What are the Left and Right AIC Patterns? (think Brain and Senses first).

The left and Right AIC patterns refer to chains of muscle that are active when on your left and right foot. Importantly, muscles are only active when the brain tells them to be, and the brain makes that decision off of sensory input. Sensory precedes motor. So thinking about muscles and movement from a biomechanical perspective doesn't make much sense unless you understand the role of sensory input, and one of the primary sensory inputs that will stop normal biomechanics is faulty occlusion, or how your teeth touch. Molars and canines will give you hamstrings and glutes, and inhibit the hip flexors, if they are being sensed properly.

    My brain was ignoring my right eye since atleast 2017 and after chiropractic adjustments it briefly fixes it, doing exercises from optometrist or even physio pulled so many muscles which ultimately had chain reactions and threw out my right hip shoulder and pelvis tilt with rotation. I’ve seen an osteopath 3x now plus coincided with finding your vids.. I haven’t been using my left heel because I have a bone spur under it, I learned about it over 20 yrs ago and did take some homeopathic tissue salts whenever it became too sore, but realised I’ve been overcompensating my flex on left foot to outer heel to small digits rather than whole heel to big toe, which is mind blowing and May not have even noticed had I not found you on here. So I’ve consciously walked properly since, and it hurts lol but anyway now my brains ignoring my left eye and my glasses are seemingly useless now 🤷🏻‍♀️ but this concept is fascinating. I’ve been down so many expensive non fruitful paths with optometrists, ENT’s, Specialist Dentist, physiotherapists and chiropractors… no one has connected the dots to me until your videos and this new osteopath. Our whole medical system needs restructuring!

    Thank you Neal you are such a legend! I thought I'd share I've finished my expansion process and now working on shifting my teeth to the left. Right canine is so lonely 🤣 so just trying to awkwardly give it some action before it gets a chance to move down. I feel pretty good for the most part with my body but my left hip is typical. It will be amazing to see what happens when the teeth are all done. Thank you for being amazing and helping influence others like myself to go down this route. If more professionals would look at everything from a sensory perspective, I think so many problems would be solved quicker! 4 months into vision therapy for something similar to you, palate expansion and learning how sensory work is key, is a world of a difference! Keep up the great work 🙌🙏❤️

    Oh, wow. Vision and oral cavity issues are often found together (certainly in my case they were). I did some expansion but I’m not sure whether I am going to try and straighten everything. Hope you stay feeling pretty good.

    @@NealHallinan Thanks for your response back and yes I'm aware that they are related and it is very interesting! It's hard for me to tell sometimes if I'm progressing with vision therapy or if it was because of the teeth realignment lol. I'm not doing this for the sake of keeping the teeth straight but just to get them to line up accordingly because they were not making the correct contact points. I'll never know the truth of the eyes are improving due to vision therapy or if it's the jaw lol but I don't regret doing the vision therapy because it's actually made me realize how to think about using my eyes in ways I never thought. Also, it makes me think that maybe I'll end up having good vision for longer as I age because I really understand how to use the eye muscles now lol. So at least there's some pros regarding that! 🤪

    @@CelindaBelle 4 years ago i started with a +4 and added on top of it the astigmatism correction that was another +3.75 so add those together .. almost legally blind. today i find my self wearing a +0.50 on both eyes :)

    As a PT with continuing education in pri the biggest problem with pri is people thinking they understand it. It’s truly all neuro/sensory. That being said my own personal experience has been challenging even Dan houglam called me a unicorn based on special tests. Ultimately I will probably need your help. You rock Neil!

    Ah, yes, the unicorn (but not in a good way). My neck cervical lateral flexion limitation is always to the left, rather than the right🤷‍♂️. Some of us “break the rules” and it’s hard to know exactly why. But “sensory” is surely the better explanation than biomechanics. I’m always hopeful that people who claim they are doing PRI inside some online coaching group are simply naive, rather than malicious individuals trying to make a buck.     Hi Neal, this video is spot on for what I have been experiencing. I am one of those cases that puts pressure on the left canine, and it has caused me toothache for about 10 years. I learned about PRI about a year ago and have been doing online videos. I finally went to a PRI specialist, and they saw everything you described in this video. They recommended getting a orthodontic retainer to allow the pressure to be at the molars and balanced. Since I didn't hear you talk about that option in this video, I am curious about your opinion on such retainers. It would be much appreciated!!

