eco everything comes out normal but E A ratio comes at 2.2 . I am regular bicyclist n i got covid and after 3 weeks my doctor prescribed eco . She told me everything is good but then i read my eco n saw my E A ratio is 2.2 everything else is normal should i worry n at sleep my heart rate is in 40s

    Hello Doc, Hello, I'm a 22 year old asian male that has been recently diagnosed with hypertension and the highest blood pressure reading I had was 140/110 while the lowest is 120/90 and 130/80. I am relatively slim and my blood tests came out fine. I didn't had echocardiogram or holter monitor. Before I had fatigue spells that comes and goes defined as feeling lethargic and no energy to do things. My cardiologist prescribed me Nebivolol and my lowest reading was now at 100/80. I also have ectopic heartbeats that has no accompanying symptoms and Nebivolol also helped with that. I have anxiety disorder and I don't cope with stress very well. I also have insomnia but i manage to sleep well sometimes. Now I'm always bothered with my heart beat like it's beating hard ALWAYS. My diastolic bp is always 80. What are thought about this doctor? Thank you for your response

    get a stress echo if it is normal imo get on anxiety meds drop cardio meds n stop goin to DR. ask cardiologist about this