    They are probably referring to a mandibular splint, rather than a retainer. They work.

    Same here, and I’m seeing a PRI therapist an hour away from me and have been for months. I feel like if I lived in Neal’s area and was able to see him I would have gotten better by now, or at least stood a better chance. The PRI PT I’m seeing has not once tested (idk if she even knows how to) if dental, vision or feet is ruining my sensory input, because these PRI exercises are not really doing anything for me. Establishing left AFIR seems impossible.

    They don't typically measure the orientation of the pelvis pri, they assume it is a "flat turn" clockwise which it always is at first in the left AIC pattern, but there's further compensations that can go beyond that I would go to Conor Harris comprehensive guide to lateral pelvic tilt and learn how to test yourself and see where your pelvis is in space. It might be pure L AIC, or beyond. I was not getting relief from PRI. At some point people cross into secondary and third-degree compensations Beyond L AIC, t and it's true there might be a sensory problem. But knowing your own pelvis orientation I would take into your own hands. I would evaluate the orientation of your pelvis for self and check out that page

    After watching a lot of your videos, I have to still learning a lot, on your beginner program in the “left zoa exercise” you bend to the left to get rib and pelvic on the correct alignment, but if you are a right bc pattern with left posterior extension of the rib cage, it is not a good idea to stretch and inhale in that area? How can you inhale if you have closed the space?

    Hi neal, I got a palate expander last momth and it has done beautifully, now I will have to wear braces to fix the midline and the all the small open bites i have and elastics to move the jaw, do u think that will fix things? Im still in right tmcc even after palate expander

    Would it be best to not wear a retainer? I got my braces off about 2 months ago & my PRT thought I may not make as much progress if I wore retainers. He said if it were him, he wouldn't wear them. And he suggested I shouldn't put any kind of appliances in my mouth until my head and neck are neutral.

    I have right torsion but since I have a cant I feel better molar contact on the left because the right side of my teeth are higher. Would a splint fix this? I still can chew on my right side just not as good also my vision in my left eye is better.

    You’re a fantastic teacher and communicator. Love your content. I work with a technique called Proprioceptive Deep Tendon Reflex (P-DTR), which is mostly neuro/sensory/reflex based. I’m curious if you’re familiar with it at all. I’m maybe looking to explore PRI for myself and my clinical practice. Would love to see how the two worlds meet and compliment each other.     Thanks, Josh. I am not familiar with P DTR. In general, lots of things can help, but in my humble opinion the main stumbling block is the fact that other modalities simply don't recognize the "left hemisphere/right side dominance" pattern that is the foundation of human existence. Our right and left sides simply aren't the same and can't be treated the same way.

    Neal, I have the trifecta, TMJ with right molar dominance, torn left rotator cuff/major scapular dysfunction (left upper trap compensating extremely), and a torn left patellar tendon (acute pain at its worst when I put pressure on that left heel). I'm assuming I'll need to address these before I'll be able to full benefit from these PRI exercises? I have every symptom of left AIC you mention

    I’ve been watching your videos, I went on Postural Restoration’s website and I found dentist Dr. Lam in Houston. Just found out the other day I have a partial dislocation in the right jaw joint causing a lot of my patterns and I will need an ALF to help stabilize it. Along with that, I will be meeting with Michael Cantrell and James Anderson. Something I have been doing is your 90-90 hip shift and your all fours breathing which seems to help me with neck/shoulder pains.

    thank you, very helpful, never thought about the feeling of the teeth, i mean i sensed that i was comfortable feeling the left side of jaw and awkward on right so I kept feeling the left side. Now i will correct! :) Also left hip is stronger and like you said compressed more on left side and lost sens of right. My foot valve is also lower on left side. Hope I understood correctly here hehe...

    Hello sir! I am glad to have watch your video and I can totally relate with you. I am a mom and PT too but I can’t seem to figure out what’s causing my daughter’s posture and gait until this PRI explanation. I want to help My daughter she’s 11 and I hope you can help me with PRI techniques for her. Right hip seem higher right hip internally rotated, right big toe pressing on ground arching the foot. She limps when she walks and I really feel sorry for her she hates walking long distances. Pls help.

    My problem is left hip higher and right is more forward and it looks like Right AIC. I was sportsman and I used my left leg to jump. On left side gluteuses strong on right side are weak . I have pain all over the body and can't fix it :( Anything what i did make me only pain I really confused.

    So, I walked on a right foot sesamoid fracture for 6 years- docs never read my rad images correctly. So my entire right side is overwhelmed with nerve/circulation/muscle pain always flexed. I can’t feel bottom under right toe. 🙄 good times

    I agree the left and the right differences but I can’t say in term English language . eg. left lung has 2 lobes but right lung had 3 lobes. And air pass flow difference sinus in left pass frontal and maxillary and right pass Ethmiod and Sphenoid

    No. You’d want to be evaluated by a PRI therapist who has experience with dental integration before touching your teeth. You don’t want to straighten teeth on a faulty foundation. That could make things worse.

    Neil, how do we fix this problem. I have every symptom you describe here. I think I also injured my left hip flexor nine months ago doing BJJ and stretching where I felt somthing tear in my upper left thigh rec fem area. Now my ITB illipsoas and rec den are really tight and overactive rear hip capsule and glutes stiff and painfull and I feel stiffness and pressure above the left SI joint. I can’t seem to tense my left glute complex anywhere near as powerful as the right side. What can I Do!!??

    Hi Neal, you mentioned that you were able to get them to stop sensing more of their left canines. Is this something PRI institute trains you guys to do? Or is this something a dentist can see as well? I’m seeing a PRI specialist but he has not mentioned anything about jaw positioning or teeth sensing.

    Yes, it’s taught in seminars, but you’ve got to keep in mind the way I described things is the result of 10 years of experience and almost 30 PRI seminars, and my own personal experience as the ultimate PRI patient. Not everyone will see things the way I do.

    Okay, I have watched a lot of your videos and I get that you say "You are not in the right AIC", but based on the information in this video-- I am. I have a hypermobility disorder and got my jaw dislocated when I was a kid causing my jaw to sit too far to the left. While sleeping, I have ground my right canine to nothing because of my jaw trying to get to the right. My left foot is supinated and my right is pronated. My left rib will not move at all--right side over inflates. I cannot sense most things on my right side--including moving facial muscles. Can you PLEASE do some videos for the people who do not fit the pattern? I know you don't think we exist, but we do.

    I knew I couldn't be the only one. I had upper and lower jaw surgery when I was 21. Right side of my jaw still is too short and never developed properly. I am right handed but all senses are stronger on my left side. Left eye dominant, left leg dominate, I pronate on my right foot, breathe mainly through left nostril. Doesn't feel or sound like I'm in a left AIC pattern, sounds like the opposite? I'm just confused.

    I don't know the technicality of left AIC, but i would assume trauma can certainly induce right AIC-like positions? In this case, it would seem like the opposite movements would be beneficial? For instance I have a higher L iliac crest, greater IR on L side, greater ER on R, and noticed greater sensation on R molars. This seems exactly opposite to the left AIC. Ive had this asymmetry for 10-15 years so just now delving into some of the mechanics to see if this can change.

    Im really confused, my right side is duller than my left, my brains connection with my right side seems like slightly less feeling. I have noticed myself leaning over to the left side, like a side crunch while sitting. I only chew on the right side of my mouth, and I've been doing this for idk how long, at least a couple years! My left canine is super sensitive and that's why I dont chew over there really. Where do I start? Going to the dentist?

    My right canine is the issue for me if I clench my teeth so I can feel my left I can feel body straiten out a little but I dont want to have to clench my teeth all the time specially were their not in the best shape lol

    I think Neil is being modest. It’s worth trying other practitioners or feeding your current practitioner any information/senses/feelings/research you have done yourself. I have a very nice pt, who is receptive to my feedback/research and while not that strong at the PRI cranio, so had to push her into that area, and so we have made lots of progress. I am recovering from a stoke, so also have TBI, cranium, jaw, vision issues. I am very well supported by family and have been able to almost not work for the last year, working largely on PRI which has helped.

    This sounds a lot like me but i had bad bite my whole life. My front teeth sit edge to edge, theres about 2mm of space between my premolars and canines and too much pressure on my molars. I just got scanned for invisalign. Do you think bite correction will be a 1sr step to fix things?

    @nealhallinan is it normal for muscles in hips to be very sore after doing pelvic repositioning pri exercises?

    It can happen. But too much soreness may indicates that those muscles are working way too hard due to resistance/tension coming from elsewhere in the body. But you need a therapist to identify where that restriction is coming from.

    @@NealHallinan your left adductor pullback exercise helped almost immediately. I was wondering is it the chicken or the egg. Are muscles tight because of poor bone positioning or are bones out of alignment because muscles are tight, knotted and scar tissued up?

    @@HumanBodyMaintenance It is a “neurological” situation. The “patterns” are brain driven. The brain says “I want you in this position”…for some reason that we have to get to the bottom of. The muscles are not the issue, they just do what the brain tells them to do. I really do have a lot of videos on this channel talking about the how/why behind these things. It can’t be explained through comments, too many variables.

    I understand you say right ACI pattern rarely occurs. But what if on top of all the simptoms and visual body misalignments, also the cat scan confirms that my lower vertebras are twisted to the left? Can I conclude that I am that rare occurance of right AIC pattern?

    Good question. When the spine is overall “oriented” to the right, there will be segments of the lumbar spine that are in a leftward rotational state. There are always rotations and counter rotations at various levels of the spine, otherwise we’d fall over. The left AIC pattern is really describing the fact that we have a bigger right diaphragm and a sensory system that “prefers” the right side. Some people have injuries/instabilities/scoliosis that put their lumbar spine to the left, but it’s considered “pathological”. the same underlying asymmetrical brain and respiratory system are still there, though, even though the spine seems to be “breaking the rules”. It’s not that common though.

    @NealHallinan thanks for your answer! Yes, for sure. Mine is a pathological case and I have had chronic pain for 10+ years. I am shocked that your videos describe by problem and my pain so precisely.... only mine is mirrored. I am trying to do the exercises you propose but on the right side, just as a test. And I do get relief. I think it's not a bad idea to train my right leg to rotate inward and my right hip to move back. I will keep watching and learning, thanks 🙏

    Not necessarily, only testing can tell you. But in general, we should all have the ability to feel our left canines, right canines, left molars and right molars, all individually (one area at a time) without interference from the other areas of the mouth.

    It's been great discovering your channel but i don't know where to begin with your videos, i have a short left leg or i guess a longer right leg and i can't walk or stand for 10 mins without feeling pain from my low back down and sometimes my whole body.

    In my opinion trying to figure it out yourself is rather frustrating, i did all Kinds of pri techniques for about a year and finally decided to fly to the us to Take Part in the Prime program. Believe me, If you have guidance what you specifially need its much easier and i finally saw results

    I would highly suggest first ensuring 100% that you don’t have a physical leg length discrepancy before you go on the PRI journey. Scannograms, full-length standing X-rays, aid in measuring the leg length discrepancy as well as measuring spinal curvatures. Once it’s determined if you have a functional discrepancy or a physical one, only then It would make sense to go in depth with PRI and by that I mean extra devices like glasses and splints. But PRI is beneficial regardless I think for anyone who has torsion and pelvic instability.

    Hi Neal, how do you find a provider that is specialised in teeth/jaws problems. Cause you keep saying find a local trainer who can help you, but outside the US there aren't that many providers around. I know my problems are definitly jaw/head related cause i have right tmj problems and feel the left canine aswell. So what to do if there aren't any providers nearby. the closest provider is like 300km but i dont think she is qualified with occlusal problems and the guy that is is 600km away. Instructions from youtube would be helpfull just to try. I live in Rotterdam, The Netherlands.

    Well, you have to keep in mind that right TMJ and feeling the left canines may simply indicate a body that is stuck on the right side. It may not mean that you actually need dental help. Only 1 of the 4 people I mentioned in this video may need some dental help. So you can't really know unless you work with someone who can test you. I'm sorry I don't have any great answer for you.

    What does body oriented to the right or left look like? Twisting a fake pelvis doesn't cut it. Ie when I walk my and stop my left foots always 5cm back vrs my right. My left shoulder shoulders always back. My left psoas/iliacus/QL clicks on a layed down cycling motion. Been told my left leg is longer. I am left aic or right aic. You need to do a video on (what am I) and not showing a fake pelvis..

    Hi, I’m 15 years old and I’ve been watching your videos and I really need help. So I have had an extra tooth in the left side of my mouth for 4 years. It led to an asymetry in my face.I got it out of there today and here is what I have now. Left eye higher than le right, restricted breathing in the right ribs, my right shoulder higher than the left one and the nerves in my arms etc. are all tight. I had to take a break from basketball until I Get that fixed and my right hip feels uncomfortable sometimes. Please help, thank you

    I have the same issue my right mandibular wisdom tooth never fully erupted while the left one did so it is making the left side of my face get over sensed easily. Everyday I wake up patterned until I put on glasses for my myopia and sense my left foot and peripheral vision and adjust my clothing then I go neutral.

    Hey neal, what could be a possiblity for me if my anterior neck muscles don't let go, but I worked with a PRI practitioner who tested me for occlusion and vision and it wasn't any of those two things causing it

    Also its worth mentioning that for me, this pattern didn't slowly develop overtime, it was something that happened in a span of like 1 week. Am talking completely neutral to a really patterned pec where I felt like my body just wanted to push me to the ground. Its almost like it was something paranormal

    Are you or someone you trust taking patients? I live in Australia and it’s gotten so severe I will travel anywhere. I’ve spend thousand and thousands of everything else. Now thousands and thousands on glasses, split, massage, TMJ, feet in at a loss. I’ve even had balance testing, confirmed by dentist, TMJ etc that your videos is my condition. The system is broken.

    I have been going to a PRI place for almost 3 months now and I do see some improvement, but I'm not fully functional yet. Honestly, I am still getting this feeling that this method is hocus pocus, like I'm being indoctrinated into Mormonism or scientology or something. Nevertheless, I'm still willing to try it out for a bit longer. I've tried everything but MRI's and surgery so far.

    This channel wouldn’t exist, and the Institute wouldn’t exist, if it were hocus pocus. Not all PRI therapists have equal amounts of experience and people have different issues. For example, Someone w hypermobility (not saying that is you) or a history of trauma may take a long time to feel better. We are dealing with the brain and the brain’s ability to change or resist change. My tinnitus has decreased over the past four years bc of PRI, but will it ever go away completely? I doubt my brain will allow it.

    ​@Neal Hallinan what I mean to say is that I am cautiously optimistic. I have no idea what my root cause is, but I have 2 things going on. Constantly having left QL tension that can also make its way into my glute minimum and piriformis. Recently it has been giving me some IT band issues to. I found my Good Feet insoles that I started using again which seems to improve my arch and walk. I'm trying to correct my pronation and flat feet. I have the outside of my heel wearing down on my shoes. I also have some neck stuff that has been pretty bad. So much so that my nerves in my face get irritated, namely what I believe is the trigeminal nerves. My tongue would tingle and I'd have my jaw involuntarily shift to one side as if I have muscle spasms around the jaw. A doctor, a dentist and a physical therapist all ruled out TMJ. My SCM, scalenes, upper traps and suboccipital muscles are always in tension. I believe I found the issue there though , as I have been working on my lower trap activation (which I found I couldn't even really activate on my own without external stimuli at first, like my brain was telling them what to do but my muscles were on vacation). A few weeks later of working on those my neck is visibly longer, meaning my shoulders aren't hiked up into my ears as often. Nerve issues are reduced, but still have work to do. I think it also has to do with some C1 hypermobility because I can nearly go 90 degrees with neck when looking up. It all just sucks, but slowly improving.

    Unfortunately PRI can’t fix a jaw dislocation bc it can’t change a structural issue. That’s a true medical issue. PRI is good for resolving pain of unknown origins.

    Dysfunction of the TMJ is thought to be a contributing factor in some forms of tinnitus.
